Open enrollment eventsIn the days leading up to and during open enrollment, you will have the opportunity to meet with representatives from Asante Employee Health, Asante Health Promotion, Asante Physician Partners, Asante Population Health, Fidelity, Guild Mortgage, HealthEquity, MetLife, Regence/MedSavvy, Mercy Flights, Reliant Behavioral Health, The Standard, VSP and Willamette Dental Group to discuss your benefits-related questions. Spouses are welcome to attend all events.

What do employees need to do?Only employees who want to make changes to their current benefits, are enrolling or re-enrolling in a flexible spending account, or FSA, for 2020 or wish to elect earned-time-off, or ETO, cash-out will need to participate in open enrollment this year. If you do nothing, your current benefit elections and covered dependents will automatically roll over into next year, except for FSA plan participation and ETO cash-out.

under the plan and who are financially dependent on you for more than half of their support; or

Asante pays for a basic life insurance policy for all regular employees, and you are automatically enrolled on your date of hire (or benefits eligibility date). For details about this plan, please refer to myHR. Employees may purchase additional life insurance for themselves, their spouse or their child(ren). Refer to myHR for rates and biweekly premium amounts. Coverage limitations are as follows.

*For this open enrollment, all employees can elect up to the guarantee issue amounts of the plan. Any new amounts above the guarantee issue are approved following medical underwriting approval. When you or your covered spouse reaches the ages that follow, the amount of life insurance will be the amount you have, reduced to the following percentage of your benefit: ages 70–74: 67%; ages 75 and older: 50%. Limitations apply to coverage increases when the covered member is not actively at work on the coverage effective date.

†Employee coverage includes the employee’s supplemental life insurance policy in addition to the basic life insurance policy that Asante pays for.

2020 annual open enrollment, Nov. 1–13 | How to enroll

Don’tforget

If you are planning to participate in open enrollment this year, you must do so between Nov. 1 and Nov. 13, 2019. Funds for 2019 FSAs need to be used by Dec. 31, 2019, as they do not roll over from year to year. All employees who want an FSA for 2020 must re-enroll during this open enrollment period. The deadline to enroll is 11:59 p.m. on Nov. 13.

1. Go to myHR: https://hr.asante .org and click Continue to Sign In (if you’re automatically signed in, skip to step 3).

2. You will be asked to sign in: ■ If you are completing this from home, use your Asante email address (e.g., john.doe@ asante.org) and Asante network password. ■ If you are completing this from any Asante network workstation, use your Asante network username and password.

3. On the myHR home page, locate the widget titled Open Enrollment.

4. Click on My Open Enrollment.

5. If you plan to add new dependents to your 2020 benefits, click on the

Dependents tab on the menu on the left side of the screen.

6. Click the Add button.

7. To start your enrollment in benefits, click on the Enrollment tab. A list of benefits will display.

8. Select only the benefits you plan to change.

9. When finished making elections, you will land on the Review and Submit page, or you can scroll to the bottom of the menu and click on Review and Submit.

10. All of your elections will display

at the bottom of the Review and Submit page. To submit your elections, click the blue Submit button at the top of the page.

11. Confirm your submission.

Once you’ve submitted your elections, a confirmation statement will be sent to your Asante email address.

12. To make changes to elections you have already submitted, simply open the session again from the My Open Enrollment link on the myHR home page and resubmit by 11:59 p.m. on Nov. 13.

Employees are required to participate in open enrollment only if:■ The employee wants to enroll or re-enroll in an FSA for 2020■ The employee wants to make changes to current benefits■ The employee wants to add or remove dependents from coverage■ The employee wants to elect earned time off cash-out

Earn a premium credit through Know Your NumbersAs a regular Asante employee, you and your covered spouse have an opportunity to earn a wellness credit toward your 2020 Asante Health Plan premium contributions by participating in a biometric screening and online health assessment.

Step 1. Complete a biometric screening or preventive exam that includes a biometric screening by Dec. 6, 2019.Complete an onsite biometric screening at an Asante facility. Refer to myHR for appointment

times, locations and online scheduling or call Asante Health Promotion at (541) 789-5211. You may instead complete a preventive exam with your primary care provider if it includes biometric testing.

Step 2. Complete the Regence online general health assessment by Dec. 13, 2019. Your biometric values will be uploaded into your general health assessment.If you are currently enrolled in an Asante Health Plan, you must go to Regence.com and log in using your username and password. Enrolled

employees and enrolled spouses must each use their own profile. Once logged in, scroll down to the Wellness Tools section and click the link. On the Wellness home page, click Health Assessment to begin the general health assessment.

If you or your spouse is not currently enrolled in an Asante Health Plan but intend to enroll in one during open enrollment, email Asante Health Promotion team at [email protected] for instructions on completing the general health assessment.
