To register please complete and return this form to the email or postal address indicated.

If your Medical Grade is of PGY2 & above you are eligible to register for ICU Basic. All other medical officers below this level can still apply; however will be placed on a waiting

list. If the chosen course is not full you will then be accepted for the course. Preference is also given to medical and nursing staff currently in ICU and ED or those

intending to be placed there.

Please note that places cannot be reserved until payment has been received. Places will be offered on an “as required” basis; therefore each application will be

considered and ranked according to the clinical need of each individual. Please detail your need for the course in the section provided on the registration form to indicate how you

believe you will benefit from completing the BASIC course.

Applications will be considered and processed 6 weeks prior to the relevant course date. ICU & ED staff applying within the last 4 weeks will not be given priority.

Applications of less than one (1) week before the course are strongly advised against as it does not allow you sufficient time for pre-course preparation and exam.

If you are currently working at the John Hunter Hospital at the time the pre-reading

materials are released you are expected to collect them rather than opting to have them

delivered, where possible. Further, it is your responsibility to update your details such as

email address, postal address, credit card information etc. should they change at any point

after submitting your application. Not doing so may result in your payment being declined

and/or pre-reading materials not being delivered to you correct address; both of which will

hinder the time given to complete you mandatory pre-course learning and assessment.

COURSE CANCELLATION POLICY

Cancellation >21 calendar days prior - full refund Cancellation <21 calendar days prior - full refund: If your place can be filled from waiting list. *No refunds for cancellations within fourteen (14) days of the course date.

We reserve the right to cancel any course at short notice for reasons of equipment

malfunction, unavailability of instructors or other unforeseen circumstances. In the

event of cancellation, every effort will be mad e to re-schedule the course or

alternatively refunds will be provided under these circumstances.
