Dear Parents,

On behalf of the staff at Blair School, I am happy to welcome you to the 2018-19 school year! We are looking forward to

a productive partnership with you to ensure our children can achieve their highest potential. We recognize that in order

to be successful in school, our children need support from both the home and school. We know a strong partnership

with you will make a great difference in your child’s education. As partners, we share the responsibility for our children’s

success and want you to know that we will do our very best to carry out our responsibilities. We ask that you guide and

support your child’s learning by ensuring that he/she:

1) Attends school daily and arrives on time, ready for the day’s learning experience

2) Completes all homework assignments given by teachers

3) Reads daily to develop a love for reading and to improve literacy skills

4) Shares school experiences with you so that you are aware of his/her school life

5) Informs you if he/she needs additional support in any area or subject

Please consider joining our parent organizations and/or volunteer programs as our students can greatly benefit from

your involvement and contributions to the school’s program and its operations. We seek membership and volunteers to

help us with the following activities:

1) Join School Site Council, PTSA, or English Language Acquisition Committee (ELAC)

2) Reading/Math support with children who need extra help

3) School-wide events

4) Student recognition events

5) Outreach and recruitment of parent and community volunteers

I am attaching a copy of our district contracts so that you and your child can review them together. If you have any

questions about the rules and expectations, please feel free to contact me to discuss them with your child’s teachers. It is very important that you and your child are fully informed regarding standards related to appropriate behavior for a

safe and productive school year. Please feel free to review the PUSD Parent Handbook which can be accessed at

PUSD.US. The wonderful Blair staff and I feel privileged to be a part of this school family. We thank you for your support

Greetings Viking Nation! I’m looking forward to the 2018/2019 school year and I hope you are too! As the incoming PTSA President, I just want to take a moment to share with you things you may not know about PTSA (aka PTSA, what have you done for me lately?)

The funds collected by membership dues and donations are intended to be used in the same year – so your money goes directly to providing programs and supplies for your student

Our PTSA is a democratic organization – You as a PTA member get to approve all fundraising and how that money is spent – you will have a direct effect on the culture of our school and your student’s experience, (but you have to come to PTSA meetings to do so).

That S in PTSA stands for Student and that means students can hold board and chair positions – (this looks great on a college app!)

The PTSA represents the Viking Nation at many PUSD events and is even more essential in making sure that the PUSD Board of Education understands Blair’s value. Our Board members attend many district meetings so you don’t have to! We make sure that the Pasadena community understands how great our school and students are.

The PTSA provides our community with a weekly newsletter – the Blair EBlast. You can receive this by email (signup on our website or Facebook page) or by checking out our Facebook page each Sunday.

BUT - we can’t do any of this without your support. PTSA membership dues provide a large amount of our funding. At $10 a membership, if each family at Blair joined the PTSA that would be close to $10,000 we would have for programs. Student memberships are only $6! Having a large PTSA membership gives a greater voice in our community. Our school benefits greatly with your membership. I sincerely hope you will consider joining the PTSA and attending some of our association meetings (First Thursday of every month) Norah Small PTSA President, 2018/2019 Still unsure about joining? IMMUNIZATION AND TUBERCULOSIS TEST REQUIREMENTS FOR ENTRY INTO BLAIR SCHOOL

Please bring documentation to Viking Days

NOTE: There is no grace period for the Tdap vaccine prior to 7th grade entry. Any student who is missing this vaccine will not be allowed to attend classes.

For All Students New to Pasadena Unified School District and ALL 7th Grade Students

Tdap 1 Tdap given at age 11-12 years or above for all 7th grade students. This vaccination is mandatory for

entry into the 7th grade.

Polio (IPV) 3 doses meet requirement for ages 7 – 17 years if at least one was given on or after the 2nd birthday.

Diphtheria, Tetanus and Pertussis 3 doses meet requirement for ages 7 – 17 years if at least one was given on or after the 2nd birthday.

Measles, Mumps, Rubella 2 doses (1 dose must be on or after 1st birthday)

Hepatitis B 3 doses at any age

Varicella 1 dose or physician-documented history of the disease, chicken pox

Tuberculosis Test Students entering at any grade level who have never attended any school in California must present

documentation of a negative T.B. skin test (Mantoux) given at any previous time. A positive PPD must beaccompanied by a chest x-ray clearance with a medical summary within 21 days of the positive T.B. test.

Students who are entering our schools at all grade levels and are coming from a California school areexempt from this requirement.

Students showing a positive T.B. skin test must have evidence of a negative chest x-ray within 21 days ofT.B. test.

Prueba de Tuberculosis Los alumnos que están entrando a la escuela por primera vez, que nunca han asistido a una escuela en California,

deberán presentar documentación de una prueba de la piel PPD (Mantoux), recibida en cualquier tiempo previo.El comprobante mostrando la fecha en que se dio la prueba, fecha cuando se revisó, y el resultado.

Este requisito no es necesario para estudiantes que vallan a matricular en nuestras escuelas que hayan asistidouna escuela en California.

Blair School is a sixth grade through twelfth grade school. Blair provides an authorized International Baccalaureate (IB) Middle Years Programme and Diploma Programme. IB is an integrated holistic approach to thinking, teaching and learning that emphasizes international understanding and responsible citizenship. carefully designed a policy that protects the quality and safety of that environment.

PLEASE READ THE FOLLOWING CAREFULLY: There are two basic rules regarding cell phones, i-phones, i-Pods, and other devices:

1. Cell phones and other communication devices are not to be used or visibly displayed at any time during the school

day (including nutrition, lunch, and passing periods) and must always be turned off. If such a device is seen by teacher,

staff or adult representative of Blair School, it may be confiscated and then only returned to a parent or guardian.

2. IPods, Game Boys, headphones, and other portable entertainment devices are not permitted on the school campus at

any time during the school day (including nutrition, lunch, and passing periods). If these items are seen by teacher, staff

member or adult representative of Blair School, they may be confiscated and then only returned to a parent or guardian.

The school will not be responsible for replacing stolen or lost electronic devices.

Parents & Guardians Please Note: In accordance with state law, students are permitted to have cell phones and pagers in their possession for emergency

purposes only (for use on way to and from school). If a parent or guardian needs to contact a student during school

hours, they are to call the school office directly; staff will then contact the student as appropriate.

Some of our students use bicycles, skateboards, or scooters as a mode of transportation to and from school. Please read the following policy carefully:

1. Students may not ride a bicycle, skateboard, or scooter on campus at any time. 2. Students may not carry or walk a bicycle, skateboard, or scooter on campus during school hours.

3. Bicycles, skateboards and scooters may not be carried to class or stored in a classroom. Offices are also

not obligated to store these items for students.

4. If you plan to use a skateboard or scooter to and from school as transportation, then you must store the scooter or skateboard during the school day. Check with the Assistant Principal’s Office for available storage spaces.

5. If you plan to ride a bicycle to and from school, you must secure it using a bike lock to the rack provided in front

of the school.

6. If a bicycle, skateboard, or scooter is carried or used on campus during school hours, it may be confiscated, stored in the school safe, and only be returned to a parent or guardian.

The Blair community has adopted a dress code compliant with the Pasadena Unified School District guidelines (AR 5132 (a)-(f)). Parents are encouraged to engage in conversations with their children about the importance of coming to school with the appropriate school dress. Specific Minimum Dress and Grooming Standards for Grades 6-12

1. Shoes must be worn.

2. Undergarments must be worn at all times.

3. Clothing must cover all undergarments.

4. Clothing must completely cover the midriff area.

5. Tops and dresses must all have sleeves or straps.

6. Bathing suits may only be worn during approved P.E. activities.

7. Clothing, jewelry, and personal items (backpacks, fanny packs, gym bags, water bottles, etc.) shall be free of writing, pictures or any other insignia which are crude, vulgar, profane, or sexually suggestive, which bear drug, alcohol, or tobacco company advertising, promotions and likenesses, or which advocate racial, ethnic, or religious prejudice.

8. Clothing or grooming that is obscene or defamatory, or that so incites students to create a clear and present danger of the commission of unlawful acts on school premises, or the violation of school regulations, or the substantial disruption of the orderly operation of the school is not permitted.

9. Jewelry which creates a health or safety hazard is not permitted.

10. Gang related and/or obscene/profane/vulgar tattoos must be covered at all times.

11. Accessories which advocate or cause disruption on campus and/or other acts of violence or may be used as weapons are not permitted.

12. Hats may be worn during outside activities for sun protection.

No grade of a student participating in a physical education class shall be adversely affected if the student does not wear standardized physical education apparel because of circumstances beyond the student’s control. (Education Code 49066).

Código de Vestimenta

Library Books All books are checked out for 2 weeks. Books may be renewed for an additional two weeks unless the book has been reserved by someone else. Students are charged 15 cents a day for overdue books. If a book is lost or damaged, the cost will be for the replacement of that book, not the original cost.

Textbooks Textbooks are loaned to students. Students are responsible for these books and must pay for them if they are lost or stolen. Writing in the books or damaging them is not permitted and any writing or damage may be fined. If you have been issued a book with writing or damage, you must bring this to the attention of the librarian to avoid any fines during textbook return. Any books that are stolen or lost are considered your responsibility, so protect your books. It is a great idea to cover your textbooks so that you know which book is yours at all times and to protect it from any damage. If for any reason you leave Blair School, you must turn in all of your books. You will not receive an official transcript until you take care of your obligations.

Computers Any attempt to change or destroy one of the school’s computers will be a means for suspension. Priority for using the computers will go to those needing the computer for research on a specific project or to type a project. If you are creating documents, we strongly urge that you use a flash drive to save your work. Each computer’s hard drive is wiped clean and the computers are not configured to save any documents. If you don’t have a flash drive, check with the librarian. We occasionally have flash drives to loan out. It costs 5 cents a page to print; cut and paste your work into Word from the Internet when it is possible. Only print what you need and retrieve your intended items at the front desk. Do not walk behind the front desk as students are not allowed in this area.

Accelerated Reader Students must show an ID before they take a test and when they pick up their test at the front desk. There is a password on the Accelerated Reader computer. Students may not take a test for another student nor may not take different tests on the same book in order to practice. Any cheating on an AR test is considered a serious infraction that may result in suspension.

Library Rules The library is a quiet space for study, computer usage, and reading. Students are to use quiet voices at all times. Any student who is in the library and not working or reading may be asked to leave. The library will be determined by the work hours of assigned personnel. Hours of operation will be posted at the beginning of the school year. You must have proper permission to use the library—either an advance phone call from your teacher or an official pass with the teacher and student names. If you are in a group, all group member names must be on the pass. It is the responsibility of the student to let the substitute teacher know he or she must call the library before sending the student to the library. Even if you have permission, the library reserves the right to refuse your pass. If you are asked to leave the library for any reason and do not comply, you must expect to be written up. The library staff will be glad to help you work on a project, help you find a book, or give you instructions. Make sure that you have your work materials, including a paper and pen, with you.

Copier The copier for the library is being leased. It costs 10 cents a page to print. If you have a large printing, please bring your own paper.



Library/Text Book

Libros de la biblioteca Todos los libros pueden ser sacados por 2 semanas. Lector acelerado Los estudiantes deben utilizar su identificación antes de que hagan el examen, asimismo cuando recogen el mismo del escritorio de entrada. La computadora que tiene el programa para Lector Acelerado (Accelerated Reader) posee una contraseña. Los estudiantes no pueden tomar el examen por otro estudiante ni tampoco pueden tomar diferentes exámenes utilizando el mismo libro como práctica. No se tolerará engaño/copia.

Reglas de la Biblioteca Los estudiantes deben hablar en voz baja todo el tiempo. No será aceptado que un grupo de alumnos estén sentados alrededor de una mesa, hablando. En ese caso se le pedirá que se retire. La biblioteca estará abierta desde las 7:45 AM – 4:00 PM y si hay un solo bibliotecario disponible por el día, la biblioteca puede estar también cerrada durante la hora de almuerzo/comida. Usted tiene que tener el permiso adecuado para usar la biblioteca. Profesores y maestros suplentes deben llamar la biblioteca si ellos desean que sus estudiantes utilicen la misma. Cualquier estudiante que entre a las instalaciones de la biblioteca se le pedirá que se retire si su profesor no ha llamado. Es la responsabilidad del estudiante avisar al profesor sustituto que él o ella deben llamar al bibliotecario antes de enviar al estudiante a la biblioteca. Los profesores asistentes deben obtener permiso escrito de sus respectivos profesores para usar la biblioteca. Aún si usted tiene permiso, la biblioteca se reserva el derecho de no permitirle pasar. Si a usted se le pide que deje la biblioteca por cualquier razón y no obedece tal pedido, usted puede esperar que su nombre quede escrito por cometer una falta. Los bibliotecarios podrán cordialmente ayudarlo o darle instrucciones cuando usted esté trabajando en un proyecto. Traiga papel y lapicero con usted. No espere que los bibliotecarios se los provean. We expect students to be in good standing before they have the privilege to participate in activities and events sponsored by Blair Students must fully complete an “Eligibility for Extra-Curricular Activities” Form and have it cleared by an administrator prior to participation in any extra-curricular activity. The Eligibility Form must also be submitted before purchasing tickets to school events.

All students are eligible to participate in extra-curricular, school-sponsored activities (e.g. sports, performing groups, school events and dances, etc.) if they comply with the following eligibility requirements.

No more than 2 F’s in the last reporting period.

No more than 2 U’s in the last reporting period.

No more than 60 period absences (10 days), excused or unexcused, per semester.

No more than 120 period absences (20 days), excused or unexcused, for the year.

No more than 15 total tardies to all classes during the last reporting period.

No more than 4 disciplinary referrals to the Dean's office

No more than 4 dress code citations per year.

All detention hours must be cleared, including Saturday School hours.

All book fines and other financial obligations paid.

Completed Eligibility Form submitted to the assigned Assistant Principal’s office by the posted deadline.

SENIOR ACTIVITIES: Senior activities (Grad Nite, Prom, etc.) are for senior participants and their guests. Juniors may participate in senior a activities if they are eligible and are invited by a senior. Freshmen and sophomores are not permitted to attend senior events. Seniors must p purchase tickets to events in person; they may not send someone else to do it for them. G GUESTS TO BLAIR EVENTS: All guests from outside Blair to school events must be approved by the Assistant Principal’s office. Guest pass r equ est requests can be picked up in the Activities Office or Assistant Principal’s office. An administrator at the guest’s school or his/her employer must sign pass sign guest pass requests. The student submitting the request must also attach a copy of the guest’s photo I.D. Requests should be submitted two (2) two weeks prior to an event. The Assistant Principal’s office will attempt to process guest pass requests within 48 hours of receiving them. C CONDUCT AT SCHOOL EVENTS: Any student or guest whose conduct during a school event is deemed disruptive to the orderly and safe pro cee proceeding of the event may be escorted from the area and may be subject to appropriate disciplinary consequences.

ATHLETIC ELIGIBILITY Be Before a student athlete is eligible to participate on any athletic team, she/he must meet the following criteria with the Athletic Director:

1) Meet all C.I.F. and existing league’s requirements: Age Eight Semester Rule Scholastic Eligibility Residential Transfer Amateur Status

2) Meet the State of California academic requirements of 2.0, C grade point average in all classes. 3) Meet the citizenship requirements of a 2.0, C grade point average 4) A probation letter must be on file if the 2.0 GPA is not met. Probation is granted only one time in a school year, and is for 10 weeks only. 5) Have an Emergency Consent Form completed by parent or guardian, including proof of insurance, and other emergency information 6) Pass a yearly physical exam. 7) Have no outstanding debts with Pasadena Unified School District or Blair School 8) Meet ALL Blair eligibility criteria (see above).

Each athlete is responsible for picking up the proper forms from his/her coach, and for returning them to the Athletic Director. The Athletic Director will pro will process each athlete before he/she may participate on any sport team.

Esperamos que todos los estudiantes sean “buenos alumnos, con buena referencia” antes que tengan el privilegio de participar en actividades y eventos patrocinados por Blair IB. Los estudiantes deben llenar completamente un formulario de “Elegibilidad para actividades extra-curriculares” y éste debe ser aprobado por un administrador antes de participar en alguna actividad extra-curricular. El formulario de Elegibilidad debe también ser presentado antes de comprar boletos para los eventos escolares.

Todos los estudiantes son elegibles para participar en actividades extra-curriculares, actividades patrocinadas por la escuela, (deportes, grupos de arte dramático, eventos escolares, bailes, etc.) si llenan y mantienen los siguientes requisitos de elegibilidad:

No más de 2 Fs o 2 Us en el último reporte de calificaciones. No más de 60 ausencias de un periodo (10 días), justificadas o sin justificar en un semestre. No más de 120 ausencias de un periodo (20 días), justificadas o sin justificar en el año. No más de 15 retrasos en todas las clases en el último reporte de calificaciones. No mas de 4 sanciones disciplinarias con la Oficina del Decano. No más de 4 citaciones de vestimenta por año. Todas las horas de detención deben de ser cumplidas incluyendo las horas de escuela de sábado. Todas las multas de libros y otras obligaciones financieras deben de ser pagadas. El Formulario de Elegibilidad Completo debe de ser entregado a la oficina del Asistente del Director en la fecha

especificada.

ACTIVIDADES DE ALUMNOS DEL DOCEAVO GRADO: (Noche de Graduación, Baile etc.) son para alumnos del doceavo grado y sus invitados. Los alumnos del onceavo grado pueden asistir a las actividades de los alumnos del doceavo grado si son elegibles e invitados por un alumno del doceavo grado. Los alumnos del noveno y décimo grados no tienen permitido asistir a los eventos del doceavo grado. Los estudiantes del doceavo grado deben comprar los boletos en persona; y no pueden mandar a otra persona a comprarlos/recogerlos por ellos. INVITADOS A LOS EVENTOS DE BLAIR: Todas las personas, que no pertenecen a Blair, invitadas a los eventos escolares deben ser aprobadas por la oficina del Asistente del Director. Un administrador perteneciente a la escuela de la persona invitada o por consiguiente un empleador debe firmar el pedido de pase para el invitado. El estudiante que solicita tal pedido debe también adjuntar una copia de la identificación del invitado. Los pedidos deben ser enviados con dos (2) semanas de anticipación al evento. La oficina del Asistente del Director va a tratar de procesar los pedidos de pase para los invitados dentro de las 48 horas de haberlo recibido.

CONDUCTA EN LOS EVENTOS ESCOLARES: Cualquier estudiante o huésped/invitado cuya conducta se considere molesta o que es perjudicial al orden y proceso de seguridad en un evento de la escuela, será sacado del lugar y dependiendo de la gravedad del caso se le aplicará las sanciones disciplinarias que correspondan.

ELEGIBILIDAD PARA ACTIVIDADES ATLÉTICAS Antes que un estudiante pueda ser elegido para participar en cualquiera de los equipos de atletismo, debe reunir los siguientes requisitos establecidos por el Director de atletismo:

1) Reúna todos los requisitos establecidos por la C.I.F y la Liga: Edad La regla del semestre ocho Debe cumplir con los requisitos de estudios Residencia Transferencia Debe tener status de amateur

2) Reúna los requisitos establecidos por el estado de California: 2.0 en promedio (grado “C”) en todas las clases. 3) Reúna los requisitos de ciudadanía/comportamiento con un 2.0, o promedio de grado C. 4) Si no tiene un promedio de 2.0 debe presentar una carta donde se establezca que está a prueba. Estar a prueba se permite una sola vez durante

el año escolar, y es por un máximo de diez semanas. 5) Su padre o custodio, debe completar un Permiso en caso de emergencia, donde ofrezca la información sobre seguro de salud y cualquier otra

información necesaria para una emergencia. 6) Debe pasar un examen físico anual. 7) No puede tener deudas pendientes con el Distrito Escolar Unificado de Pasadera, o con la Escuela Blair. 8) Reúna todos los criterios de elegibilidad establecidos por Blair. (vea lo mencionado anteriormente)

Cada atleta se hace responsable de recoger de su entrenador las aplicaciones apropiadas, y devolverlas al Director de Atletismo. El Director de atletismo procesará cada aplicación antes que cualquier atleta pueda participar en algún equipo deportivo.

General District Policies related to Locker Usage in PUSD: • Because lockers are under the joint control of the student and the district, school officials shall have the right to open

and inspect any locker without student permission when they have or there is reasonable suspicion that the search may disclose evidence of illegal possessions or activity or when odors, smoke, fire and other threats to the student health, welfare or safety are known to emanate from the locker.

• For health and safety reasons a general inspection of school properties such as lockers and desks may be conducted on a regular, announced basis, with students standing by their lockers or desks. Any items contained in a locker shall be considered to be the property of the student to whom the locker is assigned. Notice of this policy shall be given to all students when lockers are assigned.

Conditions for Gym Locker Usage at Blair :

1. Locker usage is voluntary.

2. Students who participate in the voluntary locker program are required pay a $10 non-refundable use fee for the lock.

3. Only assigned school locks may be used on the school lockers. Any non-school lock used to secure a locker may be cut off and the school will not be held responsible for the lock or the contents of any lockers used/secured without authorization.

4. Students are not permitted to share lockers. Students are encouraged to never share their locker combinations with anyone.

5. Students at Blair School may only visit lockers during their PE class or athletic practices.

6. Due to security, maintenance and liability reasons student lockers are available on a “USE AT YOUR OWN RISK” basis. It is recommended that important items not be left in lockers overnight, on weekends, or holidays. The school assumes NO responsibility for items left in school lockers, including but not limited to textbooks and other items of value.

7. Students must agree to abide by the rules and policies as outlined by the Pasadena Unified School District. These guidelines are distributed during registration or when the student enrolled in school. (See Student Handbook for Appropriate and Expected Behavior). Students who violate these rules and the policies will lose the privilege of using the lockers.

8. Nothing shall be placed or posted on the outside of student lockers.

9. At the conclusion of each school year, it is the responsibility of the student to clean out the locker completely. Following the last day of school, the custodians will clean out all lockers. The school shall not be held responsible for any items left inside student lockers after dismissal on the last day of school.

10. Student concerns related to lockers should be reported to the office of the Assistant Principal.

I acknowledge the rules and conditions of the voluntary locker program with the Pasadena Unified School District. I agree to follow the rules and policies of the school district as outlined in the Student Handbook for Appropriate and Expected Behavior along with the conditions outlined in this voluntary locker usage contract. l understand that a violation of these rules and policies will result in the loss of my privilege to use a locker.

Debido a que los armarios están bajo el control conjunto del estudiante y el distrito, los oficiales de la escuela tienen el derecho de abrir e inspeccionar cualquier armario sin el permiso del estudiante cuando tengan o hayan sospechas razonables de que la inspección pueda revelar evidencia de posesiones ilegales o actividades, o cuando olores, humo, fuego u otras amenazas a la salud, bienestar o seguridad del alumno, parezcan emanar del armario.

Por razones de salud y seguridad una inspección general de las propiedades de la escuela como armarios y mesa bancos puede ser conducida y anunciada regularmente, con la presencia de los alumnos. Cualquier artículo contenido en el armario será considerado ser propiedad del alumno a quien el armario fue asignado. Noticias de esta póliza serán entregadas al alumno cuando los armarios se asignen.

Condiciones de uso de armarios en Blair:

1. El uso de los armarios es voluntario 2. A Los alumnos que participen en el uso voluntario de los armarios se les requiere pagar una cuota de $10.00 de

uso del candado.. 3. Sólo los candados asignados por la escuela podrán ser usados en los armarios. Cualquier candado que no fue

asignado por la escuela será cortado y la escuela no se hará responsable por el candado o el contenido de los armarios que son usados/asegurados sin su autorización.

4. No se permite a los estudiantes compartir un armario: se pide a los alumnos que no compartan la combinación de su candado con ninguna otra persona.

5. Los alumnos de Blair pueden ir a sus armarios sólo antes de la escuela, durante nutrición y almuerzo, y después de la escuela.

6. Por razones de seguridad, mantenimiento y responsabilidad los alumnos tendrán que usar los armarios en base a “SU PROPIO RIESGO”. Se recomienda no dejar artículos valiosos por las noches, fines de semana, o días festivos. La escuela no asume la responsabilidad por artículos que se dejen en los armarios, incluyendo pero no limitado a los libros de textos y otros objetos de valor.

7. Los alumnos deben acatar las reglas y pólizas que se dan por medio del Distrito Escolar Unificado de Pasadena. Estas reglas son distribuidas durante la inscripción o cuando un alumno se registra en la escuela. (lea: Manual del Estudiante para el Comportamiento Apropiado y Esperado). Los alumnos que violen estas reglas y pólizas perderán el privilegio de usar los armarios.

8. No se permite pegar o anunciar nada en la parte exterior de los armarios. 9. Al terminar el año escolar, es la responsabilidad del estudiante/alumno limpiar completamente de artículos el

armario. Después del último día de clase, el conserje limpiará los armarios y sacará todo artículo que quede dentro de ellos. La escuela no será responsable por ningún artículo dejado adentro de los armarios de los estudiantes después del último día de clases.

10. Si los alumnos tienen algunas reservas o preguntas sobre los armarios, por favor comuníquese con la oficina del Asistente del Director.

AUTORIZACIÓN DE EMERGENCIA MÉDICA

En caso de emergencia y si yo / nosotros no podemos ser localizados, Yo, el abajo firmante del estudiante arriba mencionado, un menor de edad, por la presente

autorizo a un representante del Distrito Escolar Unificado Pasadena y / o suplentes antes mencionados para que actúe como agente (s) para consentimiento para

cualquier diagnóstico de rayos X, anestesia, médico o quirúrgico o el tratamiento y la atención hospitalaria considere conveniente y prestados por un médico o cirujano,

ya sea en su oficina o en un hospital con licencia. Esta autorización está dada por adelantado de cualquier tipo de atención necesaria para potenciar un representante

del funcionario de PUSD para dar su consentimiento para el tratamiento como el médico lo crea necesario. Esta autorización se da en apego de la Sección 25.8 del

Código Civil del Estado de California y es efectiva el año escolar 2017-2018.
