HEALTHCARE THAT WORKS. HEALTHCARE THAT IS SAFE. HEALTHCARE THAT LEAVES NO ONE BEHIND.

2017 Community Service Report www.LOURDES.com

Dear Friends,

Lourdes is pleased to report to the community the services provided directly or in collaboration

with community agencies during 2017. These services addressed the community’s health status priorities of obesity prevention and managing associated chronic diseases, falls prevention with a focus on the elderly, and access to mental health and substance use services. In addition to these community priorities, Lourdes remains committed to addressing community needs in the areas of access to health care by increasing access to health care coverage and prescription medications, cardiovascular disease services, and maternal/child/adolescent health services.

Lourdes is privileged to serve the healthcare needs of the community. The Daughters of Charity, our founding sponsors arrived in Binghamton and opened the doors of the hospital in 1925. Over the past 92 years, 366 Daughters of Charity have served in health care, education, parishes, and social ministries throughout Binghamton and the surrounding communities. Our Daughters led, worked alongside, comforted, and encouraged us in all that we have done. In May 2017, we bid the Daughters of Charity farewell as they closed their house in Binghamton, moving to other ministries in service to those who are poor, vulnerable, and in need. Their mission has become our mission; we are grateful for their dedication and commitment to our community and look forward to carrying the mission forward for the next 100 years. Providing community benefit is central to our Mission, with particular attention to the poor and vulnerable.

As a leading health care provider, each year we provide millions of dollars in charity care and health education programs to our community. We take seriously our role and responsibility to provide access and coverage to quality health care services.

Beyond the walls of Lourdes, our associates extended our ministry into the community by serving on community boards, participating in community awareness events, serving at community meals, and participating in food drives and our Medical Mission at Home. I express my heart felt gratitude to them for living our Mission.

Lourdes continued to be an active participant in transforming the Medicaid health care delivery system by working with more than 150 partner organizations. Lourdes associates are committed to participate in transformative work to manage our community’s health.

As the healthcare needs in our region continue to grow, we remain committed to transitioning our care model to support managing populations of patients. We look forward to continuing to partner with the community. Our relationships inspire us and make our community a stronger, healthier place to live. We invite you to join us as we continue to participate in furthering the healing ministry to which we’ve been called.

2017 Community Health PrioritiesDuring calendar year (CY) 2017, Lourdes collaborated with community agencies under the direction of the New York State Health Department and County Health Departments to identify and address public health issues and underlying causes and contributing factors to health status and chronic disease. This report to the Community demonstrates how Lourdes has continued to work over the last year to help the Greater Binghamton community in the following community health priority areas:

• Falls Prevention with a focus on helping the elderly

• Access to mental health and substance abuse services

• Obesity prevention and managing associated chronic diseases with a focus on diabetes and reducing hospitalizations due to heart failure

CY 2016-2018 Community Health Needs Assessment and Community Health Improvement PlanDuring CY 2016, Lourdes and the Broome County Health Department updated their respective Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). Lourdes continued to focus on the following community health status priorities in CY17, paying special attention to the socioeconomic poor and vulnerable:

Commitment to Community Health Status ACCESS TO HEALTH CARE

Increased Health Care Coverage

Healthcare Coverage for the Uninsured and Underinsured

Lourdes continues to make a significant impact on healthcare access and coverage for the uninsured and underinsured through our complement of Financial Counselors. All counselors are Certified Application Counselors (CAC) through the New York State of Health Marketplace. Financial Counselors are available in six locations: the Emergency Department, Main Campus/Patient Financial Assistance Program (PFAP) Office, Hope Dispensary, Center for Oral Health, Lourdes Shippers Rd. facility (Vestal), and Lourdes 303 Main Street facility (Binghamton).

All Financial Counselors are able to assist individuals and families in enrollment in the Affordable Care Act (ACA) plans, such as NYS Medicaid, Child Health Plus, Essential Plans, Qualified Health Plans, and the Lourdes Patient Financial Assistance Program. The number of persons enrolled in the PFAP program continues to see unprecedented increases, in part due to the addition of prescription assistance and the 340B drug program. Enrollment at the end of 2015 was 7,598 people and at the end of 2016, 10,566 people were covered. Increases continued in 2017, with total enrollment at the end of the year at 13,018 people, an increase of 24% over the previous year.

Service to the Poor and Vulnerable

Lourdes continues to be an active participant in transforming the Medicaid health care delivery system by working with more than 150 partner organizations. As part of this work, in 2017 Lourdes received funding from Care Compass Network to:

• integrate behavioral health into primary care

• assist patients as they transition care settings

• improve health outcomes for patients with cardiovascular disease and chronic obstructive pulmonary disease (COPD)

• increase the use of palliative care to manage the symptom burden of chronic disease

• help patients navigate the health system and address social determinates that impact their health (such as transportation)

• add health coaches and navigators in the Emergency Room

• provided video conferencing for patients with depression

• embed social workers at nine sites within the Primary Care Network

Lourdes continues to address community mental health needs through annual depression screening for the adult population. The social work team follows patients through the continuum of inpatient and outpatient care. Lourdes will be expanding into additional primary care locations in 2018.

Lourdes is a downstream case management provider under Catholic Charities to support the health and wellness of the Medicaid population with a focus on those with a behavioral health diagnosis and/or multiple chronic diseases. Services include assistance

with food, housing, clothing, insurance benefits and connection with medical and social services.

A Medical Mission at Home was held at Our Lady of Good Counsel Church in Endicott in October 2017, serving 83 people. Free healthcare services including influenza vaccination, a medical screening exam, mammography, dental screening exam, foot/nail care, lung cancer screening, and additional services were offered to everyone attending. In addition, winter coats, hats and gloves collected by Lourdes associates were distributed to attendees. The following week, patients in need of dental services, based on the screening exam, were treated at Lourdes Center for Oral Health.

Lourdes associates also collected food and gifts for families for Thanksgiving and Christmas, serving over 200 families.

Commitment to Community Health Status

Increased Access to Primary Care, Prevention and Specialty Services Lourdes continued to advance the care for the patients we serve by the acquisition of an otolaryngology (ear, nose, and throat) and two orthopedic practices. During 2017 Lourdes recruited 25 providers in various specialties and primary care. The “Lourdes on Demand” virtual office visit service was launched in 2017 to further meet the needs and convenience of our patients to access care. The Primary Care offices continued their advancement in population health by earning the Patient Centered Medical Home (PCMH) designation in all 13 locations. The PCMH care delivery model coordinates treatment through the primary care physician to ensure patients receive care in a timely and efficient manner. Lastly, Lourdes unified the electronic medical record system across all primary care settings integrate with the inpatient electronic medical record; thereby improving patient care continuity.

Lourdes Center for Mental Health has continued to expand to address the need for mental health services in the community. The center served over 1,200 clients in 2017. Under the newly developed Department of Psychiatry, efforts to better link mental health providers across the Lourdes continuum have increased. For example, integrating social workers into primary care sites, which has raised the awareness of the need for social management in partnership with clinical management of patients.

Lourdes continued to offer the SafeTALK suicide intervention training program and the Screening, Brief Intervention, and Referral to Treatment (SBIRT) program to associates in 2017. The Lourdes specific Behavioral Health Task Force continued its work to better address community mental health and substance abuse needs. During 2017 Lourdes’ Robinson St. Primary Care Practice and Lourdes Endicott Primary Care practice each added a mental health satellite office. Lourdes continues to provide inpatient and emergency department screening for alcohol and substance abuse.

The Lourdes Student Assistance Program (SAP) is a New York State Office of Alcoholism and Substance Abuse Services (NYS OASAS) licensed Prevention

Counseling program. The primary goal of prevention counseling is to assist students in resolving personal issues before alcohol, tobacco and other drug usage becomes a coping mechanism. In 2017, services were expanded to serve 5 additional local schools through a grant obtained by Sen. Fred Akshar to increase prevention counseling in schools within his district. The SAP program served 3,693 students clients in 9 local schools in 2017.

The Lourdes Alcohol and Drug Education Prevention Team (ADEPT) is a NYS OASAS certified Education and Training Provider. ADEPT is a proactive approach and research-based process that focuses on increasing protective factors and decreasing risk factors that are associated with drug and alcohol abuse in individuals, families and communities. The goal of ADEPT’s prevention education is to delay the onset of drug and alcohol use in adolescents. In 2017, ADEPT served 2,082 students, Lourdes associates, and local community members on topics including current drug trends, adolescent brain development, heroin and prescription drugs, gambling, and opioid overdose Prevention. The Alcohol and Drug Education and Prevention Team (ADEPT) regularly hosts naloxone trainings. These free trainings are open to anyone in the community; the Southern Tier AIDS Program (STAP) provided the training and the naloxone kits to 105 participants.

The Mission in Motion mobile mammography van provides services at 64 locations throughout the Broome, Chenango, Cortland, Delaware and Tioga counties as well as Barnes Kasson Hospital in Susquehanna, Pa. This year services were extended through collaboration with community health providers creating local access to screening mammography services. No-charge screening mammography services were offered at two Medical Mission at Home events held within Broome and Tioga counties. A focus on reaching into rural populations where transportation is not readily available continues to be a priority.

Lourdes provides the area’s only palliative care services through an Inpatient consultative team and an outpatient clinic.

Lourdes Hospice continues to provide end of life care to patients of all ages, including pediatrics, allowing people to live comfortably, and make memories with their families in the time that they have left. Lourdes is the only hospice provider for Broome and Tioga county residents, providing over 32,500 visits, serving over 700 unique patients, with ages ranging from three months to 104 years old in 2017. In addition, Lourdes Hospice organizes Camp Hope for Kids to help children learn to cope with the loss of a loved one. It is a three day camp held at Camp Sertoma in Kirkwood.

Lourdes at Home provided over 63,000 visits to persons across Broome, Tioga, Chenango, and Delaware Counties in calendar year 17. Lourdes at Home is the only certified home health agency in these areas to provide services to pediatric patients, from infancy and throughout childhood. Adults and children can receive nursing, physical therapy, occupational therapy, respiratory therapy, speech therapy, social work, and personal care services as necessary with a physician’s order.

Lourdes Center for Pain and Wellness offers a resource for patients who are frustrated with their pain or pain medication. Comprehensive, patient centered care addressing physical wellness, as well as emotional and spiritual well-being is provided. Services include massage therapy, chiropractic care and minimally invasive therapeutic interventions.

Increased Access to Behavioral & Mental Health, Substance Abuse, and Maternal, Child and Adolescent Health ServicesLourdes Youth Services provided services to at-risk adolescents through three programs in 2017:

• The Lourdes Mental Health Juvenile Justice Project (MHJJ) worked successfully to keep 100% of the youth served out of detention and out of home placement. This was accomplished by providing intensive counseling and case management as well as providing life skills enhancement groups for the participants.

• The Lourdes Detention Alternative After School Program (DAASP) provided services to 30 high risk youth during 2017. Of these youth, 100% were successfully kept out of non-secure detention facilities; providing significant cost savings to the county (average cost for secure detention for 12 months is $150,000). The Lourdes DAASP program received additional funding from the New York State Office of Children & Family Services (OCFS) to expand programming and add a family programming component to encourage continued success in keeping youth out of detention facilities.

• Lourdes Youth Services program was selected as one of nine community agencies nationwide, to receive $325,000 per year in Federal funding to implement programming geared toward reducing violence, increasing school engagement and performance, and improving health outcomes for minority youth. This grant is a collaborative effort between the City of Binghamton, Broome-Tioga BOCES and Lourdes Youth Services. In its final year of funding, the BCAST (Binghamton Community and Schools Together) program served 55 youth (and their families) in 2017.

YES Safe Choices is a unique youth leadership program, sponsored by Senator Fred Akshar and Lourdes, which is focused on drug and alcohol prevention. Through youth leadership training, like YES LEADS, students learn about the dangers of drug and alcohol abuse. 2017 saw a record number of YES LEADers participate in our program. 642 YES LEADers from 16 middle and high schools throughout

Broome, Tioga and Chenango counties presented in classrooms to over 4,600 students on the dangers of drugs and alcohol, bullying, and the importance of making safe choices. In addition, these groups held various community outreach events in their districts, including free family movie nights, clothing and food drives, and fun nights for kids. Our Afton and Bainbridge-Guilford High School YES LEADS groups teamed up to co-sponsor an Opiates and Injury forum that was attended by approximately 250 members of the community. In late 2016, YES Safe Choices was selected as one of eight organizations nationally to offer “The pHARMING Effects,” a middle school aged peer-to-peer presentation on prescription drug abuse developed by Youth to Youth International in conjunction with Ohio State University and the Cardinal Health Institute. In 2017 five of our YES LEADS groups brought this presentation to approximately 550 middle school students. Expansion of the YES Safe Choices program into Delaware County occurred at the start of the 2017-2018 school year, adding 23 YES LEADers from Hancock High School and bringing the total number of schools participating in the program to 17.

ensuring that the care provided is in keeping with The Baby-Friendly USA “Ten Steps to Successful Breastfeeding” practices as much as possible. Lourdes staff participated in Baby Friendly breastfeeding training which has become a requirement for all new Women’s & Children’s Services associates regardless of position. Lourdes has become a designated milk depot for the New York Milk Bank located in the Hudson Valley of NY. Donors approved through the New York Milk Bank can drop off their milk at Lourdes and it will be safely stored and sent on to the New York Milk Bank for processing. Human milk is distributed to NICUs across the State for the most fragile infants by the New York Milk Bank. Lourdes employs two International Board Certified Lactation Consultants who provide in house breastfeeding care and support as well as provide outpatient care for more focused and complex issues. Lourdes works to promote healthy births in the community at DeMarillac Maternity Program through clinical expertise, education, support, financial assistance and compassionate care. Lourdes DePaul Pediatrics was established to meet the health needs of children. Lourdes’ emphasis is in education, preventive care and promoting good health. In addition to providers, the clinical staff includes experienced pediatric nurses and a full time social worker.

Twenty Five middle schoolers from Broome and Tioga counties participated in the Medical Academy of Science and Health (M.A.S.H.) Camp at Lourdes

in 2017. The goal of M.A.S.H. Camp is to promote the health professions to young people before they begin selecting high school course work and setting goals for their future careers. The two-day camp is co-sponsored by Lourdes Hospital, the Central New York Area Health Education Center (CNYAHEC), and Excellus BlueCross BlueShield. Students participating in M.A.S.H. Camp made compounds in the pharmacy, used the DaVinci surgical robot to learn about surgery, spoke to two pilots while getting a first-hand view of a Life Net Helicopter and participated in activities to learn about diagnostic imaging, radiology, respiratory therapy, and more.

Increased Access to Prescription MedicationsIn 2016 Lourdes was approved to participate in a federal drug discount program called 340B which helps hospitals like Lourdes to serve rural communities and treat our most economically vulnerable citizens. Under the program, eligible hospitals receive pharmaceuticals at discounted prices from drug companies. The hospitals, in turn, use the savings incurred for community benefit. At Lourdes, savings from the 340B program helped us support the Lourdes Patient Financial Assistance Program, provide discounted and sometimes free medications for the needy, and fund services such as primary care, diabetes, heart and cancer.

Lourdes continued to increase Pharmaceutical Access in the community through the Dispensary of Hope of the Southern Tier. Together with Pharmacy Assistance Programs, Lourdes served over 1,100 persons, filling over 1,800 prescriptions, which resulted in over $1,252,753 in free pharmaceuticals to the community, a 13% increase over the previous year.

Increased Access to Disease Screening, Prevention and Management

Cardiovascular Disease Awareness and Prevention

Community nurse navigators are being integrated into the Lourdes Primary Care practices to promote cardiovascular health through patient engagement during their primary care visit and follow up phone calls. Education includes: ways to reduce the risk of cardiovascular disease, early identification, and management of the symptoms associated with cardiovascular disease in order to optimize a person’s health and wellness.

COACH (Coordinated Outreach Achieving Community Health) for Heart Failure and COPD are multidisciplinary teams that span the healthcare continuum to improve the coordination of care for this patient population. COACH provides services to patients in the acute care settings, in homes, in primary care and specialty practices. The focus of the program is engagement of persons and their caregivers in the management of their chronic disease. The programs have had success in reducing the readmission rate for these patients. Community partner collaborations are being developed to promote ongoing wellness in the community.

Obesity Prevention

Obesity prevention and the treatment of associated chronic diseases such as diabetes and cardiovascular disease is provided throughout the Lourdes system, including primary care, endocrinology, the Diabetes Center, and the bariatric surgery program.

• Diabetes Education. Forty staff members participated in a professional teaching day availing themselves on the newest in medications, updates to diabetes standards of care, insulin pumps, and role of metabolism in bariatric surgery and prediabetes initiatives. Throughout the year, educational topics addressed hypoglycemia initiatives and discharge tools.

• Lourdes’ All Spice Café has healthy offerings available on a daily basis. The Action Station offers a variety of fresh ingredients, from which healthy meal options are prepared on demand. Superfood of

the Month promotions feature a different food each month with a display and recipes in the cafeteria, and brief articles on the Lourdes Facebook page. Lourdes team of Clinical Dietitians grew in 2017 with the help of a Care Compass Network grant. A Registered Dietitian is available at all Lourdes Primary Care locations to serve more members of our community. In addition to individual counseling, Lourdes dietitians provided a number of other opportunities for our patients through group classes, increased cafeteria promotions, media spots, and increased community presence; for example, a “Healthy for the Holidays” class in conjunction with Cornell Cooperative Extension, calorie amounts listed for all foods in the cafeteria to assist customers with making healthy choices, and participation in Hancock Harvest Days to promote nutrition.

Cancer Services

Lourdes Regional Cancer Center provided outreach, information, and education for breast, cervical and colorectal cancer through the Cancer Services Program, community health fairs and events, and support groups. Lourdes associates continued a strong presence and participation in the American Cancer Society’s Relay for Life and Making Strides Against Breast Cancer programs.

Initiatives taken by Lourdes to achieve the NYS Department of Health prevention agenda items to increase access to cancer prevention services included: maintaining National Accreditation Program for Breast Centers (NAPBC) for the Lourdes Breast Care Center and Mobile Mammography program (providing 1,863 mammograms to the rural population across 64 locations). NAPBC accreditation demonstrates a firm commitment by Lourdes to offer patients every significant advantage in their battle against breast disease. As a NAPBC accredited center, Lourdes commits to ensuring patients will have access to comprehensive care, a multi-disciplinary team approach to coordinate the best treatment options, and information about on-going clinical trials and new treatment options.

In 2017 Lourdes continued as a certified member of the MD Anderson Cancer Network®, a program of MD Anderson Cancer Center. The affiliation provides

10

2017 Community Service Report www.LOURDES.com

Lourdes Regional Cancer Center’s certified physicians with access to MD Anderson’s world-renowned expertise, offering patients enhanced cancer care close to home. The Regional Cancer Center has added nurse navigators to assist cancer patients as they journey through cancer care in the Breast Care Center, Urology and General Oncology. They assist with emotional, physical and financial care for their clients and caregivers. In addition, a medical social worker is available to assist patients with transportation needs and a registered dietitian provides nutritional guidance. 246 people participated in Lourdes lung cancer screening program in 2017. Lourdes Radiation Oncology offers the highest technological cancer treatment in the area by utilizing the “True Beam” intensity-modulated radiation therapy (IMRT) cancer treatment for multiple cancer sites.

543 people attended the Living well series and 104 attended the Connecting with Love and Hope which were Cancer Center monthly support/education events. Lourdes continues to provide a monthly educational series to cancer survivors on topics that concern cancer survivorship, such as nutrition, fatigue, and psychosocial concerns--543 people attended the series in 2017. In an effort to prevent lung cancer, Lourdes also provided smoking cessation classes throughout the year. Lourdes again sponsored Shine a Light on Lung Cancer Awareness Night, an annual event to raise awareness, funds and hope, and to honor those we have lost.

FALLS PREVENTION COMMUNITY OUTREACH PROGRAMS

The need to prevent falls in the community is evident by the over 3,500 persons visiting Lourdes walk-ins or emergency room upon experiencing a fall in 2017. Ways in which Lourdes helps to prevent falls include:

• Lourdes has offered the Tai chi for Arthritis curriculum since 2015 and has reached over 550 participants by offering choices of 10 different classes held twice a week for eight weeks, supporting the practice of Tai Chi among community members to improve balance and reduce falls. In 2017, 200 individuals benefited from this program, being taught year round. During good weather, Tai Chi “Saturdays in the Park” is supported and facilitated by a Lourdes Tai Chi instructor to increase outdoor activity of participants through the practice of Tai Chi.

• In addition to Tai Chi, Lourdes also started teaching the “Stepping-On” fall prevention workshop in 2017. This is an evidence based seven-week workshop where participants learn exercises and strategies to help improve balance and prevent falls. This program is for those who have fallen or have a fear of falling to provide support and build confidence, thus reducing falls. Tai Chi and Stepping-On are being taught to improve the quality of life and help keep seniors safe and independent, through better balance to reduce falls and injuries related to falls.

• 100% of Lourdes at Home physical therapy staff received advanced training and certification around fall prevention over the last year to help meet the growing needs of the community. Fall prevention education along with specific strengthening and balance activities have been incorporated in nearly all ambulatory patient visits to help decrease falls. Lourdes at Home participated in Broome County’s “National Fall Prevention Awareness Day” providing information to the community at the Oakdale Mall. Fall risk is assessed at every home care visit.

• Lourdes Physical Therapy provides comprehensive evaluation and treatment of balance and vestibular disorders. Highly trained staff are skilled at helping people prevent falls. Persons who may benefit

from balance or vestibular treatment are those who experience balance deficits due to generalized weakness, light headedness, vertigo with or without nausea, difficulty walking in visually busy environments or on uneven surfaces, post-joint replacement, post-fracture or sprains, post-stroke, neurologic diagnoses such as multiple sclerosis or Parkinson’s disease, malnutrition, and medication side-effects. Joint Academy patients are encouraged to attend Lourdes’ Pre-op Education class where patients and family members learn about safety in the home including proper use of assistive devices and simple modifications of the home setting such as installing railings, widening pathways and removing throw rugs.

• Primary Care. Falls prevention screenings are performed as part of the Medicare primary care wellness visits, and for patients identified by their provider to be at risk for a fall.

• Retail Pharmacy & Medication Management. Pharmacists discuss falls prevention with patients as part of their visit to the pharmacy. Whether filling a new prescription that has a fall risk to it or refilling other prescriptions, our pharmacists are aware of falls risk criteria and utilize that while communicating to patients about their prescription use. The Vestal Retail Pharmacy provides counseling for falls prevention in conjunction with the Tai Chi and Stepping On programs.

COMMITMENT TO HEALTH CARE THAT IS SAFE

Lourdes is committed to providing quality care to everyone we serve. By instituting a number of quality improvement initiatives, Lourdes continues to offer improved community health status.

• Cardiovascular Disease, including stroke and heart failure, remains a significant health concern locally and nationally. Lourdes’ commitment to improving community health status around stroke care is evidenced by maintaining its New York State Health Department Stroke Center designation. Lourdes has maintained its Gold Plus award, five years in a row, from Get with the Guidelines for exceptional care in the management of patients that present to Lourdes with symptoms of stroke. The award recognizes efficient processes, evidence based care through a multidisciplinary team approach and quality outcomes.

• Lourdes adheres to a Universal Influenza Vaccination annually to provide for the safety of all those in our care. Associate Health provided over 2,800 influenza vaccinations to associates and volunteers in 2017.

About LourdesFor over 90 years, Lourdes has been a cornerstone of our community, offering resources that have made a difference. With our Catholic faith tradition, Lourdes Hospital provides the holistic, compassionate care that every patient deserves.

Lourdes is more than an inpatient acute care hospital…we are an outpatient ambulatory surgery center…we are a Regional Cancer Center. We provide Palliative Care and Hospice Care, an orthopedics program, diabetes care, home health care, and much more. Reaching beyond the boundaries of our main campus, Lourdes has established a network of primary and specialty care physician offices at convenient sites throughout the region. The Mobile Mammography and Mobile Dental

Care programs provide women’s health and mammography services and dental care through two mobile medical vans, making health care accessible to rural populations and children in need.

Lourdes is also a member of Ascension Health, the largest, Catholic, nonprofit health system in the nation with 150,000 associates and 35,000 affiliated providers serving in 1,900 sites of care across 23 states and the District of Columbia.

Much has changed in the community and at Lourdes since the Daughters of Charity opened a small 25-bed hospital on Riverside Drive 90 years ago, but one thing remains constant: the commitment and Mission of caring found here each and every day.

HEALTHCARE THAT WORKS. HEALTHCARE THAT IS SAFE. HEALTHCARE THAT LEAVES NO ONE BEHIND.
