While nurses have been instrumental to the gains made in improving the quality and safety of U.S. healthcare over the last decade and a half, recent studies suggest U.S. patients experience a far greater number of adverse events each year than even suggested by the Institute of Medicine in its landmark 1999 report To Err is Human. Along with the importance of patient safety, so too, is that of the healthcare provider. The American Nurses Association has a long-standing commitment to ensuring the health and wellness of nurses in all settings. Patients and communities, as well as the nursing profession at large, all stand to gain from greater nursing engagement in fostering a culture of safety in healthcare.

Learning Objectives:

• Cite the major policy documents that have shaped the national patient safety conversation. • Define and identify key elements of a “culture of safety” in healthcare. • Identify at least three action steps registered nurses can take to contribute to a culture of safety in their practice

Ms. Coulombe is leading the largest, national quality improvement initiative ever funded by the government—Partnership for Patients’ AHA/HRET Hospital Engagement Network project! As the project director leading over 30 state hospital associations representing over 1,600 hospitals engaged in improving quality and patient safety, Ms. Coulombe will share key insights and strategies hospitals can use to accelerate change in their organization. To date, projected cost savings amount to nearly $20-billion. She will also share some of the key lessons learned as well as project goals for the coming year—including harnessing employee energy and commitment to improving worker safety.

This presentation will include an overview of the TeamSTEPPS for Long Term Care program, including the implementation in Connecticut nursing homes. Lessons learned from the implementation process and preliminary data related to the culture of patient safety, turnover, and employee injury rates in the pilot facilities will be shared.

2:30 p.m. – 3:15 p.m. Closing PresentationShaping a Culture of Safety through Engagement Jackie Conrad RN, BSN, MBA, RCC™, Improvement Advisor, Leadership Coach, Cynosure Health Solutions

Creating a healthy culture of safety requires a top down and bottom up approach. While leadership sets the tone and creates the infrastructure from the top, it is through active engagement with the front lines that a commitment emerges, making work safer from the bottom up. This session will outline partnership strategies to align leadership and front line staff in advancing worker and patient safety.

Learning Objectives:

• Summarize the links between employee engagement, worker safety, and patient safety.• Compare and contrast structural and communication strategies to engage front lines in safety improvement.

Speaker Profiles

Mark Boutilier has over 16 years of experience in the healthcare field. He currently works in emergency services as a paramedic and instructor for the American Heart Association BLS, ACLS and PALS. Additionally, he presented his personal experiences as a patient safety advocate relating to “Nurses as Patients When Harm Happens” at Maine’s 2014 Patient Safety Academy.

Jerolyn Ireland, RN, is a patient safety advocate. Prior to sharing her experiences as an advocate, Ms. Ireland worked in clinical nursing, and as an assistant nursing instructor, healthcare educator, community health director, and tribal public health liaison author of the Maine Intertribal Health Newsletter. She served on multiple professional health committees relating to Best Practice Acts and certifications at state and federal levels. Advocating for patient-centered care has always been a priority for Ms. Ireland.

Pamela F. Cipriano, PhD, RN, NEA-BC, FAAN, is the 35th president of the American Nurses Association (ANA), the nation’s largest organization representing the interests of 3.4 million registered nurses.

A distinguished nursing leader, Dr. Cipriano has held executive positions in healthcare systems, academia and national professional organizations in her career. In 2015, she was named one of the “Top 100 People in Healthcare” and one of the “Top 25 Women in Healthcare,” both by Modern Healthcare magazine.

Prior to becoming ANA president, Dr. Cipriano was senior director for healthcare management consulting at Galloway Advisory by iVantage. She has served in faculty and leadership positions at the University of Virginia (UVA) since 2000, and currently holds a faculty appointment as research associate professor at the UVA School of Nursing.

Dr. Cipriano is known nationally as a strong advocate for healthcare quality, and has served on a number of boards and committees for high-profile organizations, including the National Quality Forum and the Joint Commission. Dr. Cipriano

was the 2010-11 Distinguished Nurse Scholar-in-Residence at the Institute of Medicine.

Dr. Cipriano holds a PhD in executive nursing administration from the University of Utah College of Nursing, a master of nursing degree in physiological nursing from the University of Washington, and a bachelor of science in nursing degree from American University.

Rachel Kaprielian is the Regional Director of the U.S. Department of Health and Human Services leading Secretary Sylvia Burwell’s efforts to improve America’s health and enhance the delivery of human services to some of the most vulnerable populations throughout New England.

Prior to her appointment by President Barack Obama in 2015, she was a member of Massachusetts Governor Deval Patrick’s Administration Cabinet as the Secretary of the Executive Office of Labor and Workforce Development where her accomplishments included streamlining an online public platform for Unemployment Insurance claimants. Before serving in executive roles, Ms. Kaprielian was a six-term member of the Massachusetts legislature where she was a leader in healthcare

policy including the landmark 2006 Massachusetts Healthcare Reform Act. Her leadership in health policy has also been recognized in prevention measures such as tobacco cessation and education, early intervention and care for children, electronic healthcare records management, job training, and service initiatives for the disabled, and those in home and rehabilitative care.

Ms. Kaprielian is a lifelong resident of Massachusetts. She holds a BA from the College of the Holy Cross, a JD from Suffolk University, and a MPA from the Harvard Kennedy School (HKS).

Charisse Coulombe, MS, MBA, CPHQ, is the Vice President, Clinical Quality, with the Health Research & Educational Trust (HRET) at the American Hospital Association, providing oversight of all areas of the HEN 2.0 project including data collection and analysis, state relations, meeting content, and operation logistics.

Previously, Ms. Coulombe was with Resurrection Health Care (RHC) for 16 years. As System Director, Informatics at RHC, she oversaw system-wide quality scorecard development, CMS core measure data submission and managed several large measurement databases. Reporting to the Chief Medical Officer, she played an active role in the integration of the quality data and

measurement systems when RHC merged with Provena Health.

Ms. Coulombe has a master in business administration degree from University of Phoenix, a master in zoology from Western Illinois University, and a bachelor of science in biology from Northeastern Illinois University. She also holds a Six Sigma Black Belt, and is a Certified Professional in Healthcare Quality.

Michelle M. Pandolfi, LMSW, MBA, LNHA, is the Director of Consulting Services at Qualidigm and has seventeen years experience in the long term care industry, twelve of which have been in managing national nursing home quality and patient safety projects.

Under Ms. Pandolfi’s leadership, the Qualidigm improvement team began the Connecticut Nursing Home Quality Care Collaborative (CTNHQCC), a national quality improvement initiative focused on improving the overall quality of care for nursing home residents by eliminating or reducing healthcare-acquired conditions. The Collaborative has now expanded to include all six New England states and will address topics such as Quality Assurance Performance Improvement (QAPI), Falls,

Physical Restraints, Readmissions/Acute Care Hospitalizations, Pressure Ulcers and Antipsychotic medication use.

Ms. Pandolfi continues to use her expertise to develop training materials and learning aids for use in the nursing home. As a TeamSTEPPS Master Trainer, she presents at national conferences describing her work on developing the TeamSTEPPS curriculum for long-term care under an Agency for Healthcare Research & Quality (AHRQ) funded contract. Michelle also led the team who worked with the University of Connecticut Health Center to develop and design “Heart Talk” and “Lung Talk,” evidence-based educational video series to guide healthcare providers, patients and their families in managing heart failure or COPD. The video series are used in facilities in 28 states and numerous countries around the world.

Ann Spenard, MSN, RN-BC, is a Vice President and Principal at Qualidigm. Throughout her extensive career as a long-term care and geriatrics specialist, Ms. Spenard has led national quality, patient safety, and data validation projects. She has also taught nationally to providers groups and the Centers for Medicare & Medicaid Services. Ms. Spenard has served as the clinical coordinator to the national Nursing Home Quality Initiative and senior clinician and the manager of the data verification project for the MDS, otherwise known as, DAVE2, for Qualidigm.

Additionally, Ms. Spenard has been involved in multiple research projects and has published on topics related to patient safety and healthcare quality improvement. She is a master trainer for

TeamSTEPPS, an evidence-based teamwork program to improve patient safety, and helped adapt this national program for use in long term care.

She has served in academic appointments at Yale School of Nursing and the Frank H. Netter M.D. School of Medicine at Quinnipiac University, where she is currently an Associate Clinical Professor.

Jackie Conrad RN, BSN, MBA, RCC™, is a Leadership Coach, Improvement Advisor, with Cynosure Health Solutions. In this role, Ms. Conrad focuses on individual and team coaching for improved outcomes in: patient safety, hospital acquired condition reduction, leadership development, change management, improvement science, interprofessional collaboration, and patient family engagement. As a national improvement advisor for Cynosure Health, AHA HRET Hospital Engagement Network Partnership for Patients Program she assists with reducing patient harm at the local, state, and national level. Additionally, Ms. Conrad serves as a clinical lead for fall reduction, pressure ulcer reduction, and critical access and rural hospital affinity groups. Her work includes coaching state hospital associations to reduce harm at the hospital level, and expert large

and small group facilitation skills in virtual and in-person settings.

Prior to her work with Cynosure, Ms. Conrad served as Chief Nursing Officer and Vice President of Patient Care at Adventist GlenOaks Hospital and Mount Sinai and Schwab Rehabilitation Hospitals.
