The mission of the ABMS is to serve the public and the medical profession by improving the quality of health care through setting professional standards for lifelong certification in partnership with its member boards. August 1st marked an important transition for the AmericanBoard of Plastic Surgery; Barry Noone retired after 18 years asthe first Executive Director of the ABPS. He represented us su-perbly at the ABMS and led the Board through countless complexsituations. Present and past Board directors established andfunded a new Noone Endowed Lectureship to be given at the As-sociation, and presented Barry with a new set of golf clubs-whichhe promptly used to hit his first hole in one!

Keith Brandt has now capably taken over the reins of the Board.He has worked with Barry for the past two years and is now tire-lessly commuting to Philadelphia twice a month. He has readevery file in the Board Office and has established excellent tieswith the ABMS and the other Executive Directors. The transitionwas flawless and we are in very capable hands!

Our greatest strength as a Board is our presence as one of the 24member boards of the American Board of Medical Specialties(ABMS). The mission of the ABMS is to serve the public andthe medical profession by improving the quality of health carethrough setting professional standards for lifelong certification inpartnership with its member boards. This relationship establishesour position as the only ABMS board with the name “plastic sur-gery” in its title.

It also requires Maintenance of Certification(MOC-PS) to ensure that board certified doc-tors continuously refresh and expand theirknowledge. Just as you want your airline pilot tobe up to date on the newest planes and airports,the public expects that we will also continue tooperate at the highest standards of care, safety,and ethics. At a time when medical mistakes areestimated as the third leading cause of death inthe United States, it is imperative for physiciansto make patient safety a priority.

After ten years of experience with MOC, theABPS is now ready for MOC 2.0! This will bean ongoing process over several years. The Boardwants to make the MOC process educational,valuable and user friendly. We evaluated whathas been working and what was not valuable.Improvements began with the elimination of thesix month case log requirement previously sub-mitted for the examination application. Costswere high to put MOC in place, but as moreplastic surgeons enter MOC, we have been ableto decrease the price with reduction of the tracerfee, reduction of the MOC article fee, and re-duction of the examination fee. We hope tocontinue this trend. We have gathered years ofdata from the MOC tracers and this has beenutilized in several scientific publications. We re-cently voted to allow practice improvement proj-ects as delineated in the ABMS MultispecialtyMOC Portfolio Program to substitute for atracer procedure, and to allow patient safety ar-ticles and MOC articles from multiple journalsto count for your MOC credit. We want to offermore choice for your MOC requirements.

The American Board of Internal Medicine hasfaced substantial criticism from their diplomates,but they have chosen to make their MOC a highstakes examination with a fail rate of 20%! Ourplastic surgery MOC examination contains 200questions that are contained in the MOC guideavailable at ASPS. If you study, you will pass. TheMOC-PS examination is meant to be an edu-cational experience, and our fail rate is 3% to 5%.

A new, rejuvenated Board website will take youright to your MOC page after you log in. Yourrequirements will clearly be flagged and yourprogress will be easily tracked.

Did you also know that MOC participation canget you CME credits? There is a link on yourMOC tracking page on the Board’s website thatdirects you as to how you can obtain 60 CME’sfor successfully taking the MOC examination.If you go to  www.ama-assn.org/go/cmeformsand click the link to direct credit application,you can create an account and use a copy of yourresult letter to obtain 60 CMEs. It does requirean AMA login and fee payment but you do nothave to be an AMA member.

MOC also continues to be recognized nationallyas a verification of continuous physician devel-opment. Eleven states currently accept MOCparticipation in lieu of the CME credits requiredfor licensure: Idaho, Iowa, Kansas, Minnesota,New Hampshire, North Carolina, New Mexico,Oregon, Texas, West Virginia and Wyoming.Thirteen states are now in the process of accept-ing MOC: Louisiana, Georgia, Wisconsin, Con-necticut, California, Arizona, Rhode Island,Virginia, Missouri, Nevada, New York, Ohioand South Carolina. MOC is evolving, and willbecome easier, more efficient, and less costly.

The Written Examination has been developed inhouse for the past three years with a cost savingsof $60,000 per year. Each question is rigorouslyexamined by psychometric analysis and poorlyperformed questions were deleted from scoring.Overall test reliability is at an excellent level of0.9 or above.

The Oral Examination will evolve in 2016 to in-clude electronic uploading of the candidate cases.Those blue folders and thick rubber bands maysoon be souvenir items on ebay! We will test thepossibility of using iPads to present the Oral Ex-amination Theory and Practice Session A and B“unknowns” next year. At the November 2015

Chair Report - Continued from Page 2Oral Examinations, the post examination candidatedebriefing session was very quiet with no majorcomplaints. Candidates praised the uniform, pro-fessional, and fair way that they were examined.

What does the Board Office do for you on a dayto day basis? The Board Office receives 58,000per year consumer web hits, 4,800 monthly webhits and another 2,600 per year of emails andphone calls from the public concerning boardcertification of their plastic surgeon. Letters aresent to physicians wrongly stating that they areboard certified when they are not. Exorbitantadvertising complaints are reviewed and unethi-cal behavior complaints are examined. TheBoard works in conjunction with the ResidencyReview Committee for Plastic Surgery (RRC-

PS) and the American Society for Plastic Sur-geons (ASPS) to represent our interests as we in-teract with otolaryngology, general surgery,dermatology, orthopedics, and ophthalmology.

At the May meeting in 2016, we will have a oneday retreat to strategically plan for the next fiveyears. The Board welcomes your input and ideasand appreciates all feedback. GIVING THANKS

So 10 weeks into my new gig as the Executive Di-rector of the American Board of Plastic Surgery, Iwas thinking, I needed to ask for a 4 month exten-sion to my contract, because nobody should haveto start this job August 1st. The selection of the 5cases for each candidate taking the Oral Examina-tion had just gone out and the Board Office wasflooded with anxious calls regarding missing pic-tures, documents or patients. The exam teams forthe upcoming Oral Examination were goingthrough layers upon layers of conflict prevention.Agenda items for the various committees, meetingduring the Semi-Annual Board meeting, were pil-ing up. A pipe leaked in the Board Office, requiringall the flooring to be ripped up and the walls to be

ripped out. I received my first hate mail, a fourpager! I felt honored that someone went to all thateffort for a guy who has only been on the job 10weeks. Despite all the fires burning, things are get-ting done with complete composure, well exceptfor that new guy. I remember looking around theoffice and thinking, this isn’t chaos this is ROU-TINE!

On an average day the Board Office phones ringnon-stop. Four immensely talented ladies field thecalls with warmth and efficiency. Melissa Hill,Gwen Hanuscin, Melissa Karch and Maria D’An-gelo, although coordinators of their own ABPSprocesses (Written Exam, Oral Exam, MOC,

Thephone rings and you get a real person within threerings and that person helps solve your issue withoutreferring you to another staff member. The ulti-mate in customer service.

Before starting, I made several trips to the BoardOffice to become oriented. I sat down with eachstaff person and said “show me what you do.” Tomy astonishment, each staff member produced adocument with all of their tasks, FOR THE EN-TIRE YEAR, laid out in chronologic order. Everytiny detail that had to happen to develop and exe-cute the four exams (Written, Oral, Hand andMOC) without a hitch was right there in print.Whoever set this up is a genius and guess what, shestill works here. Terry Cullison, RN, MSN hasbeen the office administrator for the Board Officesince 1997. To her, no detail is too small. I ask,“How do we…?”, “That’s in the operations man-ual”, she replies. “How do we determine…?”,“Look on page 23 of the Booklet of information”is the quick response. “Where can I find…?”, “It’ssaved in the server files” she answers, “How do youprepare the budget and pay all bills?”, Terry makesa spreadsheet appear. No scrambling, no hesita-tion, just everything in its own place.

Three examiners had to withdraw the week beforethe Oral Examinations. With all the potential con-flicts between examinees and examiners, I’m think-ing this is going to be a problem. Instead, threeprevious examiners who weren’t scheduled to at-tend the Oral Exams, dropped everything and gra-ciously volunteered to fill in. The exams and Boardmeetings go on without a hitch…except for thePointe Hilton at Tapatio Cliff ’s guest who crashedinto the water main of the Different Pointe of Viewrestaurant forcing the Directors to have dinner ina drab hotel ballroom.

This year’s Board meeting was filled with bold ideasfor upgrading the Oral Exam and MOC programs.Even though both programs have run smoothly for

years, the Directors focused on what’s best for thecandidates and diplomates and suggested huge ini-tiatives. The first goal will be to advance the OralExam to a digital format and convert the 3 foothigh stack of blue Case Report books the candi-dates submit on Thursday evening to just a tabletfor all three sessions of the exam. The ABPS MOCprogram has always been the envy of the ABMSmember boards but that doesn’t mean it can’t im-prove. A Board retreat is scheduled for May tocompletely reassess the program looking for newways to engage diplomates, make activities moreaccessible and more meaningful and reduce costs.

The Executive Director position is quite unlike anyother position in Plastic Surgery. Every day is filledwith activities centered around: certifying candi-dates, overseeing Maintenance of Certification, de-fending threats to our scope of practice, evaluatingstate license actions and dealing with the public, in-surers and the government. While certainly chal-lenging, I had the best mentor ever. I learned of myselection in May of 2013 and for almost two and ahalf years I was able to shadow Dr. Barry Noone ashe skillfully dealt with all these tasks. His knowl-edge of the growth and development of the spe-cialty is phenomenal as he experienced it, at thehighest level for 18 years. The respect that he hasearned at the ABMS is unmatched. The fact thathe was asked to chair the ABMS Executive DirectorCaucus of all the 24 ABMS boards for many yearsis testimony to his venerated status.

From the Executive Director - Continued from Page 4I am truly grateful to the Board Office staff and ad-ministrator, the directors and the past Executive Di-rector, all of whom have helped me so much duringthis transition. The dedication and professionalismdisplayed by everyone involved with the Board is ex-ceptional. So if you’re worried that the Board is in thehands of a rookie, fear not, I’m surrounded by the best.

NEW DIRECTIONS

What is the number of certified plastic surgeons whohave had their certificates revoked by the AmericanBoard of Plastic Surgery? Answer: 81 and almost allfor state medical license revocation for unethical be-havior. Surprised? The Board however has receivedten times as many notifications of State medical licens-ing actions short of revocation such as; suspension,probation, fines or censure. Many actions are for veryserious infractions but the state medical boards reactvery differently to similar infractions. The mission ofthe ABPS “is to promote safe, ethical, efficacious plas-tic surgery to the public” and the ABPS requirementsfor certification mandate that candidates/diplomateshave a “current, valid, full and unrestricted state med-ical license to practice medicine.”

To date, the Board has investigated all license actionsreceived but not publically reported the Board’s con-clusions. The Board’s mission to protect the public de-mands that. Going forward, the Board must fullydisclose confirmed state medical license actions. Ac-cordingly, all state actions will be reviewed by the Ex-ecutive Director and the Ethics Committee. If thecommittee agrees that the behavior warrants disci-pline, the Board will consider a similar action againstthe diplomate’s certificate. If a certificate is suspended,

the suspension may run concurrently with the statemedical board action. In some cases, the Board maynot take any action against the certificate but will poston the Board’s consumer search, Is Your Plastic Sur-geon ABPS Board Certified?, that there has been anaction taken against the Diplomate’s license. In all in-stances the Board will refer anyone looking for moreinformation back to the State Medical LicensingBoard for details regarding why the action was taken.

The ABPS is currently working with its webmaster tocreate the infrastructure to begin reporting license actionsand the new category of certificate “suspension.” Re-porting is estimated to begin in July. The Board will re-view all actions for the past five years and any action thatis still current may be reported. It should be noted thatmany states have rules that, in the case of court proceed-ings, no action will be taken on a license until the courtcase is complete, which can often be very prolonged. Be-cause of this, license action often occurs manyyears after the actual event. The Board will focus on thoseinfractions that impact patient safety. Any questionsshould be directed to the Executive Director via theBoard Office.

In an attempt to help diplomates, the Board has de-veloped case scenarios that showcase common entrap-ments that diplomates might be subjected to. Thesewere designed to help diplomates understand the op-tions available to them and to help them learn expectedbehavior. These scenarios has been included into theResident Educational Curriculum on the Plastic Sur-gery Education Network, http://www.psenetwork.org.Hopefully by starting in residency, plastic surgeons canbe guided by ethical principles and professionalismthroughout their careers.

ABPS CONSUMER SEARCH - “IS YOUR PLASTIC SURGEON ABPS CERTIFIED?” The Board’s website www.abplasticsurgery.org offersthe ability for consumers to search for board certifiedplastic surgeons. The surgeon’s certification status,dates of initial certification and maintenance of cer-tification and status of whether a surgeon is currentor not current with MOC requirements is provided.Lifetime Certificate Holders have a listing in theMOC-PS® Participation Column only if volun-tarily participating, otherwise that column remains

blank. There are approximately 185, not includingretired, Lifetime Certificate Holders currently en-rolled in MOC-PS®.

Web searches increase each year with an average of58,000 per year. An additional 2,600 consumerscontact the Board Office by phone or email to ver-ify certification of plastic surgeons. ABMS also ver-ifies certification at www.certificationmatters.org.

ABPS ADMISSIBILITY POLICY The American Board of Plastic Surgery, in accordancewith the ABMS Eligibility Policy, approved the admis-sibility policy in 2012 for surgeons seeking certificationin plastic surgery by ABPS:

Residents who complete an accredited plastic surgeryresidency training program in 2012, and thereafter,must complete the certification process within eightyears of the conclusion of residency. This allows for thepractice requirement of one year which is necessary tocollect and submit cases for the Oral Examination.Candidates currently in the examination process mustbecome certified by successfully completing the OralExamination by November 2018 to meet the January1, 2019 ABMS Policy deadline.

Candidates must successfully complete both theWritten and Oral Examinations required toachieve certification within eight years aftercompletion of plastic surgery residency training.Reapplication is required at the end of the firstfive years of admissibility.

Candidates are advised to utilize every opportunity (i.e.examination administration years) to complete the cer-tification examinations. That focus will help candidatesavoid reaching the maximum admissibility limits andbeing subject to the additional reapplication require-ments after five years of admissibility or the more rig-orous requirements for the RE-ENTRY Applicationfor Admissibility after eight years is exhausted in theexamination process. The full policy is posted on theBoard’s website under Policies.

TO OUR DIPLOMATES WHO SUCCESSFULLY COMPLETEDTHE MOC-PS® COGNITIVE EXAMINATIONIn 2015 the following 289 diplomates successfully completed the MOC-PS® cognitive examination. Diplomates who successfullycomplete all components of the MOC-PS® Program, including successful completion of the Examination, will be sent a newcertificate at the end of the ten-year cycle before expiration of the existing certificate.

In addition, *24 diplomates used the Hand Surgery Examination or Hand Surgery Recertification Examination (HSE) tocomplete the process. To date, 2,632 diplomates completed the MOC examination process including 145 via HSE.

MAINTENANCE OF CERTIFICATION (MOC-PS®) PROGRAMThe Board Office sends mailings and email notifications to the applicable diplomates prior to anydeadlines. Be sure that your e-mail address is updated in the “My Profile” tab available once loggedin to the Board’s website. E-mail [email protected] if you did not receive your MOC informa-tion for 2016-2017.

Diplomates should log in to the Board’s website to review their individual MOC-PS® Tracking page toensure they are current with requirements to maintain certification. The tracking page outlines MOC-PS® requirements by year and lists deadline dates, status and completion dates.

Once theMOC-PS® requirements are met in any calendar year, the following files are available to download: • Annual MOC Participation letter; • Annual MOC Participation Certificate verifying the MOC-PS® requirements have been met; and • The ABMS MOC Starmark logo. Diplomates are encouraged to use this logo on their website,

letterhead, advertisements and business cards.

MOC-PS® HighlightsBoard Retreat: The Directors will hold a Strategic Planning Retreat in May 2016 as a follow-up to theNovember 2014 MOC Retreat. Decisions for MOC from the Board’s ongoing assessment of all MOCprograms resulted in:

• Elimination of the six – month case list for the MOC Application• Reduction in the MOC Examination Fee by 10%• Reduction in the Part IV Self-Assessment Tracer module fee by $50.00• Worked with PSEN/ASPS resulting in a 50% reduction in tracer-related journal CME cost• Initiation of investigations to offer more options for the MOC Program• Approval of the ABMS Multispecialty Portfolio Program launching in 2016 as an alternate to

Part IV for those participating in approved QI projects at their institutions

ASPS MOC Study Guide The MOC exam study guide offered by the Plastic Surgery Education Network (PSEN) is an excellentreference for exam preparation,www.psenetwork.org/onlinecourses/selfassessments.

60 Category I CME credits for completing the MOC Examination. The American Medical Association (AMA) Physician’s Recognition Award (PRA) and Credit System pro-vides 60 category I CME credit hours for the successful completion of initial certification, maintenanceof certification and the hand surgery examinations. Contact the AMA at www.ama-assn.org/go/cmeformsfor details. The AMA does charge an application fee, but AMA membership is not required.

2016 MOC Examination Scheduling Permits were posted in mid-February for the 2016 MOC Examination held during the monthof April at Prometric Test Centers.

2016 MOC ActivitiesAn Information Packet and an Activities Booklet of Information were mailed in January for those in years 3,6 or 9 of the ten year MOC Cycle. An email was sent and all documents are posted to your tracking page.

2017 MOC ExaminationAn Information Packet and a Booklet of Information were mailed in January for those in the last fouryears of the certification cycle. Hand Surgery Examination in lieu of the MOC Hand Module Examination The successful completion of the Hand Surgery Examination will satisfy the MOC Examination require-ment. However, an application for MOC is required. Contact the Board Office [email protected] for all questions.

2016 Diplomate Contribution and MOC-PS® Annual Contribution Enclosed is one of two forms for reference: the 2016 Diplomate Contribution Invoice for Lifetime Certifi-cate Holders or the 2016 MOC-PS® Annual Contribution Form for Time Limited Certificate Holders.

Time-limited Certificate Holders – The MOC-PS® Fee is an annual requirement and is mandatoryfor diplomates with time-limited certificates who are participating in the MOC-PS® Program. Onlinecredit card payment via your tracking page is preferred by April 15th.

Lifetime Certificate Holders – The Diplomate Contribution is requested of lifetime certificate holderswho elect not to participate in MOC-PS®. Credit card payment is the preferred method for submission.However, if necessary, you can complete the form and forward a check made payable to The AmericanBoard of Plastic Surgery, Inc. Return to the Board Office by April 15, 2016.

The ongoing support of all diplomates has been gratifying. Since 1999, more than half of our diplomateswith lifetime certificates have responded to this annual request. The income from this support has greatlyassisted the Board in its operations and its mission, and has been especially helpful in defense of the stan-dards represented by the ABPS certificate and development of web-based procedures.

REVOKED CERTIFICATES

The Board continues to review and assess diplomates who have had revocations of state medical licenses.The Board revoked two certificates in 2015. To date, a total of 81 certificates have been revoked. Thoserevoked ABPS certificates in 2015 are:

1) Lawrence Hunt Foster, Jr., M.D. - CA 2) John Wallace Smith, M.D. - IL

Suspension of Certification and Consumer ReportingThe Board added the status of “Suspended” to the public reporting of diplomate certification based onState Medical Board sanctions.

ABPS will refer consumers to the Federation of State Medical Boards website to further research diplomateswith probation or other restrictions to a state medical license.
