2016-2017 Elementary School/Program Choice Form Instructions All students start with an assignment. Complete a School Choice Form only if you want to request a different school or program assignment. You keep the original school assignment unless your child is assigned to one of your choices. If you are not assigned to your first choice, your student will be placed on the waiting list for the first choice school. Section 1 – Enter the address where your child lives with his/her parent or legal guardian. It is the parent/guardian’s responsibility to keep Seattle Public Schools informed of any address changes. Providing false or misleading address information will be cause for revoking the student’s assignment. Section 2 – Enter your child’s grade for the 2016-2017 school year. Section 3 (Optional) – Complete this section if you are choosing a school where another child in your household is attending. To obtain sibling priority for a school, a student must live in the same household and have the same home address as his/her sibling(s). It is your responsibility to ensure that the home address on record for the sibling(s) you list in this section is the same as the one provided in Section 1. Conflicting addresses may prevent your child from receiving sibling priority to a school. In addition, the sibling(s) must be assigned to that school in the 2015-2016 school year and the 2016-2017 school year. Frequently Asked Questions (FAQs) about Siblings and School Choice are available on the SPS Admissions website at http://www.seattleschools.org/admissions or call (206) 252-0760. Section 4 – Use the Elementary School Choice Worksheet provided on the back of these instructions to help you fill out Section 4. Schools on the Worksheet are listed alphabetically by Service Area. Montessori, Spectrum, and HCC programs are listed separately from the schools’ regular programs, where applicable. On the Worksheet, number your school choices in order of preference, starting with “1” for your first choice. You may choose any of the schools listed. Use your completed Worksheet to fill in your school and program choices in Section 4. Start with your first choice and fill in the School Name for each choice. Leave the optional program boxes empty if you are applying for a regular (General Education) program. If you are applying for a Montessori, Spectrum, or HCC program, fill in the appropriate optional program box. Students must be eligible for HCC and Spectrum. If you rank these choices and the student is not eligible, the choice will be deleted from processing. Contact the school for Montessori information. The example below shows how to complete Section 4 if your first choice is Catharine Blaine regular program, second choice is Lawton Spectrum, and third choice is Lawton regular program: Section 5 – Sign and print your name. 2016-2017 Elementary School/Program Choice Form Instructions

All students start with an assignment. Complete a School Choice Form only if you want to request a different school or program assignment. You keep the original school assignment unless your child is assigned to one of your choices.
