Key Points: Elective Activity • This report focuses on hospital activity trends and rates. It is largely based on SUS data up to January 2016

(YTD Month 10) and includes both specialised and non-specialised activity.

Outpatient Attendances

• Year-on-year GP referrals to Outpatient first appointments in 2015/16 YTD are 1.8% lower than in the same

period last year. The cumulative variance from last year, however, continues reducing month by month.

• In 2015/16 YTD there has been an increase in GP Referrals to the following specialties:

• Gastroenterology; Respiratory Medicine; Obstetrics; Nephrology; Hepatology

• Overall, year to date Outpatient first attendances from all referral sources are down by 0.8% from last year.

NNE QIPP target for 2015/16 is -3.9%

• Fast Track data from NUH and Circle continues to show higher level of OP 1st activity in January & February

2016 compared with last year.

• Outpatient Follow-up attendances (all providers) are 1% up from last year. The increase has occurred in:

– T&O; Pain Management; Gynaecology; Respiratory Medicine; Paediatrics; Ophthalmology; Gastroenterology; ENT

Day Case & Elective Admissions

• SUS data shows that Day Case (DC) admissions have increased by 6% compared with last year (all

providers). The increase is seen mainly at NUH (8%) and Ramsay (23%) . There has been a sharp increase in

June and July this year and activity has remained higher than the previous two years between September

and January. DC activity did not drop in December 2015 as it did in previous years

• DC activity has increased in the following specialties:

– Gastroenterology; Pain Management; T&O; Clinical Haematology and Oncology; Hepatology; ENT.

• Fast Track data from NUH & Circle continue to show an increase in year to date DC activity (7.6%)

• Year-on-year Ordinary Elective admissions with overnight stay (EL) have increased by 1.4% (all providers).

The increase has occurred across several specialties, in particular:

– Cardiology; Geriatric Medicine; Clinical Haematology; Paediatrics; Vascular Surgery; Spinal Surgery

• Unlike DC, EL admissions dropped In December 2015 compared to December 2014.

• Fast Track data from NUH & Circle shows a slow-down in February 16

Key Points: Emergency Activity A&E

• The number of ED attendances at Type 1 (24/7 consultant led Major ED), Type 2 (Mono-specialty consultant

led ED, e.g. Eye Casualty) and Type 3 (Minor Injury) Departments in 2015/16 YTD is 3.2% higher than last

year. Attendances have remained relatively stable since April 2015.

• The increase has mainly occurred at both QMC (2.5%) and Kings Mill Hospital (4%) Emergency Departments

• The increase appears to be across most age groups, particularly 0-4 and 20-39, 55-59 and 75+

• In the same period, apart from Rushcliffe CCG, all other Notts South CCGs have seen an increase in ED

attendances.

• Data from the London Road Urgent Care Centre is included in this report (Attendances at Minor Injury Units

and Walk-in Centres, page 15)

• The number of calls to NHS 111 in April 2015 – February 2016 this year is 9.1% higher than last year.

Emergency Admissions

• Year-on-year, there has been a cumulative 5.6% drop in the overall number of Emergency hospital admissions

(all providers) this year (n=677). There has been a noticeable increase in December 2015, but admissions

have dropped again in January 2016.

• Emergency admissions from GP are significantly lower than last two years (-26%)

• Fast Track data from NUH shows that activity in February 2016 has slightly increased but it is in line with last

year. The cumulative reduction this year up to end of February is -5.0% compared to last year

• 28-day emergency re-admissions this year are down by 5% from last year. A sharp increase occurred in

December 2015, but readmissions have gone down again in January 2016.

• Emergency admissions at Sherwood Forest Hospitals (King’s Mill Hospital) have increased by 18% (n=110).

The great majority of these (88%) are admissions from A&E.

• Oakenhall and Torkard have seen the largest increase in Emergency Admissions at SFHT (+62% and +16%

respectively). They have also seen a reduction at NUH (-4% and -7% respectively), indicating a possible

marginal shift in patient flow.

Key Points: Financial Position at the 3 Main Providers Nottingham University Hospitals NHS Trust (NUH)

• In Month 10, NNE is overspent at NUH

• Main areas of YTD over-spend are:

Day Case (DC): 10% (£479,129) – Gastroenterology; Pain Management; Hepatology, Clinical Oncology

Emergency Admissions (NEL): 2% (£273,785) – Despite a reduction in activity compared with last year,

NNE is still above plan: this is mainly due to QIPP non-delivery and case-mix.

Outpatient Follow-up (OPFUP): 10% (£374,751) – across several specialties, including Outpatient

Parenteral Antibiotic Therapy (OPAT), T&O and Fracture Clinic, Pre-op Assessments; Geriatric Medicine

Outpatient Procedures: 9% (£230,858) – Ophthalmology, Urology, Fracture Clinic, ENT, Gynaecology

Inpatient Unbundled: 24% (£186,139) – Rehabilitation

OP Unbundled: 14% (£175,711) – Diagnostic Imaging (MRI and CT scans)

Circle Ltd – Nottingham Treatment Centre

• In Month 10, NNE is overspent at Circle

• Main areas of YTD over-spend are:

QIPP (non-delivery): £511,850

Elective Inpatients: £136,000 – T&O (Hip, Knee, Shoulder procedures)

High Cost Drugs: £65,668 – mainly Cytokine Inhibitors - Gastroenterology

Sherwood Forest Hospitals NHS Foundation Trust (SFH)

• In Month 10, NNE is overspent at SFH (10%, £436,035)

• Main areas of YTD over-spend are:

Emergency Admissions (including short-stay): 25% (£277,261)

Critical Care: £89,698

High Cost Drugs: 102% (£61,671)
