You will receive an online confirmation after you complete the registration process. Call the FASET Orientation Office at 404.894.6897 with questions. THE REGISTRATION DEADLINE IS DEC. 16, 2014, at 4 p.m. (EST). Please note that campus and all offices will be closed Dec. 22-26, 2014, and Jan. 1, 2015, for winter break. TRANSFER FASET SESSION* Friday, Jan. 2, 2015 — Check-in 7:45 - 8:45 a.m. *Please note: Space is available on a first-come, first-served basis. COST* Student Fee ................................................................................. $105 (includes all FASET materials, breakfast, lunch, and welcome activities) Parent/Guest Fee.......................................................................... $65 (includes all FASET materials, Parent Calendar, breakfast, and lunch) *Please note: Scholarships, including HOPE, do not cover orientation fees WHAT TO BRING • All correspondence received from Georgia Tech. • Unofficial transcripts and course syllabi/descriptions from other colleges or universities you have attended (to assist in advisement and clarify questions related to transfer credit). • Copy of your Medical Entrance Form. • Sweater or light jacket • Comfortable walking shoes. • Umbrella STUDENT HOUSING Students who will live on campus Spring 2015 semester will be able to receive their residential hall key the morning of Friday, January 2, 2015, during FASET check in. If you have additional questions regarding move-in, visit www.housing.gatech.edu or call 404.894.2470. If you are not going to live on campus but need accommodations for the evening prior to or following Transfer FASET, visit www.faset.gatech.edu for local hotels. FINAL TRANSCRIPTS By the time you arrive at Transfer FASET, you must have submitted all final transcripts for all prior coursework to the Office of Undergraduate Admission. For contact information, visit www.admission.gatech. edu. You should bring your unofficial transcripts with you, including any current coursework and official AP scores. STUDENTS/GUESTS WITH DISABILITIES Please indicate on your FASET reservation form whether you or anyone in your party has needs relating to a disability and we will connect you with our Office of Disability Services. INTERNATIONAL STUDENTS International students attending Georgia Tech must provide the following documents to the Office of International Education (OIE) during orientation prior to course registration: a valid passport, I-94 form (you will receive this form on the plane prior to landing in the United States), and an I-20 or DS-2019 form (with proof of health insurance). If you are transferring from another U.S. institution to Georgia Tech, you will receive an email with instructions from OIE regarding the transfer of your I-20 or DS-2019 form. MEDICAL RECORDS The Medical Entrance Form, Tuberculosis Assessment Form, and Certificate of Immunization are available at www.health.gatech.edu. You must provide documentation of immunization for the following (not completing the forms could cause a delay in registration): • Hepatitis B • Measles, Mumps, and Rubella (MMR) • Tuberculosis screening • Tetanus • Varicella (chicken pox) – Tdap (adult dose) required for all students – Menactra or Menveo required for all students 21 and younger For more information, call Stamps Health Services at 404.894.1432, email [email protected], or visit www.health. gatech.edu. Always include your GTID number when communicating with Health Services. PARENTS/GUESTS Attending FASET is a great way for parents, guardians, and other guests to learn ways to support their student through his or her Tech experience. To address concerns or questions they may have, parents and guests will have opportunities to talk with current students, faculty, staff, and other parents. They will also learn about the resources, services, and opportunities available to them as well as their student. The cost is $65 per person for the one-day parent and guest program. The information you provide will supplement your student’s emergency contact information and ensure that you receive timely information from the Parents Program about important campus news, on- and off-campus events, volunteer opportunities, and more. CANCELLATION POLICY All cancellations must be made in writing at least 15 business days in advance of your session for a refund of the registration fee, less a $25 administration fee. To request a refund, email [email protected]. DIRECTIONS AND PARKING INFORMATION Find directions and parking information at www.nssp.gatech.edu. On-campus FASET parking will be available only in the Student Center Parking Deck. Once you park, you should not need to move your car again since you will check in at the Student Center Commons.
