I/We confirm that I/we have adhered to The Weed Control Act and The Pest Control Products Act and the program guidelines for the Invasive Plant Control Program. Total Eligible Grant for RM/Other

(a)

The information in the cells that are shaded light green are the specific calculated amounts being claimed for.

The total litres used = the total area in acres times the herbicide rate.

Cost per litre must come from the herbicide invoice accompanying the claim.

The Eligible Grant for public lands is 100% of the total litres times the cost per litre if "Public" is entered in the Public or Private column.

The Eligible Grant for private lands is 50% of the total litres times the cost per litre if "Private" is entered in the Public or Private column.

The information in the cells that are shaded light yellow reflect the APPROVED herbicides as well as if lands are PUBLIC or PRIVATE.

In the Herbicide Name column, enter the valid herbicide names for each weed species - for reference, the valid herbicides are listed for each weed on the Schedule A -REVISED sheet.

In the Public or Private column, the valid choices are "public" or "private". The choice determines the column used for the eligible grant and whether the eligible grant is 100% of the cost or 50% of the cost of the herbicide.

In the columns that are white, enter the relevant information. The information includes:

- The owner or occupant of the land.- The GPS coordinates or Legal Land Description for the land that was treated.- The total area in acres of the land treated. Ensure that the area is in acres.- The application rate for the herbicide used (note that each herbicide has an approved application rate).- The cost per litre for the herbicide used (note that the invoice for the purchase of the herbcide must be attached to the claim form).

Prohibited Weeds

Because prohibited weeds are rare or not currently found in Saskatchewan, registered control options may not have been developed in Canada yet. In accordance with The Weed Control Act, RMs are required to notify Crops and Irrigation Branch upon the discovery of a prohibited weed and at that time an appropriate control option will be investigated and approved. For control options for Prohibited weeds or assistance in confirming identification please contact the Provincial Weed Specialist at: (306) 787-4673 or [email protected] Note on Prohibited Weeds

Prohibited Weeds are either not currently in Saskatchewan, or are only recently introduced and very rare. If producers look through the list of Prohibited Weeds, there will be very few that they will recognize since for the most part, these species are not found in Saskatchewan yet. It is essential that these weeds are discovered as soon as possible after they enter the province, quarantined when they are found, and eradicated using the most effective means possible, to prevent them from gaining a foothold. (1) The following plants are designated as Prohibited Weeds in Saskatchewan: Common Name Botanical Name barberry, common Berberis vulgaris L., bartsia, red Odontites serotina Dum. chervil, wild Anthriscus sylvestris (L.)
