CALENDAR OF EVENTS - IMPORTANT DATES

Before"July 14, 2014

Update personal and dependent information online so you can make elections during Annual Enrollment Period. This includes any children up to age 26 who are eligible for Medical, Dental or Vision coverage.

July 14, 2014 Annual Open Enrollment begins.

July 18, 2014 Annual Open Enrollment ends at noon.

August 1, 2014New elections take effect and remain in effect through July 31, 2015 unless you change your elections due to a qualified change in family status.

What’s New for 2014? !Now it's time to update your group insurance benefits… to make sure you're enrolled in the benefits that best fit your life situation. !!!

Open Enrollment for Medical, Life, Dental, Vision, Disability, Critical Illness and Accident plans ends at noon on July 18th. Your new Plan Year elections will become effective on August 1st. Take a look at your family's needs, review all your benefit options and if changes are needed, please login at myagprobenefits.com to update your elections during Open Enrollment, July 14 - 18. !

IMPORTANT!: You must login and update any change in elections needed for

the 2014-15 Plan Year during Open Enrollment. !If no changes are needed, there is no action required and your

current elections will carry over into the new plan year. !But before making your decision,

please review the following pages for important updates: !!!!!!!2

!MEDICAL INSURANCE !Ag-Pro will continue to offer the HSA and Copay medical plans as an employer sponsored benefit. Regardless of which plan and coverage level you choose, Ag-Pro will contribute $315 per month towards the cost of your medical insurance. Please note the following: !* Starting in the new plan year (August 1st), all Deductibles, Co- insurance, and Copays will count toward your out of pocket maximum, allowing you to reach 100% coverage quicker when medical claims start to grow. !*United Healthcare will issue new ID cards. Please destroy your old ID

card and present your new card for all services after 8/1/14. !Below is a summary of the new payroll deduction and benefits for each plan. The Summary of Benefits & Coverage (SBC) for each plan is posted online at the Ag-Pro benefit website (written copies available upon request). FEATURE HSA PLAN COPAY PLAN

!DENTAL INSURANCE !The Ag-Pro Dental plan will have a slight adjustment in premiums for the 2014-15 Plan Year. The new payroll deductions are: !

$10.98/pay period - Employee Only $21.72/ pay period - Employee + Spouse $28.25/ pay period - Employee + Children

$40.08/pay period - Family !Guardian will issue new Dental ID cards. Please destroy your old ID card and present your new card for all services after 8/1/14. !VOLUNTARY LIFE & CRITICAL ILLNESS !The Voluntary Life and Critical Illness plans remain unchanged. However if you have entered a new age bracket as of August 1st, your premium will change. Please login at myagprobenefits.com for details. !Please Note: You must complete an Evidence of Insurability form if you are adding or increasing coverage. The form is available online; please print, complete and turn into your HR Department. !DISABILITY !Ag-Pro will update your earnings effective August 1st. Your disability benefit and premium will be adjusted based on your most recent earnings. In addition, you may realize a premium adjustment due to age change on the Long-term Disability plan. Details are available at myagprobenefits.com. !Please Note: You must complete an Evidence of Insurability form if you are adding coverage. The form is available online; please print, complete and turn into your HR Department. !ACCIDENT !There are no changes in rates or benefits for the 2014-15 Plan Year. !

Frequently Asked Questions !How to Make Your 2014 - 15 Elections? • Directions to enroll online are available in the back of this handbook.

• Once you login, your current Plan Elections will be shown:

- Verify your address and dependents - Review your elections and make changes as desired -Make sure you confirm your enrollments upon completion -Designate beneficiaries for Basic Life and Voluntary Life (if elected) !

What changes may be made during Open Enrollment? You may enroll or terminate employee and/or dependent coverage in all plans for the new plan year. Adding coverage may be subject to evidence of insurability.

What changes may be made outside of Open Enrollment?

Insurance plan elections may only be changed during Open Enrollment unless there is a qualifying change in status during the Plan Year. You must notify the HR Department within 30 days of a qualifying event that occurs outside of Open Enrollment if you want to make changes. A list of Qualifying events may be found on your Employee Benefit homepage, login at myagprobenefits.com to view.

How can I verify my current benefit elections?

The following pages have instructions on how to access your Employee Benefit Portal website. The website contains valuable information regarding your Group Benefits such as your current elections, benefit summaries, plan cost, SBC’s, links to carrier websites, and forms.

What Forms MUST be completed?

If you decide to keep your current Group Insurance Plan elections for the upcoming 2014-2015 Plan Year, no action is needed. Your current benefit elections will automatically renew. If you need to make election changes, please login at myagprobenefits.com and complete your new elections.

Is Evidence of Insurability required to add coverage? !An Evidence of Insurability (EOI) form is required if you have previously been eligible to enroll in coverage and are now considering adding or increasing coverage on the Voluntary Life, Disability, or Critical Illness plan. The Guardian EOI form may be downloaded on your homepage at myagprobenefits.com. !!
