Mission StatementThe mission of the Pajaro Valley Community Health Trust is to improve the health and quality of life for all people of the greater Pajaro Valley by sup-porting programs and activities that ensure access to a full array of high quality, culturally competent health care services which promote health, wellness and disease prevention.

The Health Trust Board of Directors and staff have long embraced the concept of ‘health equity’ as one of our foundation’s core values. In doing so, our goal has been to create a Pajaro Valley community where all residents are able to access high-quality, affordable healthcare regardless of race or economic status.

The funded grants and programs listed in this 2013 Annual Report (see page 4) reveal a variety of approaches that reflect this commitment. Examples include scholarships for low-income students and adults bound for careers in health; youth empowerment programs that train older students to teach younger students to control their risk factors for diabetes; health education support and case management programs for low-income families learning to use the healthcare system; community and school-based health; nutrition and diabetes prevention classes that partner with food access programs to ensure low-income families are eating healthy foods; and many more. Other grants fund access to direct medical, dental and behavioral health services so that people can go to work to help support their families.

Whether these programs increase awareness, promote prevention or treat ill-ness, all play an important role in helping to advance our goals for health equity in the Pajaro Valley. At the Health Trust we believe that with this knowledge comes the power for low-income and vulnerable residents to actively engage in determining their health and a healthy future for all in our community.

There is still a great deal of work to be done in partnership with many of you in the community. The Health Trust will be here to see it through for generations to come.

Funding Guidelines The Health Trust provides grants for projects that advance our mission to improve the health and quality of life for all people of the greater Pajaro Valley. The Foundation invites grant proposals through a Request for Proposals (RFP) process. Deadlines and award amounts vary. Complete details are outlined in each RFP. RFPs are released and publicized through our website and through targeted communications. We encourage all interested organizations to visit our website periodically for RFP notices and/or to request that their organization be added to the Health Trust’s RFP mailing list. All grants made are health-related and must address one or more of the Health Trust’s priority areas:

Diabetes & Contributing Factors • Oral Health • Health Professions • Access to Care

Grants & Program Support for Fiscal Year 2012–13Grants AwardedAptos High School, Pajaro Valley High School, Watsonville/Aptos Adult Edu-cation, Watsonville High School and Cabrillo College — $5,000 in scholar-ships for Pajaro Valley students pursuing careers in health professions.Community Action Board of Santa Cruz County — $10,000 to support the Peer Health Education: REAL Diabetes Prevention project. This project will make a positive impact on diabetes through a unique blend of core youth group train-ing and peer education and outreach at Pajaro Valley elementary schools.Dientes Community Dental Care — $7,200 to support the Dental Care for Uninsured Adults/Harvesting Oral Health project that will increase the number of Pajaro Valley residents with access to comprehensive dental care.Family Service Agency of the Cen-tral Coast — $10,000 to support the Open Arms/Abrazo Abiertos initiative that will provide free or low cost men-tal health services to cancer patients, survivors of child sexual abuse, seniors

and low-income Spanish speaking residents of the Pajaro Valley. Girl Scouts of California’s Central Coast — $7,000 to support the expansion of an all-girl recreational soccer league in the Watsonville/Pajaro Valley area; delivered in a non-competitive format. Girls will have the unique opportunity to discover their abilities to lead increasingly active/healthy lives, connect with healthy adult role models, and take action by engaging in a healthier lifestyle. Healthy Kids of Santa Cruz County — $12,000 to continue to support the outreach and enrollment to uninsured children for Medi-Cal and Healthy Fami-lies programs and provide an insurance product for children who do not qualify for public programs; they will also secure access to comprehensive medical, dental, vision and mental health services.Jacob’s Heart Children’s Cancer Sup-port Services — $6,250 to provide bilingual mental health services includ-ing emotional, social and financial sup-port to families of children with cancer through art therapy, clinical counseling

and case management. La Manzana Community Resources — $7,500 to support families through the Health Access project that will provide 300 Pajaro Valley families with individual-ized support and case management in accessing and maximizing their public health benefits. Clients learn to utilize all available services and resources, know their rights and responsibilities and reduce the number of unnecessary Emergency Room visits.Mesa Verde Gardens — $10,000 to support the peer leadership develop-ment for the established Community Gardens, which connect low-income uninsured Pajaro Valley residents with an opportunity to grow their own pesticide-free fresh fruits and vegetables though the creation of small organic gardens. PVUSD/ Starlight& Cesar Chavez Nutrition Collaborative — $15,000 to support the partnership between Starlight Elementary and Cesar Chavez Middle Schools to empower youth by providing a healthy school environment that encour-ages healthy behaviors through improving access to fruits and vegetables and teach-ing nutrition and cultural literacy through garden based learning. Salud Para La Gente — $7,200 to sup-port the delivery of oral health screening outreach to low-income farm workers and their families in the Pajaro Valley. Initial outreach will bring dental teams to the fields for screenings and cleanings. Follow-up dental care will be completed in the clinic or mobile van.United Way of Santa Cruz County — $8,000 to support the Healthy Corner Market Project that will work with two targeted corner markets near schools and parks in Watsonville to increase avail-ability and marketing of healthy foods. Watsonville/Aptos Adult Education — $6,450 to support the From Field

Hands to Helping Hands program. This vocational training class will increase the bi-lingual/bi-cultural workforce of trained caregivers in the Pajaro Valley. The training will prepare 18 students for entry level jobs as caregivers for dependent elders. Students will achieve competency in healthcare skills, communication and CPR/First Aid certification.Watsonville Family YMCA — $10,000 to support the third year of implementa-tion of the Healthy Family Home (HFH), a program designed to help low-income Latino families gain knowledge and adopt healthier lifestyle behaviors. The HFH program will offer nutrition semi-nars with the wellness coach. Watsonville High School Health Academy — $3,400 to continue to help students at Watsonville High School achieve their goals of pursuing health professions. Students will be able to participate in mentor/student dinners that the school will host semi-annually to expose students to professionals who work in the careers that students might be interested in. Funding will also sup-port purchasing materials for their class.

Program SupportDiabetes Health Center — $435,158 to reduce the incidence of acute and chronic complications of diabetes by providing free and low-cost Diabetes Prevention, Self-Management Education and Medical Nutrition Therapy services to children and adults with or at risk for diabetes in our community.Regional Diabetes Collaborative — $74,429 to convene organizations and individuals from throughout the tri-county area to work on issues such as diabetes education and prevention, public policy, clinical management and best practices.
