This Agreement is made between (Owner/Occupier)___________________________________________________________

of (address)____________________________________________________________________________________________

and the Authorized Distributor or their duly appointed agent. The duration of the Agreement is for a period of twelve calendar months.

Taylex™s Authorised Distributor or their agent, upon payment of $______ and signature of the afore-named person at the address above and upon acceptance of this application by a duly authorized employee of the Authorized Distributor agrees, for a period indicated to the following:

1. Six monthly field service inspections will be made by a factory trained employee during the twelve month period covered.Inspections include:a) Any adjustment, cleaning (excluding de-sludging) and field service of the unit necessary for proper operationb) Inspection of control panel, alarm, membrane pack, blower, filtrate pump effluent quality, irrigation pump, irrigation area,

floats2. Both labour and materials will be charged for any emergency service call out outside the two calls specified above (except

items covered under warranty)3. All repairs are on a strictly C.O.D basis4. This Agreement does not bind Taylex™ or its duly appointed agent to be responsible for the quality of the effluent.

However, it will at all times wherever possible recommend how the effluent quality can be maintained at its maximum and alter and adjust the system during these visits in an endeavour to obtain the best possible effluent standards.

MEMBRANE EXCHANGE POLICY

The Taylex™ DMS (Domestic Membrane System) uses a membrane pack as part of its treatment process. These packs require special care from both the residence of the home and the service technician.

Taylex™ have in place an exchange program that will cover the costs of providing an exchanged membrane pack during the regular service once per year. This service will be included in your yearly service contract. The Taylex™ DMS requires two services per year. Due to the cleaning requirements of the membrane pack it will be removed from the residence and taken to an accred-ited cleaning station for servicing and cleaning.NOTE: These packs are on a rotation system and your original pack will not necessarily end up back in your system.

WARRANTYS

The Taylex™ DMS is covered by a 15 year warranty on all concrete components. 1 year warranty on all electrical items, 2 years if your service contract is renewed with an Accredited Taylex™ Agent. This applies to parts only in the second year. note: All warranty’s are from date of commissioning. If the membrane pack requires exchanging between services or at the six month service this exchange is at extra cost.

The membrane pack is not covered by warranty if any of the following occur:1. Paints, oils, sharp objects such as needles, cotton buds, razor blades etc are found in your system2. The service contract has lapsed for any period of time or not been renewed with an Accredited Taylex™ Agent.

and the Authorized Distributor or their duly appointed agent. The duration of the Agreement is for a period of twelve calendar months.

Taylex™s Authorised Distributor or their agent, upon payment of $______ and signature of the afore-named person at the address above and upon acceptance of this application by a duly authorized employee of the Authorized Distributor agrees, for a period indicated to the following:

1. Six monthly field service inspections will be made by a factory trained employee during the twelve month period covered.Inspections include:a) Any adjustment, cleaning (excluding de-sludging) and field service of the unit necessary for proper operationb) Inspection of control panel, alarm, membrane pack, blower, filtrate pump effluent quality, irrigation pump, irrigation area,

floats2. Both labour and materials will be charged for any emergency service call out outside the two calls specified above (except

items covered under warranty)3. All repairs are on a strictly C.O.D basis4. This Agreement does not bind Taylex™ or its duly appointed agent to be responsible for the quality of the effluent.

However, it will at all times wherever possible recommend how the effluent quality can be maintained at its maximum and alter and adjust the system during these visits in an endeavour to obtain the best possible effluent standards.

MEMBRANE EXCHANGE POLICY

The Taylex™ DMS (Domestic Membrane System) uses a membrane pack as part of its treatment process. These packs require special care from both the residence of the home and the service technician.

Taylex™ have in place an exchange program that will cover the costs of providing an exchanged membrane pack during the regular service once per year. This service will be included in your yearly service contract. The Taylex™ DMS requires two services per year. Due to the cleaning requirements of the membrane pack it will be removed from the residence and taken to an accred-ited cleaning station for servicing and cleaning.NOTE: These packs are on a rotation system and your original pack will not necessarily end up back in your system.

WARRANTYS

The Taylex™ DMS is covered by a 15 year warranty on all concrete components. 1 year warranty on all electrical items, 2 years if your service contract is renewed with an Accredited Taylex™ Agent. This applies to parts only in the second year. note: All warranty’s are from date of commissioning. If the membrane pack requires exchanging between services or at the six month service this exchange is at extra cost.

The membrane pack is not covered by warranty if any of the following occur:1. Paints, oils, sharp objects such as needles, cotton buds, razor blades etc are found in your system2. The service contract has lapsed for any period of time or not been renewed with an Accredited Taylex™ Agent.

IMPORTANT WARRANTY INFORMATION

Installed at the above property is a Taylex™ DMS (Domestic Membrane System) this particular waste water treatment system produces a very high level of water quality and requires extreme care when disposing of liquids down your drain. Your Taylex™ DMS is covered with a 1 year warranty on parts and labour, from the date of commissioning (refer to the top of this form for the commissioning date). If you choose to renew your contract with an authorised Taylex™ Service agent, Taylex™ will cover parts only for the 2nd year. Your Taylex™ DMS has a membrane pack installed that will be exchanged with a clean membrane pack every 12 months.
