2. CDL INFORMATION - If you hold a Commercial Driver's License (CDL) and were driving a commercial or noncommercial vehicle when you received this complaint, do not mark Box B. You must choose A or C. Seek legal counsel.

3. This form (and payments for choices A or B) must be mailed to the Clerk of the Circuit Court within 7 days of receiving the Complaint. If you received a Mail-In Copy (Court Communication Copy) attached to your complaint, please detach and return this envelope.(Not required online.)

4. FAILURE TO COMPLY: If you fail to comply with the requirement of this complaint, a judgment of conviction may be entered against you in the amount of the fine, penalty and costs. If you fail to appear on an assigned court date, your driver's license may be suspended or a judgment of conviction may be entered against you in the amount of the fine, penalty and costs. Cash bail or other security deposited will be used to pay the fine, penalty and costs. If you are an Illinois driver and you fail to pay the judgment in full, your drivers license will not be renewed, reissued, or reclassified until full payment is received. FOLLOW INSTRUCTIONS. You are eligible for the 4-hour Traffic Safety School course if you are 18 years of age or older and if you have not attended or enrolled in Traffic Safety School for a violation committed within the previous 12 months. If you are 18 years or older and are found to be ineligible for the 4-hour course because you were enrolled in or have attended a 4-hour course within the past 12 months, you may be eligible for an 8-hour course. Traffic Safety School will contact you, and an additional fee will be required at that time. If you are found to be ineligible for Traffic Safety School, a conviction will be reported to the Secretary of State. Any overpayment, less a processing charge, will be refunded. 9. When you mark A or B and forward payment, keep the yellow copy of your complaint until you receive your driver's license or bond card. Once your payment is processed your driver's license or bond card will be mailed to you.

TRAFFIC SAFETY SCHOOL INSTRUCTIONS — Complete this side only if you chose Box B. CLASS LOCATIONS: Classes are offered Monday through Saturday, 8:00 am to 12:00 noon, 1:00 pm to 5:00 pm and 6:00 pm to 10:00 pm (evening classes do not meet on Saturday). The 4-hour class is completed in one session.The 8-hour class is completed on two consecutive weeknight evenings or one Saturday session. Classes are offered in English, Spanish and Polish. Sign language interpretation is available upon request. TO REGISTER: Complete the Registration Form below or go to www.cookcountyclerkofcourt.orq. Mail the form no later than 7 days after receiving the Complaint with your non-refundable fee. Check or money order must be made out to CLERK OF THE CIRCUIT COURT. Do not send cash or starter checks. AREA LOCATION TRAFFIC SAFETY SCHOOL: Your class assignment form will be mailed to you. Some class locations have limited or restricted schedules. Class locations are subject to change. Your request will be honored, if possible, or a change to the closest possible location will be made. If you are unable to attend your scheduled class, immediately call Traffic Safety School and reschedule. To reschedule at no fee, call no later than the date stamped on your class assignment form. After that date the rescheduling fee is $20. If you miss your class, you must reschedule another class and pay $20. It is your responsibility to notify the Traffic Safety School if you change your mailing address. The school is not responsible for late, lost and/or undeliverable mail.

IF YOU COMPLETE CLASS YOU WILL NOT NEED TO APPEAR IN COURT: Traffic Safety School will report to the court when you complete class. No driving conviction will be reported to the Secretary of State.
