TRAFFIC SAFETY SCHOOL INSTRUCTIONS — Complete this side only if you chose Box B. CLASS LOCATIONS: Classes are offered Monday through Saturday, 8:00 am to 12:00 noon, 1:00 pm to 5:00 pm and 6:00 pm to 10:00 pm (evening classes do not meet on Saturday). The 4-hour class is completed in one session.The 8-hour class is completed on two consecutive weeknight evenings or one Saturday session. Classes are offered in English, Spanish and Polish. Sign language interpretation is available upon request. TO REGISTER: Complete the Registration Form below or go to www.cookcountyclerkofcourt.orq. Mail the form no later than 7 days after receiving the Complaint with your non-refundable fee. Check or money order must be made out to CLERK OF THE CIRCUIT COURT. Do not send cash or starter checks. Use this envelope. OUT OF STATE/COUNTY DEFENDANTS: To attend classes in another state or county, choose Box "B" on the MAIL-IN COPY. You must pay all fees listed next to Box "B" to the CLERK OF THE CIRCUIT COURT OF COOK COUNTY. Complete the Registration Form below. Indicate the desired state or county in question #5 below. Class availability in other states and a select number of other Illinois counties is knifed. Additional fees may be required by the school offering defensive driving courses located in other states or counties not listed to the right.

AREA LOCATION TRAFFIC SAFETY SCHOOL: Your class assignment form will be mailed to you. Some class locations have limited or restricted schedules. Class locations are subject to change. Your request will be honored, if possible, or a change to the closest possible location will be made. If you are unable to attend your scheduled class, immediately call Traffic Safety School and reschedule. To reschedule at no fee, call no later than the date stamped on your class assignment form. After that date the rescheduling fee is $20. If you miss your class, you must reschedule another class and pay $20. It is your responsibility to notify the Traffic Safety School if you change your mailing address. The school is not responsible for late, lost and/or undeliverable mail.

IF YOU COMPLETE CLASS YOU WILL NOT NEED TO APPEAR IN COURT: Traffic Safety School will report to the court when you complete class. No driving conviction will be reported to the Secretary of State.
