Purpose: A visual, step-by-step document that describes how to use the changes made to specific functions and tasks for the MnCHOICES application in version 15.1.

NOTE: There are five topics included in the v.15.1 – Release Notes Companion: • PCA Total Hours per Day • App Pool Outages • PCA Provider Report • Content Changes in Two Domains • Essential Community Supports Added to LTC Document

We recommend you review the entire Companion document. If you need to return to the document for clarification at a later date and want to quickly locate one of the topics following these steps:

3. Choose the topic you want to read/reference and click the Go To button. You will be taken directly to that topic.

1. PCA Total Hours per Day If the MnCHOICES Eligibility Determination indicates the person is eligible for PCA services, the Planning Summary now displays the total hours per day for which the person is eligible based on the home care rating.

• For people under 65 years of age, the hours of PCA service eligibility are displayed on the Planning Summary Report in the section labeled Additional Programs and Services.

• For people 65 and over, the message about PCA service eligibility is displayed on the Planning Summary Report in the section labeled Programs and Services.

• In the PCA row it will read: “Eligible for (can be used flexibly)” followed by “Total hours/day is _____”which displays the hours for which the person is eligible.

2. App Pool Outages As more lead agencies launched and more users were added to the system, outages occurred due to limitations of the application pool (App Pool). This caused frequent outages for the users in Production. The application was moved to a new server and out of a virtual environment into a hardware environment. In addition, some settings were changed that relate to refreshing and recycling memory usage. These changes stabilized the App Pool to reduce and eliminate outages.

3. PCA Provider Report The PCA Process Guidelines with MnCHOICES document includes a section on page 2 about Communicating with the PCA Provider. MnCHOICES now offers a standard report that can be prepared for a PCA provider with the click of a button. Depending on the amount of information entered in the assessment, the PCA Provider Report is about ten (10) pages long. It includes:

• Directing Own Care Determination • Medications, including Medication Mangagement and Equipment • Complex Health Related Needs • Behaviors • Cognitive Impairments • Determination of Additional Time • Activities of Daily Living, including Challenges, Strengths, Preferences, Support Instructions and

Need for Training/Skill Building • Authorization Summary • Recommendation to DHS • Referrals Needed • Certain Notes/Comments in report categories above

To use the PCA Printout for Providers function, complete the assessment, Determine Eligibility and then:

1. Click the Print tab on the Assessment menu (green rectangle)

2. Click the View PCA Printout for Providers (orange rectangle)

3. Each page of the report includes identifying information in the upper left and right corners (green rectangles)

4. Use the slide bar or up/down arrows on the right side of the screen to scroll up and down the page (orange rectangle)

5. Use the arrows on the tool bar to change pages up/down (red rectangle)

6. Use the magnifying glasses on the tool bar to change the width/size of the report information (green rectangle)

7. Click the PDF icon to export the PCA Provider Report as a PDF or the printer icon to send to a printer for a paper copy (orange rectangle)

8. Clicking the PDF icon displays the PDF Export Options above. To make a copy of the entire PCA Provider Report, click OK

9. A message will appear asking if you want to save the PCA Provider Report. Click OK

10. Follow your standard process for saving documents to your computer files. This allows you to email a PDF attachment to a provider through a secure email message.

11. Clicking the Printer icon on the tool bar will alert you that the document is being prepared followed by a message that the pages are ready. If you are ready to continue printing click Print.

12. Follow your standard process for printing documents. This allows you to mail a paper copy of the report to a provider.

13. When you finish workingwith the PCA Printout for Providers function, click Close PCA Provider Report to move to another function

4. Content Changes in Two Domains The release of v.15.1 provides content changes within the Quality of Life and Safety/Self Preservation Domains. The changes impact New Assessments and Reassessments differently.

Quality of Life: Future Plans New Assessments

• There are no content changes to the Quality of Life domain Future Plans question group for new assessments

Reassessments • The Quality of Life domain Future Plans question group includes an additional question with a

question indicator about the person’s current support plan.

Safety/Self Preservation: Self Preservation New Assessments

1. When conducting a new Assessment, the Safety/Self Preservation Domain has a total of five question indicators

2. There are no question indicators in the Personal Safety question group 3. All five question indicators are in the Self Preservation question group 4. Personal Safety question group in the Safety/Self Preservation Domain does not have any

question indicators 5. The content of the Personal Safety question group has been reorganized and revised slightly to

prompt a conversational assessment interview Reassessments

1. After the release of v.15.1, in a new Assessment the Self Preservation question group in the Safety/Self Preservation domain has nine (9) question indicators of which six (6) are new question indicators that do not appear in a new assessment

2. The remaining indicators are the typical question at the end of each question group indicating that the question group was reviewed and updated

3. The purpose of the questions is related to quality assurance measures about any services the person receives on their plan

4. All question indicators must be reduced to zero (0) before the Certified Assessor can determine eligibility

5. Essential Community Supports Added to LTC Document The Essential Community Supports (ECS) program started January 1, 2015 to provide transition support to people affected by changes to the nursing facility level of care (NF LOC). It is also available to support people age 65 and older who do not meet NF LOC and are not eligibility for Medical Assistance, but who have a need for community support. MnCHOICES release v.15.1 adds ECS as a Program Type to the Screening/Assessment Information Results question group in the LTC Screening Document Domain, under Field 104: Program Type.

Assessors now have the options to select: • Program Type 29: Essential Community Supports • Program Type 30: Essential Community Supports Transition

1. By selecting either: a. Code 29 – Essential Community Supports; or b. Code 30 – Essential Community Supports Transition

2. The LTC Screening Document will be populated with the selection and can be used to enter necessary information into MMIS.

6. Content Changes in LTC Screening Document eDocs released an updated version of the Long-Term Care (LTC) Screening Document (SDoc). Content changes in the LTC SD, requires that MnCHOICES reflects the changes. Content was both added and deleted in the Service Plan Summary Question Group of the LTC SDoc Domain.

In MnCHOICES, information for the LTC SDoc comes from three locations: • Person Information • Assessment Information (Quality of Life through Assessor Conclusions) • LTC Screening Document

Content changes related to the LTC SDoc only were made in the LTC Screening Document domain in the Service Plan Summary question group. The changes are as follows:

Additions The additions are all related to fields 108 and 110 of the Long-Term Care (LTC Screening Document (SDoc) and include:

Field 108 • A1 – Individual Community Living Supports (new choice) • A2 – Community Living Assistance (new choice) • 80 – Home care nursing (formerly private duty nursing) • 81 – Extended home care nursing (formerly extended private duty nursing)

Field 110 • 08 – Person is receiving temporary AC

Deletions The deletions are all related to field 108:

Field 108 • 64 – 50 hour Direct staff//Medication management assistance • 85 – 24 hour supervision for 50 hrs/ADLS/medication management
