President’s Message

I know that by the time this is published that

the holidays will be over but just wanted to say it anyways. I hope everyone has had a wonderful holiday season and hopefully had a wonderful 2014. As I refl ect on my past year, I can’t help but feel grateful for how it has turned out.

This past year was fi lled with great CE and 2015 will be no exception. Dr. Bob Margaes will have an excellent lecture and hands on weekend covering Esthetic resin techniques and also veneer preparation and cementation during our June meeting. Dr. Richard Mounce will cover updated endodontic techniques and will also have a hands on portion during our Fall meeting. I would like to encourage signing up early as these courses will fi ll fast.

I would like to congratulate our honored members who have anniversaries. There

are also members who have passed the Fellowship exam this year. Congratulations also to the members getting their Fellowship or Mastership in San Francisco this summer. If you are thinking about attaining this elite status within the AGD, you should consider the MasterTrack program offered within the NAGD.

As you know our profession is constantly evolving and there are a few issues being presented to the Board of Dentistry. I would like to thank the NAGD board members who have been involved with the hearings and putting the AGD, and every dentist in Nebraska, best interests as a top priority.

On a fi nal note, I would like to thank you for being your President and wishing everyone a wonderful 2015.

Hopefully everyone had a

truly blessed holiday season, I know I did. I always enjoy the holidays, seeing all of my friends and family. Last year we had amazing meetings with great speakers such as Dr. Koerner on, ”Oral Surgery for the General Dentist,” and Dr. Joseph Massad on, “Current Concepts in Implant

Prosthodontics.” It was such a great opportunity to have CE of this caliber available so close to home!

As we enter a new year, we are always looking to our next meeting fi lled with exciting continuing education. We kick it off in June with Annual Meeting hosting Dr. Bob Margeas for a Friday

lecture and hands on training on “Everyday Dentistry — More than Everyday Esthetics Results”. Then in the fall course we have Dr. Richard Mounce coming in to do a lecture and hands on course on Endodontics. I hope to see all of you there!

The AGD’s advocacy efforts for the third quarter largely focused on appropriations, specifi cally actions taken by the AGD to retain the legislative prohibition on use of HRSA funds to advance the Alternative Dental Provider Demonstration program and an increased emphasis on promoting the importance of oral health literacy. Other signifi cant advocacy activities included weighing in with lawmakers on the Children’s Health Insurance Program (CHIP), as well as working with Senate staff to generate support for the Dynamic Repayment Act as part of the AGD’s ongoing efforts to address the issue of dental student debt. A great deal of the AGD’s advocacy focus this quarter was enhanced by in-person meetings with key Hill and Agency offi cials and staff.

In light of the Senate Democratic appropriators’ decision to lift the prohibition on funding for HRSA’s Alternative Dental Health Care Providers Demonstration program for the fi rst time, Dr. Brown conducted a series of meetings in Washington, D.C. with infl uential Members of Congress to reinforce the need to retain the prohibition. While on the Hill, Dr. Brown met with staff from the Offi ces of Rep. Jack Kingston (R-GA), current Chairman of the Labor-HHS Subcommittee; Rep. Mike Simpson (R-ID), fellow dentist and senior appropriator; and Rep. Kevin Brady (R-TX), Chairman of the Ways and Means Subcommittee on Health. Dr. Brown also met with Rep. Paul Gosar (R-AZ), another fellow dentist and AGD ally. On the whole, the meetings were productive and substantive, but both Dr. Brown and the AGD’s Washington Representative came away from the meetings concerned that the “fi rewall” that has retained the prohibition may be weakening. The AGD is increasing its ongoing efforts to preserve the legislative ban.

Another major advocacy highpoint of this quarter was Dr. Brown’s visit with Dr. Katherine Weno, Director of the Division of Oral Health at the CDC in Atlanta. The purpose of this meeting was to discuss oral health literacy and ways in which the AGD and CDC can partner together on shared interests. By all accounts, Dr. Brown’s presentation was very well received. Dr. Weno especially appreciated Dr. Brown’s overview of the many activities AGD has underway for advancing oral health. Overall, the meeting raised many opportunities for collaboration between CDC and AGD on several issues, including oral health literacy, anti-smoking efforts, and fl uoride.

To further raise the issue of oral health literacy and potential partnership opportunities, AGD member, Dr. Susan Bishop, met

with Rep. Robin Kelly (D-IL) in her district offi ce as a follow up to Dr. Brown’s June visit. Dr. Bishop also took this opportunity to reiterate the AGD’s support for the Action for Dental Health Act, legislation introduced by the Congresswoman that focuses largely on oral health literacy and prevention, and reducing barriers to care via public/private partnerships.

The AGD was also active on the issue of CHIP funding. Coinciding with a report issued by the Medicaid and CHIP Payment and Access Commission, several bills have been introduced calling for extended funding for the program. Created in 2007, CHIP fi nances coverage for approximately eight million children nationwide. Specifi c to dentistry, in FY 2012, 43 percent of children in Medicaid and CHIP received preventive dental services, and 24 percent received a dental treatment service. Funding for CHIP is only secured through Sept. 2015, however, and conversations surrounding its future are ramping up. The AGD has issued letters of support to the cosponsors of legislation calling for extending funding as well as a general letter to Congressional leadership asking that this issue be taken up before the new Congress is sworn in. In each of these letters, the AGD again emphasizes the role oral health literacy and prevention plays in combatting the issue of chronic oral health ailments in the U.S.

Regarding dental student debt, the AGD sent letters of support to Senators Marco Rubio (R-FL) and Mark Warner (D-VA) regarding their introduction of the Dynamic Repayment Act, companion legislation to AGD-endorsed House bill, the ExCEL Act. The AGD also connected with staff in both offi ces to offer its assistance in generating support for the legislation and to serve as an ongoing resource on issues related to higher education and student debt.

In Sept., fl uoridation opponents gathered in Washington, D.C. to lobby Congress on the need to end community water fl uoridation programs. They also asked lawmakers to defund the CDC. Members of the anti-fl uoride groups told Members and their staff that commu-nity fl uoridation is outdated, ineffective, and dangerous. In response to this development, the AGD issued an action alert, encouraging members to let their legislators know of their support for commu-nity fl uoridation programs. Further, Dr. Brown raised this issue in his meeting with Rep. Mike Simpson’s senior staffer to reiterate the importance and success of fl uoridation programs to date.

Scientifi c Session to Split from Governance Meeting

T he Academy of General Dentistry (AGD) House of Delegates (HOD) recently voted to split the AGD annual meeting into two separate meetings beginning in 2017. The scientifi c session and convocation

will take place in the summer, while the HOD and governance will take place later in the year.

This change will allow the AGD to concentrate more resources toward the scientifi c session, and provide the time and space fl exibility to create new, dynamic education offerings and learning opportunities. The Annual Meetings Council, the Board, and others already have begun discussing ideas for these transformations.

Moreover, with the HOD occurring later in the year, AGD leaders and staff will have more time to evaluate the budget, allowing them to be more cost-effective in developing timely and valuable programming for AGD members.


Dr. MargeasSpeaker

D r. Margeas graduated from the University of

Iowa College of Dentistry in 1986 and completed his AEGD residency the following year. He is currently an adjunct professor in the department of Operative Dentistry at the University of Iowa. He is Board Certified by the American Board of Operative

Dentistry. He is a Diplomate of the American Board of Aesthetic Dentistry, a Fellow of the Academy of General Dentistry and International Team of Oral implantologists (ITI). He has written numerous articles on esthetic and implant dentistry, and lectures and presents hands-on courses nationally and internationally on those subjects. He serves on the Editorial Advisory board of Inside Dentistry, Compendiuim and is a contributing editor to Dentistry Today and Oral Health in Canada. Dr. Margeas maintains a full-time private practice focusing on comprehensive restorative and implant dentistry in Des Moines, Iowa.

Numerous cases utilizing direct composite resin will be shown step by step. These include class IV restorations, diastema closure, and direct veneers. You will gain an understanding of resins that can offer an alternative procedure to porcelain. The techniques are minimally invasive, yet highly esthetic and long lasting. The hands on portion will consist of creating a direct composite veneer that can rival a porcelain veneer, creating an invisible class IV restoration, and performing a diastema closure. This interactive course is done in a step by step manner utilizing a visualizer, so that the participants can watch every aspect of the procedure and then create their own composite masterpiece.

Trustee Report

D uring this end of year and Holiday

time, refl ecting on what has been accomplished and what has been set up to move forward, succeed and making plans for the future is important for all of us. We have much to be thankful for. Having now served for a little over 3 years as your Region 10 Trustee I am just getting to learn and understand the ropes and have seen a lot of exciting things happen at the Academy. With all that I have seen, heard and experienced, I am still excited about the Academy and our future.

Over these years, we have successfully purchased, designed the layout, built out the facility and moved into our own offi ce building at 560 West Lake Street. This move has been a money saver for us already. Since we moved in, Google has moved into the area in Chicago and the west loop area is growing and developing at quantum speeds and our building value continues to rise at a rapid rate. On an operational side, we are going to have a number of our board meetings on site so this will save the

Academy a good amount to allow more to go to member benefi ts. Certain council meetings can take place at this site also. Holding meetings with other organizations and conferences along with providing national CE is in the works to raise the Academy to a new level.

I had the good fortune of being asked to help run an Educational Summit that utilized the new building and we were looking well into the future for the direction of education for the Academy. We brought in 5 futurists that educated us as to what trends and learning methods are appearing on the horizon. Our councils are now busy working on the information that was gleaned. Our future here is bright as education is the foundation of our Academy.

Our relationships with other dental organizations have been strengthened through a series of alliances and we will see future information in our publications, Impact and General Dentistry, of these collaborative efforts.

At our last House of Delegates in Detroit we were fi nally able to separate

Governance from the Annual meeting. Our next annual meeting will be in San Francisco in 2015 and then Boston in 2016. We are working hard to come up with a great venue for 2017 with this new format. Working forward with the HOD meeting looks like we will be moving it to Chicago but the time frame isn’t set in stone. There will be a bit of transition as we move forward. The Annual Meetings Council can now focus on providing our members with an educational meeting that will make this meeting a must attend for members. The AGD team can work with our leaders to focus on a separate governance meeting to be sure that we are running on all cylinders and moving in the right direction towards better goals.

Yes there is a lot to be thankful for and I want to thank our Region 10 members for allowing me to represent them on the Board of Trustees. I look forward to the remainder of my tenure on the Board and helping to make this a better Academy for all of us.

Areas covered will include the latest concepts in diagnosis, case selection, and cleansing, shaping and packing of the root canal system in three dimensions. Techniques discussed in the lecture will be demonstrated in both plastic blocks and extracted teeth. Participants will have ample time to practice instrumentation and obturation of canals utilizing the methods and equipment demonstrated.

F ebruary, as many of you know is National Children's Dental Health month. The Oral Fact sheets located on the AGD website, (http://

www.agd.org) have many resources to help educate your patients not only on children's oral health but topics from A to Z. Many questions or subjects that come up in conversation during the appointment are covered with a fact sheet. They are easy to find, print, and understand for your patient. They even come personalized which is in a way free marketing. I encourage you to utilize these to help promote good oral and overall health.

In the June news-letter I highlighted a 407B proposal

currently under review to redefi ne sedation for Nebraska dentists. The fi nal mold has not yet been cast, but a topic that cur-rent sedation dentists most likely will need to address in the near future is: Where can I get sedation train-ing that meets the new requirements?

The proposed requirements insist upon stringent train-ing for a practitioner to be certifi ed for moderate sedation in either IV or oral form. For oral sedation, the DOCS course appears to fall short of the proposed requirements and would only be suitable for a single dose protocol. Barring train-ing received in residency, many sedation doctors will need to look for a reputable program that can provide the credentials needed to continue providing impor-tant sedation services.

Multiple opportunities exist to obtain training in the United States outside of residency. A solid choice comes from the Oregon Academy of General Dentistry in combination

with the Oregon Health and Science University School of Dentistry (OHSU). This program from a cursory evaluation appears to be among the more rigid offerings available, but is a program that would pass the hurdles that are cur-rently being laid down. Dr. Kenneth Reed, a periodon-tist with a second residency in dental anesthesia, directs the course. The program has multiple renowned faculty members, includ-ing co-director, Dr. Stanley Malamed.

At $12,950, the OHSU pro-gram provides 103 CE hours with 60 didactic and 43 clinical. The course is broken down into three visits, two didactic and one clinical visit, each lasting four days. Prior to attendance, each clinician must have BLS, ACLS, and DEA Schedule II/IIN- V. The clinical aspect of this course is very demand-ing. Each dentist is required to bring two dental assis-tants, armamentarium, and supplies to treat 20-22 patients. To graduate the program, each dentist must sedate and perform clinical dentistry on each patient

to simulate real workplace conditions.

Dr. Reed states implicitly in the program information that he has in the past and may in the future refuse to write a letter to the state board stating competence. Caveat emptor.

If the commitment to become IV certifi ed is unob-tainable, another option is to have a nurse or dental anesthetist come into your offi ce and perform the sedation. In this scenario, the dentist can focus solely on the sedated patient and would have freedom to exit the operatory leaving con-tinuous monitoring to the dedicate anesthetist.

Fellowship Review Course & ExamBy Julie Berger, Executive Director

O n December 5 & 6, the Nebraska and Iowa AGD offered for the first time, the AGD Fellowship Review Course in Omaha. The two day course is an intense review course of all the

disciplines covered on the test. The exam was offered on December 7th. I am happy to report that the class had a 96% pass rate. This is a testament to this group of doctors in the class and who had been studying for weeks prior to the course and exam.

Congratulations to the following Nebraska AGD members who passed the exam: Drs. Travis Antholz, Kent Dodson, Amy Fournier, Nicole Lecci, Kate Olechoski, Kyle Papousek, Tom St. Germain and Michael Sesemann.

Aside from these Nebraska AGD members, there were also 14 other doctors from Iowa, South Dakota and Kansas who passed the exam.
