This form will be used after the student has received the 10th day of Removal.
Length of Removal: Special Circumstances or Change of Placement
Indicated the Number of days removed Attach Goals/Objectives to be Addressed Indicated how the student will progress within the General

Purpose of the IEP
To provide the parents a chance to meet the new providers, discuss the new goals, and tour the building
To make sure all parties are on the same page with hours of service, goals, and data
To answer questions

Setting up the Level Change IEP
Coordinate IEPs with a middle school building representative so that the IEPs are done in one day for that building (you will have to agree to a date and times)
In Notice of Meeting check the other box and add: Level Change from elementary to middle school
Email middle school representative the final dates/times (the middle school will email the ancillary staff needed)
Confirm with parents that the IEP is at the middle school
Meet in the designated place

Pg. 1 Purpose: Annual Other -- Level Change from Elementary to Middle School
Pg. 2 PLAAF Statement -- specific to the student includes:
the disability how it impacts them in the classroom accommodations needed in order for the student to be successful
Pg. 3 Accommodations: specific to that student and related to their eligibility area(s).
Pg. 4 Hours: This is specific to the middle school’s program. Ask the building
representative. Pg. 5 MEAP -- MEAP with accommodations.
accommodations must match the PLAAF. Math tests cannot be read for students who don’t have a reading disability or receive this accommodation regularly.
