ELECTRICAL ACCIDENTS !! WHY IS THE COMMISSION WORRIED ?

• Newspaper reports & reports of the Chief Electrical Inspector indicate –

• Too many electrical accidents causing death of human beings / animals and damage to property

• The Commission finds - •The system is not functioning, Safety Rules / practices as per I E Rules and other relevant Regulations not followed•Commission can not remain a silent spectator to such a situation•Thus, steps in through the omnibus provision under the Electricity Act, 2003.

SAFETY, WHY IT IS NEEDED ?

• Loss of human and animal lives and damage to property has enormous costs.

• The increasing number of accidents indicates-– negligence, non-serious, callus attitude, ignorance of the

employees of the Licensees about the Safety Rules– carelessness of the people who faces the accidents.

• Hence, – Requires much stricter supervision by the field officers of

the Licensees. – Minimum level of preventive maintenance can restrict the

number of accidents.

CARELESSNESS/LACK OF PRECAUTIONS

• Municipalities/Urban Development Authorities / Roads & Building / High ways etc. not adhering to the rules while implementing schemes without consulting concerned Distribution Licensee.

• Raising level of roads without caring for the clearance under electric lines.

• Constructing buildings under electric lines • Extending portions of buildings/ roofs without

leaving required side clearance from the electric lines.

CARELESSNESS/LACK OF PRECAUTIONS …..CONTD..

• Heavy vehicles, raising the height of loaded goods without caring for the required electrical clearance

• Processions for immersions, marriage etc. are being accompanied by tall gates/ arch, hopper of concrete mixture machines loaded on trucks / tractors,

• Loading on the roof of bus to excessive height without looking at the required electrical clearance

RELEVANT ENABLING PROVISIONS UNDER THE LAW • Sec-53 of the EA,2003:

(a) protection from dangers arising out of electrical installation,(b) eliminating and reducing the risks of personal injury and

damage to property etc.• Regulation 49 of the OERC Distribution (Condition of Supply)

Code, 2004 : envisages that • “Maintenance of all installation in good order- not to cause any

electrical or mechanical accident or damage to consumer’s/public property.

• Licensee to repair or replace the damaged parts immediately on getting intimation of damage or danger to life.

• Sec-142 of the EA, 2003 empowers the Commission to punish any person for non-compliance of the provisions of the Act/Rules/Regulations/Directions.

CODAL PROVISIONS

• Sec.73 - CEA to specify Technical Standards for construction of electrical lines and Safety requirement.

• Sec.160 – Supply licensee to take precautions not to injuriously affect, by induction or otherwise, the working of any wire or line used for telegraphic or telephone or electric signaling communication.

• Sec.161 – Electrical accident during supply or use of electricity resulting in loss of life or injury to any human or animal. Licensee shall give notice of such accident to the Electrical Inspector.

• Electrical Inspector to make an enquiry into the accident and report on the cause of accident and whether the safety provisions have been complied with or not.

• Indian Electricity Rules, 1956 deals exclusively with the safety precautions, accidents etc.

CODAL PROVISIONS …..CONTD..

• Section 68 of EA, 03 and Clause 82 of I.E. Rules, 1956 deals with procedures, for dealing with subsequent placement/ alteration of structures/ objects/trees etc. near existing overhead lines.

• Condition 10.4 of the Distribution License • Commission may by order, provide compensation to

persons, who suffer substantial injury or to the heirs of those who lose their lives caused by any act of commission, omission or negligence on the part of any of the employees or agents of the Licensee.

• The above provisions are not exhaustive but illustrative. A full list will be circulated later.

SAMPLE CASES OF ELECTRICAL ACCIDENTS

• Licensees’ non-adherence to rules/not taking adequate precautionary measures -On 07.12.2007, Sri Purna Chandra nayak of WESCO was electrocuted and died at the 11/.4 KV S/S near Tarini Mandir, Rourkela while repair the jumper in the S/S and sudden charging of the line.

-This was due to adoption of improper method of obtaining shut down and allowing of trainee technician without having authorised permit to work in the S/S.

- On 19.01.2008, one Sri Srinibas Bhuyan of SOUTHCO died while working in the 11/.4 KV S/S, Purusottampur.

- Due to sudden charging of the line by another workman of SOUTHCO. This is also due to the same reason as cited above.

- The Commission had taken cognizance of the matter and taken suo-motu proceeding under Section 142 of the Act and had also directed the Electrical Inspectorate to proceed under Section 146 of the Act.- On 16.10.2006, three elephants died near Pentabahal under Nakti Deula, Sambalpur

- due to electrocution for the low ground clearance of the 11 KV line.

SAMPLE CASES OF ELECTRICAL ACCIDENTS ….CONTD..

• Users of electricity / general public not being vigilant

– On 16.05.2008, seven persons were electrocuted while transporting one concrete mixture machine below the 11 KV line at Talaberena, Parjang, Dhenkanal.

– On 30.10.2008, two persons died while the hopper of the concrete mixture machine came in contact with the low clearance 11 KV line at Hirmula Village of Khurda.

• CASE -1• An electrical accident occurred on 16.05.08 in Ramalenka

village under Malud Electrical Section of SOUTHCO. • One cow and three bullocks were electrocuted. • The fact - It was found that the “R” phase of 11 KV Bajrakote

feeder slipped and was hanging three feet high on the ground level due to detachment of pin insulator from GI pin which was broken completely due to corrosion of iron materials.

• Reason - Contravention of the provisions of the Safety Rules.• 11 kV supply without pin insulator - for want of a G I Pin.• Officers of SOUTHCO responsible for the fatal accident:• The JE/SDO could have arranged a GI Pin even from a

defunct line to replace and rectify the defect.

• CASE -2• On 16.10.2007, there was a snapping of LT conductors

in the LT line emanating from the 100 KVA 11/0.4 KV S/S, Baghadi.

• At about 8.20 hrs. on the said date, Sri Biswanath Sahu came in contact with the snapped conductor and was electrocuted.

• The fact - Due to non-blowing of the LT fuse (aluminium conductor was being used as LT fuse wire), non-functioning of the A.B. switch of the 11/0.4 KV S/S (blades of the AB switch were tied with GI wire) and absence of earthing of the S/S, Sri Sahu died.

CASE -2 …CONTD..

• The reasons – • 1. AB switch was not in working condition.• 2. Aluminum conductors were used in place LT fuse

wires.• 3. S/s/ earthing was disconnected from the earth.• These activities essentially fall within the domain of the field

officers in charge of power supply. • Safety aspects are being overlooked by the supervising

officials • Amounts to contravention of the provisions of Indian

Electricity Rule, 1956

• CASE -3• On 19.01.2008 at about 20.30 hrs., the LT cable

of 2X100 KVA 11/0.4 KV S/S situated adjacent to the Police Station, Purusotampur started burning.

• The fact - The line clear on the 11 KV line from the 33/11 KV S/S, Purusotampur was not properly taken by the J.M. Sri Jugal Kishore Mallik The ITI Trainee Lineman on duty at the 33/11 KV S/S was telephonically instructed by the J.M. to hand-trip the 11 KV Purusotampur feeder and not to charge the same till the telephonic clearance is given.

CASE -3 …contd.

• The line man, Sri Bhuyan was electrocuted at about 21.10 hrs. • Sri Bhuyan was having competence to work as Lineman-C

upto 650 volts only but was allowed to attend the work at the 11/0.4 KV S/S in violation of Rule-3(2) of the I.E. Rules, 1956.

• The 11/0.4 KV S/S of Purusotampur was having 2 nos. of 100 KVA transformers in one D.P. structure with one H.G. fuse in utter disregard of the provisions of Rule-50(1)(b)(i) of the I.E. Rules, 1956.

• There was no link switch with fuse or circuit breaker of adequate capacity provided to the transformers of the D.P. structure.

• The inspection formality of the said 11/0.4 KV S/S was not being followed by SOUTHCO.

CASE-3..contd.

• The reasons -• (i) persons with inappropriate permit licence were

entrusted the maintenance work in Purusotampur area of SOUTHCO;

• appropriate line clear issue and return procedure were not followed;

• the substations were constructed in violation of the provisions of the I.E. Rules, 1956;

• authorities did not check and rectify the above errors; and

• the line was charged by the person manning the 33/11 kV S/S 2 Kms. away without confirming whether a person was still working on the line.

I In the backdrop of the above codal provisions, the Commission has been receiving the enquiry reports of the Electrical Inspectorate on various electrical accidents occurred over the period. We have summarized below the individual cases of Electrical accidents.

After receiving various enquiry reports on electrical accidents, the Commission found that most of the accidents were due to non-adherence of standards and ignorance of Rules/Acts by field employees. The Commission advised the GoO vide its ltr. No.1829 dt.17.10.07 to conduct professional examinations for the engineers of the licensees/OHPC/OPGC to create awareness.

After replacement of fuse, the deceased tried to check the fuse of 11/0.23, 16 KVA Stn. Trf. & got electric shock due to non-isolation of the Trf. Accident occurred due to non-isolation of the Stn. Trf. & maintenance by the personnel not having the valid workmen permit.

Came in direct contact with the live conductor charged through hooking, which was laying naked around the pond.

Fatal - The miscreants who had hooked the LT line are responsible for the accident. Regular patrolling & hook detection by CESU is required.

Electrocution of Gopal Kalet & one Goat.

‘R’ phase of the LT line was hanging to ground at touchable height due to damage of LT pin binding. The deceased came in contact with the line to save goat.

Yadav, truck helper electrocuted while coming in contact with the loose 11 kV line with low ground clearance.

Electrocution of Alok Ku. Mohanty.

Electrocuted while the Auto in which the victim was sitting dashed against the electric pole causing the conductor came down to a height of 5 ft.

Fatal - Poor quality of pin binding and loosely secured cross arms to the poles.

Victim. Timely replacement of old distribution elements is required.

21.06.07 at/PO: Kuarmunda,PS: Kuarmunda,Sundargarh. (WESCO)

Electrocution of Dhanpati Karali

Electrocuted while touching live Y phase at 11/0.4 kV S/S of M/S Bharati Airtel Ltd.

Fatal - Died on the spot WESCO and the person engaged by electrical contractor (M/s Akhandalmani Electricals & Construction).

1.7.07 between 3 to 4 PMAt:Jatala Srigida, Bargarh(WESCO)

Electrocution of Ratish Barik and one bullock.

A bullock was electrocuted by the charged conductor fallen at the ground. To save to bullock, the deceased person came in contact and the mishap occurred.

- The particular accident occurred due to supply in under size conductor & without any proper protection arrangement.

Electrocution of a cow

The snapped live conductor touched the lower part of the stay wire. A cow came in contact with the charged stay wire and died on the spot.

Fatal - Adequate protection should be provided to the lines & S/Ss to isolate the system during fault.

15.7.07 at 6.30 PM At. Gudgudipada, Atabira, Bargarh (WESCO)

Electrocution of two buffalos

The 11 KV conductor snapped as the pin insulator punctured and the live line fell on the buffalos tied below the line.

- No proper protection arrangement and regular inspection by WESCO

General public awareness be created by WESCO
