V Neighbor Notification Form: Obtain the names, addresses, signatures and contact information (optional) ofaffected neighbors acknowledging they received notice of the improvement Owners must make an attemptto obtain signatures from owners of adjacent homes that are unoccupied or rented out to meet submittaLrequirements. Please note neighbor awareness does not constitute neighbor approval or disapproval. Theintent of the Neighbor Notification Form serves to make neighbors aware of the proposed improvement.

Concrete Setback Form (Concrete Additions Only): If submitting an application for concrete additionsincluding walkways, patios and driveway expansions, fitt in the required dimensions of your tot noted on theform.

Plans: DetaiLed plot plan drawn to scate and/or CAD drawing showing tocation, size, dimensions, setbacks,identified scale, and description of proposed improvements. Plans and specifications for improvements mustbe prepared with sufficient clarity and compLeteness to enabte the ACC to make an informed decision onthe request Said plans shall include a ptot plan drawn to scale showing the fottowing:

• Alt proposed Improvements and relevant eLevations, together with the desired Location of suchImprovements as they relate to the Residence and adjacent fence/tot lines (setbacks).

• Complete dimensions of the proposed Improvements.• Drainage plans (if applicable) where the established drainage pattern might be altered by the

proposed Improvement If drainage is to be connected to the existing system as buitt, a cLear notationmust be made on the plans.

• Any pLans larger than 11” x ‘7” must be accompanied by a digital copy.

Application Condition for ApprovaLIt is the Owner’s responsibility to supply any and alt details required, as stated in the Home Improvement Application.This gives the Architectural Controt Committee tACC) the needed information to determine compliance with theCC&Rs and the ArchitecturaL Control Guidelines regarding this proposed Home Improvement Apptication. The ACCreserves the right to request additionaL information when necessary.

The Owner is responsible for the accuracy of the information supplied within this Home Improvement Application andits compliance with any and all CC&R and Architectural Guidelines, If the construction or installation of any homeimprovement item(s) is tare) deemed non-compLiant to the CC&Rs or Architectural Guidelines after being constructedor installed, even with an approved Home Improve Application.
