medications for secondary prevention of coronary heart disease
and other artherosclerotic vascular diseases, recommendations
are not applied properly.
The reduction in CV events is an essential component of cost-effective health strategy.
All this demonstrates the potential of a product that may enhance adherence in order to reduce direct health costs.

ΣΤΑΘΕΡΟΣ ΣΥΝΔΥΑΣΜΟΣ
ΑΣΠΙΡΙΝΗΣ(ASPIRIN) + ΜΠΙΣΟΠΡΟΛΟΛΗΣ (BISOPROLOL)

• Τα αιµοπετάλια είναι ιδιαίτερα ευαίσθητα στη δράση της ασπιρίνης.
• Αυτό οφείλεται στο ότι, σε αντίθεση µε άλλα κύτταρα, τα αιµοπετάλια δεν
έχουν την ικανότητα να επαναπαράγουν κυκλοοξυγονάσες.
• Στην πράξη αυτό σηµαίνει, ότι µία δόση µόλις 40 mg ασπιρίνης θα
αναστείλει την κυκλοοξυγονάση των αιµοπεταλίων για 8 έως 10 ηµέρες, που
είναι ο µέσος χρόνος ζωής τους
Ασπιρίνη & Kαρδιαγγειακά επεισόδια• Χαµηλές δόσεις ασπιρίνης για µεγάλο χρονικό διάστηµα
διακόπτουν τον σχηµατισµό θροµβοξάνης Α2 (TXA2) στα
PLT, αναστέλλοντας τη συσσώρευση των PLT
•Καθηµερινές δόσεις 40 mg ασπιρίνης παρεµποδίζουν σε
µεγάλο ποσοστό την απελευθέρωση TXA2, ενώ η βιοσύνθεση της
χρήσιµης προσταγλανδίνης Ι2 (προστακυκλίνη: δραστικός αντιθροµβωτικός και
αγγειοδιασταλτικός παράγοντας) επηρεάζεται ελάχιστα • Ωστόσο, σοβαρή αρνητική επίπτωση της µακροχρόνιας
λήψης ασπιρίνης είναι η µείωση της πηκτικής ικανότητας του
αίµατος και η αύξηση της πιθανότητας αιµορραγικών
επεισοδίων ανάλογα µε τις λαµβανόµενες δόσεις

! Πολύ ετερογενείς πληθυσµοί
! Πληθυσµοί πολύ χαµηλού έως χαµηλού κινδύνου στις περισσότερες µελέτες
! Πολύ ετερογενές σχήµα χορήγησης ασπιρίνης (από 75mg µέρα παρ’ ηµέρα έως 500mg/ηµέρα). Γνωρίζουµε πλέον ότι αρκούν χαµηλές ηµερήσιες δόσεις για την αντιαιµοπεταλιακή δράση & µειωµένο κίνδυνο αιµορραγίας

As the number of patients who had a previous CV event was small ~ 9% (1.6% had a previous MI, 1.2% had a previous stroke, and approximately 6% had other previous coronary heart disease [CHD]), the HOT study can be regarded as a major primary prevention study.

Ασπιρίνη στην κλινική πράξη
! Η Ασπιρίνη χρησιµοποιείται σταθερά στην πρώϊµη φάση των οξέων στεφανιαίων συµβαµάτων, µαζί µε νεώτερους παράγοντες (κλοπιδογρέλη, πρασουγκρέλη, τικαγκρελόλη)
! Συνιστάται σε όλες τίς οδηγίες σε ασθενείς µε καρδιοαγγειακή νόσο, όταν δεν υπάρχουν αντενδείξεις
! Αυτό είναι σε αντίθεση µε τίς οδηγίες στην πρωτογενή πρόληψη, εξαιτίας τής διαφορετικής αναλογίας κινδύνου:ώφελους (risk:benefit ratio)
