Sir/Madam,

This has the reference of this Ministry's letter of even number datedE·I 1.2017.conveying the approval of the Government for implementation of Scheme forAdolescent Girls (SAG) for out of school girls in the age group of L1-14 years with itsphased expansion for a period of one year i.e. upto 30.11.2018. The schemeoperational in ~05 districts has been extended cover additional 303-districts in 2017-18.List of the additional 303 districts is enclosed at Annexure-l.

2. SAG would replace the existing Kishori Shakti Yojana (KSY) in selected 303districts. In the remaining districts, KSY where operational, would continue for out ofschool girls of age 1I -14 years and will be phased out with expansion of SAG. Theguidelines for implementation of the Scheme are being finalized and would be sent

~.3. State Government/UT Administration are requested to start a base line survey forigentification of the out of schoo I girls of age 11-14 years in. the selected districts to rollout the sch~me The instructions along with the format for conducting the base linesurvey are enclosed at Annexure-2.

4. The States Govt. IVT Administration is requested to carry out the base linesurvey in the selected districts expeditiously and intimate the Ministry about thebeneficiaries expected to be covered under SAG during 2017-18 in the format of.Summary: ~?y 31.12.~017 positively. A copy of the same may be sent throughemail mksingh.ofb@nic.in and arora.prabha@nic.in. Funds would be made availableto States/UTs under SAG for Nutrition component based on this information andissuance of Notification w.r.t revised rates of nutrition for out of school girls of age _!_I-14years (reference MWCD's letter of even number dated 06.10. 2017).

:1 . Annexure-ItInstructions for Base line Survey of Adolescent..G:lrls

1. What is the purpose of the Survey?

The Base line Survey of out of school girls of age 11-14 years is meant to help tQ_pState/ DT identify and list every out of school girls of age 11-14 years in the area ofthe AWCs where SAG is to be implemented.

2. How is the format organized? How ts it to be used? Whose names should beentered in this format?There are two parts of the base line format:

a. Part 1 is used to record the details of out of school girls of age 11-14 yearswho live in the area covered by the AWC.

b. Part 2 is the sUmmary of total out of school girls of age 11-14 years in thearea.

Part 1:Details of Adolescent Girls

1. Part I of the format is to be used to collect and write down some simple detailsabout the AWC, the families and out of school girls of age 11-14 years living inthe area of the AWC. All out of school girls of age 11-14 years are eligible toreceive services from the AWC and should be included in the survey. At the top ofeach page, a few details of the AWC are to be entered.

i. Location: Here, the brief address of the AWC should be entered. This mayinclude the LGD Code, name of the street or tola or mohalla, as the case maybe.

ii. Date of start of survey: This is the date of the first day of the survey. Tbis isa permanent date, and should not be changed.

Ill. Date of end of survey: This should be written aft~_the survey is completed andshould be counter signed by the supervisor after ensuring that all girls arecovered.

IV. Name of Surveyorj The name of the Anganwadi Worker or Surveyor (if otherthan the Anganwadi Worker) should be mentioned in the name column andsigned by her.

2. How is the survey to be conducted?On receiving instructions, the AWW should go from house to house to meet every familyin the area covered by her AWC, and gather information about the details of each out ofschool girls of age 11-14 years.

Details to be entered column wise.

1. Col. 1: S.No. - ~rial number of the entry.

ii. Col 2: Name of the out of school girls of age 11-14 years: Full name of the out ofschool girls of age j 1-14 years should be written (Surname, name) as the person

would normally write it in official documents for example Gupta Sunita.

iii. Col 3: Aadhaar Number of AG: The Aadhaar Number of the Ag to be recorded inthis column.

iv. Co14: li'ull Name Ofl?ather/Guanlirul of Family: The full name of Father shouldbe written. as he would normally write it in offieial documents, such as the rationcard. This usually includes the first name and surname for example Ram KumarGupta. The person/Guardian by whose name the family is known in the village tobe given, in case the family is looked after by a person other than father/mother.The Guardian may be a man or a woman.

_ v. Col 5: Full name of Mother: The full name of Mother should be writt~hewould normally write it iI2....<?f~cial'!.~ts, such a_!the ~ car,whis usuallyincludes the first name and surname for example Sushma Ram Kumar Gupta orSushma Devi Gupta as may be applicable.

'- vi. Col 6: SC / ST (Others: There are many categories of caste, but here, only tickunder "ST" or "SC" as the case may be. If the family belongs to neither of these­categories, then tick under "Other". All General categories, OBCs, Minorities etc.will be ticked under' Other'.

vii. Col 7: Date of Birth; The date of birth of the out of school girls of age 11-14 yearsis to be recorded (if available) in this Column in dd/mm/yyyy format. For example26.10.2006.

viii. Col 8: A2e as on 1.1.2018: The age of the AGs as on 1st January 2018 should beentered here, in completed years. For instance,

IX. Col 9 (a) and 9 (b): out-Qf-.school datil; The AWW has to carefully tick about theAG being out-oY-school. OOS will include AGs who have never gone to school asalso the dropout girls. For example, .if a girl has never attended sohoel, the entryunder this column should be "0". If the AG bas dropped ·out after class 5 or 6 , theentry under this column should be Class 5 or 6 what ever the case may be.

x. Col 10: Reason for leayine the school; The column of Education status will reflectthe class completed by AGs in columns in 9 (b). Reason for leaving the school to beentered in col (10).

l. The survey would be carried out by the AWW. In areas where the AWW is notadequatelY qualified, the State Government may get the survey done bySupervisors/others.

2. The entry of only those AGs should be made who have stayed in that village forthe past 6 months and have intention of staying in the same village.

3, The Supervisor would check and authenticate 20% of the entries in the surveyregisters.

4. The Base line survey would be conducted by the AWW as a onetime a_c1iY.i!Y_fordetails of age, schooling status, reasons for leaving the school. This would be thebase for the AG Register, to be updated on a quarterly basis.I' 5, The Part 2 may be summarized by the AWW. If, she is unable to do it this may be

I done by Supervisors/others.6. The cut-off date for validation of data would be 31112/2017. That is, it should be

certified that the data is correct as on 3111212017.7. The date of end of survey should be countersigned by the Supervisor.
