in recovery seeking to be reunited with children who have been placed in foster care.

Fees: Clients are responsible for a program fee. This amount is 1/3 of their total

monthly income. Additionally, utility bills in excess of apartment allotment will be charge to the client.

Services: Providing transitional housing, life-skills counseling, nutrition, parenting, abuse,

education and education support, transportation, AA/NA meetings and recovery support and other necessary referrals.

of treatment. The staff should be able to direct the necessary steps to take to obtain treatment.

Services: The Department of Health has designated Single County Authorities across the

state to be responsible for administration, planning and funding of publicly funded drug and alcohol abuse prevention and treatment services in their local area. Services include screening to determine the presence or absence of alcohol or other drug use in an effort to ascertain the need for emergent, urgent or routine services, Level of Care assessment, validation, authorization and placement determination, level of case management determination, referral, continued stay review and client liability determination.

discipleship to men interested in addressing life issues such as bondage to alcohol, drugs, gambling, pornography, and unemployment. Every participant is provided a safe place to live, food to eat, opportunities to learn good work habits, personal and group bible studies, discipleship, educational opportunities, and access to a compassionate and caring staff.

as well as their significant relationships. Fees: None Services: Co-Dependents United is an open, interactive self help group offering Stage 1 and

Stage 2 recovery as described by Charles Whitfield, Pia Melody, John Bradshaw, and others. The Stage 1 program focuses primarily on current problems as they relate to our co-dependency issues. The Stage 2 program is directed to the underlying feelings which are at the root of the co-dependency unresolved painful feelings from childhood. Co-dependents United is an interactive group, (not a twelve step group). Co-Dependents United is planning to open another group in Sewickley sometime in late 2010. Call the above number for more information. Many participants are from Beaver County and ride sharing may be a possibility.

other drug problems. Accepts all insurances and is able to access county funding for those with no insurance. DASBV has a sliding scale fee for those ineligible for other funding.

Services: Out-patient counseling for individuals, families, groups and adolescents at various

locations throughout the surrounding Beaver County area.

and adolescent behavioral problems. Fees: Pyramid participates in most commercial insurance plans and works with clients who are

un-insured to receive funding for treatment that is deemed appropriate. Services: Behavioral healthcare services for men and women including a full continuum of adult

and adolescent drug and alcohol rehabilitation programs, methadone maintenance programs, therapeutic group homes and alternative schools.

such as, but not limited to, drug and alcohol abuse. Applicants must test drug and alcohol free to enter the program and remain drug and alcohol free for the entire program.

Fees: none Services: Christian resident Drug and alcohol recovery program. The program includes resident

care, work therapy, personal counseling, group discussion, activities, instructional classes and personal interviews. All clients attend Bible Studies, Daily Devotions, Worship Services, Narcotics Anonymous, Substance Abuse and Alcoholics Anonymous meetings. The normal program length is 120 days although it can last as long as necessary for recovery.

test drug and alcohol free to enter the program and remain drug and alcohol free for the entire 4 phase program.

Services: Intensive outpatient, outpatient, residential services, HIV/early intervention.

child age 12 years or younger, or be actively seeking custody of a child, or be pregnant. Pregnant women who are IV drug users or substance abusers will receive priority admission. Sojourner House admits women from Allegheny County, the surrounding region and the state.

Services: Sojourner House is a faith-based residential rehabilitation facility where addicted

women learn to break the inter-generational cycle of poverty and chemical abuse while in their own apartment with their children.

Sojourner House responds to admissions inquiries from women and their loved ones, other drug and alcohol programs, case workers, social agencies, physicians, prison and probation officers, counselors, and Children, Youth and Family courts. Sojourner house has 14 apartments. If full, there is a waiting list. To provide continuity between the times a woman is determined to be eligible and the time of admittance, the Clinical Supervisor will require the applicant to attend some type of outpatient treatment and/or a required number of NA or AA meetings per week. Women on the waiting list must also call Sojourner House a specified number of times per week, during which they can receive additional support. Priority of admission is determined by a number of factors, including order on waiting list, clinical issues (e.g. whether or not a woman is following through with treatment recommendations, severity of emotional problems), and environmental issues (e.g. lack of housing, domestic violence). Women on the waiting list are also offered referral to other treatment programs and options and information about interim services. The length of stay varies for each woman, but the program is designed for a five to six month stay. Woman cannot work or go to school while they are at Sojourner House.

alcohol addiction located in a rural setting about 30 minutes northeast of Pittsburgh, PA.

Eligibility: Admission to Turning Point is available to male and female adults (18 years and

older) who are chemically dependent. Fees: For information regarding programs, admissions and fees, or any questions, call

the admissions department. Many insurance companies provide coverage for most or all of treatment at Turning Point. For the uninsured, competitive self-pay rates are offered in an effort to make treatment affordable. Prospective candidates should also contact their local county drug and alcohol agency for placement.

Services: Turning Point is a 65 bed inpatient facility, and our services and continuum of

care include medically monitored detoxification (using Suboxone® for opiate addicts, when appropriate), short term inpatient treatment, long term inpatient treatment, a partial hospitalization program, intensive outpatient program, and individual counseling on an outpatient basis.

The Way Out Workshop and Common Solution Recovery

:

The Way Out is a one day workshop that seeks to answer three questions: 1. What is the exact nature of addiction? 2. What is the solution? 3. How do you find a way out?

The Workshop identifies both the problem and the solution of drug and alcohol addiction in order for one to work through the hard part of an addicts recovery. The Way-out Workshop was originally developed by alcoholics and addicts in the Church Army Branson, Missouri base, in order to carry the message of the Common Solution Recovery to those still trapped in addiction. Based upon the Alcoholics Anonymous "Big Book" the workshop explains steps 1 through 4. All participants receive a "Big Book" along with all other necessary materials. The workshop and corresponding Common Solution Recovery Classes (CSR), as presented through Church Army, have been successful in helping hundreds of suffering alcoholics find sobriety and a way out. CSR is a series of classes presented in the weeks after a workshop which prepares an individual for their 5th Step and then Steps 6 through 12. The CSR classes are meant to present a model of unity, fellowship and common cause necessary for an individual to begin life long sobriety. Follow-up classes with our Church Army leaders are offered to help you continue through the hard work of recovery. The Way-Out workshop has been conducted with the Bridge to Recovery and on a regular basis at Beaver County Jail.

The Al-Anon Family Groups, which includes Alateen, are a fellowship of relatives and friends of alcoholics who share their experience, strength, and hope in order to solve their common problems. We believe alcoholism is a family illness and that changed attitudes can aid recovery. Alateen is a fellowship of young Al-Anon members, usually teenagers, whose lives have been affected by someone else's drinking. "NA is a nonprofit fellowship or society of men and women for whom drugs had become a major problem. We meet regularly to help each other stay clean. ... We are not interested in what or how much you used ... but only in what you want to do about your problem and how we can help." Membership is open to all drug addicts, regardless of the particular drug or combination of drugs used. Narcotics Anonymous provides a recovery process and support network inextricably linked together. One of the keys to NA’s success is the therapeutic value of addicts working with other addicts. Based on their employment interests and goals, participants are assigned to qualified nonprofit organizations or government agencies on a part-time basis where they are trained and receive work experience by performing their community service training assignment. While participants receive training, supervision and experience directly from their host agencies, their training wages and fringe benefits are paid by Experience Works. The training assignment is designed to help a person upgrade their skills, learn new skills, and ultimately find employment. The training assignment will be approximately 20 hours per week. As the training progresses, Experience Works staff will help the participant turn their newly acquired skills and experience into a full time or part time job off the program. The participant may also receive additional education, training and work experience through supplemental programs.

Angel Food Ministries is a non-profit, organization dedicated to providing grocery relief and support throughout the U.S. Angel Food is available in a quantity that fits into a medium-sized box at $30 per unit. The menu changes every month and consists of both fresh and frozen items with an average retail value of approximately $60. Generally, one unit of food assists in feeding a family of four for about one week or a single senior citizen for almost a month. The food is high quality not second hand. There are no applications to complete or qualifications to meet. Angel Food participates in the Food Stamp program, using the Off-Line Food Stamp Voucher system. Angel Food partners with churches and community organizations throughout the country to help distribute food to those in need.

The Center for Hope program provides an outreach and service to a community that has undergone economic hardship and demographic change in what has been designated a distressed community. In Beaver County, LSS coordinates through the Salvation Army for distributions to 48 different communities. TEFAP now distributes a wide variety of nonperishable food products, including canned and frozen meats; fruit juices; canned and frozen vegetables; infant formula; instant potatoes; instant baking mix; rice; noodles; pasta; peanut butter and egg mix, among others. HACB provides housing without regard to race, color or national origin and does not discriminate on the basis of physical or mental handicap or disability. and meet income guidelines). There are maximum net income limits (after deductions) established by HUD for HACB housing. Call the office or see website for detailed information.

Fees: Generally, the monthly rent for HACB housing is 30 percent of the family's net

income (gross income minus deductions) or a minimum rent of $50 per month, whichever is greater. A flat monthly rent based on apartment size and location may also be offered. A $99 security deposit and the first's month rent are due when you sign your lease. The security deposit, less any charges for rent or damages, is refunded when you move. In some HACB neighborhoods, residents pay their utilities. For these residents, we provide a monthly utility allowance in the form of a rent credit that is subtracted from your rent every month. However, rent credits for utility payments are not available to residents who select the flat rent option.

The Stone Harbour program is a 12 bed facility that provides homeless individuals with co-occurring disabilities such as mental illness and/or substance abuse and/or legal issues a safe, therapeutic, regimented, transitional environment that promotes independence and recovery, that guarantees dignity of self and others, teaches the value of hard work and discipline, and assists persons in developing useful skills through a broad needs based “phase” program (if applicable). Stone Harbour prepares the individual for positive reintegration into the community. Criteria for Eligibility:

Veterans Supportive Housing (VASH) Program Butler VA Medical Center

What is HUD/VASH? Housing and Urban Development (HUD)’ VA Supportive Housing (VASH) is a joint program of the U.S. Department of Housing and Urban Development (HUD), Section 8 “Housing Choice” rental assistance vouchers and the Butler VA Medical Center. Locally, this program is administered through Butler Public Housing Authority (PHA) and VA Medical Center’s HUD/VASH Program. Who May Be Eligible? 1) Veterans who are homeless, as defined by the McKinney Act are: An individual (or family group) who lacks a fixed regular and adequate night time residence and who has a primary night time residence that is supervised publicly or privately operated shelter designed to provide temporary living accommodations including welfare hotels, congregate shelters, and transitional housing for the mentally ill; an institution that provides a temporary residence for individuals intended to institutionalized; or a public or private place not designed for, or ordinarily used as, a regular sleeping accommodation for human beings. 2) Homeless veterans who are eligible for a full range of VA services. 3) Homeless veterans who are in need of case management for substance abuse recovery, medical and/or mental health and are willing to accept this as part of their treatment. 4) Homeless veterans who are willing to participate in outpatient programming. Highest Priority for this program is for chronically homeless disabled veterans and families (defined as consistently homeless for one year or 4 episodes of homelessness in the past three years). Note: The term “homeless” or “homeless individual” does not include any individual imprisoned or otherwise pursuant to an Act of Congress or state law. Among the homeless population not identified as chronically homeless, the following groups should be considered and offered preference, if possible, under existing guidelines:

The following information is from the Pennsylvania Guidebook for Incarcerated Veterans: Determining VA Eligibility: VA health care: 1-877-222-8387 - VA benefits: 1-800-827-1000 Eligibility for most VA benefits is based upon discharge from active military service under honorable conditions. Active service means full-time service as a member of the Army, Navy, Air Force, Marine Corps, and Coast Guard. Honorable and General discharges qualify a veteran for most VA benefits. Dishonorable and bad conduct discharges issued by general courts-martial may bar VA benefits. Veterans in prison and parolees may be eligible for certain VA benefits. VA regional offices can clarify the eligibility of prisoners, parolees and individuals with multiple discharges issued under differing conditions. VA benefits will not be provided to any veteran or dependent wanted for an outstanding felony warrant. Those seeking a VA benefit for the first time must submit a copy of their service discharge form (DD-214, DD-215), which documents service dates and type of discharge, or give their full name, military service number, branch and dates of service. To apply for services veterans must complete a VA Form 10-10EZ, Application for Health Benefits. The form may be obtained from any VA health care facility or regional benefits office, or by calling the VA Health Benefits Service Center toll free at 1-877-222-VETS (8387) or online at (http://www.va.gov/1010ez.htm). Once enrolled, a veteran is eligible to receive services at VA facilities anywhere in the country. Eligibility for VA Benefits during Incarceration: Veterans incarcerated and incarcerated dependents may apply for the same compensation, dependency and indemnity compensation (DIC) – service connected death benefits – and pension benefits as veterans who are not incarcerated. However, Congress has greatly restricted the amount of benefits that may be paid to a veteran or dependent while he or she is incarcerated. These benefits are institutionalized as part of law: 38 U.S.C Sec 5313 (a), 38 C.F.R., Sec.3.665 (a), (d), which reads as follows:

If a veteran is incarcerated as the result of a “felony” conviction as defined by law: “Any offense punishable by death or imprisonment for a term exceeding one year, unless specifically categorized as a misdemeanor under the law of the prosecuting jurisdiction,” Then, the amount paid to a veteran incarcerated for a service-connected disability is generally limited by law to the 10 percent disability rate, or half the amount of the ten percent rate if the veteran’s disability rating is 10 percent. (If the veteran is rated before incarceration as 20 percent disabled or higher, he will receive only the amount payable to a 10 percent disabled veteran.) Incarcerated DIC recipients will receive one-half the amount paid to a veteran receiving compensation payments for a 10 percent-rated disability. A veteran may not receive non-service connected VA pension benefits, or any portion of these benefits, while incarcerated for a felony. However, his family may receive an apportionment of such benefits under the procedure described above. (See 38 C.F.R. Sec.3.666)

One important requirement for eligibility for VA benefits is that the veteran has to have been issued either an honorable or general discharge, or would have received one if not for reenlisting. If a veteran had two periods of service, one honorable and the other less than honorable, he may still be eligible for VA benefits based on the honorable period of service. VA Medical Care is not provided to veterans in prison, but VA health facilities may provide care to you after your release.

Benefits Payments While Incarcerated: There is a 60-day "grace period" following a conviction when you may still receive full benefits. To avoid an overpayment, it is important that you notify the VARO immediately when you go to prison if you are receiving payments. If you do not notify the VA and receive overpayment, you and your family will lose all financial payments until the debt is paid.

For example, Joe is a veteran who receives a VA pension of $807 per month. He commits a crime and is incarcerated, but doesn't tell the VA right away and keeps getting paid for 6 months. Joe is overpaid a total of $4,872. After serving his sentence of 18 months, he is released and applies to the VA to have his pension restarted. The new pension rate is $830 per month, but the VA will use that amount to start repaying the $4,872 debt. Joe's first real check will start 5 months after he is released and will be for $30! Joe has to go at least 5 months without that income.

Your award for compensation or pension benefits should resume from the date you are released, as long as the VA receives notice of release within one year. This includes placement in a community treatment center or halfway house. Remember, you must notify the VA when you are released to restart your payments. Apportionment: Although legally, the veteran can only receive a portion of the full amount payable for his or her disability rating, the remaining balance may be “apportioned to the individual’s dependent family”. To apply for apportionment, send a letter to the VA Regional Office (VARO) that has jurisdiction over the veteran’s case. VA regulations clearly specify this apportionment amount will only go to family members if they can show financial need for such amount. This applies to the spouse, children, or dependent parents who are involved in the application. In deciding appropriate apportionment the following factors are considered: The family member’s income and living expenses; The amount of compensation available to be apportioned; The needs and living expenses of other family members; and Special needs of any of the family members.

For example: a veteran incarcerated rated as 80 percent disability can only receive the amount he or she would get if he or she were 10 percent disabled. However, his or her family may be apportioned up to 70 percent, the difference of the 80 percent rating. (DIC may also be apportioned with similar restrictions.) There is a 60-day “grace period” following conviction where the veteran, or DIC recipient, may still receive full benefits. If the veteran continues to receive benefits after the 60-day period, it will result in an “over-payment”. The VA considers it to be the recipient’s responsibility and fault if this occurs because the recipient failed to notify the VA of his or her incarceration. Attempts to obtain a waiver in these situations of overpayment are often unsuccessful. As a rule, the veteran loses most, if not all, financial benefits until the entire overpayment is recovered by the VA. It has also been a standard procedure that the family will not be entitled to receive an apportionment until the debt is completely recovered.

jail verifying his/her identity and official information at least a week BEFORE they are released. The request must be submitted in writing, using the Inmate Request Form, to the Director of Treatment.

3. Upon release, the inmate should gather any additional identity records, proof of address

and court, probation and/or parole documents he or she may have and take them with their jail identification to the Pennsylvania Division of Vital Records office.

How to Obtain a Pennsylvania Photo ID Card PennDOT Photo & Exam Center

An identification card can be issued to any Pennsylvania resident who is at least 16 years of age. Step 1: Visit the East Rochester Driver License Center and take along:

1. Proof of identification (see attached form) 2. Social Security Card 3. Check or money order in the amount of $12 4. 2 acceptable forms of address verification if applicant is 18 years or older.

Step 2: Complete Form DL-54A "Application for Pennsylvania Photo Identification Card". Step 3: Upon handing in the proper identification requirements, a camera card is issued to you. Step 4: The camera card is taken to the photo technician co-located within the Driver License Center for

the photo to be taken. Step 5: A Photo Identification Card is issued to you. What If You Do Not Have All of the Required Proof of Identification?

1. The inmate being released may use his or her Beaver County Jail identity wristband. This is an official county government issued photo identification that may be accepted by the Department of Transportation as proof of identity.

2. The inmate must ask to keep their wristband and request a corresponding letter from the jail verifying his/her identity and official information at least a week BEFORE they are released. The request must be submitted in writing, using the Inmate Request Form, to the Director of Treatment.

3. Upon release, the inmate should gather any additional identity records, proof of address and court, probation and/or parole documents he or she may have and take them with their jail identification to the Pennsylvania Division of Vital Records office.

4. If you were issued a Pennsylvania Driver’s License or Photo ID in the past, ask if they can locate your old records and verify your identity from the records and/or photos on file.

How to Obtain a Replacement Social Security Card Social Security

An Application for a Social Security Card, Form SS-5 must be submitted to the Social Security office to get or renew a social security card. Please be sure to look at the form instructions (see attached form SS-5). There is no charge when applying for or renewing a card. To renew a card (lost card or new name), you must present recently issued documents that prove your identity, and if born outside the U.S., proof of U.S. citizenship or lawful alien status. Take original or certified copies of your supporting documents along with the application. Acceptable proof of age includes a birth certificate, hospital records, or passport if born

outside of the U.S. Acceptable proof of identity includes a driver's licenses, passports, marriage or divorce

record, school identification card, employer identification card, military identification card, health insurance card (not a Medicare card), life insurance policy, or adoption record.

Acceptable proof of U.S. citizenship includes U.S. consular report of birth, passport and

certificate of citizenship or naturalization. What If You Do Not Have All of the Required Proof of Identification?

Identification and Residency RequirementsFor U.S. Citizens

As part of a nationwide effort to enhance the issuance of secure identification credentials,Pennsylvania requires the following documentation from U.S. Citizens applying for a PennsylvaniaDriver’s License, Learner’s Permit or Identification (ID) Card or U.S. Citizens transferring an out-of-state Driver’s License or ID Card.

LIMITS ON REPLACEMENT SOCIAL SECURITY NUMBER (SSN) CARDS

16. If you are age 18 or older, you must sign the application. If you are under age 18, you or a parentor legal guardian may sign. If you are physically or mentally incapable of signing the application,generally a parent, close relative, or legal guardian may sign the application. If you cannot signyour name, you should sign with an "X" mark and have two people sign as witnesses in the spacebeside the mark. Please do not alter your signature by including any additional information on thesignature line as this may invalidate your application. Call us if you need clarification about whocan sign. (See the "IMPORTANT" note under evidence of IDENTITY on page 3.)

Most items on the form are self-explanatory. Those that need explanation are discussed below. Thenumbers match the numbered items on the form. If you are completing this form for someone else,please complete the items as they apply to that person.2. Show the address where you can receive your card 10 to 14 days from now.3. If you check "Legal Alien Not Allowed to Work," you must provide a document from a U.S.

Federal, State, or local government agency that explains why you need a Social Security numberand that you meet all of the requirements for the U.S. government benefit. NOTE: Not all U.S.State or local benefits are acceptable for non-work SSN purposes. Contact SSA to see if yourreason qualifies.If you check "Other," you must provide a document from the U.S. government agency thatexplains why you need a Social Security number and that you meet all of the requirements for aFederal benefit except for the number.

5. Providing race/ethnic information is voluntary. However, providing this information helps usprepare statistical reports on how Social Security programs affect people. We do not revealthe identities of individuals in these reports.

6. Show the month, day and full (4 digit) year of birth, for example, "1998" for year of birth.

8.B. You must show the mother's Social Security number only when the application is for an originalSocial Security card for a person under age 18. However, this item may be left blank if the motherwas never assigned a Social Security number, or if you do not know the mother's Social Securitynumber and are unable to obtain it. We will still be able to assign a number to the person underage 18.

9.B. You must show the father's Social Security number only when the application is for an originalSocial Security card for a person under age 18. However, this item may be left blank if the fatherwas never assigned a Social Security number, or if you do not know the father's Social Securitynumber and are unable to obtain it. We will still be able to assign a number to the person underage 18.

13. If the date of birth you show in item 6 is different from the date of birth you used on a priorapplication for a Social Security card, show the date of birth you used on the prior application andsubmit evidence of age to support the date of birth in item 6.

Public Law 108-458 imposes limits on the number of replacement SSN cards you may receive at 3 peryear and 10 in a lifetime. In determining these limits, SSA will not count changes in legal name (i.e., firstname or surname), or changes to a restrictive legend (i.e., Valid for Work with DHS Authorization, NotValid for Employment) shown on the SSN card. In addition, we may grant exceptions on a case-by-casebasis if you provide evidence to establish a need for an SSN card beyond these limits (e.g., a letter froma social services agency stating you must show the SSN card in order to get benefits).

CHANGE OF INFORMATION: If you need to correct information on your SSN card, or information shownin our records (e.g., a name change, or corrected date of birth), you will need to prove your identity andprovide documents that support the change and establish the reason for the change (e.g., a birthcertificate to show your corrected date or place of birth). A name change document (e.g., marriagedocument) must identify you by both your old and new names. If it does not have enough identifyinginformation (See IDENTITY, Page 3) , we will request an identity document in your prior name and anotherin your new legal name in addition to the name change document. If you were born outside of the U.S.,you also need to prove your U.S. citizenship or current lawful, work-authorized immigration status.

EVIDENCE DOCUMENTS WE NEED TO SEE

AGE: In general, we must see your birth certificate. In some situations, we may accept another document that shows your age. Some of the other documents we may accept are:• U.S. Hospital record of your birth (created at the time of your birth) • Religious record established before age five showing your age or date of birth • Passport • Final Adoption Decree (the adoption decree must indicate that the birth data was taken from the original birth certificate)Call us for advice if you cannot obtain one of these documents.

U.S. CITIZENSHIP: In general, we can accept your U.S. birth certificate or U.S. Passport. Otherdocuments we may accept are a Consular Report of Birth, Certificate of Citizenship, or Certificateof Naturalization.

IMMIGRATION STATUS: We need to see a current document issued to you by the Department ofHomeland Security (DHS) showing your immigration status, such as Form I-551, I-94, I-688B, orI-766. We CANNOT accept a receipt showing you applied for the document. If you are notauthorized to work in the U.S., we can issue you a Social Security card only if you need the numberfor a valid nonwork reason. (See HOW TO COMPLETE THIS APPLICATION, Page 2, Item 3.)Your card will be marked to show you cannot work. If you do work, we will notify DHS.

IDENTITY: We must see evidence of identity in your legal name. Your legal name will be shownon the SSN card. Generally, we prefer to see documents issued in the U.S. Documents submitted toestablish identity must show your legal name AND provide biographical information (your date ofbirth, age, or parents' names) and/or physical information (photograph, or physical description--height, eye and hair color, etc.). Additionally, if you send a photo identity document but do not appearin person, the document must show your biographical information (e.g., your date of birth, age, orparents' names). To protect your Social Security card and number, identity documents must be ofrecent issuance.

• U.S. driver's license; or• U.S. State-issued non-driver identity card; or• U.S. passport

WE CANNOT ACCEPT A BIRTH CERTIFICATE, HOSPITAL SOUVENIR BIRTH CERTIFICATE,SOCIAL SECURITY CARD OR CARD STUB, OR A SOCIAL SECURITY RECORD as evidenceof identity.

IMPORTANT: If you are applying for a card on behalf of someone else, you must provideevidence that establishes your authority to sign the application on behalf of the person towhom the card will be issued (e.g., a minor child's birth certificate establishes the authority of aparent to sign on behalf of the child). In addition, we must see different documents as proof ofidentity for both you and the person to whom the card will be issued.

WE MUST SEE YOUR:

If you do not have one of these documents, or cannot get a replacement within 10 days, we mayaccept other documents such as a U.S. military identity card, Certificate of Naturalization, oremployee identity card. For young children, we may accept medical records (clinic, doctor, orhospital) maintained by the medical provider. We may also accept a final adoption decree, or aschool identity card or a school record maintained by the school.

If you are not a U.S. citizen, we must see your current U.S. immigration document and your foreignpassport with biographical information or photograph.

THE PAPERWORK/PRIVACY ACT AND YOUR APPLICATION

The Privacy Act of 1974 requires us to give each person the following notice when applying for aSocial Security number.

Sections 205(c) and 702 of the Social Security Act allow us to collect the facts we ask for on thisform.

We use the facts you provide on this form to assign you a Social Security number and to issue you aSocial Security card. You do not have to give us these facts, however, without them we cannotissue you a Social Security number or a card. Without a number, you may not be able to get a joband could lose Social Security benefits in the future.

The Social Security number is also used by the Internal Revenue Service for tax administrationpurposes as an identifier in processing tax returns of persons who have income which is reported tothe Internal Revenue Service and by persons who are claimed as dependents on someone'sFederal income tax return.

We may disclose information as necessary to administer Social Security programs, including toappropriate law enforcement agencies to investigate alleged violations of Social Security law; toother government agencies for administering entitlement, health, and welfare programs such asMedicaid, Medicare, veterans' benefits, military pension, and civil service annuities, black lung,housing, student loans, railroad retirement benefits, and food stamps; to the Internal RevenueService for Federal tax administration; and to employers and former employers to properly preparewage reports. We may also disclose information as required by Federal law, for example, to theDepartment of Homeland Security, to identify and locate aliens in the U.S.; to the Selective ServiceSystem for draft registration; and to the Department of Health and Human Services for child supportenforcement purposes. We may verify Social Security numbers for State motor vehicle agenciesthat use the number in issuing drivers' licenses, as authorized by the Social Security Act. Finally,we may disclose information to your Congressional representative if they request information toanswer questions you ask him or her.

We may use the information you give us when we match records by computer. Matching programscompare our records with those of other Federal, State, or local government agencies to determinewhether a person qualifies for benefits paid by the Federal government. The law allows us to do thiseven if you do not agree to it.

Explanations about these and other reasons why information you provide us may be used or givenout are available in Social Security offices. If you want to learn more about this, contact any SocialSecurity office.
