INTRODUCTION

Almost every patient who is admitted to a hospital receives drug therapy. Drug therapy accounts for a substantial percentage of cost to an institution while drugs are lifesaving. They also have the potential to cause morbidity and mortality. The Pharmacyand Therapeutic (P&T) Committee stands as the decision-making body concerning drug use for most institutions.

The Kalamansig Municipal Hospital Drug Formulary is published by the Pharmacy and Therapeutic Committee, in cooperation with the Department of Pharmacy. Members of the Pharmacy and Therapeutic Committee include:

I- MEDICATION USE POLICIES AND OTHER IMPORTANT INFORMATION

A. Objectives and statement of purpose of the formulary

General Objectives:

Our institution, Kalamansig Municipal Hospital, will have anadequate medicines and medical supply for the constituent of Kalamansig.

Specific Objectives:

1. To establish a functional Therapeutic Committee for Kalamansig Municipal Hospital

2. To ensure adequate stock level of commonly used medicinesat all times

3. To ensure adequate stock level of intravenous fluids at all times

4. To ensure adequate stock level of emergency medicines at all times.

5. To ensure adequate stock level of medicine supply at all times.

The hospital formulary represents the official listing of drug approved for use at Kalamansig Municipal Hospital. The purpose of the hospital formulary is to ensure the availability

of safe, effective and cost-efficient medications. The hospital formulary is established by the medical staff a described in medical staff policy and procedure. A formulary is subjected to change at any time.

While the purpose of the formulary is to help ensure the availability of medications, at times there may be external factors that impact inventory levels of certain medication. Problems at the level of the pharmaceutical manufacturer or FDA after impact our ability to procure medications. Shortage of important medication are communicated to the medical staff whenever this occur.

B. Policies Formulated by Therapeutic Committee

1. The Therapeutic Committee (T&C) established should meet every 2nd Monday of every other Month.

2. The stock level of common oral medicine is at least five ___prior to purchase.

3. The stock level of common intravenous medicine is at least 50% ampule prior to purchase.

4. The stock level of emergency medicine is at least 10% ampuleprior to purchase except:

Strontium Sulfate (SrSO4) Sodium Bicarbonate ( Na HCO 3)

5. The different department heads may request things needed fortheir department 1st week to 3rd week of each month.

6. The pharmacist will report the stock level of pharmacy stocks during the monthly meeting.

7. The pharmacist will report expirable medicines/medical supplies within 3months prior to expiration date during the monthly meeting.

8. The pharmacist prepares request for purchase of medicines and medical supplies.

9. The pharmacist submits a monthly report of all consumed medicines and medical supplies to the RHU of Sultan Kudarat.

CHAIRMAN:

1. The chairman is responsible to call a regular meeting and emergency meeting as need arises.

2. The chairman will approve resolutions after deliberation of all the members.

3. The chairman shall check proper implementation of policies formulated.

VICE CHAIRMAN:

In the absence of the chairman, the vice chairman willtake over.

MEMBERS:

1. All members shall attend regular meeting as agreed.2. The member will follow the policies implemented.

3. The member shall suggest medicines and medical supplies to be purchase.

Narrative Report:

On the 3rd day of admission, patient complained of itchiness of face and neck;

redness followed. The attending physician was then notified.

Above medicine was discontinued and Diphenhydramine syrup was given p.o

stat as ordered.
