Plan Highlights

Eligibility All employees must meet the eligibility requirements outlined in the Adopting Employer Elections Agreement. Generally, any employee of an Adopting Employer who has met the age and service requirements may participate in the Plan. Any self-employed Minister shall also be eligible to participate. The Plan does not allow participation by an independent contractor.

Enrollment Period Upon your satisfaction of the eligibility requirements, you may participate in the Plan immediately.

Contributions You are eligible to make either a pre-tax 403(b) salary deferral or an after-tax Roth contribution once you meet the eligibility requirements outlined in the Adopting Employer Elections Agreement. You may increase or decrease the amount of your contributions at anytime. You may stop your contributions at anytime. Any changes made will be effective as quickly as can be administratively accommodated by the payroll.

How Much May I Contribute

You may elect to save from 1% to 100% of your compensation to this Plan (stated in whole percentages only or as a specific dollar amount). An Internal Revenue Service (IRS) dollar limit applies. The current dollar limit is $16,500 for 2010 (this amount is indexed and periodically may increase) each calendar year. You may also qualify for "catch-up" contributions" which may allow you to significantly enhance your retirement savings. There are two different types of catch-up contributions that may apply—the 15-year catch-up and the age-50 catch-up. You may take advantage of both types of catch-up contributions, but the age-50 catch-up can only be utilized after you first maximize the 15-year catch up available to you for a year in which you qualify. If you qualify for the 15-year rule, your Salary deferral under this limit can be as high as $19,500 for 2010, but various factors, such as your years of service with the Employer and prior catch-up contributions, may affect the maximum amount available. If you have 15 years of service with your Adopting Employer and wish to make contributions in excess of $16,500, please contact your Plan Administrator for assistance in determining your maximum amount. The age-50 catch-up contribution is available if you will be age 50 by December 31 of a plan year and are already contributing the maximum allowable deferral to your plan. You are entitled to contribute an additional catch-up contribution. The maximum catch-up contribution is $5,500 for the calendar year 2010. Please see your Plan Administrator for more details.

Employer Contributions

The Plan provides for a discretionary matching contribution and a discretionary profit sharing contribution in an amount to be determined each year by your employer.

Vesting Vesting refers to your 'ownership' of a benefit from the Plan. You are always 100% vested in all accounts.

Withdrawals Funds may be withdrawn from your account for one of the following events:

• Qualified Retirement • Death • Disability • Termination of Employment • Attained age 59½ or older (restricted to one withdrawal per calendar year) • Financial Hardship (limited to pre-tax salary deferral accounts and no earnings) • In-Service Withdrawal at anytime from your Rollover Account (limited to

one withdrawal each calendar year)

Loans Yes, you can apply for a loan by completing an application on the Participant Website. An application fee (currently $130) is charged for each loan application. You are permitted to only have one loan at a time. The minimum amount of a loan is $1,000. The total amount of the loan is limited by rules under the tax laws. In general, a loan is limited to the lesser of one-half of your vested account balance or $50,000. All loans are generally required to be repaid within five years.

How can a participant apply for a loan

You can select Loan Modeling/Request from the options under Plan Services • Loan Modeling Form allows you to model and/or request a loan • Loan Summary located under Account Summary allows you to review your

loan summary such as interest rate, loan balance, next payment date, etc.

Once you have modeled a loan and want to request a loan, you can select the Loan Application button or request a loan application through the Call Center (800) 442-4015. You will be able to complete the Loan Application on line and an automated email will be generated to your Employer notifying that you have requested a loan. You will be able to view your loan packet on line prior to automated email being generated to Employer. Employer will review the loan application and verify your employment status and ability to afford the payment and will approve or decline the loan. Both you and the Employer will be able to track the status of the loan request through the Website or the call center. If loan is approved, you will be notified and a distribution will be requested. If the loan is declined, the Employer has the option to provide an amount that they will approve. You will be notified that your loan request has been declined but that you can re-apply for a loan of the different amount. If you wish to cancel the amount of a loan request, you will be required to cancel your current request and then re-apply for the new amount.

Rollovers/Transfers You are allowed to rollover money into your account at anytime even if you have not met the eligibility requirements to participate in the Plan. This Plan also accepts direct transfers from other 403(b) plans. See your Plan Administrator for rollover details.

Different investors have different risk tolerances. Much of the difference stems from time horizon. That is, someone with a short investment time horizon is less able to recover from losses. The remainder of the difference is attributable to the individual’s appetite for risk. Volatility can be nerve-wracking for many people, and they are more comfortable when they can avoid it. However, there is a definite relationship between risk and return. Investors need to recognize this risk/return trade-off.

Use your score from the previous page to identify the corresponding profile that may be best suited for your retirement investments.

Investor profile types

Note: These risk profiles are samples only. The proper asset allocation for your situation may differ. In applying any one of the profiles to your individual situation, you also should consider your other assets, income, and investments (including home equity, savings accounts, and other retirement plans), as well as your needs, goals, and aversion to risk. If any of these factors change, you should review your investment allocation.

While diversification through an asset allocation strategy is a useful technique that can help to manage overall portfolio risk and volatility, there is no certainty or assurance that a diversified portfolio will enhance overall return or outperform one that is not diversified. An investment made according to asset allocation models neither guarantees a profit nor eliminates the possibility of loss.

MINISTRY EMPLOYEES RETIREMENT PLAN SPOUSE'S AGREEMENT TO WAIVE THE RIGHT TO BE THE

BENEFICIARY OF A PARTICIPANT

1. What Rights Do You Have to Benefits after Your Spouse Dies?

Your spouse has an account in the above plan. The money in the account that your spouse will be entitled to receive is called the "vested account". Federal law states that you will receive the vested account after your spouse dies. Example: Pat Doe dies at age 45 and Pat's vested account in the plan was $10,000 at the time of Pat's death. The plan will pay the $10,000 to Pat's spouse, Robin Doe (adjusted for gains and losses after Pat's death).

2. Can Your Spouse Choose other Beneficiaries to Receive the Account?

Yes. However, under federal law, your right to your spouse's vested account cannot be taken away unless you agree. If you agree, your spouse can elect to have all or part of the vested account paid to someone else. Each person your spouse chooses to receive a part of the vested account is called a "beneficiary." For example, if you agree, your spouse can have the vested account paid to his or her children instead of you. Example: Pat and Robin Doe agree that 1/2 of Pat's vested account will be paid to Robin and 1/2 of the vested account will be paid to Pat's child, Chris. If Pat's vested account at the time of his death is $10,000, the plan will pay $5,000 to Robin and $5,000 to Chris (each amount adjusted for gains and losses after Pat's death). Your spouse cannot have the vested account paid to someone else unless you agree and sign the spousal consent.

3. Do You Have to Give Up Your Right to Your Spouse's Vested Account?

No. Your choice must be voluntary. It is your personal decision whether you want to give up your right to your spouse's vested account.

4. Can Your Spouse Change the Beneficiary in the Future If You Sign this Agreement?

If you sign this agreement, your spouse cannot change the beneficiary named in his or her Beneficiary Designation Form to anyone other than you, unless you agree to the new beneficiary by signing a new agreement. If you agree, your spouse can change the beneficiary at any time before your spouse dies.

5. Can You Change Your Mind after You Sign this Agreement?

You cannot change this agreement after you sign it. Your decision is final. However, you and your spouse can decide later to change the beneficiaries.

6. What Happens to this Agreement If You Become Separated or Divorced?

Legal separation or divorce may end your right to the vested account even if you do not sign this agreement. However, if you become legally separated or divorced, you might be able to get a special court order (which is called a "qualified domestic relations order" or "QDRO") that specifically protects your rights to the vested account. If you are thinking about separating or getting a divorce, you should get legal advice on your rights to benefits from the plan. You must acknowledge reading this agreement and consent to your spouse's designation of a non-spouse beneficiary on the Spousal Consent Form. To be valid, your signature must be acknowledged before a Notary Public or Plan Representative. McCready and Keene Notice Regarding Spousal Death Benefit in a Defined Contribution Plan (7/09) DC5A
