events that impact us now and often need more immediate attention/action
Risks that materialise have the potential to become issues if

is closely linked.

Project Risk/Issue Log:
raises awareness of a problem
formally raises the risk/issue within the project
provides a central point of reference for all project risks and
issues
provides a log of agreed actions and their outcome
provides a tool to manage the progression, escalation and
resolution of a problem/potential problem/opportunity
Escalating Risks & Issues
Projects may need to escalate risks/issues up to programme or
executive level
Recording and escalating risks/issues is the responsibility of
each project
The Project Management Office (PMO) or designated project
support can oversee and assure the management of
risks/issues in line with local Risk and Issue Management
