  2. 2. General Information This site produces plastic containers of various shapes and sizes for a wide range of consumer products. The base materials are recyclable be them pellet, scrap or finish product. All materials must be kept free of contaminants that might be generated by your activities. It is your responsibility to maintain a clean, orderly and safe work site. Contractor Orientation Program Products: *NO containers found or brought on site are to be used for any purpose other than their original intent. The storage of materials in non-identified containers is prohibited.

  4. 4. Contractor Orientation Program Site Access: Sign-In Site Security Entry to this site requires checking in with your site contact. Signature IN upon arrival & OUT upon exiting. Displaying of visitors / contractors badge. Access limited to assigned work area only. All exterior doors are secured entry. Upon exiting, re-entry into the facility will require contacting of site contact and or identification card swipe. No exterior doors are allowed to be propped open. If you have an access card to the facility, this is yours alone. Allowing your badge to be used by another is a violation of policy. *NO contractors are allowed site entry without first under going site orientation.
  5. 5. Contractors Information: Contractor Orientation Program Provide The Following Information, Company Name Your Name Site Contact Name Provide Verification of the Following, Company Certificate of Insurance (C of I). Any specialized licensing or job task specific work permits.

Contractors Guide: General Site Rules No cameras or taking of photographs No chemicals without prior approval Good Manufacturing Practices No loose items are allowed to be worn above the waist. This includes jewelry, pens, pencils, phones and badges. Wearing of hairnets, including beard nets No food or drink outside of break room Tobacco use is restricted to designated areas only. Antibacterial hand washing is required before entry to manufacturing areas. Personnel with wounds and or known carriers of disease are prohibited from product contact. Loss of wound coverage and or bandage must be reported. PPE: Amcor Provided Hearing protection, ear plugs. Hair & beard nets. PPE: Contractor Responsibility Eye protection, safety glasses with side shielding Head protection, hard hat or bump cap Hand protection, gloves are to be worn where appropriate for the hazard Foot protection, protective toe foot wear High visibility clothing, vest / shirt. Contractors Guide (cont): House Rules Compliance with all site rules Compliance with your employer rules High hazard work will require the issuance of a permit and possible additional PPE Non-compliance with rules and requirements will result in site removal Site Locations Exits Parking Designated areas: Evacuation, Take shelter Contractor Orientation Program

Chemical Control: Material Identification Chemicals brought on site must be, Declared Reviewed and approved for use Accompanied by an MSDS, if chemical is not already in our database Stored and transported in identified containers Material Use and Disposal Contractors are responsible for, Safe use Providing any specialized PPE Containment and approved disposal Safe storage Removal from site upon departure Contractor Orientation Program * All Federal, State and Local, (Hazcom, OSHA, Environmental, etc.) regulations are to be followed and you are responsible for any special licensing.

Specialized Work Requiring Permits: Hot Works Cutting, welding, grinding Elevated Heights (including roof) Aerial lifts (boom, scissor, articulated), cranes, scaffolding, roof surface / edge Energized Systems Electrical *Excavation / Demolition Exterior / interior building and or property modifications *Confined Space Limited access, hazardous atmospheres, entrapment, engulfment Contractor Orientation Program *Normal site operations do not include these hazards. Only approved / trained contracted personnel are allowed to work under these conditions.

Specialized Work: Hot Works Hot Works Review task for alternative to performing hot works outside of designated work shop area Obtain approval and permit prior to starting task Follow all safety standards Specialized PPE: Eye, face, hand, clothing Ensure operational fire extinguisher is readily accessible Establish a safe perimeter clear of flammable materials Establish a safe perimeter including flash curtain / screen placement Secure area and systems when not present Complete fire watch safety sweep of area following task completion Contractor Orientation Program

Specialized Work: Elevated Heights: 10> Elevated Heights (including roof) Review task for alternative to working at height Obtain approval and permit prior to starting task Follow all safety standards Specialized PPE: Harness, Lanyard Use secured anchoring points Establish a safe operating perimeter Secure area and systems when not present Roof access requires a minimum of 2 persons with established means of communication and contact schedules. Working within 10 feet of unprotected roof openings and or roofs edge requires fall protection devices. Contractor Orientation Program

Specialized Work: Energized Electrical Energized Systems Review task for alternative to working on energized equipment Obtain approval and permit prior to starting task Follow all safety standards Specialized PPE: arc flash rated Use rated insulated tooling Establish a safe perimeter Secure area and systems when not present Follow LOTO guidelines Contractor Orientation Program *Working on any energized electrical systems is not desired and all efforts must be made to de-energize systems prior to servicing. Only approved / trained contracted personnel are allowed to work under these conditions.

Specialized Work: *Excavation / Demolition Excavation / Demolition Review task Obtain approval and permit prior to starting task Follow all safety standards Specialized PPE: Eye, face, hand, clothing Establish a safe perimeter Secure area and systems when not present Contractors are responsible for materials containment and disposal Trenching at a depth of 4 >, requires stabilization and securing of excavation site Contractor Orientation Program *Normal site operations do not include these hazards. Only approved / trained contracted personnel are allowed to work under these conditions.

Specialized Work: *Confined Space Confined Space Review task for alternative to confined space entry Obtain approval and permit prior to starting task Follow all safety standards Specialized PPE: Harness, Lanyard Specialized Equipment: Atmospheric testing meter, air circulation device, personnel retrieval equipment Establish a safe perimeter Secure area and systems when not present Requires a minimum of 2 persons with established means of communication and contact schedules Contractor Orientation Program Confined Space Type #2 Permit Required Confined Space No Permit Required Confined Space Type #1 Permit Required Site Labeling *Normal site operations do not include these hazards. Only approved / trained contracted personnel are allowed to work under type #2 conditions.

Specialized Equipment: Forklift Powered material handler Aerial lift Powered elevated work platform Powered Industrial Tools Specialized Cranes Moving & loading equipment Contractor Orientation Program All specialized equipment and tools brought on site must be approved for use and meet safe operating conditions as defined by federal, state, local, mfr. safety codes. Use of Amcor specialized equipment and tools must be prearranged by your site contact prior to use.
  16. 16. Specialized Equipment: Forklift Safety Guidelines Powered lift trucks, used in all areas of the operations, are to be operated within defined site safety parameters. These include reduced speeds of < 5mph. (Example: No faster than a brisk walking pace). Only propane and / or electric powered vehicles are allowed to be operated inside of any facility. Amcor forklifts are restricted to Amcor employee use, unless otherwise prearranged. Pedestrians and drivers share responsibility to establish eye contact and interact safely together. Pedestrians are to use pedestrian doors when traveling between areas, not rollup doors Wearing of a high visibility vest is required when in areas of lift truck operations If job task requires working in areas of frequent travel establish a safety perimeter for both motorized vehicles and pedestrians. Specialized Equipment: Aerial Lift Safety Guidelines Powered aerial lifts used in all areas of the operations Only propane and / or electric powered vehicles are allowed, review of which required upon delivery to site Use restricted to trained / certified Amcor employees unless otherwise prearranged through your site contact Pedestrians responsibility to establish eye contact with driver alerting them to your site presence Wearing of a fall safety harness and lanyard attached to an anchorage is required Standing on the railings is strictly prohibited Platform floor is to remain clear of trip hazards Use restricted to flat level surfaces Establishment of a highly visible safety perimeter is required at ground level for pedestrian safety Contractor Orientation Program

Specialized Equipment: Manufacturing Machinery Safety Guidelines Keep body and tools clear of operational machinery Some system are fully automated and may start without an area presence LOTO requirements must be followed to prevent unexpected machine operations Establish a safety perimeter around your work area that prevents or limits machinery access Establish a safety perimeter around your work area that prevents product contamination Contractor Orientation Program

Specialized Equipment: Semi Trucks & Trailers Safety Guidelines Contractors are required to check in with warehouse / material handling department personnel prior to performing any work in the loading docks (exterior and interior) High visibility vests are required to be worn at all times while in this area of the operations. Placement of a barrier, ie. your vehicle, lift truck, cones tape must be in place to alert and prevent trailer movement in the area of the loading dock you are working in. Short term work / inspection / observation in a loading bay, without any of the afore mentioned barriers, requires a second person spotter to stand alert and guard from trailer movement. Notification of area cleared, work completed, must be relayed to department upon vacating the area. Loading docks are high traffic areas and are restricted. No loitering is permitted. Specialized Equipment: Exterior Operations Safety Guidelines Deliveries / Pickups Landscaping Lawn Care / Snow Removal Pest Control Work being performed around the exterior of the facility requires, Notification of a site contact upon arrival and departure. Remaining clear of site operations, including loading docks and rail spurs, unless authorized to do so and provided the necessary safety training. Contractor Orientation Program

Incident Reporting: Injuries Medical Treatment Injuries occurring on site MUST be reported immediately to your site contact and your employer Appropriate medical treatment will be determined and then provided for Trained Amcor ERT members are on site to provide assistance Off site emergency services can be contacted and treatment arranged A site 1st Aid station and emergency medical response bags containing basic medical supplies are available AED and CPR equipment is available BBP kits including universal PPE and cleanup supplies are available Contractor Orientation Program Pill medications are available, only in single use dosage and are not allowed in the manufacturing areas.

Incident Reporting: Property Damage Equipment Damage Damage to Amcor machinery, site structures and product MUST be reported immediately to your site contact Appropriate actions to secure the operations and provide for safety will be determined Property Damage Damage to Amcor and or employee property MUST be reported immediately to your site contact Appropriate actions to secure the property, provide for safety and notify effected employees will be taken Contractor Orientation Program Always practice safe driving techniques while on Amcor property, watching for pedestrians / moving vehicles, and operate equipment at controlled speeds.

Housekeeping: Maintaining Work Area Established Perimeter Identify work area by using cones, fencing and or caution tape where possible Orderly Work Area Maintain work area to provide for safe travel and to protect product from contamination Waste Disposal Materials brought on site are the responsibility of the contractor Disposal must be in accordance with site requirements Recyclables are to be placed in identified containers Hazardous waste is to be disposed of per regulations Contractor Orientation Program Energy conservation and scrap reduction are priorities of Amcor. Your assistance in protecting the environment is appreciated.
