What is implant?

It's screw fixed in the bone made from titanium. Implant has three components :

1 -Implant itself replacing the root part of the tooth.

2-Abutment is the supporting the coronal restoration.

:3 -Restoration which could bea)Fixed prosethises crown or bridge ,b)Removable denture

Why we do implant ? 1-Missing single tooth between

two healthy teeth. 2-Multipal missing teeth for

young patients witch doesn’t wants removable denture.

3 -Patient has free end sadle and they wants fixed prosthetics.

4 -Nice appearance cosmetic.

Contra Indications -Absolute Contraindications:

1-Osteoporosis .2-Nephropathy.

3-Ischemic cardiopathy .4-Severe pathoiogies of breathing.

History of implant ?

In 1956 branamark has discovered that the titanium Is combatable and Osseointegrated with human being bone.

Mechanism of implant: The osteointegration based on the

:surface treatment of the implant 1-Surface 2- geometry.

1-Treatment of implant surface, in the past they treat the implant surface by Hydroxyaptite ( Calcium hydroxide & calcium phosphate ) .

They developed and they start treat the surface to get fastest osseointegration by Acid etch & sand blast (Macro cavity ),Later on they found the implant surface which is treated only by Bio-organic Acid etch it get more osseointegration (Micro cavity ) & good distribution for the osteoblast cells and good healing.

2 -It's important of geometry is to gave us

the primary stability.

Implant form: They design the implant as cylindrical with external Hex for abutment connection and 1mm polish nick ,they discovered the connection between the abutment and the implant with external hex is broken ,They developed the implant design to cylindrical implant with inertial hex 1mm polished nick ,After that they found the forces coming to the implant due to mastication has not good distribution to the bone and could have bone resorption around the implant. so they developed the design of the implant to cylindrical implant internal Hex with 1mm polish nick and tapered apix 2 mm, They developed the design of the implant to get the shape root of the natural tooth and introduce the tapered implant. The indication of tapered implant, 1-in case of space less than 6mm between the natural roots. 2- In case of Immediate implantation

after root extraction .

Implant surgery :

One step surgery technique they open the flap and insert the implant in the bone and closing the flap around the implant waiting for the osseointegration then we did the prosthetic phase .

Two Steps surgery we open the flap and insert the implant , put the cover screw close the flap over the implant ,waiting for the osseointegration ,then we exposure the implant from the gingiva then we did the prosthetic phase.

Surgical stent ( Guide)

Surgical technique1-Open the flap start drilling with

deferent sizes of drill bit. 2-By using the connectors to hold the

implant from the vial to the implant socket.

3-Tiete the implant by ratchet to the bone level, remove the connector.

4-Un screw the mount ,we put the cover screw after that we close the flap.

5-After the osseointegration we Exposure the implant using the tissue

punches. 6-We remove the cover screw ,inserting

the healing screw (gingiva former ,gingival healing )for 10-15 days.

7-Remove the healing screw ,inserting the Impartation transfer in the implant.

8-We take Impartation by using special tray with inside of it the Impartation transfer.

9-Insert the healing screw in the implant10-We insert Laboratory analogue with

implant transfer.11-Send it to the lab ,when the Laboratory

made the study model, Inside it the implant analogue, we select the right abatement .
