Health Care Providers

In-network services are covered at 80/20 after the deductible has been met

Out-of-network services are covered at 60/40 after the deductible has been met

Office Visits

$50 per visit will be paid for office visits for new or existing patients for which patient history and medical decision making are fairly straightforward

50% per visit will be paid for office visits requiring a more complex medical history and medical decision making

$500 out-of-pocket limit for office visits per person, not a part of the medical deductible or OOP

Prescription CoveragePrescription Co-pays:

The +20% coinsurance is applied to the remaining cost of the Rx

Once your out-of-pocket cost for prescriptions reaches $1,500/calendar year, the 20% coinsurance stops

Co-pays continue until your total out-of-pocket for prescriptions reaches $2,000/calendar year. Prescriptions are then paid at 100%

Completion of Health Risk AssessmentParticipants that join after July 1 will not be

charged an additional $25 in 2015 if they don’t complete the steps

Routine vision exam-plan pays up to $50 for employee, spouse, and dependent children in 2014

One baseline mammogram for female employee/spouse ages 35-39

Routine mammogram paid at 100% for women 40 and above

Colonoscopy for employee/spouse age 50 and above: Plan pays first $1,200 with balance at 80% after deductible has been met, etc. (once very 10 years)

Dental InsuranceDeductible: $30/year with maximum or $90/family/yearAnnual benefit limit: $1,000/personCoverage for exams, x-rays, cleanings, fillings,

surgery, root canals, gum disease treatments and denture repairs.Percentage paid by plan for above services:

70% 1st year (September – December 2014), coverage percentage increases by 10% each year until by your 4th year in the plan you have 100% coverage

Flexible Spending AccountMoney placed in this account is not subject to

Life Insurance

The college provides basic life insurance in the amount of twice the annual basic salary at no cost to the employee

Accidental death and dismemberment (AD&D) coverage is also offered at twice the basic salary at no cost to the employee

Optional supplemental life insurance is available for new employees without a health screening

Disability InsuranceLong term disability insurance (LTD) is

provided for faculty teaching at least 4 classes Salary continuation is provided for the first 6

months of leave equal to ½ annual salaryLong term disability benefits pay 60% of pay

Luther’s Retirement Programwith TIAA-CREF

After completing 1-year of eligible employment, the college remits 10% of an employee’s base salary to TIAA-CREF

The 1-year waiting period may be waived if you met your 1-year of eligibility with another employer

All faculty, regardless of course load, are eligible to participate in a tax-deferred annuity (TDA) or a Roth 403(b) with TIAA-CREF

Confidential Reporting Hotline

Occasionally, an employee has a concern that he or she would like to express anonymously. These concerns might include ethical concerns, theft, sexual harassment, etc. For these occasions, the college participates in a service called the In Touch Hotline.

This system is available for your use around the clock, seven days a week. Because the hotline is operated by an independent organization, any calls made through this hotline are completely confidential.
