What we will cover…

How Much Time Can I Take?

Provision: Allows for 12 weeks of job-protected time off for the following reasons:

• Birth and care of the eligible employee's child, or placement for adoption or foster care of a child with the employee;

• Care of an immediate family member (spouse, child, parent) who has a serious health condition; or

• Care of the employee's own serious health condition.

Eligibility: employed by salesforce for at least 12 months and have worked at least 1,250 hours in the past 12 months.

***If you have not met the eligibility for FMLA, you are still eligible to take leave; however, your leave will not be subject to the provisions of FMLA.

How Will I Receive Pay?

Short Term Disability (STD)

• Company Paid Benefit

• Eligibility: must satisfy 7 consecutive calendar days of waiting period. The waiting period is unpaid unless PTO is used.

• Covers 80% of weekly earnings* up to $5,000 per week for 180 days maximum

Earnings* = On-Target Earnings

Paid Benefits

SFDC Paid Parental Leave

• 4 weeks of pay at 80% of your weekly on-target earnings up to a maximum of $5,000 per week

• For adoption and paternity leaves, there is a 7 calendar day waiting period

• This benefit is coordinated with other income replacement benefits, such as New Jersey Family Leave Insurance (FLI) or Rhode Island Temporary Caregiver Insurance (TCI).

• For maternity leaves following pregnancy leave period, the waiting period is waived.

Benefits & Resources

• While you are on leave, your health benefits will be maintained during the protected leave period given that you continue to make benefit contributions.

• During any unpaid leave time, the employee will not accrue PTO, make 401(k) contributions, and is ineligible for company 401(k) match and paid holidays. If you have a 401(k) loan, please email 401K_Stock_Email2Case@salesforce.com for instructions on repayment.

• Stock Options and RSUs will continue to vest during the protected leave period.

• ESPP – the money you have contributed toward the ESPP will be used to purchase stock during the purchasing period.

• During leave, you will not be able to submit toward fitness or education reimbursement programs.

• Educational information on prenatal care, labor and delivery, newborn and baby care, post-partum depression, breastfeeding and any pregnancy related issues

Benefits & Resources

Meals on salesforce:

• Salesforce provides a $250 allowance toward take-out meals when you are home caring for your newborn or newly adopted child. You do not need to apply for this benefit. A separate check will be direct deposited to your bank account at the end of the month in which you began leave.

Adoption Assistance Reimbursement

• Reimbursement up to $5,000 per child for eligible adoption expenses related to adopting

• Eligible child(ren) must be under 18 years of age (or over 18 years of age if they are physically or mentally incapable of caring for themselves)

Bright Horizons Back-Up Care Advantage

• Includes both back-up child care and adult/elder care.

• 7 back up care days available per calendar year

• Co-pay of $15 per child or $25 per family for the center-based care option; in-home care is $6 per hour, with a four-hour minimum.

Applying For Leave

Step 1: MUST BE COMPLETED PRIOR TO STARTING LEAVE

• Call the Larkin Company at (650)938-0933 or (866)923-3336 to file your leave claim

• If you are planning to take any PTO during the eligibility waiting period or any unpaid time throughout the leave, please provide that time to the Larkin Company.

STEP 2: ADDING THE BABY OR CHILD IN BENEFITS

• Enroll baby in benefit by logging in to Workday within 30 days of event. You must use your salesforce.com laptop and VPN to access Workday.

• Use Aloha to log into Workday. Please reference this guide. The event date is the date the adoption is finalized.

 

STEP 3: RETURNING FROM LEAVE

• Please let the Larkin Company know once you are ready to return to work.
