FIRST TIME USER REGISTRATION Regardless of whether you were screened at the Expo last year, you must create a new account

to schedule your 2020 appointment. Click Create Account

CREATE USERNAME AND PASSWORD

ACCEPT TERMS AND CONDITIONS After reading, click I Agree

WELCOME TO HEALTHALIGN After reading, click Ok

PERSONAL INFORMATION UPDATE Fill in your Date of Birth in MM/DD/YYYY format and your Sex

SAVE YOUR PERSONAL INFORMATION Click Save Changes

SELECT A SERVICE Click Select a Service

SELECT ON-SITE SCREENING Click the “+” next to Biometric Screening to open the drop down

Select the On-Site Event Option

Then click Select

SELECT ON-SITE EVENT Click on the event you wish to attend

SELECT APPOINTMENT TIME Click on the time that you wish to attend the event

Appointments will take approximately 15 minutes be available from 9:00 a.m. – 1:45 p.m.

After reading Protection From Disclosure of Health Information click I Agree

After reading Screening Consent Agreement click I Agree

SELECT APPOINTMENT TIME Once you confirm your appointment details, click Schedule

CONFIRM APPOINTMENT A confirmation email will be sent to your preferred email

Your healthAlign Home Page will reflect your appointment

You can receive notifications via email,

text message or both

Click on your username on the upper

right hand side of the screen to update

your notification preference

RESCHEDULING APPOINTMENT If needed you can reschedule your appointment on the Home Page

Click Reschedule Appointment

AFTER SCREENING You will receive your results at the Total WellBeing Expo

To access your results online, you will receive an email, following your screening, letting you know your results are available to view

Your username and password are what you established when creating your account

Click Biometric Results

VIEW SCREENING RESULTS The results from your Biometric Screening will be here for you to view
