How Can We Help?
Step 1: A member of the relocation team will meet with you for a personal interview.
Step 2: You and the team will determine your needs, strengths, community supports
and issues that enable or prevent you from moving to an independent setting.
Step 3: If you decide you want to move - you and the team will develop a plan.
How Can We Help Cont.
Step 4: You can choose not to continue in the process at any time.
Step 5: Relocation resources will be available to you to help with
moving expenses.
Step 6: Inform you of what we can’t do for you.
