• Invasion, riot, civil commotion or rebellion, any war (declared or undeclared) or act of
war, violation or attempted violation of the law or resistance to arrest;
• Suicide or any attempted suicide or self-inflicted injury or illness, while sane or insane;
over indulgence in alcohol, drug-taking unless taken under the direction of a
Registered Medical Practitioner;
• Pregnancy or childbirth, or any illness or injury associated with pregnancy or
childbirth;
• Venereal disease, AIDS, AIDS related complexes and all illnesses or diseases

What is LOG?
What is the purpose of Letter of Guarantee (LOG) ?
• LOG acts as a waiver of deposit upon admission into hospitals and payments will
be billed to Insurer directly.
• LOG can only be used for in-hospitalization cases
• LOG covers only patient’s room & board entitlement and other hospital services
up to a maximum of S$10,000 and does not include Professional Doctor’s Fees.
• It is important to understand that issuance of a LOG does not guarantee that the
claim is payable.
• Admissibility of the claim will be subject to Insurer’s assessment upon receipt of
claim form after discharge. In the event that Insurer rejects the claim, employee is
liable for any outstanding liability owing to the hospital or the Insurer.

Q1. If I am overseas, am I covered?
A This policy is 24 hour worldwide. You are also covered outside office hour whether on holiday or at
work. If you are hospitalized, you should pay the bill and seek reimbursement when you are back in
Singapore. Please ensure that you obtain the medical report from the attending doctor and submit
together with the original final bills and medical claim form. All medical report and bills are
recommended to be translated in English to expedite in claims processing.
It is advisable that you purchase your own travel insurance if you are going for holiday overseas.
Q2. Can I continue the medical coverage if I resign from my company?
A This group insurance benefits is bought by your company as the main policyholder and coverage will
cease upon leaving the Company. It is, therefore, advisable that you purchase your own individual
for yourself and your dependents to ensure adequate protection.
Q3. Is benefits pro-rated for dependents?
A All benefits are on per insured basis and it is not shared.
Q4. If I go for medical treatment at A&E in Singapore government or private hospitals, will I be
reimbursed?

Q5. If I have another personal medical policy or my spouse’s company policy also covers me – can I
claim from these policies?
A Yes, if your claim is not paid in full by VMware’s medical plan, you can submit the balance amount from
GHS and Major Medical not reimbursed to the other insurers for review. Please obtain a copy of the
“certified true copies” of bills and payment statement which the other insurers would require to review the
claims. Insurers will only reimburse you for the amount not paid by the 1st insurer.
Q6. Can I cover my parents?
A We define dependants as your legal spouse and children between 15 days old (must be discharged well
from the hospital) and 25 years old if the child(ren) is/are still in full-time education and unmarried.
Parents are not considered as dependents in Singapore Group Insurance.
Q7. Is pre-existing condition covered?
A Pre-existing condition will be covered if you are with Vmware for a period of 12 months from the effective
date of coverage. Not applicable to cancer treatments and kidney dialysis.
Q8. Is Hepatitis B Screening, Health Screening and Vaccinations covered?
A No, these are not covered as they are considered as preventive in nature.
Q9. How long does it take to receive my claims?
A You will receive your claims within 4-6 weeks subject to submission of full claims documents. The claims
will only be assessed by the insurer if the documents required are complete.

Q10. When I utilize my Medisave/ Medishield Integrated Plan to pay for my treatment, how will my
claim be reimbursed?
A Payment that was made through Medisave and Medishield Integrated Plan will be refunded to the
respective Medisave Account and Medishield Integrated Plan to make good the lifetime limit.
Q11. How do you reimburse claim amount payable?
A The insurer will pay according to the priorities listed below:
1. To the hospital if there is any outstanding amount billed to the insurer (If LOG is utilised).
2. To the claimant via GIRO for any cash payment made on eligible medical bills.
3. To the claimant’s Medisave account.
4. To the claimant’s Medishield plan.
