School-to-Work Transition

Agency Goals Related to Transition: Improve School-to-Work Transition

Services and outcomes Improve collaboration between all

agencies involved in transition activities Develop IPE for all eligible students prior

to exiting high school

Transition Student:

A secondary school student with a disability pursuant to IDEA and/or the Rehabilitation Act, who is an applicant or eligible individual for Vocational Rehabilitation services and becomes a signed applicant for services before exiting a secondary school program.

VR Counselor Assignments

Develop and maintain effective working relationship with assigned high school

Provide consultative services Ensure timeliness in processing referrals,

determining eligibility and working with student to develop IPE

DVR TIMELINES:

Referral – September through October Eligibility – By December Comprehensive Assessment/Career

Counseling/Guidance and Counseling - January through March/April

IPE Development – March through May

Eligibility:

Individual with a physical or mental impairment; which results in a substantial impediment to

employment; who can benefit in terms of an employment

outcome from vocational rehabilitation services; and

VR services are required in order for the individual to prepare for, enter, engage in, retain or regain employment

Guidance and Counseling/Career Counseling On the Job Training Support in Post-Secondary Education Apprenticeships Assistive Technology Job Seeking Skills (soft skills) Job Placement and support Job Coaching
