Generalidades • LMA está caracterizada por la acumulación de células
anormales que fallan en diferenciar en granulocitos o monocitos funcionales
• Estadística: 1/10,000 hab., predominio en adultos
• Clínica de presentación muy variada
• Históricamente el diagnóstico era morfológico y descriptivo
• Clasificación FAB usada en los últimos 30 años no correlacionaba con el pronóstico de SLE y SG
• Posteriormente la clasificación de LMA se fundamenta en los hallazgos citogenéticos y moleculares
• Prima el concepto de ESTRATIFICACION por RIESGO

Analisis de la mutación en LMA
Mutación genética en pacientes
con LMA-CN
• 90% de pacientes jóvenes con LMA-CN tienen al
menos una de las cuatro mutaciones: NPM1, FLT3-
ITD, CEBPA y DNMT3A
• En general, al menos 1 mutación ha sido encontrada
en más del 99% de los casos
• Hay al menos dos mutaciones en el 72% de los
pacientes con citogenética normal
• La presencia de mutación en WT1, TET2, DNMT3 en
riesgo favorable e intermedio le da peor pronóstico
• La sobreexpresión de BAALC, ERG y micro RNA en

Resumen
• Nuevos marcadores genéticos pronósticos en LMA permitirán
nuevas estrategias terapéuticas que conducirán al incremento
de los pacientes curados
• Los marcadores moleculares que son factores pronósticos son
rutinariamente utilizados para la selección de terapias y
estratificación en varios subtipos de LMA
• La mayoría de estudios clínicos están incluyendo marcadores
genéticos
• La efectividad de terapias alternativas para casos de LMA con
factores pronóstico adverso, tienen que ser probadas. Sin
embargo, hay evidencia que la mejor opción es el uso de TPH +
terapia molecular dirigida.
