The people who manage and deliver health
services in the Black Country are making
services better for people with learning
disabilities and who have behaviour that
challenges
This is part of a national plan called
Transforming Care. This is happening
everywhere in the country
It will -
• Improve the quality of care
• Improve quality of life for people with
learning disabilities
• Provide more care in the community
• Reduce the hospital stays in assessment
and treatment units
We have developed a community model
for the Black Country which has community
teams and specialists who are trained to
meet the needs of service users
There will still be some assessment and
treatment beds for people who need them.
They will all be at Penrose House, Heath
Lane Sandwell
What is it ?
We have looked at the assessment and
treatment units we have in the Black
Country and believe the best one is Penrose
House in Sandwell
People can get services when they need
them and at anytime. It has separate male
and female wards
We think that Penrose House is safer than
Ridge Hill in Dudley and Orchard Hills and
Daisy Bank. Both are temporarily closed
because they were unsafe
We have tested the community model and
believe it is making a difference to people’s
lives
We want to know what you think about the
services and what is important to you
The Transforming Care Programme only
includes the low and medium secure units
The Transforming Care Programme is about
people getting the help and support they
need in their community and close to their
homes
This will be provided by community teams
who are specially trained
People will only go into hospital if the care
they need cannot be given safely in the
community
The hospital care will be high quality
specialist care and patients will be there for
the shortest time possible

The Black Country Transforming Care
Partnership has worked with people with
learning disabilities and their families and
carers to develop the new community
model
The new community model will maintain
people’s rights, respect and dignity
There will still be beds in the Black Country
for people who need to go to the
assessment and treatment unit
We will need less hospital beds in the Black
Country
The unit that we will have all of the hospital
beds in will be Penrose House in Sandwell
The new community model for people with
learning disabilities
The closing of Ridge Hill Hospital in Dudley
and Orchard Hills/Daisy Bank in Walsall

The Black Country has seventeen thousand
adults who have a learning disability and
are over 18 years old
We already have a community model to
work with people with learning disabilities in
the Black Country
The community team work together with
other services to provide people to stay near
their families and friends
The community model has an intensive
support service to avoid staying at the
hospital
The community service has a forensic model
for people who are involved in the criminal
justice system
What we have now ?
There will be a short term inpatient hospital
for people with learning disabilities who
cannot have treatment in the community. It
will have ten beds
In the Black Country there is one assessment
and treatment unit at Penrose House, Heath
Lane in Sandwell. Both women and men
can be treated there
There are also community teams from the
Black Country Partnership Foundation trust
who support people with learning disabilities
There are two other hospitals called Ridge
Hill in Dudley and Orchard Hills/Daisy Bank in
Walsall. They are not used and all patients
are now treated in the community
In 2016 the assessment and treatment beds
at Pond Lane Hospital Wolverhampton was
closed
The Black Country TCP has been working
with people who have learning disabilities,
their families and carers to get the
community model right

Support for families and carers to stay in their
home
We are making sure services are available
for people with learning disabilities at the
right time, in the right place and in the right
way

We believe that services for people with
learning disabilities should be delivered in
the community

How the Community Model works
There are four community learning teams.
One in each area. Walsall, Dudley, Sandwell
and Wolverhampton
The teams will provide a service for adults
over eighteen years old who have a
diagnosed learning disability and cannot
use mainstream services
The service will support families and carers
by making the assessment process easier
It will also support staff and professionals
who deliver the service by providing
specialist training
Community Learning Disability Service
Be as independent as possible
Stop hospital admissions when they are not
needed.
Be discharged in the shortest time possible from
hospital care.
Be supported to look after their mental health
and physical care needs.
To live as independently as possible in the
community.
Be involved in decision making about their care.
To get the help and support as quickly as
possible when they are in crisis.
The service will support people to
This is a 24 hour inpatient service for people
with learning disabilities and complex health
needs
There will be three main functions of support
An assessment of the needs and treatment
for emotional and behavioural distress
A safe place where people can get the
support they need
Support when people leave the assessment
and treatment unit to go back into the
community
They will be supported by the community
disability team and the intensive support
teams
Assessment and treatment beds
Be supported to look after their
mental health and physical care.
The Community Intensive Support Team is a
service for people who cannot use
mainstream services with a diagnosed
learning disability who are over eighteen
years old
The service will stop unnecessary hospital
admissions
Support people back into the community
Support people to live as independently as
possible
Intensive Support Service
The Intensive Support service will
Support people to understand their triggers
and get support earlier
Respond when in crisis
Work with other teams to plan support when
managing challenging behaviours
Plan to prevent future crisis and work with
community teams to do this
Assess family carers’ needs to support them
when crisis happens
To get the help and support as quickly as
possible when they are in crisis
Forensic Support Service
The Community Forensic team is a service
for people who are over 18 years old and
have a diagnosed learning disability
They will be a part of the criminal justice
system or at risk of becoming a part of it
They will be unable to access mainstream
services and need a forensic team
Plan to prevent future crisis and work with
community teams to do this
The Forensic Support service will
Provides support to people with learning disabilities
who are a part of the criminal justice system or at
risk of it
Will promote the qualities and values of the Good
Lives Model of offender rehabilitation
Will promote independence in a way that is possible
Prevents and avoids unnecessary hospital stays
Make sure that when you are in hospital you can
get out as soon as your treatment is completed
Supports families and carers in getting the support
they need when dealing with the criminal justice
system
The best place for assessment and treatment beds
There will be times when some people with a
learning disability will need to go to an
assessment and treatment hospital
We know that people want to stay near their
community and friends and family. That is why
it is important to keep an assessment and
treatment hospital open in the Black Country
area
We want to make this one hospital the best it
can be and provide a high level of care for the
people who need it
We have looked at the three hospitals we have
in the Black Country and have decided that
the best place is Penrose House in Sandwell
The best place for assessment and treatment beds
This is because Ridge Hill and Daisy
Bank/Orchard Hills are
• not in community areas
• will not work in an emergency for people
who need emergency care
• they both need a lot of work to make them
the best place we can offer
• Penrose House has support from other units
on the site if they need it
This means that it has been decided that
Penrose House is the safest site for the
Transforming Care Partnership assessment and
treatment beds

right one
We want to make sure that we know all of your
thoughts and concerns before we ask the
clinical commissioning groups to agree to close
Ridge Hill and Daisy Bank/Orchard Hills for
good
This will help us to make sure that we can
support the new model of care in the best way

engagement
This will include GPs, local councillors, specialist
schools and community groups
