I. Executive Summary
The Mines and Geosciences Bureau Regional Office No. 12 is located
at the Prime Regional Center, Carpenter Hill, City of Koronadal under the
leadership of Engr. Efren B. Carido as OIC-Office of the Regional Director
pursuant to Special Order No. 2017-525 issued by the DENR Secretary on
July 10, 2017. OIC-RD Efren B. Carido was manned by four divisions
namely :

1. Mineral Reservation identified this year was gold and silver
prospect located in the municipalities of Kalamansig and Lebak,
Province of Sultan Kudarat with an area of 24,140.14
2. Communication Plan for Mineral Development - All targets were accomplished except on the demand driven and request by the LGUs and linkages with the other agencies, coordination and joint activities this office participated. Aside of intervening trainings, workshops, and as lectures and resource speaker.

Geohazard ssessment and Mapping
c) Vulnerability and Risk Assessment (VRA) – MGB frontline activity in cooperation with the LGUs and other concerned agencies. It’s assessed and quantified the vulnerability of cities and municipalities based on their exposure to natural hazards. It defines the degree by which a sector can be affected considering its existing and potential impacts or losses, and capacities to adapt to or mitigate these hazards. For the year CY 2017 a 100% accomplished .With a target of 12 municipalities within Region 12 . Three (3) municipalities in Sultan Kudarat ; Four municipalities in North Cotabato;Three (3) municipalities in South Cotabato and Two (2) municipalities in Sarangani Province as shown below:

conducted on January 23-February 15,2017.It covered the 27 Barangays. The assessed areas were rated as having low, moderate, higher very high (critical) susceptibility to landslide.
Detailed Sub-surface Assessment-was conducted on May 5 to May 31, 2017 in the municipality of Maasim, Saranggani. The conduct of sinkhole inventor, and ascertain the presence and location of these sinkholes and caves using geomorphological information and the Ground Penetrating Radar. (GPR). A total of 808.75 survey line meters were made and conducted to 17 sites including elementar , ,secondary school and areas near government establishments. Eleven recommendations were made based on the conclusions generalized from the findings and observations.
Quadranggle Mapping- conducted on April 20 to May 29,2017 of Mt. Isarog Quadrangle Quadrangle Number 3660-1,Naga City Quad number 3660-1V,Tinambac –QUAD No.3660-II,Libmanan Quad No. 36650-III and Tambang Point Quad No. 3661-1. A total of 55 samples were collected and total of 97 observation points mapped and surveyed.Only five (5) samples per quadrangle will be sent to MGB-CO Petrolab for assaying ,petrology and paleontological studies.
Coastal Geohazard and Impact of Climate Change-
• Region XIII-3 ( Region 13-Surigao City,Malimono,San
Francisco,Surigao del Norte conducted on August 30 –Sept. 16,2017 submitted on December 6,2017.)
• Region XI-11 ( Malita and Sta.Maria on October 2-8,2017) (Hagonoy, Padada,Sulop and Malalag Davao del Sur- October 9-14,2017) (Don Marcelino and Jose Abad Santos -October 16-23,2017),and (Davao City-and Samal City on Nov. 6-14,2017 ).
A total 126 barangays were distributed on November 13 -14,2017 with Coastal Geohazard Threat Advisories on the following :
Province Davao Del Sur = 18 barangays
Davao Occidental = 51 barangays
Island Garden City of Samal = 31 barangays

Groundwater Resource Assessment –Focus on assessing and evaluating the country’s groundwater resources , and come out with a 1:250,000 scale groundwater availability map ,including the determination of the availability and quality of groundwater in the various rock units /geologic formations ,established the quality /chemistry of groundwater basins ,establish the potentials and limitations of available groundwater in the various groundwater basins. This year two provinces were targeted the Sarangani Province and Sultan Kudarat Province.

II. Assessment of Accomplishments
Geosciences Development Services
The CY 2017 was great challenge for the Geosciences Development Services
Almost all targets were 100% accomplished the personnel had to double time
and exerts efforts to accomplish their targets. For only this year some of our
personnel experiences the cross region on the quadrangle mapping were
conducted to Region 5 and Coastal Assessment were conducted in Region 13
and Region 11 aside of accomplishing the target of our region. Limited time
aside of intervening trainings and workshops to be attended by the technical
personnel. More demand driven activities on geological site scoping and
invitation as a resource person in seminars and trainings.
Quadrangle Geological Mapping and Surveys
This year quadrangle mapping was conducted in Region V. Five (5)
quadrangles were targeted for the first semester of 2017, and these were all in
municipalities/city within the province of Camarines Sur in Region 5. The
following quadrangles were updated and field assessed, namely; Isarog
Quadrangle targeted on April 20-May 9; Naga City Quadrangle on May 10-29;
Tambang Quadrangle on May 30-June 10; Tinambac Quadrangle on June 11-
20 and Libmanan Quadrangle on June 21-28. The activity involves geological
assessment, mapping, rock sampling and inventory of geologic structures were
observed within the quadrangles

Activities conducted were the following:
1. Information and Education Campaign (IEC) were scheduled on respective SP/SB sessions: October 2, 2017-SP of Sultan Kudarat; October 3, 2017-SB of Lebak, Sultan Kudarat. October 4, 2017-Barangay Council of Salaman; October 24, 2017-SB of Kalamansig, Sultan Kudarat and on November 9, 2017-Barangay Council of Obial, Kalamansig, Sultan Kudarat.
2. Environmental Resource Mapping, Semi-Detailed Geological Mapping/Sampling and assay analysis of collected samples.
Vulnerability and Risk Assessment (VRA)
For the year 2017, there are twelve (12) municipalities that were targeted for the Vulnerability and Risk Assessment (VRA). In South Cotabato, Tampakan, Tupi and Polomolok were targeted; in Sultan Kudarat, Lebak, Bagumbayan and Senator Ninoy; in Sarangani, Malungon and Alabel; and in North Cotabato, Antipas, President Roxas, Alamada and Banisilan were targeted and were conducted starting February to August of this year.
The VRA was conducted by updating the existing landslide and flood
hazard map of the municipalities and doing anecdotal interviews to
barangay officials tracing histories of flooding and landslide
occurrences within the area and data gathering for the latest
municipality’s 2015 Community Based Monitoring System (CBMS)
population statistics, administrative maps and other pertinent data for
the generation of report and maps. Prior to the assessment existing
maps and related literatures were reviewed. Exit conference will be

conducted on the last day of every fieldwork, Information and
Education Campaign (IEC) on the results of VRA were scheduled.
Detailed Sub-surface Assessment
The municipality of Maasim in Sarangani Province, the lone target
for Subsurface Hazard Assessment for the year 2017. The
assessment was conducted by doing anecdotal interviews to
barangay officials tracing histories of sinkhole occurrences within the
area. Field geological assessment is conducted by doing
observations and measurements of sinkhole surface manifestations
such as shallow holes and ground displacements. Ground
Penetrating Radar (GPR) was used to determine the subsurface
structures and strata. Collected data will be interpreted by a GPR
software to produce plan and section view of the subsurface
structures and strata. Prior to actual field survey, existing maps and
literatures are reviewed.
Coastal Geohazard Assesment and Mapping
Coastal Geohazard Assessment and Mapping activities were
conducted in Thirteen (13) local government units outside Region 12.
The location of the targets are within the jurisdiction of Regions 13
and 11. In September 2017, two municipalities and a city were
targeted in Region 13, namely Malimono, San Francisco and Surigao
City, all within Surigao del Norte Province. While the remaining ten
were in Region 11, namely Malita, Sta. Maria, Jose Abad Santos and
Don Marcelino in Davao Occidental; Hagonoy, Padada, Sulop and

Malalag in Davao del Sur; Davao City, and Samal City in Davao del
Norte were targeted in October and November
The conduct of this activity includes interview and anecdotal accounts (data/information gathering), coastline tracking, getting the locations of coastal puroks per barangay and beach profiling. The output of this activity are Coastal Geohazard Assessment Report, Coastal Erosion and Accretion Map (also coastal shift maps) using DSAS software and Threat Advisories given to barangay heads for their proper guidance and information.

Groundwater Resources Assessment
For the groundwater resources assessment this year’s target was
one (1) for regular budget. The province targeted was Saranggani
Province covering the seven municipalities conducted on February
27-March 21, 2017 for Kiamba and Maitum, on, March 27-April 26,
2017 in Malungon and Malapatan, Alabel on April 28-June 2, 2017
and Maasim and Glan on June 5 to July 1,2017. The physical
conditions of the study area in terms of topography,slope ,drainage,
and watershed ,rock type , and rainfall had been established. The
groundwater availability of the Province had been mapped
(1:250,000 scale) .The marine clastics and pyroclastics were
identified as the main groundwater resources aquifer potential having
high porosity and permeability. One target for Tier 2 is the province
of Sultan Kudarat. The province is composed of twelve (12)
municipalities namely Tacurong, Columbio, Lutayan, Lambayong,
Isulan, Esperanza, President Quirino, Bagumbayan, Lebak, Senator
Ninoy Aquino, Kalamansig, and Palimbang. The first phase is
composed of the municipalities of Tacurong, Columbio and Lutayan
on July 5 to August 6, 2017. Esperanza, Lambayong and President
Coastal tracking using a handheld GPS along the beach of a certain barangay in the Malimono municipality of Surigao del Norte.

Quirino were assessed during the second phase on August 8 to
September 8, 2017. The third phase is composed of Senator Nino
Aquino, Isulan and Bagumbayan conducted on September 13 to
October 13, 2017. The fourth phase is comprised by the
municipalities of Lebak and Kalamansig on October 18 to November
21, 2017. The last phase is the municipality of Palimbang assessed
on November 22 to December 12, 2017.

Communication Plan for Minerals Development
Stakeholders Forum were conducted during the regular SB meeting to
address the issues and concerns related to mining activities. An open forum
is conducted after the power point presentation in order to clarify issues and
concerns. Question that were raised during the open forum were
subsequently answered by the present. Most of the questions raised were
the benefits of mining in terms of revenue shares, Social Development as well
as environmental protection and the life of the community after mining. There
were also questions raised relative to the impact of small-scale mining
operation.

Innovations
• MGB XII had created on May 5 the Information Communication and Education (ICE) Network through MGB XII Special Order No. 94, series of 2017 to serve as the public information and quick response arm of the region. ICE network is responsible to sustain consultation process with stakeholders to collect opinions and continue to engage them. The team was utilized when invite for resource person are requested by other line government agencies and stakeholders ;
• MGB XII also formally presented its facebook account “Mines and Geosciences XII” to apprise the public on the activities of the regional office and showcase educational materials about mining and the environment. Likewise, hotline number 09195980526 was also established to attend to queries, tips or complaints on mining and geohazard related concerns and issues.
• Trainings and internal audits were conducted for Integrated
Management System for ISO accreditation covering quality, environmental, occupational safety and health management.
• Partneship with SMI and freedom for socio economic impact study of mining during the exploration period.
• Mobilization of the monthly Officer of the day
• Localized Human Resources Information System (HRIS) which will be the baseline of the Personal Data Sheet (PDS)

On June 29,2017 in the celebration of Environment month, MGB XII conducted tree planting

November 7, 2017 in the celebration of November as Mining Month 2017, MGB XII conducted
clean-up drive and tree planting activity at Mary Mount School of Koronadal,Inc.
November 28, 2017 Mining month 2017 conducted blood-letting activity in partnership with the
Philippine Red Cross South Cotabato Chapter held at MGB XII Office, Carpenter Hill, Koronadal
City. Thirty (30) successful blood donors and eleven (11) were not able to donate because of
high blood pressure. They were served with balut, arroz caldo and soft drink. All were given T-
shirt and eco bag.MGB XII extend much gratefulness on the support of Small Scale Miners
Association of Tboli (SSM) and BJMP XII.
