Case study: Flight X at Airport A

The flight was cleared to land on RWY 28R but instead lined up with parallel TWY C

RWY 28L was closed (promulgated by NOTAM), pilots mistaken RWY 28R as RWY 28L and TWY C as RWY 28R

The NTSB identified:

Need for more effective presentation of flight operations information to optimize pilot review and retention of relevant information.

Although the NOTAM about the runway 28L closure appeared in the flight release and the 4 ACARS messages that were provided to the flight crew, the presentation of that information did not effectively convey the importance of the runway closure information and promote flight crew review and retention.

“At global level ICAO shall expedite the establishment of a working arrangement to address AIM domain-specific subjects with high priority given to support implementation.”

“Particular attention should be directed to the development of guidance material, the need to update the roadmap for the transition from AIS to AIM, revise and/or enhance existing requirements”

“facilitating the migration of existing paper-based aeronautical charting products into the digital environment and identify requirements for the graphical representation of aeronautical information in a SWIM environment.”
