The drug references known âs "compendia" for pu{poses of determining Medicaid coverage for
off-labetprescriptions are incorporated by reference in 42 U.S.C. $1396r-8(9)(1)(B)(i). This
satisfies the requirement to provide a description by category and location'
Notwithstanding the foregoing, the Relator previously provided defendant with copies ofthe DRUGDEX monographs for the prescriptions identified in (a) paragraph24 of the Complaint
in this matter, Docket No. 1, and (b) Docket Nos. 46-1, 46-2, and 46-3 (Cunently Identified
Prescriptions). In addition, the location of a copy of the DRUGDEX Consult, Recommendation,
Evidence and Efficacy Ratings is provided below. Relator is also providing below the location
of copies of American Hospital Formulary Service Drug Information (AHFS) monographs forthe Currently Identified Prescriptions.
With respect to the United States Pharmacopeia-Drug Information (or its successor
pubtications), until recently Relator believed that publication had been discontinued and there
was no successor. Hov/ever, it has recently come to Relator's attention that DrugPoints, currently publishecÍ by Truvex, previously published by'fhomson Reuters, may be the successor
publ.ication. Unfortunately, the publisher has heretofore not been able to provide documentation
io demonstrate this. Despite the uncertainty over DrugPoints being the successor publication to
United States Pharmacopeia-Drug Information, the location of copies of DrugPoints
monographs for the Currently Identified Prescriptions is also provided provided below, along
with Appendix A, Strength of Recommendation and Evidence.
2, Medicaid Coverage for Prescriptions Not for a Medically Acceptedfndication

CV - Matthew Joy
Since 1996I have been the technical lead for forensic computer cases which have been handled by Bright
Solutions Inc.
Bright Solutions Inc. is an Alaskan computer and lnternet commerce consulting fiLm with a great deal ofexpãrience working in data recovery and conversion. In the past year our team has worked as an advisor and/or
expert witness on approxim ately a dozen computer forensic examinations for a range cases involving digital
evidence.
Over the years we have been involved as expert witnesses in a number of legal cases where we helped to collect
and interpret the digital tecords. Bright Solutions has done contract work for the Federal Court System, Alaska
State Crime Lab, yãkutat police, Whittier police, and St. Paul Police, Federal and State Public Defender offices
in Anchorage, Palmer, Koãiak, Kenai and the Office of Public Advocacy and with many law firms. The staff
has been thiough fingerprinting and background checks and have all signed non-disclosure agreements so that
we have clearance to work in sensitive environments or with sensitive subjects.
I was first recognizedby the court as an expert witness in my first of many cases in 1996 SOA vs. Michael
Braddock, a former Anchorage Police f)epartment of'ficer accused of relations with a minor. After the case twas
settled, I was asked by Sergeant Casper to present a seminar on the proper handling of computer evidence for
the APD. I was also ãskcd to take a job with the Federal Bureau of Investigation based upon the work I did in
that hrst ApD case however I decided to remain independent since working as computer consulting across a
wide variety of environments since it keeps expertise sharp. Over time my staff and I have assisted with legal
oases in matters such as these:
Acquisition of Forensic Digital DataAnalysis of phone and email and network records
Audio and video analysis & EnhancementCell phone recordsCollection and Interpretation of GPS InforrnationCorporate DisputesDigitalVideoMilitary JusticePublic Defender cases such as Murder or child porn Defense

Certifred Þ'raud Examiners. Bright solutions Inc. has also presented a session regarding cell phone forensics to
The Anchorage Association of Trial Lawyers and at the Alaska Public Defenders Conference.
