• Form Information Required From Contractor/Consultant/Municipality to District Designee Preferably No Later Than the 3rd of Each Month:– State Project Number (Column 5)– DBE Information (Total Payment to Date for DBE’s Utilized in

Column 14))

• District Designee Must Submit Completed 1585 to:– Ted Szymanski (theodore.szymanski@ct.gov)- Due no later than the 5th day of each month

• Total number of employees utilized during the noted timeframe

• Total Work Hours charged to the ARRA project• Total Labor Cost (hours x hourly rate = total labor cost)

• District Designee Must Submit Completed 1589 No later Than the 5th of Each Month to:– Ted Szymanski (theodore.szymanski@ct.gov)
