WHY WE NEED EFFICENT DIAGNOSTIC
MICROBIOLOGY LABORATOREIS
Dr.T.V.Rao MD Life is an experience to learn from many, In 1994 in a credited Hospital a Director a known famous
Cardio thoracic surgeon visits the department and says you people look at Urine and stool, I gave a
reply I am sorry Sir, we are qualified in Medical Microbiology beyond the imagination of a Physician
thinks I stood to my words and lived a life of simplicity and learning microbiology with and from all
resources and learned people in the profession Today Medical profession is Money making
machine, lesser value for human life and many wish to become rich at the cost of human suffering
faster than many around by so many vagaries in the System, Government of India has started Many
Medical colleges, to stop the quackery, and to replace the Medical Profession with qualified Doctors,
However the Laboratory system never touched by the Medical council of India or Ministry of health,
there are many dangers await to the innocent people by the inefficient and commission oriented
Laboratories, Today many qualified Doctors are least interested in doing a proper clinical evaluation,
and have become believers of the Laboratories, never forget the laboratory specialists take matters
casually increasing the morbidity and mortality, However many conflicts going to harm many
innocent people when the conflicts of interests are very high, as when the Doctors send for MRI, and
CT scans many can get faster easier money than the consultation. The condition of private
laboratories are really pathetic as far concerned with Microbiology. However whatever said and
fighting the system is in vain nobody cares the life of the patient and getting money makes them rich
to life. Today many perceive in many developing countries life in danger with SUPERBUGS AND
FAILED ANTIBIOTIC POLICY and pharmacies have become money spinners to many. Antibiotic
stewardship failed to make an impact as the matters have become out of control, in this process
India and like some developing Nations will become a paradise for SUPERBUGSCHANGING ROLE OF
DIAGNOSTIC MICROBIOLOGY Microbiologists to live in the system with competency should be
knowledged and practice the academic changes going in the profession rather arm chair Professors
should be familiar with documentation to view and show the clinicians how dangerous the microbes
when we have no proper accountability to our work and Documentation Requirements Laboratory
documents and record about each specimen and result including, specimen and result including,
specimen source & condition, Pathogens identified and Susceptibility testing results –LIS, Logbook
PLAN YOUR LABORATORY Stages of laboratory activities The QC program must ensure optimum
patients specimens & result integrity throughout the 3 stages processes: stages processes:
•1.Pre-analytical
•2.Analytical
•3.Post-analytical
PLAN WITH GOOD LABORATORY PRACTICES Three phases of testing:
1) before testing (test ordering and specimen collection),
2) during testing (control testing, test performance, and result interpretation and recording), and
3) after testing (result reporting, •documentation, confirmatory testing, and •biohazard waste
disposal
WHY WE NEED INTERNAL AUDIT Aim: Monitoring the performance of the whole procedures
Method: •Laboratory activities (pre-analytic analytic & post analytic) were examined •Standards
were set using laboratory standard operating •Standards were set using laboratory standard
operating procedures •the findings were discussed the measured performance was reviewed an
explanation for any deficiencies sought.
TRY PRACTICE YOUR KNOWLEDGE Proficiency testing/ quality assessment both scheme act as an
indicator of the •effectiveness of internal quality control program Advantage of external quality
assessment assessment •provision of wide variety of organisms •stable specimens •chance to
compare individual performance with other participants
Never forget we are all potential future patients and SUPERBUGS do not spare us, and we will kill
many of our patients, if there is no proper beginning with implementing minimal standards in the
Diagnostic laboratory system, which depends on reliable assessment of testing and calibration
laboratories, inspection agencies and fabricator inspection programs.
HOWEVER TODAY OUR LABORATOREIS NEED MORE ACCOUTABILITY KNOWLEDGE AND ETHICS TO
SAVE MANY PATIETNS.
