Introduction

Further to a request made by Mr A N Other of Any Town Hospital NHS Trust,

Oakleaf Technical Services Ltd has completed a re-inspection of previously identified

asbestos materials (ACMs) in A Block.

A survey report (produced by Aspect Contracts Ltd) was provided by the Trust and

used as the base for the re-inspections. All asbestos information has been checked

and updated to provide the Trust with accurate and compliant risk assessments.

Any caveats from the original reports are still important and should be considered

when managing asbestos on the site.

The inspections were completed in August of 2010. At the time of the survey, the

building was occupied and fully functional. Access to the site was arranged by Mr A

N Other who coordinated with local building users and the Estates team.

For details of the asbestos contained within this building, see section 1 and 2 of this

report.

Method of Survey

The building was inspected in a professional and methodical manner, using the

drawings and existing survey reports provided by the Trust. Access to the site was

arranged through Mr A N Other.

The survey was carried out in accordance with the Oakleaf Technical Services Ltd in

house asbestos survey procedure. This procedure is based upon and meets the

requirements of HSG 264 Asbestos: The survey guide.

Where possible, all areas of asbestos identified in the previous report have been

accessed and their current condition recorded.

Material risk assessments for all asbestos containing materials (ACMs) have been

produced, to show current levels of damage and subsequent treatments.

Recommendations of future actions for each asbestos containing material (ACM)

have been provided. Any actions taken must be in full accordance with the Control

of Asbestos Regulations 2006 (CAR 2006).

Risk Assessment

The risk assessment method used is the HSE approved ‘materials risk assessment’, as

described in the HSE document HSG 264. This is used to assess all ACMs on a like for

like basis, using an algorithm that for each ACM looks at:

� Product type – Cement roof sheets or gaskets etc.

� Extent of damage/deterioration – High or low etc.

� Surface treatment – Painted, exposed etc.

� Asbestos type – Chrysotile etc.

Each of these parameters is awarded a score, with the scores then combined to give

the final material risk assessment score of between 2 and 12. Materials that do not

contain asbestos are not given a score.

ACMs with a score of 10 or more should be regarded as high risk with a significant

potential to release fibres if disturbed.

ACMs with a score between 7 and 9 are regarded as medium risk items.

ACMs with a score between 5 and 6 are regarded as low risk items.

ACMs with a score of 4 and below are regarded as very low risk items.

Survey Information

Sealed voids; partitions; cavity walls; behind cladding; behind Asbestos Containing

Materials (ACMs); under floorboards; safes/fire cabinets; plant and equipment;

above solid ceilings, where to gain access would have caused destruction, have not

been surveyed.

The extent of the survey did not include items on the surface of or within the ground

beneath or adjacent to the survey area. The grounds of the building and

inaccessible areas such as under floor ducts, tunnels and sewers have not been

assessed. Confined spaces of any kind will not have been accessed. This is not a

contaminated land survey.

This also applies for re-locatable items held in storage facilities (for example,

asbestos gloves in boxes or draws).

Representative checks will have been made above suspended ceiling grids where

access is available, i.e. where the ceiling tiles were not sealed with paint or fixed

with screws/nails.

Loft and roof spaces will have only been surveyed where safe access was available.

If crawl boards or secure flooring were not present, the area would have been

deemed to be unsafe.

Flooded basements, pipe runs, troughs or other areas that were covered with water

could not be fully assessed. These areas may contain asbestos that is not identified

in the survey.

Fuse boxes will not have been opened up as they remain ‘live’ and would constitute

a safety hazard. Some older type fuse boxes have been known to contain chrysotile

asbestos rope used as fuse insulation. It will be necessary to presume that these

contain asbestos and investigate the areas with the help of an electrician before any

future works are carried out.

Fixed items of plant, heaters, services or equipment will not have been surveyed

internally, as surveyors are not qualified to shut down, dismantle and restart these

items. The survey was limited to the accessible external casings of these items.

Oakleaf Technical Services Ltd make every effort to fully survey buildings and rooms,

but cannot be held responsible for asbestos products located behind or underneath

stored items. This is particularly important in areas of excessive or untidy storage.

As a qualified engineer was not present on site, the lift shafts have not been fully

inspected. The inspection will have been limited to gaps in-between the lift doors

and the lift shaft were lifts are present.

Within a survey, Oakleaf Technical Services Ltd will not have removed insulation or

its coverings, such as metal or plaster cladding. The inspection will have been

carried out where the underlying pipe was visible (normally at areas of damage,

flanges or gaskets) and the outer casing of boilers, pipes and other plant machinery.

General Comments:

Where drawings have been provided by the client, Oakleaf Technical Services Ltd

does not accept liability for recording errors due to inaccurate drawings.

The results in the survey report relate to prevailing conditions on site at the time of

survey. Alterations to the building or to its use and occupancy may affect the

accessibility of ACMs contained within. In these circumstances, the report should be

reviewed in accordance with HSG227 ‘A comprehensive guide to Managing Asbestos

in Premises’.

Due to the limitations described above, the survey information should not therefore

be solely relied on when undertaking building works. Before any building works,

refurbishment or re-wiring etc are undertaken a fully invasive demolition or

refurbishment survey must be carried out in the location of the works to ensure that

workers are not put at risk.
