Getting StartedThis Provider and Pharmacy Directory lists Primary Care Doctors and other health care providers available to our Members. You can view the most current Doctor listing on our website at www.iehp.org. Or call IEHP Member Services. A. Choosing Your Primary Care Doctor

As a Member, you need to choose a Primary Care Doctor, usually referred to as a PCP. You should select your Doctor within 30 days of enrollment. If you do not choose a Doctor, IEHP will choose one for you within 40 days of enrollment. Each enrolled family member can select a different Primary Care Doctor.Here is a list of the types of Primary Care Doctors you can choose:

y Family and General Practice Doctors can usually see the whole family. y Internal Medicine Doctors usually see only adults and children 14 years or older. y Pediatricians see newborns to individuals age 18 or 21. y Obstetricians and Gynecologists (OB/GYNs) see only women and specialize in women’s health and maternity care. Only those OB/GYNs

listed in the “Primary Care Doctors and Hospitals” section can be chosen as your Primary Care Doctor. y Nurse Practitioners (NP) can usually see the whole family.

If you have any questions about age restrictions, Doctor availability, your Doctor’s medical school education, residency or board certification status, call IEHP Member Services.

B. Getting the Hospital You WantYour choice of Primary Care Doctor will determine which hospital you will go to if you need inpatient or outpatient care. If you want to be able to use a specific hospital, select a Doctor who is associated with that hospital. You can find the Doctor’s hospital association on the same line as the Doctor you select. Doctors who are associated with more than one hospital are listed twice with different numbers.

C. Getting the Specialist You WantSpecialists are Doctors who practice in a medical specialty. All IEHP specialists are linked to Primary Care Doctors. OB/GYNs are the only specialists listed in the Provider and Pharmacy Directory because they are the only type of specialist that you can see without a referral. If there is a specific specialist you would like to see, talk to you Doctor.Your Primary Care Doctor can provide services such as physical exams and preventive health services. For other services you may need to see a specialist. Your Doctor will tell you if you need to see a specialist. Your Doctor will submit a “referral” (i.e., a request) to the IPA/Medical Group or IEHP. You must have a “referral” to get specialty services.

D. Need Help Selecting or Changing your Doctor?Whether you’re choosing a Doctor for the first time, or want to change Doctors, call IEHP Member Services.

More than 120,000 Medi-Cal beneficiaries with disabilities have joined IEHP. Here are some of the actions we’ve taken to improve the communication, access, and health of our Members with disabilities:

Office Accessibility Guide—The Provider and Pharmacy Directory gives you information on the basic physical accessibility features of our Primary Care Doctors’ offices.

Access Level/Indicator Description: The meaning of each accessibility code is described in the chart below:B - Basic Access Demonstrates facility site access for the members with disabilities to parking, building, elevator, doctor’s office, exam room, and

restroom.L - Limited Access Demonstrates facility site access for the members with a disability is missing or is incomplete in one or more features for parking,

building, elevator, doctor’s office, exam room, and restroom.P - Parking Parking spaces, including van accessible space(s), are accessible. Pathways have curbed ramps between the parking lot, office, and at

drop off locations.EB - Exterior Building Curbed ramps and other ramps to the building are wide enough for a wheelchair or scooter. Handrails are provided on both sides of

the ramp. There is an “accessible” entrance to the building. Doors are wide enough to let a wheelchair or scooter user enter, and have handles that are easy to use.

IB - Interior Building Doors open wide enough to let a wheelchair or scooter user enter, and have handles that are easy to use. Interior ramps are wide enough and have handrails. Stairs, if present, have handrails. If there is an elevator, it is available for public/patient use at all times the building is open. The elevator has easy to hear sounds and Braille buttons within reach. The elevator has enough room for a wheelchair or scooter to turn around. If there is a platform lift, it can be used without help.

R - Restroom The restroom is accessible and the doors are wide enough to accommodate a wheelchair or scooter and easy to open. The restroom has enough room for a wheelchair or scooter to turn around and close the door. There are grab bars which allow easy transfer from wheelchair to toilet. The sink is easy to get to and the faucets, soap, and toilet paper are easy to reach and use.

E - Exam Room The entrance to the exam room is accessible, with a clear path. The doors open wide enough to accommodate a wheelchair or scooter and are easy to open. The exam room has enough room for a wheelchair or scooter to turn around.

T - Exam Table/Scale The exam table moves up and down and the scale is accessible with handrails to assist people using wheelchairs or scooters. The weight scale is able to accommodate a wheelchair.

A Bus Route Guide — The Provider and Pharmacy Directory lists bus stops within blocks of our Primary Care Doctor’s office, so you can choose a Doctor that is easy for you to reach. We encourage you to call your Doctor’s office for accessible pathway and curb information. Ask your local transit agency for wheelchair lift information.

All Primary Care Doctors and specialists you see should be within the same IPA or Medical Group.

Independent Physician Associations (IPAs) or medical groups are a group of Doctors in private practice in your community who form an association (or team) to contract with health plans, like IEHP. These Doctors practice in their own independent offices, clinic, or medical groups.

Your Primary Care Doctor, along with the IPA or medical group, takes care of all your health care needs. This includes getting authorization to see a specialist or for medical services such as lab tests x-rays, and/or hospital admittance. IEHP does not provide health care to you directly.

Contact your Primary Care Doctor to coordinate these services or if you have any questions or concerns about the coordination of your care.

When choosing a Primary Care Doctor, make sure that the Primary Care Doctor and hospital you select are located within 10 miles or 30 minutes of your home.

When making your choice, you might also consider the following factors: y If the Doctor’s office is open evenings or weekends y Which languages the Doctor speaks y If the Doctor takes walk-ins or same-day appointments y Where the Doctor sends patients for lab work

At IEHP, you always have a choice. If you decide to change your Primary Care Doctor, go to www.iehp.org or call IEHP Member Services.

Note: When you fill out the Plan Choice Form, use the DOC # affiliated with the IPA that you want on your form, instead of the NPI#.
