Critical: Senior and his/her parent(s) review the

Step 1- call, write, or use the Internet to request or download & print your college applications. Pay particular attention to mailing instructions,
required recommendations, etc. Note due dates & allow at least 30 school days for
processing the Transcript Packet.

Step 2- Submit the following completed Transcript Packet Request items to the registrar:
Check Off List: A signed Transcript Request/Release of Info. form (pg.
4) A Student Resume (suggested format pg. 5) A completed Counselor Input Form (pg. 6) A completed Parent Questionnaire (pg. 7) Any Secondary School Report Forms/Counselor
Recommendations. A signed waiver form (pg. 8) $4.00 for each transcript packet requested (cash or
check payable to HHS) A large addressed envelope, for each school, with
three stamps per envelope.

Mid-Year Reports and Final Transcripts
• Provide the Registrar with a stamped, addressed, business envelope and any additional counselor forms for each school requiring a mid-year report at the beginning of February.
