Erysipel / Phlegmone
Erysipel (1) scharf begrenzt, «flammend»
Streptokokken Gr. A Strept. Gr. B, C, G, Staph. aureus
Phlegmone = Zellulitis (2) weniger scharf, ödematös
Staph. aureus > Streptokokken andere Keime bei besonderer Exposition:

Schwierige SSTI
Nicht heilend:
Abszess, Osteomyelitis in Tiefe Sono, MRI
schlechte Durchblutung Angiologie
resistente Keime (CA-MRSA…) Bakteriologie
Rezidiv. Furunkel / Abszesse:
S. aureus-Trägertum Abstriche (25% der Bevölkerung) Nase/Rachen/Inguina
meist kein Immundefekt BZ, BB, HIV-Test
DD: Acne inversa (= Hidradenitis suppurativa)

Frage 2: Scabies
Welche Antwort ist korrekt ?
1) Ivermectin p.o. ersetzt Permethrin als first line Therapie.
2) Auch asymptomatische, enge Kontaktpersonen von Patienten sollen untersucht und behandelt werden.
3) Die Symptome verschwinden typischerweise rasch unter adäquater Therapie.

Praxisrucksack Hautinfektionen
Scabies
Diagnose Anamnese + Befund
ev. mehrere Wochen asymptomatisch
Therapie Permethrin Crème 5%
am wirksamsten
1x Hals bis Fuss, über Nacht ev. nach 7 Tagen wiederholen
wichtig
Juckreiz persistiert oft über Wochen
asymptomat. enge Kontaktpersonen mitbehandeln (1x)
Therapieversagen: falsche Anwendung, Reinfestation
Alternative: Ivermectin p.o.
