Purpose. To issue enclosure (1), the Navy and Marine Corps Public Health Center (NMCPHC) Organization Manual, consistent with reference (a) and aligned to the mission, functions, and tasks delineated in reference (b). This instruction is a complete revision and should be read in its entirety. 2. Cancellation. NAVMCPUBHLTHCENINST 5450.1R. 3. Scope. The provisions of this instruction are applicable to the Navy and Marine Corps Public Health Center. 4. Action a. All NMCPHC personnel shall familiarize themselves with this instruction in the performance of their duties. b. Special Assistants and directors assigned ownership of the various sections in enclosure (1) will review their respective sections at least annually for accuracy and relevance. This review will be initiated by the Director for Administration (DFA). c. The commanding officer (CO) will approve all change transmittals and modifications to the organizational structure. 5. Introduction Command History In 1964, the idea for a comprehensive Navy occupational health program originated at what was then the Navy's Bureau of Weapons. The Bureau recognized the need for an occupational health program, which would encompass all Fleet Readiness and Training Ordnance field activities. It directed the CO, Naval Ammunition Depot (NAD), Crane, Indiana to address this need by broadening that command's occupational health function. The expanded mission, which came under the supervision of the Senior Medical Officer (SMO) at NAD Crane, included assistance to all naval ammunition depots and naval stations in coordinating occupational health programs. Under the SMO's supervision, the program's first industrial hygiene survey was done. In May 1967, the Bureau of Weapons formalized the program function it had assigned to NAD Crane with the establishment of the Naval Ordnance Systems Command Environmental Health Center, a division of NAD Crane Medical Department. In June 1970, the Center was disestablished as an NAD Crane program and on July 1, it became a Headquarters Detachment (HQ DET) of the Naval Ordnance Systems Command (NAVORD), Cincinnati, Ohio. In October of that year, The HQ DET became known as the Naval Ordnance Environmental Health Center (NOECH), NAVORD. A year later, the Bureau of Medicine and Surgery (BUMED), the Naval Material Command, and NAVORD signed a memorandum of agreement which brought NOEHC under the aegis of BUMED, renamed the “Navy Industrial Environmental Health Center. They continued to provide industrial environmental health services primarily to BUMED and NAVORD. In July 1974, the Center became the "Navy Environmental Health Center" (NEHC), an Echelon 3 shore activity under the command and support of BUMED. A move by the Center to Norfolk, Virginia was initiated in the fall of 1978. Relocation was undertaken in response to an increase in requests for fleet support and further expansion of the command's occupational health mission. The expanded role included responsibility for the Navy Occupational Safety and Health Inspection Program, analytical lab services, radiation health, hazardous materials identification, asbestos hazard control, preventive medicine,

epidemiology, and hearing conservation. (The Hearing Conservation Division of the Naval Aerospace Medical Institute, Pensacola, Florida became NEHC's Hearing Conservation Branch in November 1978). The move was completed when the Hearing Conservation Branch and NEHC were consolidated in Norfolk in July 1979. In 1981, NEHC's mission was further expanded to coordinate and provide centralized support and occupational health, environmental health, and preventive medicine services to medical activities ashore and afloat. Navy Environmental and Preventive Medicine Units (NEPMU) and Navy Disease Vector Ecology and Control Centers (DVECC) were placed under NEHC as Echelon 4 commands. These activities included: NEPMU 2, Norfolk, Virginia; NEPMU 5, San Diego, California; NEPMU 6, Pearl Harbor, Hawaii; NEPMU 7, Naples Italy; DVECC, Jacksonville, Florida; and DVECC, Alameda California. BUMED reorganized in 1982 to establish the Naval Medical Command (NAVMEDCOM), with NEHC being one of its 15 subordinate Echelon 3 commands. In 1983, NEHC functions were once again expanded to include coordination and review of all occupational health and preventive medicine programs under the direction and management of NAVMEDCOM. NEHC established many programs, which continued to have a direct impact on day-to-day work of Navy personnel. For example, 1986, it established the Navy Preventive Medicine Information System, a computerized information system designed to support the Fleet and Fleet Marine Force with consistently updated preventive medicine and vector-borne disease profiles of countries of military importance. In 1987, NEHC also assumed its role as Project Manager for Navy Occupational Health Information Management System site implementations. In 1989, NEHC moved into ample, modern spaces, specifically designed to accommodate the command's needs in meeting a vastly expanded mission. The much needed move consolidated all NEHC departments and staff, which had operated out of three separate, temporary facilities for the past ten years at Norfolk Commerce Park, 2510 Walmer Avenue, Norfolk, Virginia. In October 1990, NEHC assumed the additional responsibility for the management of the Navy Drug Screening Laboratories (NAVDRUGLAB). This brought the total number of field activities under the command's guidance up to ten.

In 1995, the command was restructured to provide better customer service. Directorates for newly established programs in health promotion and chemical biological radiological environmental warfare were established. In 1999, NEHC prepared for its relocation to the Naval Medical Center Portsmouth Compound, Building 215. Planning of this move actually began in the early 1980's when it was decided that Naval Medicine could better serve the Fleet through the availability of all Navy Medical and Preventive Medicine Commands in one central location. On February 11, 2002 the NEHC relocated to the Navy Medical Center Portsmouth Compound occupying the 10th and 11th floors of Building 215. The NEPMUs took on a major role at the start of Operations Iraqi and Enduring Freedom. Their Forward Deployable Preventive Medicine Units (FDPMUs) began an ongoing deployment rotation in support of those operations as well as supporting numerous humanitarian events around the globe. In order to better meet the demands of an ever-increasing operational tempo, the Navy DVECC, Bangor, Washington was disestablished on 30 September 2005 and assets redistributed among the various NEPMUs and the other NDVECC located in Jacksonville, FL. In October 2006, subsequent to realignment within Navy Medicine, NEHC was designated an Echelon 4 Command under the Navy Medicine Support Command, Jacksonville FL. NEPMU SEVEN, Sigonella Italy was disestablished to reduce the Navy’s European footprint. NEPMUs TWO, FIVE and SIX were realigned under Naval Medical Centers Portsmouth and San Diego. During this same time frame, the Navy DVECC, Jacksonville was designated a center of excellence for Navy entomology and re-named the Navy Entomology Center of Excellence (NECE). In November 2007, NEHC was re-named the “Navy and Marine Corps Public Health Center” in keeping with its ever-changing and growing mission to provide leadership and expertise to ensure mission readiness through disease prevention and health promotion in support of the National Military Strategy. In 2010, NMCPHC regained cognizance over the NEPMUs and the Navy Bloodborne Infection Management Center (NBMIC) in Bethesda, MD.

18 Aug 14 Command Organization. The NMCPHC staff is organized into two major functional areas: the Command Suite and six directorates. The Command Suite (CS) is composed of the CO, Executive Officer (XO), Command Master Chief (CMC), CS Administrative Staff, and Special Assistants. Each of the six Directorates is headed by a director. Each Directorate is further divided into departments and in some instances into divisions.

a. Commanding Officer (1) The CO is responsible for the organization, management, and mission accomplishment of the NMCPHC. The CO exercises complete military jurisdiction and is responsible for the safety and well-being of the entire command. He or she has the authority to demand from all personnel under his or her jurisdiction, compliance with federal statutes and regulations, pertinent orders, and instructions of BUMED, the Secretary of the Navy (SECNAV), and other competent authority. (2) The CO is the immediate superior in command for its ten field activities, exercising command and control over the

NECE; Naval Dosimetry Center; three NAVDRUGLAB; NBIMC; and NEPMUs 2, 5, 6 and 7. (3) The duties and responsibilities of the CO are prescribed in Navy Regulations and the Manual of the Medical Department. The responsibility for the efficient organization, safety and well-being of the entire command is absolute. The CO may, with discretion and when not contrary to law or regulations, delegate authority and duties to the executive officer and other subordinates as appropriate for the execution of command functions or operations. Such delegation of authority in no way relieves the CO of his or her continued responsibility. (4) The CO is also charged with mission specific preventive medicine responsibilities, including: (a) Promoting and advocating for integration and alignment of preventive medicine, occupational health, health promotion, and population health efforts throughout Navy Medicine. (b) Ensuring preventive medicine and population health issues are adequately addressed throughout the Budget Submitting Office for level programming and resource decisions. (c) Providing expertise and guidance for service-wide initiatives and strategies to improve the health of the population as a whole. The CO also facilitates occupational health, preventive medicine, health promotion, population health, industrial hygiene, environmental health, and medical surveillance efforts of BUMED activities by providing program management, coordination, and technical expertise. (d) Supporting collaborative efforts with other Navy and Department of Defense (DoD) components on multi-service occupational health, preventive medicine, health promotion, population health, drug testing, industrial hygiene, environmental health, and medical surveillance issues. (e) Providing integrated planning for Navy deployable preventive medicine systems and advising on appropriate research, development, and testing requirements for the above functions.

b. Executive Officer (1) The XO ensures implementation of the CO’s orders and policies. Normally, all official communication between the CO and other command leaders occurs through the XO. The XO is the direct representative of the CO in coordinating the administrative and management functions of the command. (2) All orders issued by the XO are regarded as proceeding from the CO. The XO acts independently with regards to administrative matters, which do not require the CO’s personal attention yet keeps the CO apprised of the action taken. (3) The XO is the point of contact and coordinating authority for the command’s ten field activities: NECE; Naval Dosimetry Center; three NAVDRUGLAB; NBIMC; and NEPMUs 2, 5, 6 and 7. The field activities shall report directly to the XO for all administrative matters and general support. c. Command Master Chief (1) Advises the CO in matters pertinent to the welfare and morale of all assigned enlisted personnel. (2) Exchanges ideas and disseminate information and instruction, which affect the enlisted community, and ensure that both written and oral lines of communications are available to the CO and all enlisted personnel of NMCPHC. (3) Serves as NMCPHC’s primary focal point on the professional development of preventive medicine technicians at the NMCPHC and field activities. d. Command Suite Administrative Staff (1) Manage the daily activities of the CO and XO; to include coordinate appropriate arrangements for travel, schedule visits, prepare travel vouchers, arrange meetings and prepare all required documents, and submit appropriate reports. (2) Manage central repository of official incoming and outgoing tasks assigned to or by the CO. Perform final review of all incoming and outgoing general command correspondence that has been prepared for the COs signature.

(3) Manage the command’s Electronic Knowledge Management (eKM) System, in receipt, distribution and tracking of all taskers. Review submitted staff work for correct content and format, and return for re-work if necessary. (4) Exercise budgetary control of expenses for the Command Suite. Approve requisitions for operating supplies, equipment, and services within the allocated budget allowance. (5) In coordination with the Command Legal Officer, perform duties and functions related to assisting with the preparation of legal cases. Consult with appropriate Staff Judge Advocate, when circumstances permit, when preparing documents or other related activities associated with the administration of Military Justice and JAG Manual investigations. (6) Serve as the civilian and military awards coordinator. (7) Manage the command’s military fitness reports and evaluations program. Notify staff members of reporting schedule and input requirements, ensures that reports are accurately prepared and timely submitted. e. Command Special Assistants (1) Civilian Personnel Liaison. NMCPHC point of contact with the Human Resources Office on all civilian personnel matters and manages all civilian personnel programs within NMCPHC and its field activities. (a) Provide human resource guidance and technical support to NMCPHC and its field activities. (b) Manage Position Management Board (PMB) and all requests from NMCPHC and its field activities. (c) Track recruits, job announcements, and certificates for hiring new employees. (d) Maintain files of command position descriptions. (e) Manage civilian performance appraisal process.

(f) Initiate civilian personnel actions and maintains up to date status. (g) Handle civilian non-salary related inquiries. (h) Maintain bulletin board on personnel information. (2) Command Managed Equal Opportunity (CMEO) Manager. Consults and advises the CO and XO in matters pertaining to the CMEO program. (a) Oversee the CMEO Program for NMCPHC and its field activities, Command Assessment Team (CAT), and Command Training Team requirements. (b) Coordinate and report CMEO training requirements. (c) Ensure Defense Equal Opportunity Management Institute organizational climate surveys are conducted and completed within required time frames and requirements. (d) Prepare NMCPHC policies and procedures for submitting and processing discrimination and sexual harassment complaints for military personnel. (f) Ensure NMCPHC equal opportunity (EO) program complies with all items cited in the CMEO Checklist, enclosure (3) of OPNAVINST 5354.1F CH-1. (g) Review and monitor all command investigations into EO issues with judge advocates or legal officers and provide assistance as needed. (h) Maintain CMEO records files for 3 years. (3) Command Inspections/Freedom of Information Act (FOIA) Coordinator (a) Command Oversight, Internal Audit Programs, Medical Inspector and Inspector General/Hotline Representative. 1. Manage the command oversight inspections and assist visits of NMCPHC’s field activities. Oversee the

implementation and execution of OPNAVINST 5200.25E both within NMCPHC and at the field activities. Based on findings from on-site inspections and/or analysis of MIC internal audits, provide actionable recommendations for improving organizational performance, safety, readiness, efficiency, effectiveness, and staff quality of life. Promote enterprise-wide organizational improvement through dissemination of benchmark processes, innovations and shared accomplishments. 2. Liaise with the Medical Inspector General to provide continuous oversight and review of the integrity of NMCPHC and its field activities operations; especially those designed to prevent fraud, waste, loss, unauthorized use, or misappropriation of assets. Coordinate and/or conduct investigations to determine compliance with agency regulations, laws, procedures, and sound management. 3. Prepare recommendations to correct deficiencies which adversely impact mission accomplishment, command integrity, or the uneconomical use of command resources. (b) Special Assistant for Standards of Conduct/Ethics, FOIA requests, Privacy Act (PA) requests, and congressional inquiries. 1. In conjunction with BUMED provides guidance to NMCPHC and its field activities regarding Standards of Conduct and Ethics. 2. Receive, analyze, and coordinate responses for all requests received under the FOIA, PA and Congressional inquiries. (4) Command Information Officer (CIO). The CIO has direct access to the CO in matters relating to information technology (IT)/Information Assurance (IA) systems security. The CIO serves as the senior Information Assurance Specialist responsible for management of and compliance with Navy IA Program for all IT assets under the purview of the Commanding Officer of NMCPHC. (a) Manages IA programs that meet current and future business requirement. Administers and monitors compliance of programs which address IA operations and readiness, including certification and accreditation responsibilities and

requirements for information systems, as prescribed by SECNAV M-5239.1. Ensure adequate security for IT systems in accordance with SECNAVINST 5239.3B. (b) Develop and maintain the Command’s Life Cycle Management (LCM) Plan. This 5-year IT Strategic Acquisition Plan describes the current IT hardware environment, plans for the replacement of older, outdated computer hardware and the acquisition of new computers resources. (c) Provide guidance to the command to ensure compliance with security directives and serve as the major proponent of IT system security in discussions concerning Information Assurance and the operation of the IT system. (d) Maintain inventory of all mobile computing and portable storage devices capable of storing Protected Health Information (PHI), Personally Identifiable Information (PII) or sensitive Data at Rest (DAR) as defined in BUMEDINST 5239.2. (5) Legal Officer. The legal officer, who shall be a commissioned officer, is an advisor and staff assistant to the commanding officer and the executive officer concerning the interpretation and application of the Uniform Code of Military Justice, Manual for Courts-Martial, JAGINST 5800.7F, and other military laws and regulations. (a) Consult a judge advocate, when circumstances permit, when preparing documents or other related activities associated with the administration of Military Justice and JAGMAN investigations. (b) Draft charge sheets, the orders convening courts-martial, and the appointing orders of those assigned to conduct investigations in accordance with JAGINST 5800.7F. (c) Supervise the technical and clerical preparation of all court-martial documents and administrative documents related to legal matters. (d) Ensure that officers and enlisted personnel assigned to courts-martial, investigations, and other legal duties are familiar with those duties.

(e) Collaborate with the training officer to ensure that all officers and enlisted personnel are fully acquainted with their rights and obligations specified in the Uniform Code of Military Justice. (f) Participate, as required, in processing cases involving non-judicial punishment and recommend that appropriate cases be referred to trial by courts-martial. (g) Review court-martial records, and prepare a recommendation in accordance with the Manual for Courts-Martial to assist the convening authority in deciding what action to take on the findings and sentence. Consult a judge advocate when circumstances permit. (h) Review for accuracy, clarity, consistency, completeness and compliance with applicable directives, investigative reports prepared in accordance with JAGINST 5800.7F and prepare command endorsement, as appropriate. (i) Refer personnel needing assistance with personal legal matters to the attending Region Legal Service Office, Defense Service Office, or nearest Armed Forces legal office, as appropriate. (j) Draft orders convening formal pretrial investigations pursuant to Article 32, of the Uniform Code of Military Justice. (k) Draft the Convening Authority's action on the record of trial and the court-martial issuing order with the assistance of the servicing Region Legal Service Office. (6) Public Affairs Officer (PAO). The PAO is responsible for implementing and maintaining internal information, community outreach and public information programs that are consistent with the guidelines set forth in SECNAVINST 5720.44C and the objectives established by the CO. The PAO reports to the CO concerning all public affairs matters. (a) Serve as the principal advisor to the CO for public and internal information, as well as clearance of information for public release, community relations, and audiovisual matters.

(b) Provide timely, concise, and accurate information to local media when proper queries from national media will be quickly coordinated through the chain of command. (c) Actively engage in a continuing program to highlight and publicize the achievements of command through all current and applicable mediums to include Navy.mil, Navy and Marine Corps Medical News, military installation newspapers and local press, and the Fleet Home Town News Program. (d) Operate and maintain an internal public affairs program to keep staff members and their family members fully informed of command activities and overall Navy policies and programs. (e) Maintain an active involvement with local civic organizations, medically related societies, and other interested groups. (f) Coordinate media visits and other public affair events. (g) Review and certify as releasable all command website information. (h) Manage command visual information program with oversight responsibility for video products, photographic imaging and all associated written information. (7) Safety Manager. Principal advisor to the CO on safety and occupational health matters and aggressively manages the Navy Safety and Occupational Health (SOH) Program per applicable Department of the Navy (DON), DoD, and federal directives. The safety manager reports to the CO, for matters regarding safety and to the XO for matters relating to the administration of programs and the correction of deficiencies. All hands have direct access to the safety manager. The following core responsibilities are provided below. (b) Plan, conduct and document workplace inspections of all buildings, grounds, facilities, materials, equipment, devices, operations and conditions to ensure compliance with applicable policies, laws, regulations, and standards. (c) Provide consulting services to activity organizational elements and all levels of supervision on safety principles and technical aspects and their application to employees and workplaces. (d) Coordinate the investigation of all mishaps. (e) Implement requirements and procedures for employee hazard reporting. Manage the program for the correction of workplace hazards. (f) Prepare annual self-evaluation(s) of program and program elements following Program Review and Measurement System Self-Assessment Model guidelines. (g) Coordinate SOH program training and educational programs. (h) Periodically monitor the Navy‘s Medical Surveillance Programs. (i) Collaborate with all directors, department heads, and department safety representatives in safety matters and safety program management. (j) Ensure Enterprise Safety Applications Management System, the standard data management system is used to record, monitor, assess, and measure safety programs, is implemented and used to record safety actions throughout the command. (8) Security Manager. Principal advisor to the CO and direct representative in matters regarding the eligibility of personnel to access classified information and to be assigned to sensitive duties. The security manager reports to the CO, on matters of security and to the XO for administration of the information and personnel security programs. Security Program and the DON Information Security Program. The following core responsibilities are provided below. For a complete description of the Security Manager’s responsibilities and authority, refer to SECNAV M-5510.30 and SECNAV-5510.36. (a) Develop written command information and personnel security procedures, including an emergency plan with provisions for safeguarding classified information during military operations or emergency situations. (b) Formulate and coordinate the command's security awareness and education program to include regular or special briefings for all command personnel, as described chapter 4 of SECNAV M-5510.30. (c) Ensure security control of visits to and from the command when the visitor requires, and is authorized, access to classified information. (d) Ensure that all personnel who will handle classified information or will be assigned to sensitive duties are appropriately vetted through coordination with the Department of Defense Consolidated Adjudication Facility – Navy Division and that requests for personnel security investigations are properly prepared, submitted and monitored. (e) Ensure that access to classified information is limited to those who are eligible and have the need to know. (f) Ensure that personnel security investigations, clearances and accesses are properly recorded in the Joint Clearance and Access Verification System (JCAVS), and that subordinate commands are properly registered for JCAVS, as necessary. (g) Serve as the principal advisor and representative to the CO in matters pertaining to the classification, safeguarding, transmission, and destruction of classified information. (h) Coordinate the command program for continuous evaluation of eligibility for access to classified information or assignment to sensitive duties.

(i) Coordinate with the command IAM on matters of common concern. (j) Ensure that all personnel who have had access to classified information who are separating or retiring have completed a Security Termination Statement. (k) Ensure all personnel execute a Classified Information Nondisclosure Agreement (SF 312) prior to granting initial access to classified information and record execution of the SF 312 in JCAVS. (l) Ensure that all security violations or incidents involving the possible compromise of classified information are investigated and reported. (m) Provide advice and guidance to subordinate command security managers in the conduct of inspections, assist visits, program reviews, and for the completion of tasks and reports as may be required by a higher echelon. 2. Director General Responsibilities. The directors, subject to the orders of the CO and XO, shall: a. Exercise such authority with respect to NMCPHC as the CO may delegate. b. Ensure that the directorate is properly organized and up to date on required training for all personnel and up to date on medical readiness for all military personnel. c. Prepare and maintain the instructions and notices for the organization and operation of the directorate. d. Assign personnel to duties within the directorate. e. Be responsible for the effectiveness of the directorate. Plan, direct, and supervise the work and training of directorate personnel. f. Ensure the preparation, promulgation, and execution of pertinent directives, plans, and other work products as well as ensure they are prepared and executed in conformity with the CO’s policies and intentions. g. Assume responsibility for the proper operation, maintenance, preservation, custody, and inventory of equipment and other material assigned to the directorate. i. Keep the PAO informed of activities for assisting in effective public affairs activities. p. (13) Materiel Management Division Functions (a) Direct, manage, coordinate, and provide guidance and expertise on material management, supply operations, and procurement management for the command and field activities. Environmental Programs Department Functions (1) Execute two Inter-Service Support Agreements between the Chief, BUMED and Commander, Naval Facilities Engineering Command (NAVFACENGCOM/Base Realignment and Closure (BRAC) Program Management Office (PMO)) and U.S. Fleet Forces Command (USFFC) to provide a wide range of public health services including health risk assessment, risk communication, toxicology, health and safety consultation and other public health consultative services supporting Navy and Marine Corps medical and environmental programs. Specific support for the Department of the Navy (DON) Environmental Restoration (ER) Program is conducted in accordance with OPNAVINST 5090.1D and OPNAVINST M-5090.1. (2) Interact with other federal agencies and military services to develop procedures, policies and activities pertaining to health and environmental risk communication, public health, risk assessment, and training. (3) Medical Risk Communications Functions. Provide expert-level risk communication training and technical assistance on environmental and public health issues for the Navy and Marine Corps and other DoD activities. This effort includes: (a) Develop risk communication strategies and products to communicate environmental, and public health issues to potentially impacted military and civilian communities with multimedia communications at public availability meetings, Restoration Advisory Board meetings, and other interactive forums. (b) Provide consultative assistance to the Office of the Under Secretary of Defense (e.g., Acquisitions, Technology & Logistics), Deputy Assistant Secretary of the Navy (e.g., Installations & Environment), CNO (N45), BUMED, CNIC, NAVFACENGCOM, BRAC PMO, or installation commanders regarding appropriate risk communication techniques and protocols to PAOs, remedial project managers for national priorities list (NPL) and BRAC sites, and other internal DoD and external customers to ensure that Navy and Marine Corps communications to the public regarding health and environmental risk are appropriate and maximally effective.

(c) Develop and provide general and topic-specific training tailored to specific Navy and Marine Corps projects and project teams, as well as cooperative training with the Civil Engineering Corps Officers’ School, contractors and others to help ensure competency and awareness of public health and environmental communication practices and principles to effectively communicate with the public, the media, regulators and other stakeholders. (d) Support Navy Medicine, in effectively communicating results of disease cluster investigation, pandemic influenza and other crisis situations, and occupational health issues. Activities include: 1. Review draft public health assessments, health studies, and other ATSDR draft public health assessment documents. Provide expert consultation for health and safety programmatic issues for environmental programs to include support for chemical specific remediation under the Comprehensive Environmental Response, Compensation, and Liability Act, such as asbestos and lead in soil; vapor intrusion issues; and construction activity on or near hazardous waste site. 3. Provide industrial hygiene and medical expertise to activities during spill events and other environmental emergencies.

(5) Reimbursable Risk Communications Functions. Functions for this division are identical to those listed in Medical Risk Communications Division. (6) Risk Assessment and Toxicology Functions. Provide expert-level risk assessment and toxicology support to the Navy, Marine Corps and other DoD organizations related to potential health impacts posed by releases or potential releases of hazardous substances such as chemicals, biologicals, and disease vectors into the environment to assist with risk management decisions. This effort includes: (a) Assess human health risk in accordance with acceptable protocols such as those developed by the U.S. Environmental Protection Agency (EPA) and U.S. Public Health Service by assembling and evaluating site-specific sample data and calculating estimated human health risks to specific contaminated media. (c) Provide health/medical evaluation of the various cleanup program documents including risk assessments, work plans, sampling plans, remedial investigation documents, quality assurance plans, and health and safety plans as requested by NAVFACENGCOM, BRAC PMO and others. (d) Recommend actions to reduce human exposure and eliminate or mitigate risk in situations where health risk assessments indicate increased or unacceptable risks. (e) Provide support to the Office of the Under Secretary of Defense (Installations & Environment) by monitoring the status of the EPA and other federal and state agency chemical toxicological studies and methodologies that can impact: the development of toxicity values used to establish site-specific risk assessments and occupational health standards; military acquisitions; and military operations. (f) Provide consultative support involving quantitative human health risk assessments and chemical toxicity to Navy and Marine Corps facilities and program offices and

Specific Responsibilities. The Director for Population Health, subject to the orders of the Commanding Officer will perform the general responsibilities delineated in section 3. c. d. EpiData Center Department Functions (1) Serve as the designated conduit for all Navy Medicine reportable medical event data transmitted to the Armed Forces Health Surveillance Center. (2) Serve as one of the DoD Pandemic Influenza Surveillance Centers, tasked to develop influenza surveillance tools using Health Level 7 data streams. (3) Serve as the DON communicable disease surveillance center to provide senior medical and preventive medicine officers with periodic and special trend analysis reports of communicable diseases and conditions that may impact operational readiness including malaria, tuberculosis, sexually transmitted infections, and upper respiratory illness. (7) Monitor and report analysis of indicators that impact hospital risk management including but not limited to patient and healthcare worker safety, hospital-acquired infections, and antibiotic resistant microorganisms. Such programming includes, but is not limited to, health promotion calendar of events, linkage to national health resources, supporting tools and resources for field implementation, and supporting health promotion excellence through award recognition. 6. Preventive Medicine Directorate

Programs and Policy Support Department Functions (1) Serve as the DON’s medical surveillance hub to determine baseline medical surveillance and reporting requirements, guidance, and procedures. (d) Maintain a list of the holders of U.S. Public Health Service seals provided by the CDC for issuance of Navy Ship Sanitation Certificates (SSCs) to specific Navy unit identification codes.

(e) Provide CDC a listing of ports where authorized agents may issue certificates including periodic updates of the list, as necessary. (f) Maintain a database of all SSCs issued by authorized Navy inspectors. (c) Conduct training for the Navy’s DRSi user community. (14) Work with stakeholders to develop risk communication strategies and products that meet the needs of the DON and are in full agreement with state and federal environmental health and public health laws and regulations. (15) Provide support to emergent risk communication needs to ensure communications are appropriate and maximally effective. (16) Provide technical review of operational risk management matrices and other associated documents to ensure compliance with the Food and Drug Administration, EPA, CDC other pertinent requirements. (17) Provide environmental health and preventive medicine technical support, training, guidance and evaluation in support of Naval hospitals, clinics, research and dental facilities. (18) Assist Naval activities with unique environmental health and preventive medicine matters. (19) Provide professional support and assistance to medical department and operational Navy planners to develop doctrine for applying surveillance, food safety, water safety and preventive medicine principles and practices to planning and mission requirements. (20) Work with the EDC and other epidemiology experts in the DoD to conduct meaningful studies and use statistical analyses of existing databases to identify and monitor trends in disease and injury, and provide reports to headquarters and activities upon request.

7. Laboratory Services Directorate

a. Mission. Enhances force health protection and readiness by supporting the DoD Drug Demand Reduction Urinalysis Program, the DON Bloodborne Infection Management Program. b. Specific Responsibilities. The Director for Laboratory Services, subject to the orders of the CO will: (1) Perform the general responsibilities delineated in section 3. (2) Provide input to the fitness report submissions of the three NAVDRUGLABs COs. (3) Be appointed the Navy Drug Testing Program Manager. c. Provide administrative and technical oversight of the three NAVDRUGLABs. Function as the Navy’s ultimate technical authority on drug testing matters for the Navy and Marine Corps. (1) Provide professional guidance and assistance to the NAVDRUGLABs regarding implementation of policy, resolution of complex technical problems and interpretation of the scientific literature relating to drug testing. (2) Provide direction, coordination, and oversight for program-wide research projects.

(3) Evaluate new testing methodologies and equipment that may be used to update and improve the efficiency and integrity of drug testing. (4) Provide all aspects of quality assurance monitoring for the Navy Drug Testing Program. Conduct technical inspections of the laboratories with representatives from DoD, Army, Air Force, Armed Forces Medical Examiner System, OPNAV (N17), Office of the Judge Advocate General and contracted experts in forensic toxicology. Monitor external proficiency testing programs. (5) Provide oversight of DoD technical evaluation committees. Provide support to the DoD Drug Testing Program Contracting Team at the Naval Medical Logistics Command on major equipment procurement, maintenance, and supply contracts. Seek cost-savings initiatives, standardization, and adoption of best practices. (6) Liaise with the Director, Drug Testing and Program Policy in the Office of the Under Secretary of Defense for Personnel and Readiness, Armed Forces Medical Examiner System, BUMED, Chief of Naval Personnel (Alcohol and Drug Abuse Prevention), Deputy Undersecretary of the Navy (Counter Narcotics), Headquarters Marine Corps (Drug Demand Reduction), and other civilian and military agencies on matters regarding drug testing. (7) Represent the Surgeon General of the Navy as a member of the DoD Biochemical Testing Advisory Board. d. Provide centralized oversight and support of the NBIMC including resource needs and measures to ensure efficient, effective, and compliant operations.

8. Resource Management Directorate

a. Mission. To optimize the use of resources within NMCPHC and its field activities through the formulation of effective financial and resource management principles, policies, and procedures. b. Specific Responsibilities. The Director/Comptroller for Resource Management Directorate, subject to the orders of the CO will: (1) Perform the general responsibilities delineated in section 3. (2) Provide technical guidance, advice, and detailed reporting of financial matters throughout the organization. (3) Perform fund and property management responsibilities as the designated fund administrator for the command in accordance with 31 U.S.C. 1517 and volume 14 of DoD 7000.14-R. (4) Formulate and prescribe procedures and systems to exercise effective control over the fiscal issues and resource management concerns relating to NMCPHC and its field activities. (5) Justify and ensure optimum use of financial resources to effectively deliver NMCPHC’s public health services. (6) Develop and maintain an integrated fiduciary system that is both accurate and responsive to BUMED.

(7) Advise the CO and NMCPHC principals on the financial effects of proposed directions and policies of higher authorities. Recommends position on proposals; develops counter proposals, if appropriate; and recommends the most effective means of implementing approved directions and policies. (8) Represent the CO on all financial matters at levels including, but not limited to BUMED, and other Uniformed Services. (9) Support NMCPHC with resource information to assist in reviewing and assessing existing programs and developing emerging programs. (10) Advise and assist field activities with resource management issues or concerns. (11) Issue annual planning figures and allocates funding to subordinate activities. Monitor resource execution. (12) Integrate planning and programming into the budget process by translating manpower, material resources, and facility requirements into phased financial resources. c. Financial Management Department Functions (1) Provide financial management and direction in all fiscal operations including accounting, budgeting, and payroll. (2) Prepare program and budget estimates, special budget exhibits, and justification for Operation and Maintenance, Defense Health Programs; Operations and Maintenance, Navy; and other appropriations issued by higher authority. (3) Prepare program objective memorandum (POM) guidance and perform technical POM issue paper preparation and review in full consultation with NMCPHC/BUMED leadership. (4) Receive and review requests for adjustment of resource authorizations and coordinates with program managers for recommendations. Issue modifications of resource authorizations as appropriate when directed by the Comptroller.

(5) Conduct research on resource management policy, legislation, and regulatory matters. Recommend appropriate action when circumstances warrant. (6) Provide oversight of various budget and accounting tools and systems used by NMCPHC and its field activities. Maintains program and budgetary control records. (7) Prepare financial information for use by the Comptroller in determining command resource execution in relation to planning figures by program. Provides periodic status reports on execution versus the financial plan. (8) Conform to DoD standards for accountability and separation of duties among accountable officials and ensure internal controls are fully established and maintained. (9) Initiate appropriate action when there is noncompliance with a statutory regulation pertaining to the obligation or expenditure of funds. (10) Develop, manage, and maintain an automated system for the accumulation, validation, and reporting of detailed cost elements used in performance, execution, and trend analysis of all levels of NMCPHC operations. (11) Provide budget and accounting direction to NMCPHC leadership and to the field activities based on the NMCPHC Business Plan major product lines and departmental products and services. (12) Provide guidance and instruction for annual budget preparation and review the resource requirements and justifications of various programs of the NMCPHC and field activities. (13) Oversee the development and consolidates NMCPHC and field activities’ annual budgets for submission to higher authority. (14) Distribute available funds and recommend reprogramming of funds to accomplish changes to mission requirements.

(15) Serve as the SME for resource management issues during command inspections of subordinate activities. (16) Monitor adherence to statutory regulations as prescribed in Assistant Secretary of Defense-Health Affairs (ASD-HA), DON, and BUMED financial guidance and evaluates the effectiveness and adequacy of the general financial reporting environment. (17) Coordinate and monitor execution of civilian full time equivalent work years for NMCPHC and the field activities. (18) Liaise with Defense Finance and Accounting Services to provide current pay information and issue resolution for military and civilian staff. (19) Operate and maintain the field-level accounting system used to collect and report obligations, expenditures, and other related financial data for NMCPHC operations and centrally managed programs. (20) Monitor special interest financial programs including the government credit cards, electronic billing, travel and transportation accounting. (21) Monitor adherence to statutory regulations as prescribed in ASD-HA, DON, and BUMED financial guidance, and evaluates the effectiveness and adequacy of the general financial reporting environment. (22) Oversee the implementation of timekeeping procedures and manages the processing of payroll for civilian employees. (23) Review all requests for overtime, compensatory time, and awards and personnel actions for civilian employees. (24) Manage the Standard Labor Data Collection and Distribution Application financial system and the Defense Civilian Payroll System. (25) Monitor all civilian pay programs for compliance with guidance.

(26) Collect and reconcile fund transactions of assigned expense operating budget and related operating targets in meeting the reporting requirements of higher authority. (27) Review requests for financial data from agencies outside of NMCPHC and provide data submissions when appropriate. (28) Oversee and facilitate the development and tracking of NMCPHC’s Business plan and performance metrics. (29) Facilitate the product line development through planning initiatives that result in the development of a detailed alignment of projects to product line, measured within the Surgeon General's guidelines for strategic initiatives. (30) Manage the support agreements program for NMCPHC and its field activities. (a) Develop, negotiate, review, and submit for approval proposals for new and amended intra and inter-agency support, training, and research and development. (b) Conduct annual reviews of reimbursable and non-reimbursable agreements. (c) Identify, coordinate, and resolve impediments to successful implementation of agreements between parties and approval authorities. (31) Manage the official temporary duty travel program for NMCPHC and its field activities. (a) Ensure all temporary duty travel requirements comply with agency, service, and major command travel policies and procedures. (b) Maintain accountable official appointments, training records and plans. (c) Manage preparation of travel documents and reports. (d) Provide assistance in submitting travel requests, arranging travel requirements, obtaining funding, and timely completion of travel claims.

(e) Manage the identification, notification, and collection of traveler debt. (f) Assist travelers in preparing official no-fee passport and visa applications where needed. (g) Maintain a system of internal controls that enforces: 1. Properly appointed and trained personnel perform accountable official duties. 2. A reviewer who is separate and distinct from the certifying/approving official for each document that approves travel or obligates funds have. 3. Regular review of available reports to prevent accounting errors, delayed vouchers, and collection of traveler debt. (32) Administer the Defense Travel System (DTS) for NMCPHC and its field activities. (a) Provide local level system support to accountable officials and travelers. (b) Maintain permission levels and accesses to enforce separation of duties and protect Personally Identifiable Information. (c) Maintain organizations, routing lists, and groups. (d) Process rejects from accounting and disbursing. (e) Maintain lines of accounting and budgets. (f) Update personal profiles. (33) Serve as the Hierarchy Level 5 Government Travel Charge Card (GTCC) Agency/Office Program Coordinator (A/OPC). (a) Ensure all travelers possess a GTCC. (b) Maintain training records and statements of understanding.

(c) Ensure all A/OPCs and GTCC holders complete mandatory refresher training. (d) Use the GTCC contractor's electronic account access to monitor card usage to prevent delinquencies, fraud, waste, and abuse.
