Welcome!

Welcome to the Lac La Biche Dance Society! We are pleased you are joining us and wish you an exciting and rewarding year! The exceptional quality of our program is due in part to the hard work of our students and our parents - you are an important part of our success. This handbook has been designed to assist you with procedures and events throughout the year. The content is based on information taken from the Lac La Biche Dance Society’s handouts from previous years, as well as information and guidelines from the Edmonton School of Ballet and Athabasca Dance School. Please read it carefully, keep it handy, and refer to it when you have questions.

Mission Statement of the Lac La Biche Dance Society “Promote dance within our community by providing students of the dance club with a strong foundation in dance fundamentals through the employment of instructors who are qualified to teach in their chosen

discipline.” We are a dance society run by a volunteer board of executives, committees and volunteers. Information & Newsletters- It is your responsibility to inform yourself about the goings on of the club. Mail for each student will be available in a file box that the Instructor will make available at the beginning of each class. Parents/ Students will be responsible for checking and picking up their own mail. Expect to see monthly newsletters, fundraising information, attendance slips, etc in this box. The Dance Society will provide a monthly newsletter. It is the responsibility of all parents to read each newsletter as they contain vital information. Questions- If you have a question and are unsure of where to find the answer you can contact your Day rep or email the society at [email protected] and your question will be answered as soon as the manager is able. If you need to speak to an instructor, please contact them via their preferred contact method as per the instructor’s information. Grievances- Should you want to make a grievance or concern known you must compose a letter to the President of the Society. The Executive will not acknowledge any spoken or second-hand grievances. Letters can be emailed to [email protected] or Attn: President, The LLB Dance Society Box 2244 Lac La Biche, AB T0A 2C0. Registrations- Registration fees must be paid by one of the three options provided on the registration form before classes start or BEFORE THE STUDENT PARTICIPATES IN A CLASS. Please note that no student will be admitted to class unless the manager has received the completed registration form, including signed waivers, and all required cheques. Instructors will not be accepting registration forms/ payment. Any registration forms/fees are to be mailed directly to the Manager, Lac La Biche Dance Society, Box 2244, Lac La Biche AB T0A 2C0. A service charge of $25.00 will apply to all NSF or returned cheques. Kidsport - Registration fee subsidies may be available to families who meet eligibility criteria for Kidsport. You could eligible for up to $200 per child per year. Contact the LLB County office at 623-4323.

Fees and Cheques - As a non-profit society we strive to keep our fees low, therefore we ask our dancers/parents to both volunteer and complete fundraising activities throughout the year. Cheques are required in the following amounts as commitment to these activities. These cheques should be undated (no date on cheque).

These cheques serve as an insurance policy for volunteerism, to keep the club running smoothly. The fundraising cheques will be returned upon completion of the required amount of fundraising (see fundraising section). The volunteer cheque will be returned upon completion of your chosen volunteer commitment (for example helping at the year end show, pictures, a fundraising event etc.) If you choose to write “cash my cheque” on your cheques, they will be cashed in September. If you are unable to participate in volunteer or fundraising activities throughout the year but did not let us know to cash your cheques, they will be cashed in May. In the event you do not show up for a year-end commitment, a bingo you have signed up for, or a fundraising event you have committed to volunteering at, your cheque will be immediately cashed. The club endures fines from the Indoor Sports Association if our volunteers do not show up for bingos and it is an embarrassment to the club. When and if this occurs the club will pass the ensuing fines on to the parent that was responsible for the fine. Fundraising - The Dance Society regularly completes several fundraisers each year. Some examples of fundraisers from the past include bingos, barbeques, raffles, Every Day Style, Tupperware, Chocolate, Stawnichy Sausage, bottle drives… We welcome new ideas for fundraisers! Please email the club at [email protected] with your suggestions. We also invite all local businesses to approach the club with their suggestions as well. Don’t want to complete fundraisers? You can volunteer for the Executive- tier one positions (President, Vice President, Secretary, Treasurer, Director At Large, Community Liaison, Grants & Donation Coordinator); which will earn you back all of your fundraising cheques. Tier two positions (Day Rep, Committee members, Campaign managers) will earn you back one cheque (once you have fulfilled your commitment). When selling goods for a fundraiser you can deliver your monies directly to the manager, campaign managers, or at the designated venue (if one applies to the particular campaign). If you can’t deliver the monies at the scheduled time, please call the manager to make arrangements - do not give money to the instructors. Once all fundraisers have been chosen by the executive, you will be informed

about the fundraisers chosen and when they will be run. This way you can choose the fundraisers you would like to participate in. You do not have to participate in every fundraiser. We have several fundraisers to give everyone choices of what they would like to sell. However, the more money raised with the fundraisers, the easier it is for us to keep fees low. If you are unable or would prefer not to do any fundraising, please let us know you would like us to cash your two $50 fundraising cheques. Each family is expected to sell a minimum of $200 of fundraising products. The $200 can be a combination of many fundraisers or all from one. You may also choose to work at a Bingo which counts for half of your fundraising and earns you back one fundraising cheque. Examples: Returned Cheque(s) $100 sold on one or more fundraisers = $50 Fundraising cheque Completed Bingo = $50 Fundraising cheque Join and complete term as Executive = Two $50 Fundraising cheques Committee, campaign manager, day rep = $50 Fundraising cheque

Costumes are required for year-end recital (unless enrolled in a non-performance class) and additional costumes are required if performing a solo, duet or small group number. The costume deposit for each class is $50. If you are performing a solo, duet or small group number, an additional $50 deposit will be require before costumes are ordered. Withdrawals - Withdrawals will only be accepted in writing mailed to the dance society at the above address postmarked by September 30, 2010. Written withdrawals will only be accepted after this date for the following reasons: moving (change of address proof required) or medical (Dr’s note will be required). If the dancer does not attend class and proper withdrawal notice is not given, the fees and volunteer commitment cheque still apply. Withdrawals after September 30, 2010 but before December 31, 2010 will be responsible for one fundraising commitment. Any withdrawals after December 31 are responsible for full fundraising commitment. Once costumes are ordered you will be responsible for all costume expenses. Instructors and executive will not accept any withdrawal verbally.

Absence/ Attendance Policy - Attendance at each and every class is imperative for the dancer to reach his or her potential. Missed work is difficult or impossible to regain due to the physical and artistic nature of dance. All students are required to attend all rehearsals and performances. Students and their parents are expected to be responsible in this regard. Dance is a discipline and a responsibility. It is your choice as to whether or not you want to be involved but, if you do make that commitment we expect you to follow through. Please show your respect to your teachers, the dance and especially your group! Parents will be informed, either by the instructor or executive, in the event that student absences become a concern, usually if the student misses 3 classes. Please be sure that you call or email your instructor if you or your child is unable to attend class for any reason.

Class Times – You will be called at the beginning of the year with a day, time and start date for your child’s class. You will be informed of any changes to your child’s class assignment or schedule. A student may be moved to a different level during the first few weeks of class, at the Instructor’s discretion (see below). All dance students have until September 30th to decide whether they wish to continue- after that NO refunds will be made. Please see Withdrawals section for more information. Artistic Discretion - The Instructors, in collaboration with the manager, make all decisions with respect to assignment of class levels, eligibility for exams, readiness for Pointe work, and roles in performances. For example, a student may be moved to a different class because their training and progress would benefit from such a move. Also, since each student develops differently due to stages of bodily development, coordination, level of interest and ability, advancement is not automatic and each student’s progress from year to year will be assessed with considerations given to all of these factors. Class Cancellations/ School Closures - The Lac La Biche Dance Society will publish class closures for holidays, etc. in the monthly newsletters. These closures will vary with instructor and program. Closures due to inclement weather, highway conditions, or illness will be announced on our website and facebook group. Attempts will be made to phone dancers, but due to time constraints or volunteer capacity, this may not always be possible. If you are uncertain of whether there will be

class that day and do not have internet access it is YOUR responsibility to phone your Day rep, instructor, or fellow parents/students to find out whether or not there is class. Solos, Duets and Groups - Near the beginning of the dance season dancers may inquire about performing a solo, duet or small group. These opportunities will be offered only at the discretion of the instructor. Ages six and up only can perform solo. Should you choose to take this opportunity you will be paying for extra instruction time with the instructor- this is payable directly to the instructor. The club will have standardized costs for said instruction: $140 for a solo, duet are $100 each, trio $75 each- this gets you 3 sessions, possibly more should venues become available. Separate costumes are also required when performing a solo, duet, or trio- dancers are also responsible for this cost. There will be a $50 costume deposit for each solo/duet/trio required before the costume will be ordered. Should the student choose to perform their solo/duet/ trio at festival there are entry fees that apply that the student must pay before their entry to festival is submitted. Performance Opportunities - Students in performance classes are expected to know and perform the choreography with accuracy. Students should strive to meet the technical and artistic requirements of the piece. Students may be removed from choreography at the discretion of the instructor at any time. Please be aware that practice by oneself and knowledge of the choreography is not necessarily sufficient to perform. Just like team sports or other group activities, dance requires group performance to achieve excellence in timing, spacing, staging, and emotion by every performer. If a student requires extra attention to achieve excellence in choreography that student may purchase extra lessons from the instructor at their own cost. Please note that regardless of past participation, students who are unable to dance full out or are absent from final rehearsals, dress rehearsals, and/or stage rehearsal will not be permitted to perform at the year-end recital or festivals, whatever is applicable at the time. This is for the professionalism and safety of all involved. Public Performances - Individual or small groups of dancers may be asked to perform locally during the dance year at Lacalta Lodge, Hospital, Heritage Centre, Schools, Fundraisers, etc. Notes will be sent to parents on place and time if your student is participating. PLEASE NOTE: Students wishing to perform any dances acquired through instruction received by our dance instructors must seek their approval in order to perform these dances outside of the Dance Society agenda. Further, it is a courtesy to discuss with the instructor any classes/ workshops that the student may wish to attend so that they might provide feedback to you with regard to those programs. Festivals - Festival offers an opportunity for dancers to recognize and acknowledge other dancers, teachers and choreography. The dancers are learning from others while challenging themselves which is a valuable experience. Focus will not be placed on marks. At festival we want our dancers to try their best, represent the Lac La Biche Dance Society with their impeccable behavior and most of all HAVE FUN! “Healthy competition is a way of being on top of one’s own progress, to better one’s self. Do not improve in order to win over someone else. Improve to win over yourself.” Age six and up will attend festival upon instructor’s discretion. The club pays for group entry fees to the first two festivals of the year. The first festival we will be attending is in Athabasca which takes place April 8th – 10th. Our second festival will be announced once it is chosen. Once we have registered the groups we do not receive a refund if dancers do not go. If you are unable to commit to the festivals, please contact the manager so we can register accordingly. Registrations for festivals are done in

November. Dancers wanting to perform a solo/duet/small group at festival(s) need to pay the fees before entries are sent it. Please contact the manager for fee amounts and to arrange payment. Year-End Recital - The Club performs a year-end recital for the community showcasing each class’s efforts throughout the year. Due to the large growth our club has seen over the last few years, changes need to be made to our year end show. After it is discussed and at the general meeting, you will be updated regarding our 2011 Recital dates. Tickets will be made available to each family to purchase before tickets go on sale to the public. Students must purchase a ticket in order to view the performance. Exams - The instructors may recommend students for exams toward the end of the dance year (ballet program only). Students will be provided with study material and hair and apparel requirements for the examinations. There is an exam fee, payable by the student/ parent, which must be paid in full prior to the student taking the exam or the student will not be permitted to participate. There will also be additional dance fees to cover extra practice classes. Instructors 2010-11 - We are fortunate to have some of the most qualified teachers in the province to work with our students. Each instructor brings unique talents to our club, allowing students to explore various areas of dance with skilled instructors. Our Instructors are in Lac La Biche to teach our students DANCE. Please treat them with respect. Parents are asked not to disrespect any decisions made by Instructors and Executive. The Instructors are not to handle any dance business- no fundraising monies, fees, etc. Please see the fundraising section or your fundraising contact person for information on how to hand in monies. Please welcome Donna Pederson, Megan Horn, Melissa Horn, Fatima Elladan, and Melissa Staroszik. Please see Instructor Bios for more information. Executive 2010-11 - The dance society is run by a volunteer group of executive. Each year this executive must be re-elected. Positions on the executive include: President, Vice President, Treasurer and Secretary, Director At Large, Community Liaison, Grants & Donation Coordinator. Other volunteer positions include, but are not limited to, Year-end committee, Costume Manager, Day Reps and Fundraising Campaign Managers. In order to serve on the Executive, volunteers must sign a confidentiality agreement to protect our dancers and parents financial and personal information. Executive members must also complete both criminal and child welfare checks as they will be working with both money and children. This is for the safety and wellbeing of all involved. Involvement on the executive includes committing to monthly meetings, weekly (if not daily) emails and actively participating in not only the role you volunteer for, but whatever “needs to be done”. The following is a brief (non-inclusive) description of the executive positions:

President- calling and chairing monthly meetings, as well as 1-2 general meetings per year. Organizes the work of the dance club to ensure everything gets done that needs to be done. Maintains ongoing communication with instructors and staff, organizes staff-executive meetings, hires new instructors, and renews and negotiates employment contracts with instructors and staff. Brings forward parent concerns to the executive. MC the year end recital or find someone who is willing to.

Vice President- chairs meeting in the president’s absence. Acts as an advocate for the dance society in the community. Assists with organization and completion of workload.

Treasurer- reviews work of manager. Signs cheques and reviews payments. Presents treasurer’s report for monthly meetings. In collaboration with the Executive and manager, prepares budget. Assists with organization and completion of workload. Audits books when needed.

Secretary- takes minutes of each meeting. Coordinates travel arrangements, accommodations, etc for instructors. Assists with organization and completion of workload.

Grants & Donation Coordinator – helps the manager to find and apply for grants. Helps to find donations for fundraising events etc. (Internet access is a must.)

Community Liaison – a volunteer to further the dance society’s involvement with the community, to initiate programs and events involving the dance society that would benefit the community at large. Ex. Dancing at the lodge.

Directors At Large – attends monthly meetings to help make decisions, share ideas etc. Assist with organization and completion of workload.

Committees and some other specific positions involve less time than executive positions and are therefore considered tier two positions (worth one fundraising cheque). These positions only have to attend meetings where their business is being discussed, not monthly meetings. The following volunteers are needed:

Year End Committee- responsible for the successful production of the dance society’s year-end recital. Duties include, but are not limited to: Prepare the year-end volunteer sign up sheets. Work with the executive to book venues. Book sound and lighting technicians. Get mics and equipment out of storage; ensure batteries are ready for technical rehearsal. Arrange for fans in dressing rooms. Coordinate the design, format, and printing of the program. Coordinate the design, format and printing of tickets for all performances.

Costume Manager- assist the general manager by obtaining measurements of students for costumes, assists with the distribution of costumes upon arrival. Assists with organization and completion of costume workload.

Fundraising Campaign Managers- assist the manager with the massive workload of organizing and completing fundraising events and campaigns. Campaign managers will be paired up so we require two managers for each fundraiser.

Special Events Committee (to be formed just prior to events) – Postings will be put up requesting volunteers to sit on sub-committees, along with the executive and manager to organize special events. Will be responsible for participating in the organization of any special event throughout the year.

Dance Reps - may attend executive and general meetings. They are the “voice” for their dance night’s parents and dancers. Obtain class lists from Manager and provides a class phone list to each class phone rep. They complete a “master list” of all the phone parents for their night and provide copies for the manager and instructors. Help the manager make supervision rosters. Calls class phoning parents when parents need to be phoned, eg. To notify them when classes have to be cancelled or venue changed. Acts as a liaison with parents and president when there are concerns or problems that need to be addressed. The dance reps are the first point of contact if you have a concern! We will need one Dance rep per instructor per night.

Other volunteer positions include, but are not limited to,

Phone parents - to make phone calls to parents when classes are cancelled, or at the request of the president, manager, day rep or instructor. (1 per class)

Injury Policy - The club takes the prevention of injuries very seriously. In the event that a student becomes injured, careful thought must be given to the management of the problem. It is extremely important that the dancer seek qualified medical assistance promptly to ensure that acute injuries do not become chronic problems.

It is expected that students will follow their health professional’s advice carefully. Students should participate in class as much as possible and then continue with their rehabilitation methods/ exercises in class. If a student is not able to participate in class they should provide a note explaining why from their parent or doctor. Students are still expected to attend all of their classes. Students should attempt to not simply view the class- they will be required to participate in whatever way they can. Injured students must still wear their dancewear to all classes. Students who are injured or ill have the added responsibility of staying in shape and learning choreography while they cannot participate in class. Any student who is chronically ill or injured may have an altered role or be removed from the class’s choreography on the instructor’s discretion. Parents and students will be notified if there are concerns about progress or performance opportunities. Parent Expectations General Parents must escort their children inside the building and ensure that they arrive safely at their class. Please wait with your child until they are in their class before leaving the building. Instructors will bring out the student’s mailbox folder at the beginning of class and parents are responsible for checking and picking up their mail from these boxes. Parents are asked to leave the classroom immediately after preparing their children/ child for class. Parents are not permitted to observe regular dance classes, as this can be very distracting to students and instructors alike. The Dance Society offers two opportunities for parent viewing- one in November and one in April- during which parents are welcome to sit and observe their child’s class. Discipline If a student receives a discipline slip they cannot return to class until said slip is signed by the parent (if student is under 18) and returned to the instructor. Supervision A parent supervisor is required at all classes (with the exception of adult classes) to ensure the safety of our dancers. A parent supervisor is required to guarantee that no unauthorized persons enter the building, that no students leave the building unattended, that all students are picked up from class, if a student is injured there is help for the instructor, etc. The manager or Day rep will draw up supervision rosters after registration, please watch for your schedule in your folder. If you cannot perform your supervisor duties for the day it is YOUR responsibility to find a replacement. Theatre Etiquette Please remember when watching your child’s performance several important factors help everyone enjoy the show. Turn off all cell phones and pagers. Do not enter or exit during the performance and do not move around the theatre. Please stay and enjoy all the performances so that each student in our school has an audience for their dance. Please be as quiet as possible during the performance. These small points educate our students on theatre etiquette and make for a positive and enjoyable dance experience. There will be a zero tolerance policy for violence, threatening, cursing or vulgar language used towards instructors, dancers, the manager, other parents, executive and any other volunteers. Everyone in and involved with our dance club needs to be treated with respect. The dancer will not be removed from the club but the parent will be banned from all dance activities including but not limited to the building where classes are being held, festivals, the building where the year-end shows are being held, extra performances, pictures etc. You will have to find someone else to help with your child. If you choose to remove your child you will be responsible for all fees, costumes, fundraising and commitments.

Student Expectations - The Lac La Biche Dance Society expects students to behave professionally at all times. This includes being considerate and respectful of their peers, instructor, volunteers and parents as well as facilities and equipment at all times. Classes When an instructor enters the class, students should be ready to start. The students should not talk in class. Water bottles and towels may be brought into class and used at appropriate times. Students should finish the class with a thank you to the instructor and pianist (if available). When a guest enters the room and is introduced, students will greet them appropriately and treat them with respect. Entry and Washrooms Students should take pride in the facilities by putting garbage away, keeping clothes in bags, and maintaining a neat hallway and washroom. Quiet, well-mannered behavior is expected at all times. Inappropriate language, yelling, or running is prohibited. Performances and Festivals When attending or participating in festivals or performances students are acting as representatives of the Lac La Biche Dance Society. Impeccable behavior is expected both in the audience and backstage. Students should clap enthusiastically to show their appreciation and support, but screaming, calling out a dancer’s name, or whistling is inappropriate. Wardrobe The instructor will choose group dances and the costume which she feels will be appropriate for the music, in consultation with dancers and parents. Cost of the costume is the responsibility of the student/ parent/ guardian. The dance society has costumes in storage which students may wish to buy for solos, duets, or trios. Older dancers may also have costumes that have only been worn a few times that they may be willing to lend or sell. A costume deposit of $50 per class is required with registration(s). Measurements will be taken of each dancer in October and costumes ordered. Costumes will only be released when they are paid in full (watch for costume pick up night in your newsletters). Please remember that there is absolutely no eating or drinking (except water) while in costume. Students will be responsible for the cleaning and repair of their own costumes if they are damaged before the end of the season. If a costume is borrowed and is damaged or in need of cleaning or repair the student will be responsible for the cost of the repair. Students must return borrowed costumes in the condition they were received. Note: Costumes are charged at a cost which includes the costume, shipping and handling, duty, and all applicable taxes. If a student withdraws from class after the costumes have been ordered, that student is still responsible for the cost of their costume. Dress Code The dress code is an extremely important tool for the correct development of technique and assists in preventing injuries. When the instructor can view the dancer’s body clearly, they are able to make subtle adjustments that may eliminate future problems. Students / parents/ guardians are expected to purchase appropriate attire for class and are expected to wear it to every class, workshop, practice, etc. Students will be excused twice during the year for improper attire, after that it will be a discipline issue. No jewelry is permitted for any of the classes. Please see handout for 2010-2011 dress code. Students should be properly groomed at all times. Students should keep extra dancewear in their bags in case of emergencies and ensure that their dancewear is laundered on a regular basis. Makeup is not required for regular classes, but will be required for pictures, festivals and performances. Instructors will hand out makeup sheets beforehand and students will be responsible for purchasing their own make-up. The instructors may alter the dress code at any

time. There are several dance stores in Edmonton that carry dancewear, and bodysuits, tights, and ballet slippers, can be found at Wal Mart. Some bargains can also be found on Ebay. The dance society also has a page on facebook where dancers can offer their old outfits, shoes, etc for sale. A list of dancewear stores is posted on our bulletin board at McArthur place. Dancer Expectations 1. Please be on time to all classes. Being late is disruptive to the rest of the class. 2. Chewing gum is not allowed 3. No juice, pop, food, etc. permitted in the dance class… water only please 4. Dress Code is to be followed at all times- instructors may alter it at any time 5. Cell phone use/ texting/ updating statuses, etc. is strictly prohibited during class time. Leave

cell phone on silent in bags or in cell phone basket. 6. Dancers coming to class under the influence of alcohol or drugs will be asked to leave the room

and wait for their parent to pick them up. 7. No swearing, foul language, or disrespectful behavior will be tolerated 8. Respect for instructors, parents, classmates, volunteers is expected at all times. 9. Abide by instructor’s rules 10.Actively participate in class Discipline

Camcorders may be used in the classroom - both to record choreography and document discipline incidences. We had several incidences where a camcorder would have been essential to enforcing our policies.

Discipline- 3 strikes- 1st is verbal and a slip (if merited), 2nd written notice (letter home to parent and executive), 3rd strike is dismissal from class and the student will need to have a consultation with the parents, instructor and executive before they are permitted to return to class.

In the event of a serious incident with a student, the student may be asked to leave the club. Meetings and discussions with parents will be set up to discuss behavior should a problem occur.

There is a zero tolerance policy for violence, cursing or vulgar language used towards instructor or classmate, threatening, and substance abuse/usage. Any of these actions will result in immediate dismissal from the club. The student will not be permitted to return to the club until certain conditions are met after consultation with Instructor, Student, and Executive.

Parents who do not uphold and support the rules and policies of the Lac La Biche Dance Society may be asked to withdraw their family from the club.

If a student is dismissed from the club due to discipline issues NO REFUNDS will be issued and you will be responsible for all fees, fundraising commitments, costumes and your volunteer

Dancewear – Shoe Swap & Remote Sale!

Please see the Dress Code handout for the 2010-11 dance year requirements.

For your convenience we have set up a Shoe Swap and a Remote sale so you can pick up shoes, tights, bodysuits etc right here in Lac La Biche.

‘All 4 Dance’ will be setting up their remote sales on September 8th, 2010 at McArthur Place in the Tea Room from 3pm to 8pm. They have been provided a copy of our dress code and will be able to help you with your child’s dance wear needs.

Shoe Swap & Costume Sale! - Bring your old shoes and sell them before you pick up your new ones or find the shoes you need used! We will be set up at McArthur Place in the hallway & coat room. The Lac La Biche Dance Society will be selling some of our old costumes – pick them up for dress up, Halloween costumes etc! We will be there from 5pm to 8pm.

If you can’t make it on the 8th, the following is a list of companies in the Edmonton area that carry dance apparel. Past experience has offered great customer service and dance wear could probably be sent out via Canada Post or Greyhound. For the younger kids, the bodysuits and tights required are quite basic and run in sizes 2-4, 4-6, 6-8, 8-10, 10-12, etc. so there isn’t a lot of fitting required. Instructors can provide details on style and colour of bodysuits and tights and you can also refer to the 2010-11 Dress Code hand out. Please note that pink tights and ballet pink tights are different. Some stores offer discounts – mention Lac La Biche Dance Society when ordering. If parents are unable to travel to Edmonton to purchase ballet slippers or jazz shoes, you can trace your child’s feet while standing (barefoot) and fax the sheet to the store. Camcorder Agreement

I____________________________________________ have read the Lac La Biche Dance Society Handbook and understand that students of the Dance Society may be filmed for the purposes of recording and subsequently practicing choreography. In the event of an incident in the classroom that requires discipline, the video may also be used to back-up claims of said incidents. I give my permission for my child to be videoed for the above purposes. In the event of using the videos for the purpose of discipline, the videos will only be available to the instructors, executive, manager and the parent who has child having discipline issues. If the video is being used for practicing choreography, only the choreography will be given. It will only be available to students in that class. No video footage will be put on the llbdance.ca website, used for TV advertisement etc without further permission from you.
