1.1 PROJECT INFORMATION

A. Project Name: Wehner Park Festival Area Playground Improvements

B. Owner: City of Shrewsbury

E. Date of Addendum: June, 19, 2017

1.2 NOTICE TO BIDDERS

A. This Addendum is issued to all registered plan holders pursuant to the Instructions to Bidders and Conditions of the Contract. This Addendum serves to clarify, revise, and supersede information in the Project Manual, Drawings, and previously issued Addenda. Portions of the Addendum affecting the Contract Documents will be incorporated into the Contract by enumeration of the Addendum in the Owner/Contractor Agreement.

B. The Bidder shall acknowledge receipt of this Addendum in the appropriate space on the Bid Form.

C. The date for receipt of bids is unchanged by this Addendum, at same time and location.

1. SKL-001. REVISION to base bid. Existing water fountain and concrete pad to remain, and Contractor to pour ADA accessible standard concrete pad to connect existing water fountain concrete pad to proposed concrete walk as part of base bid.

2. SKL-002. ADD ALTERNATE. As an add alternate, Contractor to remove existing water fountain and concrete pad. Contractor to pour new concrete pad and install new water fountain in location shown in drawing. Contractor to connect new water fountain to existing water line that was serving existing fountain. New water fountain model to be

MDF Drinking Fountain Model 440 SMFA (Front Approach). Existing water fountain to be provided to City for reuse.

3. SKL-003. REVISION to playground underdrain system.

4. SKL-004. REVISION to base bid. For base bid, existing concrete walk adjacent to parking

lot shown in drawing to remain. As an add alternate, City to remove existing concrete walk and subbase, and Contractor to pour new standard concrete walk and base materials in location of existing concrete walk.

1.4 CLARIFICATIONS

A. REVISION to base bid. Concrete pads for benches (2) and trash receptacle (1) to be included in base bid. Benches (2) and trash receptacle (1) to be add alternates. SUBSTITUTE Beacon Hill Recycled Plastic 6’ Contour Bench with Commercial Steel Outdoor Bench with Straight Back by Belson Outdoors Model #CBPB-6SB-BK. SUBSTITUTE Beacon Hill 35 Gal. Recycled Plastic Open Top Trash Receptacle with Flare Top Trash Receptacle w/ Liner and Rain Bonnet Cover by Belson Outdoors Model #CBTR-FTRB-BK.

1.2 BID FORM SUPPLEMENT

A. This form is required to be attached to the Bid Form.

1.3 DESCRIPTION

A. The undersigned Bidder proposes the amount below be added to or deducted from the Base Bid if particular alternates are accepted by Owner. Amounts listed for each alternate include costs of related coordination, modification, or adjustment.

B. If the alternate does not affect the Contract Sum, the Bidder shall indicate "NO CHANGE."

C. If the alternate does not affect the Work of this Contract, the Bidder shall indicate "NOT APPLICABLE."

D. The Bidder shall be responsible for determining from the Contract Documents the effects of each alternate on the Contract Time and the Contract Sum.

E. Owner reserves the right to accept or reject any alternate, in any order, and to award or amend the Contract accordingly within 60 days of the Notice of Award unless otherwise indicated in the Contract Documents.

F. Acceptance or non-acceptance of any alternates by the Owner shall have no effect on the Contract Time unless the "Schedule of Alternates" Article below provides a formatted space for the adjustment of the Contract Time.
