WINSTON HEALTH POLICY BALL What Is It? The Health Policy Ball is the primary fundraising source for the Fellowship. The Ball takes place on the first Saturday of February ever year and has been held at the Ritz Carlton, Tysons Corner, for the last 15 years or so. The event is sponsored by about 80 organizations each year, paying between $5,500 and $15,000 for a table. These organizations include associations, corporations with health policy interests, and other non-profit groups with interest in health policy. Many organizations have sponsored since the event was founded 25 years ago. There is substantial competition for tables and there is typically a significant waitlist for each Ball. WINSTON HEALTH POLICY BALL

What Is It?

The Health Policy Ball is the primary fundraising source for the Fellowship. The night of the Ball, the first thing that happens, starting around 5:30, is the Fellow-hosted reception. One of the current Fellows gets a suite and the fellows host the former fellows for a party in that room. It is their responsibility to send invites, buy food and drink, and host the party.

The Ball officially starts at 6:30 pm with two simultaneous receptions. The main reception is in the foyer of the Ballroom on the 5th floor. All invitees are invited to that reception, and it usually gets rolling around 6:15. By 7, the room is totally packed. A second “private” reception to honor the Fellows is held in the Old Dominion Room which is on the 4th floor off the hotel lobby. That reception is intended to be an opportunity to recognize the Fellows, so it is important to ensure that all of the current and former Fellows are in that room not later than 6:50 pm. In addition, all of the people that met with the Fellows in the prior year are invited to that reception, along with the gold and platinum sponsors and their guests. We typically get about 150 people at that reception. There will be remarks and the Fellow Gifts will be presented to the Fellows.

The dinner begins at 7:30, though it takes about 20 minutes to get everyone into their seats. The band can help get people to sit down. It isn’t worth worrying too much about whether or not people are eating. The bigger issue is making sure everyone has a seat. Once all of the government guests are seated, the hard work is done.

After the dinner, people can stay and dance to the band, they can go out to the foyer for a conversation (it is too loud in the ballroom for conversation generally), or they can go to the Old Dominion Room on the 4th floor where there is a jazz piano and some food (cheese and crackers / chocolate covered strawberries). The band in the ballroom wraps up at 11:30, and the jazz lounge closes at 12 pm.

8/2/2013 The committee should meet in early September to discuss the materials to be sent to the sponsors and invitees and determine if there should be any changes to prior practice or pricing.

9/4/13 There are prior reservation forms in surveygizmo.com. Just copy the prior year’s survey and update with appropriate dates, contact info, policies and procedures, etc. Test it several times to make sure it is working and is sending the confirmation email upon submission.

Hunt down any bounce backs and update the database with any changes you receive. If there are likely to be any changes to pricing or policies, it is a good idea to include that in the initial email, so if you can, wait until after the fall Board meeting.

Provisionally assign tables 10/16/2013 As requests come in, “assign” them to a table so that you can seehow you are progressing and give the best tables to those that come in early. Start Waitlist 10/20/2013 Around day 4 you will run out of tables. Hold back about 4 tablesfor long-time sponsors that may not have responded (in spite ofyour attempts to reach them). Also make sure all Board membersthat want tables have one, even if you haven’t heard from them.Let sponsors on the waitlist know that they are on it right away,but not where on it. Once you’ve dealt with any fall out over thenext couple of days from sponsors that thought they had a tablebut didn’t, etc, you can tell sponsors where they are on thewaitlist.

Request Ethics Committee 10/29/2013 Sample letters from 2014 are attached. You will send these lettersLetters via email and/or fax. The Senate will send a response back, the

House will not, but they will make note of the event and you canlet attendees know that they are aware. All other ethics folks willcontact you directly.

Contract Photographer 10/31/2013 Book Pablo through Freed Photography, unless you can findsomeone good who is cheaper (he’s very expensive). You will wanthim from 6-10. Make sure the contract includes high-res photosdelivered either on desk or uploaded, not more than 5 businessdays after the event.

Request bio and room 11/20/2013 The program for the Ball includes a bio of each Fellow. I usuallypreference from Fellows send the last year’s bio (or one from the past that was really good)

to give them an idea of what to write. Count the words in lastyear’s so you know how many words to tell them it should be.When you get it and lay it out, if you need to edit it substantiallyyou should send it back to them to review again. At the same timeask which one wants the suite for the former fellows’ party.

Order Fellow Gifts (Good 12/1/2013 Order from Salisbury Pewter, John Will Tankard. Photo ofthrough 2015) engraving attached. It makes life easier if you order for a couple of

years in advance just so you don’t have to do it every year.

Finalize invitee list with input 12/2/2013 By this point you need to have a finalized list of invitees,from various people prioritized into first and second wave, with reliable contact info.

Use Leadership Directories (username and password below) tomake sure the contact info is correct.

Email invitations to wave 1 12/3/2013 Send out the first wave of invitations by email. I create an emailmerge so that it doesn’t go out as a big batch which often getsstopped by spam filters. I include the PDF invite in the email.

Manage Bounce Backs 12/3/2013 A lot of invites will bounce back. You may have bad addresses orthe person may have moved. Use Leadership Directories, yoursubcommittee, and google to see if you can identify why themessage bounced and what you should do about it.

Mail printed invitations 12/5/2013 Once you’ve dealt with the fallout of the email messages, you’llwant to send out the printed invitations to the same list youemailed. A lot of these invites may never get where they’re sent,and they’re pretty much a formality, but sometimes they reachpeople the email doesn’t, and folks like to have the printed invitein hand because it has the details of the event on it.

Send sponsors follow-up tax 12/4/2013 The letter used in 2014 is attached. Mail these letters along withletter with Invites 10 printed invitations and envelopes per table reserved.

Schedule program printing 12/17/2013 Get in touch with Good printers to let them know you’ll have thewith printer program ready for printing the first week in January and get it on

their calendar.

Update Program 12/17/2013 The program needs to be updated with the Fellow bios, the menu,the year’s sponsors, including gold and platinum, 10 and 20 years,Ball committee, etc. At this point all of that info should be writtenin stone, though sponsors still change at the last minute so wait aslong as possible to finalize. A copy of the 2013 Program isattached.

Send Program to Ball 12/17/2013 The Ball Committee has asked to review the program. You may notCommittee need to send it to everyone. Ask in advance who wants to review

it.

Collect names of Sponsor 12/23/2013 There is a SurveyGizmo form that you can use to have sponsorsGuests send you the names and emails of their personal guests. You will

use this list to let everyone know about last minute logistics forthe event. For those sponsors that are gold and platinum level youwill also use that info to invite the private guests to the receptionin the Old Dominion Room for the Fellows.

Develop pre-reception invite 12/23/2013 In addition to the Gold and Platinum sponsor lists for invitation tolist and check it with fellows the fellows’ reception in the Old Dominion Room, you will need to

develop a list of the current and former fellows, and everyone thatthe current fellows interviewed with that year that h a s alrea d y b ee n invi t ed and r s v p ’d y es fo r the Ball . Send the list to the fellowsto make sure nobody is left off.

Invitee response deadline 1/7/2014 Once the response deadline for invitations arrives, take a look athow many yes RSVPs you have and how many non-responses youhave. You’ll probably want to send a reminder to your non-responses that they need to rsvp, indicating that they won’t havea seat if they don’t respond by the deadline. If, after the deadline,you still have some empty seats, go back to your committee andask if there are folks that still need invites.

1/8/2014 Create some kind of spreadsheet in excel that will allow you to randomly assign guests to sponsor tables. Enter that info into the database as you make the assignments then print the list and take a look. If you see any glaring problems (high level person at table in the back corner), make adjustments. This will be reviewed by the Ball Committee Chair so this is only a draft. The important thing is to make sure you have every empty seat filled, if possible. If not, know where your empty seats are so you can fill them as people RSVP.

1/9/2014 If there are any higher ups that were invited and haven’t responded, follow up to determine if they plan to attend.

1/9/2014 Send an email to all of your confirmed guests letting them know about the discounted rate at the Ritz and how to book it.

Plan Winston Tables 1/17/2014 There are about 20 people that we invite to the Ball that we can’tseat with sponsors. They are current and former Board members that are not with sponsoring organizations, Don Yesukaitis from the Finance Committee, the current fellows, and any former Fellows that ar e with sponsoring organizations but don’t have a seat. We hold back 2 tables for the Fellowship for these people and then pray we don’t have more people than seats. You’ll need to decide who sits at which table so you can send them that info.

Schedule Ball Committee 1/26/2014 Schedule this for as soon after the Ball as possible so theMeeting Debrief after Ball outcomes are clear in everyone’s mind.

Develop reception script and 1/28/2014 A copy of last year’s script is attached. This will include theball timing photographer schedule.

Email all Ball attendees with 1/28/2014 Send an email to all attendees of which you are awareinfo about Fellowship (government guests and sponsor guests, plus all sponsors) lettingprogram and Jazz bar them know about the Fellowship, about the Jazz Bar, etc.

Develop special request list 1/29/2014 Your emails and contact requests to your guests includes a requestfor special meal requirements. You’ll want to note these in thedatabase then put together a list for the hotel. Ask the hotel tomake about 30 more vegetarian meals than are requested byguests. We always need more than we have.

Manage Seating 1/7-2/1/2014 Throughout the month before the Ball you will be receiving rsvp’s,extending new invitations, and dealing with cancellations. You’llconstantly be assigning and reassigning seats. Your goal is to haveevery seat filled and no extra people at 7pm on the day of the Ball.

Print seating lists 2/1/2014 At the last possible minute, print your list of guests with assignedsponsors and sponsors with assigned guests. Give everyoneworking on site a copy

Review and pay bill 2/15/2014 The bill typically comes about 2 week after the event and the Ritzis rabid about getting it paid fast. Make sure the deposit iscredited. Resolve any issues with the hotel quickly and get it paid.The check requires 2 signatures so that process takes a bit of time.

Negotiate contract for next 3/1/2014 As of this writing the Ball is contracted at the Ritz through 2015.open date This should be revisited as soon as the ball is over to make sure

you’re booked 2 years out at all times. Consult with theCommittee Chair before finalizing. The Board Chair should sign thecontract.

At the Winston Fellowship, we make every effort to ensure that the Health Policy Ball complies with all government ethics laws. Our policies and procedures are designed to protect our guests and sponsors from inadvertently violating these rules. They also assure the ethics officers and government officials we invite are attending a legitimate, ethically appropriate event. If you have any questions about our policies, please to not hesitate to let us know. In order to meet these and other practical goals, we have developed the following procedures.

Congressional and Administration guests are invited and assigned to tables by the Fellowship and should not accept invitations directly from table sponsoring organizations. Please do not invite or seat any Congressional or Administration guests at your table unless they have been assigned by the Fellowship. Doing so may place these individuals, yourselves, and others in danger of violating ethics rules.

The Fellowship will identify those government officials who will be invited to attend the Ball. Since the number of government officials invited is determined by the number of seats sponsors donate back to the Fellowship for this purpose, please consider donating a seat at your table to the Fellowship.

The Fellowship extends Ball invitations directly to government officials and receives their RSVPs.The Fellowship then assigns government invitees and their guests to sponsor tables throughout the room.

After the Fellowship assigns the government guest to a table, both the table sponsor and the government guest are notified of the seating assignment. The Fellowship cannot, of course, guarantee the ultimate attendance of the government guest and encourages table sponsors to coordinate with the assigned guest(s) prior to the Ball to ensure attendance and to coordinate a meeting place. Prior to the Ball, if the government guest notifies the Fellowship that they will not attend, the Fellowship will contact the sponsor as soon as possible. If the government guest notifies the table sponsor that they will not attend, the Fellowship requests that the table sponsor notify us as soon as possible so that we can attempts to assign another guest to the table.

If a table sponsor invites a government guest directly, the Fellowship may have to consider withholding an invitation to that sponsor in the future.

Payment for table reservations is due to the Fellowship by January 10, 2014. T a b le re s er v a ti o n s that are not paid by January 10th will be released to the waiting list.

Cancellation Policy: Cancellations received prior to November 15, 2013 will not incur a cancellation fee. Cancellations received between November 15, 2013 and December 15, 2013 will be assessed a 15% cancellation fee. For cancellations received between December 16, 2013 and January 10, 2014, a 25% cancellation fee will be assessed. Tables cancelled after January 10, 2014 are not eligible for a refund. Should the cancellation fee not be paid in full before March 1, 2014, the Fellowship may consider withholding an invitation to sponsor a table in the future.

o Ethics approval for government guests to attend the Ball extends only to the reception, dinner, and dance hosted by the Fellowship. Table Sponsors who hold a separate event must inform invitees that they should obtain separate ethics approval to attend that event.

o Table Sponsors who opt to hold an event must send a copy of the invitation list for government invitees to the Ball staff ([email protected]) by later than January 10, 2014 and before sending invitations to their event. Our staff will review the list and indicate to the sponsor any government invitees on the invitation list who have not been invited to the Ball. To avoid confusion over seating at the Ball, Table Sponsors are asked to invite only government guests to sponsor-hosted events who are also invited to the Ball.

o To avoid confusion, Table Sponsors may not reference the Winston Health Policy Ball or use the name “Winston,” “Health Policy Ball” or “Winston Health Policy Fellowship” in their invitations.

Thank you for your support of the David A. Winston Health Policy Fellowship through your purchase of 3 Silver Level table for the 2014 David A. Winston Health Policy Ball, which will be held on February 1, 2014 at 6:30 pm at the Ritz Carlton at Tysons Corner, Virginia.

Every year the David A. Winston Fellowship, a 501(c)3 educational organization, hosts an event known as the David A. Winston Health Policy Ball. The Ball serves as both as a networking and public policy discussion opportunity for members of the healthcare industry, as well as a fund raising effort for educational fellowships and scholarships awarded annually to promising young students of health administration to spend a year in Washington governmental positions learning about the health policy– making process.

In past years, we have liaised with your office prior to the event to make you aware that it will be taking place and that the Fellowship will be inviting staff members from House offices to the event as guests of the Fellowship.

For the 2014 Ball, which will take place February 1, 2014, at the Ritz Carlton Tysons Corner, I am writing to again make you aware of the event and request the appropriate indication of acknowledgement. We believe that this event qualifies as a “widely attended event” under the House rules. Attendance at the event will include more than 25 persons from outside of Congress and the event is open to a range of persons interested in the health policy arena. 2014 WINSTON BALL FAQ’S

1. What is the nature of the event?It is a networking event and charitable fundraiser.

2. Who is the beneficiary of the event?The David A. Winston Health Policy Fellowship is the beneficiary of the proceeds of the event. The Fellowship is a 501(c)3 organization with an educational mission.

3. Who determines the government guests to be invited?The Fellowship Board and the Ball Planning Committee develop the list of individuals to be invited.

4. Who determines where guests sit?The Ball Planning Committee makes seating assignments

5. How are seating assignments determined?Seats are assigned through a random assignment process.

6. How many people attend the Ball?Approximately 1030.

7. How many of those individuals are government invitees?In 2013 there were 165 government invitees in attendance.

8. Who owns the seat where the government guest is seated?The Fellowship asks sponsors to donate seats back to the Fellowship at their reserved tables, so the Fellowship owns the seats.

9. What is the cost / value of a seat?A table may be reserved for $5,500. Individual tickets are not available for sale. There are 10 seats at a table. The value of a seat for tax purposes, based upon the cost of the meal and entertainment, is calculated at $250.

8. Who plans the event?The event is planned by a subcommittee of the Winston Fellowship Board of Directors. That subcommittee includes ball sponsors, ball guests, and event staff.

9. Are other events held in conjunction with the Ball?Occasionally sponsors will opt to host an event either prior to or following the Ball itself. The Fellowship does not encourage this and has strict guidelines around these events and how they must be managed in order that attendees remain in compliance with ethics rules. Details of these guidelines may be found in the Policies and Procedures attached.

Planning for the 2014 Winston Health Policy Ball (to be held on February 1st at the Ritz Carlton atTysons Corner) is underway!

Tables are sold on a first come, first served basis and sell out quickly (in just two days last year). We want to assure that all sponsors receive invitations in the same timeframe and have the same opportunity to sponsor a table if they respond quickly.

As a first step in our process, we are emailing you to confirm the contact information we have for your organization. At the end of this email you will find the contact information that we currently have for you in our database.

Please reply to this email indicating whether or not the information is correct. If necessary, please provide corrected information so that we can update our database before sending sponsor invitation letters. If you would like us to remove you from our list, please let us know.

The letters of invitation to organizations that have sponsored a table in the past three years will be sent via E M A IL to both the primary and secondary contact (if any) on October 15th. If you are a recent sponsor and do not receive an invitation to sponsor on the 15th please contact me at [email protected] and I will send your invitation again.

Every year the David A. Winston Fellowship, a 501(c)3 educational organization, hosts an event known as the David A. Winston Health Policy Ball. The Ball serves as both as a networking and public policy discussion opportunity for members of the healthcare industry, as well as a fund raising effort for educational fellowships and scholarships awarded annually to promising young students of health administration to spend a year in Washington governmental positions learning about the health policy– making process.

In past years, we have liaised with your office prior to the event to make you aware that it will be taking place and that the Fellowship will be inviting staff members from Senate offices to the event as guests of the Fellowship.

For the 2014 Ball, which will take place February 1, 2014, at the Ritz Carlton Tysons Corner, I am writing to again request a letter of approval (or the appropriate indication of approval) in advance from your office for those individuals that we invite from the Senate to attend this event.

We believe that this event qualifies as a “widely attended event” under the Senate rules. Attendance at the event will include more than 25 persons from outside of Congress and the event is open to a range of persons interested in the health policy arena. Attendance at the event is appropriate to the representative function of the members and staff because it allows them to meet their colleagues in a collegial setting, as well as to conduct discussions with industry and government personnel involved in health policy about many issues under consideration in the legislative and regulatory process.

2014 WINSTON BALL FAQ’S

1. What is the nature of the event?It is a networking event and charitable fundraiser.

2. Who is the beneficiary of the event?The David A. Winston Health Policy Fellowship is the beneficiary of the proceeds of the event. The Fellowship is a 501(c)3 organization with an educational mission.

3. Who determines the government guests to be invited?The Fellowship Board and the Ball Planning Committee develop the list of individuals to be invited.

4. Who determines where guests sit?The Ball Planning Committee makes seating assignments

5. How are seating assignments determined?Seats are assigned through a random assignment process.

6. How many people attend the Ball?Approximately 1030.

7. How many of those individuals are government invitees?In 2013 there were 165 government invitees in attendance.

8. Who owns the seat where the government guest is seated?The Fellowship asks sponsors to donate seats back to the Fellowship at their reserved tables, so the Fellowship owns the seats.

9. What is the cost / value of a seat?A table may be reserved for $5,500. Individual tickets are not available for sale. There are 10 seats at a table. The value of a seat for tax purposes, based upon the cost of the meal and entertainment, is calculated at $250.

8. Who plans the event?The event is planned by a subcommittee of the Winston Fellowship Board of Directors. That subcommittee includes ball sponsors, ball guests, and event staff.

9. Are other events held in conjunction with the Ball?Occasionally sponsors will opt to host an event either prior to or following the Ball itself. The Fellowship does not encourage this and has strict guidelines around these events and how they must be managed in order that attendees remain in compliance with ethics rules. Details of these guidelines may be found in the Policies and Procedures attached.

At the Winston Fellowship, we make every effort to ensure that the Health Policy Ball complies with all government ethics laws. Our policies and procedures are designed to protect our guests and sponsors from inadvertently violating these rules. They also assure the ethics officers and government officials we invite are attending a legitimate, ethically appropriate event. If you have any questions about our policies, please to not hesitate to let us know. In order to meet these and other practical goals, we have developed the following procedures.

• Congressional and Administration guests are invited and assigned to tables by the Fellowship and should not accept invitations directly from table sponsoring organizations. Please do not invite or seat any Congressional or Administration guests at your table unless they have been assigned by the Fellowship. Doing so may place these individuals, yourselves, and others in danger of violating ethics rules.

• The Fellowship will identify those government officials who will be invited to attend the Ball.Since the number of government officials invited is determined by the number of seats sponsors donate back to the Fellowship for this purpose, please consider donating a seat at your table to the Fellowship.

• The Fellowship extends Ball invitations directly to government officials and receives their RSVPs.The Fellowship then assigns government invitees and their guests to sponsor tables throughout the room.

• After the Fellowship assigns the government guest to a table, both the table sponsor and the government guest are notified of the seating assignment. The Fellowship cannot, of course, guarantee the ultimate attendance of the government guest and encourages table sponsors to coordinate with the assigned guest(s) prior to the Ball to ensure attendance and to coordinate a meeting place. Prior to the Ball, if the government guest notifies the Fellowship that they will not attend, the Fellowship will contact the sponsor as soon as possible. If the government guest notifies the table sponsor that they will not attend, the Fellowship requests that the table sponsor notify us as soon as possible so that we can attempts to assign another guest to the table.

• If a table sponsor invites a government guest directly, the Fellowship may have to consider withholding an invitation to that sponsor in the future.

• Payment for table reservations is due to the Fellowship by January 10, 2014. T abl e r ese r v a t i o n s t ha t ar e n o t pai d b y Janua r y 10 th will be released to the waiting list.

• Cancellation Policy: Cancellations received prior to November 15, 2013 will not incur a cancellation fee. Cancellations received between November 15, 2013 and December 15, 2013 will be assessed a 15% cancellation fee. For cancellations received between December 16, 2013 and January 10, 2014, a 25% cancellation fee will be assessed. Tables cancelled after January 10, 2014 are not eligible for a refund. Should the cancellation fee not be paid in full before March 1, 2014, the Fellowship may consider withholding an invitation to sponsor a table in the future.

The Fellowship recognizes that some sponsor’s consider hosting parties held in conjunction with the Health Policy Ball. Our policy on sponsor hosted parties is consistent with our policy for the Ball itself.

I am pleased to inform you that planning for the 2014 Health Policy Ball is well underway. We anticipate another exceptional turnout for this special event. I am writing today to invite you to sponsor a table this coming year.

Please read these materials carefully as some policies and procedures have changed.

The Ball will be held on Saturday, February 1, 2014 beginning at 6:30 pm at the Ritz Carlton Hotel at Tysons Corner, Virginia.

We are offering thre e levels of sponsorship and each sponsor may purchase up to a maximum of three tables in combinations described below:

Platinum Sponsorship ($15,000) – as a Platinum Sponsor you receive premier placement close to the dance floor for your table seating 10 people. Your sponsorship will be recognized in the program and in signage. Additionally, you will be recognized as a co spo‐ nsor of the post‐dinner reception in the Old Dominion Room. You may purchase up to two Platinum sponsorship tables.

Gold Circle Sponsorship ($11,000) – as a Gold Circle Sponsor you receive a table for 10 people in the first three rows adjacent to the dance floor. Your sponsorship will be recognized and highlighted in the program and in signage. You may purchase up to two Gold Circle sponsorship tables.

Silver Sponsorship ($5,500) – As a Silver Sponsor you receive a table for 10 people. Your sponsorship will be recognized in the program and signage. You may purchase one Silver sponsorship table (but may purchase additional tables at a higher level if you want more than one table).

The Fellowship has succeeded in holding sponsorship costs steady for five years. In 2014 you will note a modest price increase, the proceeds of which will support expanding the Winston

Fellowship's program to include the awarding of ten $10,000 scholarships to promising healthcare policy students.

We expect a high demand for tables and want to make them available equitably. That is why we are limiting the number of tables a sponsor can purchase. We also try to distribute table locations among all of our sponsoring organizations, and so may not be able to assure sponsors that if they purchase more than one, those will be adjacent to each other. Additionally, starting in 2014 we will be treating companies that submit reservations separately but that are under a single corporate umbrella as one company for the purposes of table limitations.

Please reserve your table(s) soon because they sell‐out very quickly. Please follow the link below to reserve your table(s) online. We will send you a confirmation within two business days. If you do not receive one, please check back with us by emailing [email protected] to ensure we received your reservation.

As in previous years, we are offering you the opportunity to host Congressional and Administration invitees and their guest at your table(s). In your reservation, please indicate how many seats at your table(s) (in multiples of two) you would like to donate back to the Fellowship for government guests we will seat with you.

As a table sponsor, we will seek your recommendations about prospective government official invitees. As a reminder, we adhere to government ethics rules and consistent with those rules, all invitations to government guests must be extended solely by the Fellowship. The Fellowship will extend invitations to a broad array of individuals and then assign them to sponsor tables. To ensure compliance with ethics rules, the Fellowship will not fulfill requests for specific government guests to be seated at a particular organization's table. Please do not invite or seat any Congressional or Administration guests yourself, as this may place you and these individuals in danger of violating ethics rules.

We will send printed invitations for non‐government guests in early December and will advise you of which government guests will be seated at your table in late January. Please refer to the Health Policy Ball Policies and Procedures for more information regarding how the Fellowship invites and seats government guests and additional important information about the Ball. Ill. Acts of God Cancellation: If an act of God, terrorism, riots, tabor strikes, fire accidents, illness, litigation, inclement weather,or mechanical or transportation failures, renders the performance of services of provision of materials impossible, the Client or the Producer, as the case may be, upon prompt written notice to the other, shall be excused from performing this Agreement. In such case, the liability of the Client shall not exceed the actual expense incurred by the Producer.

IV. Employee Status and Insurance: GS's personnel working on this project pursuant to this Agreement are not employees of AUPHA. The Producer carries all insurance necessary to comply with the Workmen's Compensation and the employer's Liability laws of the states in which the Producer's work is to be performed.

This policy provides the necessary safeguard that can make the difference between financial success and failure of your important event. EXPO-PLU$ provides comprehensivecoverage for lost revenue and tor additional operating expenses incurred because of an unexpected disruption. You arecovered from the time the premium is accepted until your event Is over. For example, let's say you purchase this protectlonIn April and your event is planned for November. If In July, a hurricane hits destroying the event's venue, coverage would apply avoiding a potential significant flnandalloss.

• Communicable Disease- Limited or Full Options available• Cost of Remedial Action - Underwriters and the representatives will work with you to attempt to remedy and avoid,

where possible, a situation that could result in a dalm under your policy. If this fails and you still suffer a claim the policy limit is still there in full to respond to your covered loss and has not been eroded by the cost of remedial action.

• Insured Commlbnents - In addition to limit of liability up to 20% extra is available for those finandal commitmentsyou have entered into and undertaken by others on your behalf. Let us know If your commitments are greater than 20% and our underwriters will conslder extending this to suit your needs.

• Automatic Worldwide coverage for additional indoor events that are related to your prindple events that are under $100,000 budgeted gross revenue and/or expenses during the policy term.

• Future Marketing Expense - In addition to the policy limit you purchase and without dollar limitation, subject to their approval underwriters and their representatives will work with you to positively promote your future event should a loss occur.

• Golf Event cancellation- if your event Includes a Golf Event, we provide up to $100,000 due to adverse weather should your day be cancelled

• Golf Hole in One- up to $25,000 If you arrange a "prize" for par three hole not less than 175 yards, or 165 for theladles, should you wish to enhance your Goff Event

• Team Building/Recreational Event- any outdoor pre-arranged team building or recreational events solely for the partidpants at the insured event up to 5% of the limit of indemnity or $75,000, whichever Is lesser, should the event be cancelled for reasons beyond your control.

Indemnification under this section is limited to $710,000.00.

HOTEL PENALTY CHARGES

It is hereby understood and agreed this policy is extended to indemnify the Named Insured in respect of the penalty imposed by the hotel in respect of each hotel room falling short of the Hotel Room Commitment up to a sub-limit of USD10,000 in all.

It is a condition precedent to liability under this policy that:

• The attendance of participants at the Event was not below the Named Insured's estimate, or prior three years average.

• The number of Hotel Room Commitments is not greater than last year's actual, or an average of the last three years actual rooms rented at the host hotel.

• The Hotel Room Commitment rate is not greater than the previous Event plus up to ten percent

Definition:Hotel Room Commitment- The number of rooms the Named Insured reserves for the Event. This commitment is discharged from the Named Insured as they are booked by participants in the Event and the Insurers liability is similarly discharged.

About the Fellowship

The David A. Winston Health Policy Fellowship is a distinguished postgraduate fellowship in health policy. The Fellowship was established in 1987 to commemorate the contributions and personal commitment of David A. Winston who, for 20 years, played a significant role in shaping American health policy at both the state and federal levels.

The purpose of the Fellowship is to provide a unique opportunity for promising students to learn about the political system through direct exposure to the roles of both the public and private sectors in health policy development. The Fellowship provides a monthly stipend and related expenses for a 12 to 16-month period starting in June of each year. The Fellowship presents a unique opportunity to learn and develop experience through systematic exposure to individuals, organizations, associations, and coalitions in both the private and public sectors. The distinguished Fellowship Board of Trustees selects the Fellows and helps to assure the quality of the experience.

In addition to the Fellowship experience, the Winston Board also funds ten graduate-level scholarships per year awarded to students in health policy. The scholarship program recognizes student excellence and achievement based upon the applicant’s record and recommendations from faculty and colleagues. Students are awarded $5,000 each and attend a day-long Health Policy Symposium hosted by the Winston Board.

The Health Policy Ball was initiated by David Winston in 1985 as a social gathering among friends and colleagues in the healthcare field at the beginning of each new year. In his memory, the Ball has become the principal annual fundraising event for the Fellowship and related programs established in his name.

The Fellowship is a non-profit organization that is governed by a Board of Trustees and is administered by the Association of University Programs in Health Administration (AUPHA).

2012-2013 Fellow ProfilesElizabeth “Issie” Karan, JD, MPHElizabeth comes to the Winston Fellowship after completing her juris doctorate and master’s of public health in Public Health Administration and Policy at the University of Minnesota. Previously, Elizabeth received her bachelor’s of science with honors in Biology and Political Science from the University of Wisconsin. While at the University of Minnesota, Elizabeth was selected as the American Society of Law, Medicine & Ethics Student Award winner and completed over 100 hours of volunteer legal services. Elizabeth’s enthusiasm for health policy stems from her desire to improve the lives of those impacted by the health system.

While at Minnesota, Elizabeth focused on health law, health policy, and public service. She was a staff member and Managing and Research Editor for the Minnesota Law Review and served as the Community Service Coordinator for the Women Law Students Association. During law school, Elizabeth volunteered on public service projects related to asylum law, mental health advocacy, and interned for Judge Schiltz of the US District Court. She spent a semester studying international law at Tilburg University in the Netherlands. She also worked with Professor Ralph Hall to develop his health law compliance capstone course and with Professor Lynn Blewett on comparative health systems research. She graduated with honors and her thesis, “Churning under the ACA: a New Spin on an Old Problem,” was selected as the best master’s project of her graduating class.

Elizabeth’s work experiences have provided insight into law, health policy, and her career goals. She clerked with Kaplan, Strangis and Kaplan where she gained a keen attention to detail but also learned that corporate law was not her passion. Elizabeth sought experience in public service and took a position with the Minnesota Department of Health and Human Services. There she worked with the legal management division and the licensing division on issues ranging from immigration law to eligibility determinations to criminal background investigations. At the Department, Elizabeth saw health policy in action and found value in serving the public. Elizabeth also clerked with the legal department at the Cleveland Clinic. At the Cleveland Clinic, she saw the challenges of health law compliance for providers and gained exposure to patient care. Elizabeth believes that these experiences fostered her passion for health policy and public service.

Currently, Elizabeth is working with the majority staff of the Senate Finance Committee’s health team. She feels honored and privileged to work with her inspiring colleagues and to be a Winston Fellow. She wishes to express her deepest appreciation to the Winston Board for their insightful advice and

2012-2013 Fellow Profiles2012-2013 Fellow ProfilesYajuan Lu, JDYajuan Lu is a native Beijinger and an American-trained attorney. Yajuan developed an interest in health policy while pursuing a bachelor’s degree in Biomedicine & English at Peking University in Beijing, China. She later expanded that interest by pursuing legal studies at the University of Pittsburgh School of Law with a concentration in their Health Law Certificate Program.

Before serving as a Winston fellow, Yajuan had shown her ability to take up leader- ship roles in various settings. She was elected as the President of the Pitt Health Law Society while obtaining her juris doctorate degree, and was selected as one of the three recipients of the Pitt Law Alumni Association award in 2012. Yajuan expanded her horizons in the area of health law through internships at the University of Pittsburgh Medical Center and North Penn Legal Services.

Prior to devoting herself to the study of domestic health policy, Yajuan worked exten- sively on global health issues. In 2010, she received a research grant from the Center of Global Health of University of Pittsburgh to conduct research on Chinese migrant workers’ healthcare seeking behaviors. She conducted a study in the metropolitan area of Beijing through interviews with over 300 immigrants. Yajuan later worked at the legal department of the Pan American Health Organization, regional office of the World Health Organization, headquartered in Washington, D.C. There she focused on legal matters arising out of international healthcare transactions. Former Dean and Professors of Pitt Law have commended Yajuan for her resourcefulness, self- reliance, and her ability to combine social, scientific, policy, and legal perspectives across national and cultural boundaries.

Yajuan is an active member of the Asian Pacific American Bar Association D.C. Chapter, where she and her husband, Alan Leung, have volunteered and provided mentorship for D.C. law students.

Yajuan started her placement as a health policy fellow at Senator Grassley’s office in September 2012. She has been learning from and working with the Senator’s Health Policy Director, Rodney Whitlock, Ph.D. Yajuan describes her experience as “baptiz- ing by the holy fire of health policy” over the past few months as she assists with the development of an entitlement reform project.

Yajuan expresses her deepest thanks to the Winston Board of Trustees for select- ing her as a 2012-2013 David A. Winston Health Policy Fellow. It is because of the thoughtful guidance of the Board, and particularly, her mentor, Chip Kahn, that she is able to work alongside inspiring leaders and effective and passionate members on important healthcare legislative matters. Swarna Vallurupalli, MPH-HP, JD, LLMSwarna comes to the David A. Winston Healthy Policy Fellowship after completing her master of public health, health policy in St. Louis. During graduation ceremo- nies, Swarna was presented with Outstanding Student in Health Policy Award and was inducted into the Delta Omega Honor Society. Swarna also was awarded Great Distinction during her oral competency exams.

While completing her degree, she served as the President of the Graduate Students in Health Policy Association as well as the President of the Academy Health Student Chapter. In both of these roles she worked to enhance the student experience by starting a research program for health policy students, restructuring a career launch panel, and coordinating student-faculty events. Swarna also served as a Graduate Research Assistant in which she worked with faculty members on Alzheimer’s issues, diabetes research, and provided assistance with departmental matters.

Swarna also completed an internship with the Association of Maternal and Child Health Programs (AMCHP) during her time in school. The internship served as an introduction to Washington’s health policy world.

Swarna graduated from Vanderbilt University Law School where she also received a certificate of specialization in Law and Business along with her juris doctorate degree. She attended Vanderbilt on an Honor’s Scholarship. During law school, Swarna served as a research assistant for Professor Jeffrey Schoenblum and worked on issues related to estate planning and taxation. While in law school she served as a member of the school’s Honor Council and as the President of the South Asian Law Student’s Associa- tion. Swarna also received her LLM in Taxation from New York University School Law.

Swarna’s undergradute degree is from the University of Chicago where she graduated with both general academic honors and honors in Economics. Her graduate thesis focused on the effect of deregulation of the electricity industry on consumers. While at the University of Chicago she was inducted into the Maroon Key Society for her leadership and scholarship. Swarna was on the Dean’s List and served as a student leader during Orientation and was the Co-Chair of Senior Week for her class. She also volunteered at the student crises center.

Swarna expresses her deepest appreciation and gratitude to the Board of the Winston Fellowship for selecting her as a Fellow. With the Board’s support and encouragement, she accepted a placement with the Senate Finance Committee’s tax team. She would like to thank everyone who has taken the time to make her fellowship year an excep- tional one: the Board, everyone who took the time to speak with her this summer, and the members of the Finance majority staff on the
