- Tzippys Tidbits - A few extra pounds...-getting enough minutes of sleep or good quality sleep, your metabolism will not

function properly, says the clinical director of a sleep division in Glendale, Ariz.

On average, we need about 7.5 hours of quality sleep per night, he says. If you are get-ting this already, another half hour will not help you lose 10 pounds, but if you are a five-hour sleeper and start to sleep for seven hours a night, you will start dropping weight.

Exactly how lack of sleep affects our ability to lose weight has a lot to do with our nightly hormones.

The two hor-mones that are key in this process are ghrelin and leptin. Ghrelin is the go hormone that tells you when to eat, and when you are sleep-deprived, you have more ghrelin. Leptin is the hormone that tells you to stop eating, and when you are sleep deprived, you have less leptin.

More ghrelin plus less leptin equals weight gain.

You are eating more, plus your metabo-lism is slower when you are sleep-deprived.

Provocative new research involving data from nearly 3 million adults suggests that a having an overweight body mass index (BMI) may be linked to a longer life than one that puts you within a normal weight range.

The research, which analyzed 97 studies in all, found that people with BMIs under 30 but

above normal (the overweight range) had a 6 percent lower risk of dying from all causes than those who were normal weight, while those whose BMIs fell into the obese range were 18 percent more likely to die of any cause. The researchers wrote:

Relative to normal weight overweight was associated with significantly lower all-cause mortality.

Do a Few Extra Pounds Make You Healthier?

The study results imply, at least superfi-cially, that carrying some extra weight may help you live longer or at the very least may

not be as unhealthy as its made out to be. A JAMA editorial highlighted:

The presence of a wasting disease, heart disease, diabetes, renal dialysis, or older age are all associated with an inverse relationship between BMI and mortality rate, an observa-tion termed the obesity paradox or reverse epidemiology.

The optimal BMI linked with lowest mortality in patients with chronic disease may be within the overweight and obesity range.

mechanical effects with some types of trau-matic injuries, and convey other salutary effects that need to be investigated in light of the studies

Indeed, it is quite possible to be overweight and healthy, just as its possible to be normal weight and unhealthy. But for the vast majority of those who carry around extra pounds, health problems will often result.

The study has been heavily criticized for painting an overly simplistic picture of a very complex situation. For instance, it doesnt tell you whether those living longer were afflicted with more chronic disease or whether their quality of life was otherwise impacted. And even more importantly, it used only BMI as a measure of body composition, and this is a highly flawed technique.

Many studies have actually found that a high BMI was associated with a lower risk of death, a phenomenon known as the obesity paradox. But these findings are typically only examples of how BMI is such a flawed measure-ment tool

Why BMI is a Flawed Measurement Tool

If youd like to know how much body fat you have and whether or not your levels put you into a weight category that might lead to health problems, most public health agencies, and therefore most physicians, promote the use of the BMI, which gauges weight in relation to height. But this method is quite flawed, as research suggests it may underestimate obesity rates and misclassify up to one-quarter of men and nearly half of women. According to the president of the nonprofit Path Foundation in New York City:

Based on BMI, about one-third of Americans are considered obese, but when other methods of measuring obesity are used, that number may be closer to 60%.

One of the primary reasons why BMI is such a flawed measurement tool is that it uses weight as a measure of risk, when it is actually a high percentage of body fat that makes a person have an increased disease risk. Your weight takes into account your bone structure, for instance, so a big-boned person may weigh more, but that certainly doesnt mean they have more body fat.

Athletes and completely out-of-shape people can also have similar BMI scores, or a very muscular person could be classified as obese using BMI, when in reality it is mostly lean muscle accounting for their higher-than-average weight. BMI also tells you nothing about where fat is located in your body, and it appears that the location of the fat, particularly if its around your stomach, is more important than the absolute amount of fat when it comes to measuring certain health risks, especially heart disease.

Do You Know Your Body Fat Percentage?

This is another useful tool that is leaps and bounds ahead of BMI as far as gauging your weight-related health risks is concerned. It is FAR better to monitor your body fat percentage than it is your total weight, as the body fat percentage is what dictates metabolic health or dysfunction not your total weight.

Too much body fat is linked to chronic health problems like high blood pressure, high cholesterol, heart disease, diabetes, and cancer, while too little body fat is also problematic and can cause your body to enter a catabolic state, where muscle protein is used as fuel.

Body fat calipers are one of the most trusted and most accurate ways to measure body fat. A body fat or skinfold caliper is a lightweight, hand-held device that quickly and easily measures the thickness of a fold of your skin with its underlying layer of fat. Taken at three very specific locations on your body, these readings can help you estimate the total percent of body fat within your entire body.

face, and mumbled an unconvincing fine when I asked him how he was doing. It was obvious that he was lost

without his wife.

The sorrow I felt was the kind you experi-ence when someone you consider a gitteh neshama does something foolhardy and ends up damaged, like failing to wear a seatbelt and becoming paralyzed in a car crash. I felt anguish because my friends death was a tragedy that might have been prevented.

She had ignored an in your face indication of a potentially life-threatening problem. She had found a lump that should have been investigated in a timely manner, but she ignored it for a very long time. It was almost as if my friend had resurrected and embraced the childhood belief that if you close your eyes, the monster does not exist. If you cant see it, then it isnt there.

But that does not work in the real world. Refusal to acknowledge danger signals do not make them go away.

Months later, when she was, by then, racked with physical weakness and pain, she opened her eyes. And the boogeyman was still there and had grown more menacing. It is hard to say if early intervention would have made a difference in the ultimate outcome. But I do know that it is easier to put out a small campfire than a forest fire. The devastation to her family could have been postponed and she may have been able to walk more of her children to the chuppah and cuddled a few more grandchildren. Instead, several bear her name.

The price of deliberate obliviousness is very high emotionally, physically, socially, and financially. Widowed spouses buckle under the heavy responsibilities and burdens of day-to-day living. Simchos are minimized by empty chairs, and grandchildren are deprived of creating memories because magic moments cannot be shared with those who no longer are there.

Sometimes, the inclination to take care of a potential problem is there, but people refrain from doing so for various reasons. This reality can result in avoidance and ultimately gehakte tzores la.

I was told by a friend that her machutun, a diabetic with no health insurance delayed getting treatment for a small cut that did not heal, and ultimately it infected his whole body. A big part of the problem was that he didnt even know he was diabetic. He had not had blood work done for years. He had no clue how dangerous a small cut could potentially be. His first grandchild was born a few months later. He and his siblings only know him through photos.

There are so many middle class families who are playing Russian roulette with their lives because they have no health coverage. Getting coverage should be their number one priority. In far too many cases, however, money that should be set aside for this purpose is used for expenses that have more to do with vanity than with actual need like updating furniture, spending Yom Tov in hotels, and trips.

Unfortunately, people think that if they feel well, then they dont need to go to the doctor, dentist, or optometrist for an annual checkup. (Brain, eye, facial and oral tumors can be revealed during routine eye tests and dental work). But thats exactly when you should go. Because if you are feeling well when a medical problem is discovered, chances are that any problematic find can be resolved successfully because it is still just a low-flame and not yet a conflagration. The streets were crowded with people. Priests were passing in processions, beating their dreary tambourines; police and custom-house officers with pointed hats encrusted with lac and carrying two sabers hung to their waists; soldiers, clad in blue cotton with white stripes, and bearing guns; the Mikados guards, enveloped in silken doubles, hauberks and coats of mail; and numbers of military folk of all ranksfor the military profession is as much respected in Japan as it is despised in Chinawent hither and thither in groups and pairs. Passepartout saw, too, begging friars, long-robed pilgrims, and simple civilians, with their warped and jet-black hair, big heads, broad torso, slender legs, short stature, and complex-ions varying from copper-color to a dead white, but never yellow, like the Chinese, from whom the Japanese widely differ. He did not fail to observe the curious equipagescarriages and palanquins, barrows supplied with sails, and lit-ters made of bamboo; nor the women who took

little steps with their little feet, whereon they wore canvas shoes, straw sandals, and clogs of worked wood, and who displayed tight-looking eyes, teeth fashionably blackened, and gowns crossed with silken scarfs, tied in an enormous knot behind an ornament which the modern Parisian ladies seem to have borrowed from the dames of Japan.

Passepartout wandered for several hours in the midst of this motley crowd, looking in at the windows of the rich and curious shops, the jewelry establish-ments glittering with quaint Japanese ornaments, the restaurants decked with streamers and banners, the tea-houses, where the odorous beverage was being drunk with saki, a liquor concocted from the fermentation of rice, and the com-fortable smoking-houses, where

they were puffing, not opium, which is almost unknown in

Japan, but a very fine, stringy tobacco. He went on till he found himself in the fields, in the midst of vast rice planta-tions. There he saw dazzling camellias expanding themselves, with flowers which were giving forth their last colors and perfumes, not on bushes, but on trees, and within bamboo enclosures, cherry, plum, and apple trees, which the Japanese cultivate rather for their blossoms than their fruit, and which queerly-fashioned, grinning scarecrows protected from the sparrows, pigeons, ravens, and other voracious birds. On the branches of the cedars were perched large eagles; amid the foliage of the weeping willows were herons, solemnly standing on one leg; and on every hand were crows, ducks, hawks, wild birds, and a multitude of cranes, which the Japanese consider sacred, and which to their minds symbolize long life and prosperity.

As he was strolling along, Passep-artout espied some violets among the shrubs.

Good! said he; Ill have some additional supper.

But, on smelling them, he found that they were odorless.

No chance there, thought he.

The worthy fellow had certainly taken good care to eat as hearty a breakfast as possible before leaving the Carnatic; but, as he had been walking about all day, the demands of hunger were becoming importunate. The one solitary kosher bagel he was given by the Jewish peddler was obviously not enough to satisfy his cavernous appetite. He observed that the butchers stalls

contained neither mutton, goat, nor pork; and, knowing also that it is a sac-rilege to kill cattle, which are preserved solely for farming, he made up his mind that meat was far from plentiful in Yo-kohamanor was he mistaken; and, in default of butchers meat, he could have wished for a quarter of wild boar or deer, a partridge, or some quails, some game or fish, which, with rice, the Japanese eat almost exclusively. But he found it necessary to keep up a stout heart, and to postpone the meal he craved till the following morning. Night came, and Passepartout re-entered the native quarter, where he wandered through the streets, lit by varicolored lanterns, looking on at the dancers, who were executing skillful steps and poundings, and the astrologers who stood in the open air with their telescopes. Then he came to the harbor, which was lit up by the resin torches of the fishermen, who were fishing from their boats.

The streets at last became quiet, and the patrol, the officers of which, in their splendid costumes, and surround-ed by their suites, Passepartout thought seemed like ambassadors, succeeded the bustling crowd. Each time a company passed, Passepartout chuckled, and said to himself: Good! Another Japanese embassy departing for Europe!

Chapter 23

PASSEPARTOUTS NOSE BECOMES OUTRAGEOUSLY LONG

The next morning poor, jaded, famished Passepartout said to himself that he must get something to eat at all hazards, and the sooner he did so the better. He might, i n -deed, sell his watch; but h e would have starved first. Now or never he must use the strong, if

not melodious voice which nature had bestowed upon

him. He knew several French and English songs, and resolved to try them upon the Japanese, who must be lovers of music, since they were forever pounding on their cymbals, tam-tams, and tam-bourines, and could not but appreciate European talent.

It was, perhaps, rather early in the morning to get up a concert, and the audience prematurely aroused from their slumbers, might not possibly pay their entertainer with coin bearing the Mikados features. Passepartout there-fore decided to wait several hours; and, as he was sauntering along, it occurred to him that he would seem rather too well dressed for a wandering artist. The idea struck him to change his garments for clothes more in harmony with his project; by which he might also get a little money to satisfy the immediate cravings of hunger. The resolution taken, it remained to carry it out.

It was only after a long search that Passepartout discovered a native dealer in old clothes, to whom he applied for an exchange. The man liked the European costume, and before long Passepartout issued from his shop outfitted in an old Japanese coat, and a sort of one-sided turban, faded with long use. A few small pieces of silver, moreover, jingled in his pocket.

Good! thought he. I will imagine I am at the Carnival!

His first care, after being thus Japanesed, was to enter a tea-house of modest appearance, and, upon half a bird and a little rice, to breakfast like a man for whom dinner was as yet a prob-lem to be solved.

Now, thought he, when he had eaten heartily, I mustnt lose my head. I cant sell this costume again for one still more Japanese. I must consider how to leave this country of the Sun, of which I shall not retain the most delightful of memories, as quickly as possible.

It occurred to him to visit the steamers which were about to leave for America. He would offer himself as a cook or servant, in payment of his pas-sage and meals. Once at San Francisco, he would find some means of going on. The difficulty was, how to traverse the four thousand seven hundred miles of the Pacific which lay between Japan and the New World.

Passepartout was not the man to let an idea go begging, and directed his steps towards the docks. But, as he approached them, his project, which at first had seemed so simple, began to grow more and more formidable to his mind. What need would they have of a cook or servant on an American steamer, and what confidence would they put in him, dressed as he was? What refer-ences could he give?

As he was reflecting in this wise, his eyes fell upon an immense placard which a sort of clown was carrying through the streets. This placard, which was in Eng-lish, read as follows:

He followed the clown, and soon found himself once more in the Japanese quarter. A quarter of an hour later he stopped before a large cabin, adorned with several clusters of streamers, the exterior walls of which were designed to represent, in violent colors and without perspective, a company of jugglers.

This was the Honorable William Batulcars establishment. That gentle-man was a sort of Barnum, the director of a troupe of mountebanks, jugglers, clowns, acrobats, equilibrists, and gym-nasts, who, according to the placard, was giving his last performances before leaving the Empire of the Sun for the States of the Union.

Passepartout entered and asked for Mr. Batulcar, who straightway appeared in person.

What do you want? said he to Passepartout, whom he at first took for a native.

Would you like a servant, sir? asked Passepartout.

A servant! cried Mr. Batulcar, ca-ressing the thick grey beard which hung from his chin. I already have two who are obedient and faithful, have never left me, and serve me for their nourishment and here they are, added he, holding out his two robust arms, furrowed with

veins as large as the strings of a bass-viol.

So I can be of no use to you?

None.

Shucks! I should so like to cross the Pacific with you!

Ah! said the Honourable Mr. Bat-ulcar. You are no more a Japanese than I am a monkey! Who are you dressed up in that way?

A man dresses as he can.

Thats true. You are a Frenchman, arent you?

Yes; a Parisian of Paris.

Then you ought to know how to make grimaces?

Why, replied Passepartout, a little vexed that his nationality should cause this question, we Frenchmen know how to make grimaces, it is true but not any better than the Americans do.

True. Well, if I cant take you as a servant, I can as a clown. You see, my friend, in France they exhibit foreign clowns, and in foreign parts French clowns.

Some Chevra Kadishas force families of niftarim Rl to use only a certain funeral home. In the emotionally vulnerable period before burial such pressure succeeds almost 100% of the time.

THIS IS ILLEGAL! It stifles competition and inflates funeral costs for members of our community.

See New York State Insurance Law Article 32, Section 3208 (d): No personshall in any way deprive the personal representative or family of the deceased of the advantage of competition in procuring and purchasing supplies and services in connection with the burial of such deceased.

It is disgraceful that some Hatzalah, Bikur Cholim or Chevra Kadisha members roam around in hospitals, to direct business to their favorite chapel. Also, there are Hatzalah members, who "volunteer" to take charge of funeral arrangements for the family and steer them to a chapel when a petira takes place in a private residence.

Be aware: The families must have free choice to choose the funeral services provider they want. One consideration may be cost.

Four funeral homes in Brooklyn are owned and operated by frum families, ie: (in alphabetical order; 1) Kehila Chapels, 718-332-3311, owned by Jacobson Family-Brooklyn. 2) Shomrei Hadas Chapel, 718-436-8700, owned by Seidenfeld Family-Brooklyn. 3) Shomrei Hachomos Chapel, 718-435-8100, owned by Friedman Family-Chicago. 4) Yereim Chapel, 718-384-6784, owned by Brach Family-Brooklyn). The prices they charge for funeral services may vary widely and could make a difference in thousands of dollars.

Most Chevra Kadishas are honest and selfless. However, if a representative insists that you utilize only his recommended funeral home, and especially if he threatens not to issue a burial permit, you are essentially dealing with a CROOK. In that case it is up to you to compare prices and act appropriately.

Sometimes the representative will suggest a compromise that you only do the Tahara at his facility. Dont bite the bait, because that funeral home will then transport the niftar to its own facility and proceed to bill you on the basis of their inflated funeral costs for all the services PLUS the rental of your own desired funeral home for conducting the services. Inform the Chevra Kadisha representative that the Halacha (Rema Yore De'ah 371:4) forbids transporting a niftar even for the purpose of allowing a Kohein to be able to enter his own house. If he remains obstinate, tell him in no uncertain terms that the funeral home YOU choose will conduct the funeral, no matter what. If he doesn't issue the permit, the entire funeral party will remain with the casket outside the headquarters of the Chevra Kadishas parent organization (shul, yeshiva, etc.), even if it's necessary to spend the entire night there! And of course the media will be notified.

Families have every right in the world to institute an action against a Chevra Kadisha organization, chapel or their representatives if they believe they were mistreated or misled.
