    Cilji in kompetence: Objectives and competences:

    Študent bo spoznal biomedicinsko in informacijsko tehnologijo ter aplikacije na področju medicine. Na podlagi osnov se bo poglobil v nekatera specialnima področja odločitvenih in inteligentnih sistemov ter bioinformatike. Specialna področja bodo študentje usvojili predvsem s raziskovalnimi seminarskimi nalogami. To znanje bo študent lahko uporabljal pri raziskovalnem in praktičnem reševanju medicinskih problemov

    Students will learn about biomedical and information technology/application in medicine. Special attention will be focused to decision support and intelligent systems and also bioinformatics. Advanced subjects will be studied through research assignments. The gained knowledge will be used by research and practical problem solving.

    Intended learning outcomes:

    Znanje in razumevanje bioinformacijskih tehnologij s katerimi bo študent sposoben bolj učinkovito reševati raznovrstne medicinske probleme. Prenesljive/ključne spretnosti in drugi atributi: - programiranje in delo z računalnikom - poznavanje računalniško podprte

    biomedicinske tehnologije - poznavanje simulatorjev in simulacij v

    medicini - sposobnost napredne analize in razumevanja

    podatkov.

    Vsebina: Contents (Syllabus outline):

    Kemijske vezi in medmolekularne interakcije ter struktura bioloških makromolekul in supramolekularnih kompleksov. Vezava ligandov in alosterični pojavi. Voda, njena struktura in pomen za biološke sisteme. Biofizika celične membrane in celice: osmozne lastnosti in kislinsko-bazno ravnotežje, metabolizem celice, mehanske lastnosti celične membrane, transport preko celične membrane, električna vzdražljivost celice in prenos električnega impulza. Biofizika celičnega skeleta in molekularnih strojev subceličnih dimenzij (delovanje mišice). Izbrani fiziološki sistemi: kri in krvni obtok, čutila, okostje in mišice, živčevje. Regulacija bioloških sistemov (sistemska analiza, regulacija metaboličnih sistemov - kontrolna teorija). Biološki dinamični sistemi (celična signalizacija, razvoj populacije). Samoorganizacija bioloških sistemov. Teorije in modeli evolucije. Interakcija neionizirajočega elektromagnetnega sevanja s humanim tkivom. Interakcija ionizirajočega sevanja s humanim tkivom. Pregled eksperimentalnih biofizikalnih metod. Pregled osnovnih konceptov statistične termodinamike s poudarkom na obravnavi bioloških

    Chemical bonding, intermolecular interactions and structure of biological macromolecules and supramolecular systems. Binding of ligands and allosteric phenomena. Structure of water and its meaning for biological systems. Cell and cell membrane biophysics: osmotic properties, acid-base equilibrium, metabolism, mechanical properties of a cell membrane, membrane transport, electrical excitability and propagation of electric pulse. Biophysics of cytoskeleton and molecular motors. Selected physiological systems: blood and cardiovascular system; senses, skeletal-muscular system, nervous system. Regulation of biological systems (system analysis, control theory of metabolic systems). Biological dynamic systems (cell signalling, evolution of population). Self-organisation of biological systems. Theory and models of evolution. Interaction of non-ionising electromagnetic radiation with human tissue. Interaction with ionising radiation with human tissue. Overview of experimental methods in biophysics. Overview of basic concepts in statistical thermodynamics applied to biological systems.

    Cilji: Objectives:

    Cilj predmeta je obravnavati strukturo in funkcijo bioloških sistemov oziroma njihovih gradnikov na molekularni in makromolekularni ravni, na stopnji supramolekularne organiziranosti, na ravni celice in interakcije med njimi ter na ravni organov človeškega telesa. Pristop temelji na matematični formulaciji konceptov v biofiziki. Obravnavani primeri so izbrani iz humane biologije in zato posebej zanimivi za medicino.

    The main objective of the course is to discuss the structure and function of biological systems on different levels of biological complexity from a molecular, macromolecular and supramolecular level to a cellular level and tissue as well as to organs of the human body. The course is based on mathematical formulation of biophysical concepts. In particular, systems presented are selected from human biology and, therefore, they are applicable to medicine.

    Intended learning outcomes:

    Znanje in razumevanje: Osvojeno pregledno interdisciplinarno znanje o strukturnih lastnosti in delovanju bioloških sistemov na različnih ravneh organiziranosti od molekule do organizma.

    Knowledge and Understanding: Broad interdisciplinary knowledge of structure and function of different biological systems considered at different levels of complexity from molecules to human organs.

    Prenesljive/ključne spretnosti in drugi atributi: Sposobnost vključitve v poglobljeno raziskovalno delo z namenom nadaljevanja doktorskega študija in izdelave doktorata na različnih problemih biomedicine.

    Transferable/Key Skills and other attributes: Ability of a student to be involved deeply in research in order to continue his/her doctoral studies leading to PhD thesis on various problems from biomedicine.

    Metode izolacije in kvantitativnega ter kvalitatitvnega določanja strukture in funkcije beljakovin: elektroforeza, kromatografske metode, pI, spektrofotometrija, FTIR, MALDI-MS, proteomika in metabolomika, biološki modeli.

    Imunocito(histo)kemiske metode: temeljni principi in aplikacije.

    Napake v strukturi beljakovin in z njimi povezane bolezni.

    Encimi: Regulacija in klinična aplikacija: plazemski intracelularni encimi, merjenje encimske aktivnosti, serumski markerji pri poškodbah tkiva, encimi kot analitični in terapevtski reagenti.

    Heteropolisaharidi: glikoproteini in glikolipidi, proteoglikani in peptidoglikani, oligosaharidi in občutljivost gostitelja.

    Metode določanja ogljikovih hidratov.

    Objectives:

    Premet ponuja študentu pregledna in nekatera poglobljena znanja o zgradbi, organizaciji ter delovanju prokariontskega in eukariontskega genoma. Študenta seznani z vlogo genetskih faktorjev pri vzrokih humanih bolezni ter prispevku pri multifaktorjalnih boleznih, s kompleksno analizo delovanja in prenosa genetske informacije ter dedovanjem. Poudarek predavanj je na aplikativni vlogi genetike v sodobnih medicinskih tehnikah, diagnostiki in genski terapiji.

    The subject offers a review and some extensive knowledge of the structure, organization and action of the prokaryotic and eukaryotic genome. It will acquaint the student with the role of genetic factors in human diseases and with their contribution to multifactorial diseases, with a complex analysis of the action and transfer of genetic information and inheritance. The stress is on the applicative role of genetics in modern medical techniques, diagnosis and gene therapy.

    Znanje in razumevanje: Študent si poglobi znanje o genetskih mehanizmih ter razširi poznavanje uporabe genetskih tehnologij v medicini in biotehnologiji. Praktična znanja pridobi pri individualnem delu ob izdelavi raziskovalnega (doktorskega) projekta v genetskem laboratoriju, kjer se seznani z osnovnimi tehnikami genetske analize.

    Knowledge and Understanding: The student acquires knowledge of genetic mechanisms and a broader knowledge of the use of genetic technologies in medicine and biotechnology. He/she acquires practical knowledge in individual work in preparing a research (doctoral) project in the genetic lab, where he/she becomes acquainted with the basic techniques of genetic analysis.

    Prenesljive/ključne spretnosti in drugi atributi: Ob izdelavi raziskovalnega (doktorskega) projekta se nauči uporabljati metode ter rezultate genetskih analiz v različne aplikativne namene na področju medicinske genetske diagnostike.

    Transferable/Key Skills and other attributes: In preparing the research (doctoral) project the student learns to use the methods and results of genetic analyses for various applicative purposes in the field of medical genetic diagnosis.

    Cilji: Objectives:

    Snov predmeta, skupaj s seminarskimi vajami, omogoča podiplomskemu študentu pridobiti znanja molekularne biologije, molekularne genetike in predvsem razumevanje novih načinov terapij in tarč, ki izhajajao iz spoznavanja človeškega genoma. Prav tako predmet omogoča spoznavanje najpomembnejših kompleksnih zdravilnih učinkovin iz področja biofarmacevtikov.

    The content of the subject is created together with the parallel seminar course, in order to expose the knowledge of molecular biology, molecular genetics and understanding of novel approaches to gene therapy, based on the human genome sequencing programme. Furthermore, the subject enables the student to be acquainted with the most important biopharmaceuticals.

    Znanje in razumevanje: Z razumevanjem sestave in delovanja biofarmacevtikov in uporabe genske terapije se poveča praktično znanje o biofarmacevtikih, glavnih terapevtskih učinkih in razumevanje nastajanja neželjenih učinkov.

    Knowledge and Understanding: By understanding of the composition and technology of biopharmaceutical drug production, and development and use of the gene therapy, students will expand their practical knowledge and understanding of therapeutical effects as well as of undesired side effects of biopharmaceuticals.

    1. Klinično preizkušanje zdravil Ko so končane raziskave na živalih, celicah in tkivih, je mogoče sklepati na učinkovitost in varnost nove učinkovine orientacijsko v bioloških sistemih. Pri kliničnem raziskovanju oz. preizkušanju zdravil so štiri faze, po tretji pride do registracije zdravila, četrta faza pa vključuje vse raziskave od tedaj naprej. Za registracijo zdravila je potrebna kemijska analiza zdravila, pri generičnih zdravilih pa klinične farmakokinetične raziskave. Smernice za klinično preizkušanje zdravil so zajete v principih dobre klinične prakse oz. mednarodnih konferenc za harmonizacijo. Partnerji v klinični raziskavi so: državni organ oz. ministrstvo za zdravje oz. urad za zdravila in etična komisija; raziskovalci, praviloma zdravniki, največkrat v univerzitetnih ustanovah; sponzor oz. plačnik raziskave, ki je lastnik rezultatov in odgovoren za nadzor. Tipi kliničnih raziskav so: odprta, enojno slepa, dvojno slepa, randomizirana, nerandomizirana, sekvencijska, raziskava v primerjavi s skupino placebo, raziskava z dvema ali več terapevtskimi skupinami, raziskave v enem ali v več centrih, raziskava z zgodovinsko kontrolno skupino, z majhnim številom preiskovancev ali velika klinična raziskava, metaanaliza. Najnovejši diagnostični in terapevtski dosežki v dermatovenerologiji

Evropska organizacija za raziskave in zdravljenje raka Evropska organizacija za raziskave in zdravljenje raka (EORTC) je mednarodna raziskovalna organizacija s sedežem v Bruslju, ki je bila ustanovljena leta 1962. Namenjena je napredku multidisciplinarnih raziskav na področju raka in je povezana z vodilnimi raziskovalnimi ustanovami, ki delujejo na področju biomedicine. V ta namen razvija, izvaja, koordinira in vzpodbuja eksperimentalne in klinične raziskave raka v

In clinical research, or testing of drugs there are four phases, after the third, a drug can be registered; the fourth phase includes all researches after this point.     Evropi. Končni cilj EORTC je, s pomočjo razvoja novih zdravil in drugih inovativnih postopkov, preskušanja bolj učinkovitih načinov zdravljenja, kirurgije in obsevanja, izboljšati standarde zdravljenja raka v Evropi. V ta namen povezuje v omrežje številne znanstvenike, klinike in ustanove, ki organizirajo laboratorijske in klinične raziskave vseh vrst rakov. 4. Raziskave v kirurgiji Znanstveno-raziskovalna dejavnost v kirurgiji je postavila temelj sodobne kirurgije. Sodobna kirurgija temelji na izsledkih številnih vej naravoslovnih znanosti in je z njimi tesno prepletena. Po zadnjih petletnih izsledkih Angleške kraljeve kirurške šole in Evropske akademije za kirurške raziskave poteka okoli ¾ raziskav iz področja kirurgije na posameznih temeljnih področjih sodobne biomedicine (molekularna genetika, biologija celice, tkivni inženiring, biomateriali, ipd), četrtina projektov pa je povezana s kliničnim raziskovanjem. Cilji evropske akademije za raziskave na področju kirurgije so:

    Prispevati k izboljšanju medicinske oskrbe bolnika in njegove rehabilitacije.

    Razčlenjevati, razvijati in uvajati nove in sodobne kirurške postopke.

Researches in surgery Scientific research activity in surgery established basis of modern surgery. Modern surgery is founded on the results of the numerous branches of science and is tightly interweaving with them. According to the five-year results of the English Royal Surgeon School and European Academy for Surgeon Researches, about ¾ of the researches in the field of surgery are carried out in the basic fields of modern biomedicine (molecular genetic, biology of cell, tissue engineering, biomaterials etc), ¼ of the projects are linked with clinical research.     Sem prištevamo sistemski lupus eritematozus, druge oblike lupusnega sindroma, sjögrenov sindrom, sistemsko sklerozo, polimiozitis, dermatomiozitis, prekrivajoči se sindrom. Te bolezni so po kliničnih in laboratorijskih značilnostih zelo podobne, ob enem pa dovolj različne, da jih lahko ločimo med seboj. Skupne značilnosti so:

    avtoimunska patogeneza,

    prisotnost genov HLA-DR lokusa. Prizadenejo lahko različne organe, značilna in specifična so serumska avtoprotitelesa, nekatere bolezni so tudi posledica vpliva raznih zdravil. 6. Analiza slikovnih preiskav v klinični medicini Poudarek je na bolniku in raziskovalcu prijaznih metodah, kar je tudi v skladu z navodilom Etične komisije, vsekakor pa se ne bo mogoče v poglobljenem delu izogniti obremenilnih preiskav in preiskav, ki imajo določen riziko, tako za bolnika, kot za raziskovalca. Osnovna slikovna prijazna preiskava je dokazano ultrazvočna analiza z uporabo kontrastov. V našem terciarnem programu klinične medicine je tudi endoskopski ultrazvok, tako s kontrasti, kot možnostjo pridobitve cisto - histo materiala, kakor tudi aplikaciji določenih kemičnih in drugih bioloških agensov v izbrano tarčo. Komplementarno bi vsekakor bila tudi vključena v raziskovalne namene kompjuterizirana tomografija, dodana s pozistronsko emisijsko tomografijo (PETCT), ki pogosto v predlagani klinični praksi dopolni znanstveno raziskovanje in objektivno analizira rezultate. Posebno področje bi bilo namenjeno magnetnoresonančni analizi, med drugim tudi uporaba določenih predproceduralnih in poproceduralnih računalniških dodelav. Prav tako se prišteva k slikovnim preiskavam izvide nuklearno medicinskih pregledov. Vse te standardne slikovne metode bi dopolnil s slikovno arhivo endoskopskih preiskav. Aktivnosti na področju video konferenc v raziskovalne namene klinične prakse - postaviti mesto in težo aktivnostim na nivoju video konferenc. 7. Epidemiološke metode – temeljno orodje v klinični medicini in javnemu zdravju V okviru tega sklopa bo predstavljena epidemiologija kot temeljna znanost ki omogoča opis, in kvantifikacijo določenih zdravstvenih problemov ter proučevanje vzrokov za pojav le teh. V okviru predmeta se bodo študenti seznanili s temeljnimi epidemiološkimi metodami in postopki. These diseases are similar according to their clinical and laboratory features, but at the same time enough different to be distinguished. Common characteristics are:

    autoimmune pathogenesis,

    presence of the HLA -DR locus genes. Diseases can affect different organs, one of the features is presence of specific serum auto antibodies, and some diseases are also consequences of the influence of different medications. 6. Analysis of the images in the clinical medicine Accent is on the patient – researcher friendly methods, in concordance with the instructions issued by the Ethical Commission. However, it will not be possible to avoid overwhelming researching and a research involving risk for the patient and a researcher. It has been proved that the only patient-friendly image examination is ultrasound analysis using contrasts. Our tertiary program of clinical medicine deals with usage of endoscopic ultrasound using contrast as possibilities of gaining cysto-hysto material, and applications of specific chemical and other biological agenses into chosen target. Complementarily, computerised tomography plus positron emission tomography ((PETCT) are often suggested in clinical practice to supplement scientific research and objective result analyses. Special field is dedicated to magnet resonance analyses, and other pre procedural and post procedural computerised analyses. Images are also nuclear medical examinations. All this standard image methodes are supplemented with archived endoscopic examination images. Activities in the field of video conferences for clinical practice research purposes – activities should gain their proper weight at the level of video conferences. 7. Epidemiological methods – basic tool in clinical medicine in public health system Within this frame, epidemiology as basic science which enables description, quantification of certain health problems and research of their causes will be presented. Students will introduce basic epidemiological methods and procedures. Principles and purposes of the epidemiological studies, abilities and limitations of these studies, and typical kinds of studies within descriptive and analytical epidemiology. Na dokazih temelječe delo v medicini Na dokazih temelječe delo v medicini predstavlja pomembno spremembo v načinu dela, učenja, poučevanja in raziskovanja zdravnikov. Opredeljeno je kot uporaba trenutno razpoložljivih najboljših dokazov v odločanju o oskrbi posameznih bolnikov. Dokaz sam po sebi ni dovolj za kakovostno oskrbo bolnika. Poudarjeno je, da prakticiranje na dokazih temelječe medicine zahteva njeno povezavo s kliničnim strokovnim znanjem, kar vključuje naslednje pomembne elemente: − strokovno jemanje anamneze in izvedbo

    telesnega pregleda; − poznavanje bolnika, družine in skupnosti, kar

    omogoča odločanje o zdravljenju v kontekstu bolnika;

    − odnos z bolnikom, ki upošteva njegove predstave in vrednote;

    − praktično znanje o dostopnosti virov v skupnosti.

    Brez kliničnega strokovnega znanja, postane delo v praksi tiranizirano z zunanjimi dokazi, kajti še tako odlični zunanji dokazi so lahko povsem neprimerni za določenega bolnika. In brez trenutno najboljših zunanjih dokazov, postane delo v praksi hitro zastarelo in v škodo bolnikov.

Evidence-based medicine work Evidence based work in medicine is an important change in the doctor's way of work, learning, teaching and researching. It is defined as use of momentarily available best evidences in patient's health care decision making. Evidence by itself is not sufficient for patient’s quality health care. It is pointed out that practicing of evidence based medicine demands liaison with clinical professional knowledge, taking into consideration important elements, as follows: professional history taking and physical examination; − information about patient, family and

    community, treatment of the patient in patient's context.

    − relationship with patient, considering their believes and values;

    − practical knowledge about availability of supportive sources in the community.

    Without clinical professional knowledge, practice becomes tyrannised by external evidences, but even excellent external evidences can be totally inadequate for particular patient. However, without momentarily best external evidences, work in practice soon becomes out of date and harmful to the patient.

    pridobivanju podatkov za razlago slikovnih raziskav, spoznavanje osnovnih značilnosti posameznih vrst slikovnih raziskav in analiz, seznanitev z veliko obremenjenostjo ponavljanja slikovnih analiz v Sloveniji – njihovo dupliciranje. Slušatelji bodo usposobljeni za pridobivanje različnih slikovnih gradiv in dokumentov po medmrežju medicinskega depoja varovanega z ključnimi vstopnimi kodami. Epidemiološke metode – temeljno orodje v klinični medicini in javnemu zdravju Seznanitev slušateljev s pristopi in problemi pri pridobivanju podatkov za epidemiološke raziskave, spoznavanje osnovnih značilnosti posameznih vrst epidemioloških raziskav, seznanitev z velikimi javno zdravstvenimi problemi v Sloveniji. Poleg tega bodo slušatelji usposobljeni za uporabo različnih meril za oceno teže posameznih bolezni ter analizo uspešnosti različnih diagnostičnih in terapevtskih metod. 8. Raziskovanje in metodologija proučevanja duševnih procesov Spoznavati merske procese na področju duševnosti in posebnosti merjenja duševnih aktivnosti (strukturirani in nestrukturirani parametri). Specifičnost metodoloških pristopov pri duševnih procesih. 9. Statistika v medicini Jasna predstava o možnostih uporabe sodobnih statističnih metod pri raziskovanju v medicini in širše. 10. Raziskovanje in metodologija proučevanja duševnih procesov Pridobljeno znanje bo vplivalo na razumevanje duševnih procesov, kandidati bodo spoznali drugačnost in specifičnost mentalnih parametrov, ter se naučili obvladovati variable, ki v laični populaciji predstavljajo relativne entitete. Spoznali bodo, da je na področju duševnosti zelo kompleksna in eksaktna metodologija proučevanja zaradi specifičnosti narave variabel. 9.     2. Najnovejši diagnostični in terapevtski dosežki v dermatovenerologiji Evalvacija pridobljenih rezultatov, njihova korelacija s klinično sliko ter pomen teh pri zdravljenju. 3. Evropska organizacija za raziskave in zdravljenje raka Poznavanje pomena EORTC pri izvajanju raziskav na področju zdravljenja raka. 4. Raziskave v kirurgiji Praktično znanje za vodenje raziskav. 5. Sistemske vezivno-tkivne bolezni Analiza in evalvacija testov, s pomočjo katerih ocenjujemo avtoimunska dogajanja. 6. Analiza slikovnih preiskav v klinični medicini Študentje lahko s pridom direktno sodelujejo v proučevanju različnosti posameznih slikovnih algoritmov značilnih za posamezne bolezenske sindrome in analizirajo uspešnost obvladovanja posameznih bolezni na analizi najbolj povednih slikovnih metod. Pridobljeno znanje je lahko aplicirano tudi za ocenitev učinkovitega zdravljenja. Nadaljnja analiza omogoča tudi možnost diferenciranja korektnosti in strokovne zanesljivosti posameznim nosilcev slikovnih diagnostično terapevtskih metod. 7. Epidemiološke metode – temeljno orodje v klinični medicini in javnemu zdravju Slušatelji lahko s pridobljenim znanjem sodelujejo pri proučevanju razširjenost zdravstvenih problemov kot tudi pri analizi uspešnosti obvladovanja posameznih bolezni in stanj ter pri ocenjevanju učinkovitost zdravljenja. Pridobljeno znanje lahko uporabijo pri zagotavljanju kakovosti posameznih procesov dela. 8. Raziskovanje in metodologija proučevanja duševnih procesov Poznavanje raziskovanja na področju duševnosti pripomore pri vsakršnem proučevanju drugih problemov, ko se pojavijo manj enoznačni, latentni ali manj razumljivi pojavi. Duševnost se v marsičem prepleta s telesnim, poznavanje raziskovanja na področju duševnosti pa poleg poznavanja raziskovanja na področju drugih medicinskih strok omogoča interaktivnejše, kompleksnejše in pogosto bolj reliabilne pristope k obravnavi problemov v medicini. 9. Statistika v medicini Uporaba statistike kot orodja pri raziskavah, razumevanje rezultatov in tolmačenje.

Acquired knowledge should enable student to ensure work quality process. 8. Researching and methodology of investigation into mental process Knowledge about researching in the mental field enable us to study about other problems/issues, when we have to deal with less obvious, characteristic, latent or less understandable issues. State of mind is in interconnection with physical state. Knowledge about researching in the field of mentality, as well as other medical fields enable more interactive, complex and often more reliable approaches to problem solving in medicine. 9.
